A Case Report of a Patient with Shoulder Subluxation induced Shoulder Pain after Intracerebral Hemorrhage Treated with Korean Medicine (뇌출혈 이후 발생한 견관절 아탈구로 견관절 통증을 호소하는 환자의 한의 복합치료 증례보고 1례)

  • Objective : The aim of this study was to report the effectiveness of Korean medicine on a Intracerebral hemorrhage patient with shoulder subluxation induced shoulder pain. Method : The patient was treated with acupuncture, electroacupunture, soyeom pharmacopuncture, electrical stimulation therapy, and rehabilitation exercise. The treatment effect was evaluated through the Numeral Rating Scale(NRS), Passive Range Of Motion(PROM) and Fingerbreadth Palpation Method(FPM). Results : After 31days of Korean Medicine including acupuncture, electroacupunture, soyeom pharmacopuncture, electrical stimulation therapy, and rehabilitation exercise treatment, the pain parameters and range of motions improved. Conclusion : This study suggested that Korean medicine might be effective for shoulder pain and range of motions in patients with intracerebral hemorrhage.

A Roentgenographic Cephalometric Study of Temporomandibular Joint Subluxation (악관절아탈구증에 대한 두부방사선계측학적 연구)

  • The purpose of this study was to assess temporomandibular joint subluxation by means of cephalometry using two lateral cephalograms from each person with in centric occlusion and wide-open mouth position, and to compare patient group with subluxation to normal control group in the measurements and correlation coefficient. The 200 cephalograms of 100 Korean adults, patient group consisted of 24 females and 26 males ranged from 17 to 63 years age and the normal control group consisted of 20 females and 30 males ranged from 18 to 56 years age, were studied and analyzed statistically. The results were as follows; 1. In the comparison of patient group vs normal control group in the measurements, statistically significant differences were found in C-C', C'-PTM, K-FH, K-PTM, Gn-Gn', C-S-C', Gn-S-Gn', Gn-K-Gn', GoGn-SN, and GoGn-Go'Gn'. K-point* of patient group was located antero - superiorly than of normal control group, and the significance level was higher in K-PTM than in K-FH. There was no statistically significant difference found in local relationship of C-point between patient group and normal control group. The values of correlation coefficient among all measurements were in 0.958≥r≥-0.760, and the highest value was in Gn-Gn' to GoGn-Go'Gn' and Gn-K-Gn' to Gn-Gn', and the lowest value was in C'-PTM to Gn-K-Gn' of normal control group. K was determined as a point of intersection by a perpendicular bisector of Gn-Gn' and a perpendicular bisector of C-C'.

Effect of Manual Therapy on a Patient With Atlantoaxial Rotatory Subluxation (환축추 회전 아탈구 환자에 대한 도수치료 효과)

  • Background: An 8-year-old girl had severe neck pain and stiffness after trauma. CT scan showed atlantoaxial rotatory subluxation (AARS). She had conservative treatment because she did not have neurological symptoms and spinal basilar artery dysfunction. Conservative therapy was halter traction twice for 4 weeks. However, pain and stiffness persisted. She had been recommended to have surgery from her physician, but she received manual therapy for non-surgical procedures. Methods: The joint mobilization, muscle energy technique, motor control exercise, and deep neck flexor (DNF) endurance exercise were applied as manual therapy and 10 session for 2weeks. Results: Clinical outcomes were measured at initial baseline, after 2 weeks, and after 6weeks. Active range of motion was completely restored after 6weeks and numeric pain rating scale was completely reduced after 2 weeks. The strength of neck flexor muscle recovered to normal after 2 weeks, and the DNF endurance was improved to 25 seconds after 2 weeks and to 42 seconds after 6weeks. Motor control capacity recovered to 30 ㎜Hg after 2 weeks. Conclusions: This case report describes the immediate and short-term clinical outcomes for a patient presenting with symptoms of neck pain following AARS. Clinical rationale and patient preference aided the decision to incorporate manual therapy as a treatment for this patient. Manual therapy has shown a successful recovery in AARS patients, more research is needed to validate the inference of this case report.

  • . Open posterior capsulolabral reconstruction(PCLR) in posterior subluxation yields very excellent clinical results if it is positional. . The main pathologic lesion is an excessive redundancy of the posteroinferior capsule. . The muscular type of posterior instability is contraindicated in PCLR.

  • The role of arthroscopy for the diagnosis of polyethylene wear after total knee arthroplasty has been reported previously. In this report, we demonstrate a case of wear of meniscal bearing in unicompartmental knee arthroplasty (UKA) and recurrent meniscal bearing subluxation which was diagnosed by arthroscopy. Arthroscopic examination has its role in diagnosing the wear and subluxation of polyethylene bearing after UKA.

  • Objectives : To introduce the medicare listing system for the clinicians that korean society of Chuna manual medicine for spine and nerves applies presently and wants to promote. Methods : Compare and analyse the differences, merits and demerits between Palmer-Gonstead listing system and medicare listing system using the publications and literatures of Chuna manual medicine and chiropractic. Results : It is easy to explain the movements and subluxations of spine when using medicare listing system. Also it has simple terminological system that can be applied when diagnosing the lesion of spinal joints with various palpations, Conclusion : Listing system used by Korean society of Chuna manual medicine for spine and nerves presently has very appropriate forms to indicate the movements and subluxations, However, it needs to spread to clinicians who still are using former listing system by continuous education.

Relation of necessity of the first aid education and attitude about traumatic dental injuries in some military men (일부 군인들의 치아외상에 대한 태도와 응급처치 교육필요성과의 관련성)

  • Objectives : The purpose of this study was to investigate the traumatic injuries by first aid education and mouth protector in some military men. Methods : Among 195 military men, a total of 164 military men filled out the self-administered questionnaire and the return rate was 84.1%. The data were evaluated statistically using chi-square analysis. Results : 81.5% of respondents agreed that first aid education is necessary and 48.2% of respondents agreed that mouth protector of is necessary. In managing tooth fracture, subluxation, and avulsion, the military men answered the necessity of the first aid education revealed 87.2%, 86.8%, and 91.5%, respectively. The questions in managing tooth fracture, subluxation showed that mouth protector is necessary 56.3%, 49.0% respectively. In managing tooth fracture with reattachment, the necessity of the first aid education showed 88.9%. Conclusions : In order to get the knowledge about emergency treatment of traumatic injuries in military men, it is necessary to develop the professional education program for the dental hygiene personnel.

  • Objectives : Yogaktong is the genernal term of lowback and sciatic pain. Simpley this is not for the syndrom or illiness but for the symptoms. In this study, the effect of Acupuncture and Moxibustion Treatment by Analysing Su-point(背兪穴) compared with X-ray measuring for three patients with Yogaktong(腰脚痛) were evaluated. Methods : After treatment with the Acupuncture and Moxibustion Treatment by Analysing Su-point(背兪穴) compared with X-ray measuring for one time, we evaluated the effect by the figure of x-ray measuring and complaints of patients. Results : In three cases, the change in the figure declined dramatically and symptoms of patients got better after one treatment. Conclusions : Acupuncture and Moxibustion Treatment by Analysing Su-point(背兪穴) compared with X-ray measuring was effective. It will be attempted to the patients with it.

  • A 23.5 kg179, 8 months olds non-spaycd female Rottweiler dog was submitted to the veterinary teaching animal hospitals Chonbuk: national university, for chronic bitten trauma on right foreleg with pain, lameness, and pronation. The patient fought with a neighboring dog about five months ago and had mild anorexia, depression, bolt normal walking at that time. CBC and blood chemical examination were in normal range. Physical examination resulted in the right foreleg with edema and interval rotation of carpal joint. Radiograph of the right carpus showed varus and subluxation. We finally diagnosed the patient as the growth deformity of distal radius and ulna caused by medial premature closure of distal radial physis. Osteotomy was performed fur reforming of the varus of the distal radius and ulna with 6-hole straight plate and six 3.5 mm screws. Panarthrodcsis of carpus was preformed for correcting subluxation using 6-hole T-shaped plate and four 3.5 mm screws with cortical onlay autograft and strengthened by two 3.5 mm screws and tension wire band in lateral aspect of the carpus. Follow-up radiographs after 16 weeds of the surgery showed complete coaptation of osteotomy sire of the radius and uIna. After 6 months of the procedurc, talc plate, screws and the wire were removed except 7-shaped plate and four screws fur arthrodesis. The patient was seen in normal forelimb and could walk and run without lameness after 7 months of the procedure.
