뇌졸중 발생위험군의 뇌졸중 발생위험도, 뇌졸중 지식과 건강증진행위 (Probability of Stroke, Knowledge of Stroke, and Health-Promoting Lifestyle in Stroke Risk Groups)

  • Purpose: This study was done to investigate the probability of stroke, knowledge of stroke, and health-promoting lifestyle among stroke risk groups. Method: A descriptive correlational design was used. Data for 110 patients were analyzed. The probability of stroke was calculated using the Stroke Risk Profile from the Framingham Heart Study (2013), knowledge of stroke was measured using a questionnaire developed by Yoon et al. (2001), and health-promoting lifestyle was measured using the HPLP-II, developed by Walker et al. (1995). Results: The average probability of stroke was 11.74, knowledge of stroke, 67.88, and health-promoting lifestyle, 2.27. Probability of stroke showed significant differences according to gender. Knowledge differed according to patients' salaries. Health-promoting lifestyle showed significant differences according to gender. There were no significant correlations between probability of stroke and knowledge of stroke or probability of stroke and health-promoting lifestyle, but there was a significant correlation between knowledge of stroke and health-promoting lifestyle. Conclusion: Results indicate the necessity of active education to increase knowledge related to stroke which will contribute to an increase in health-promoting behaviors and make primary prevention a reality in the reduction of risk of stroke among stroke risk groups.

뇌졸중 환자의 낙상 형태와 낙상 관련요인 (The Fall Circumstance and Related Factors Associated with Fall in the Stroke Patients)

  • Purpose: We analyzed the incidence of falls and the related factors, circumstances, and consequences associated with falls in stroke patients. Methods: We recruited 127 stroke patients and used a self-reported questionnaire to measure fall prevalence rates and the related factors, circumstances, and consequences of falls. The chi-square test was used to establish associations between related factors. Results: The prevalence of falls in stroke patients was 69.3%, and was associated with gender and time since the stroke. Falls occurred 2-5 times (55.4%) poststroke and most subjects first fall in the 2$\sim$6 month (46.5%) after the stroke. Most (55%) falls occurred at the hospital. Walking was the most frequent circumstance for falls (38.5%). Most (54.4%) falls led to consequences such as fractures, ligament strains, bruises, or abrasions. Conclusion: Fall-prevention strategies decrease the number of falls and the severity of fall-related injuries. These data support the concept of preventive strategies for falls in stroke patients who are at risk.

Seasonal Variation of Acute Stroke;Hospital Based Study

  • Objectives: This study aimed to investigate the seasonal variation in stroke types, ischemic stroke subtypes, stroke risk factors and Sasang constitutions. Methods: 226 patients with acute stroke within 14 days onset were included, who were admitted to Kyunghee Oriental Medical Center from November 2005 to October 2006. The year was subdivided into four parts: spring (March-May); summer (June-August); fall (September-November); and winter (December-February). Stroke types, ischemic stroke subtypes, stroke risk factors and Sasang constitutions in the four groups were examined. Results: Ischemic stroke was most frequent in summer, whereas hemorrhagic stroke was most frequent in winter. There was no significant difference in seasonal variation of stroke. The frequency of ischemic heart disease among stroke risk factors was significantly high in spring (p=0.031). The frequency of hypertension, diabetes mellitus, atrial fibrillation, and hyperlipidemia did not differ among seasons. There was no significant difference in Sasang constitution among seasons. Frequency of small vessel occlusion was highest in summer. Large artery atherosclerosis was frequent in spring and summer,but seasonal variation of ischemic stroke subtypes did not show statistical difference. Conclusion: Acute stroke demonstrates seasonal characteristics according to stroke types, ischemic stroke subtypes, risk factors for stroke, and Sasang constitutions. These results have important clinical implications in stroke prevention.

노인 뇌졸중 입원환자의 역학적 연구 (Epidemiologic Study of Geriatric Cerebrovascular Accident Inpatients)

  • This study was done to research the general information, causes of stroke, risk factors, complications during admission of geriatric CVA inpatients. We retrospectively studied 208 above 65 years old geriatric CVA inpatients treated in Ajou university hospital from 1994 to 2003 by medical record. These were divided into two groups by following years. We were gathered information about sex, onset age, cause of stroke, number of stroke, complications, housing, family, insurance, smoking, alcohol. Most of incidence of the stroke was noted in the group of young old age (65~74 years old) and old age (75~84 years old). The occurrence rate of male stroke (38.5%) was less than of female stroke (61.5%) and the ratio of male to female was 1:1.6. The occurrence rate of ischemic stroke (72.6%) was higher than of hemorrhagic stroke. The occurrence rate of ischemic stroke increased more and more at the late stage. The most common risk factors for stroke was hypertension and complications during hospitalization were neurogenic bowl and bladder. These results of epidemiologic study may help above 65 years old geriatric CVA early treatment and prevention, rehabilitation and use basic data for multiple prospective study using stroke registry.

뇌졸중 환자의 체질별 혈액학적 특성 -1개 한방병원의 입원환자를 중심으로- (Hematologic Characteristics of Stroke Inpatients according to Sasang Constitution in One Korean Medicine Hospital)

  • Objectives The purpose of this study is to know hematologic characteristics of patients with stroke in Sasang Constitutional Medicine which will be helpful for the prevention and treatment of stroke. Methods We collected data from 111 patients who received treatment as a stroke inpatient at one Korean medicine hospital in Busan, Korea. A Sasang Constitutional Medicine(SCM) specialist diagnosed their Sasang Constitutional types based on the four diagnosis standard of SCM and herbal medicine reaction. Results of hematologic tests were classified according to gender, age, stroke types and Sasang Constitutional types and analysed by SAS system. Results 1. Male's mean value of RBC count, Hemoglobin and Hematocrit was lower than normal range. 2. The mean value of hsCRP of above age 65 group was 3.19 times significantly higher than under age 65 group. 3. The mean value of platelet of cerebral hemorrhage group was significantly higher than cerebral infarction group. 4. The mean value of triglyceride of Taeeumin was significantly higher than Soyangin and Soeumin. Conclusions For the prevention of stroke, appropriate treatment of stress, trauma and infections is needed and male require a regular and balanced meals. Especially Taeeumin need to eat small meals to avoid excessive accumulation of energy.

중노년기 성인의 뇌졸중 문해력, 건강정보 문해력, 뇌졸중 지식과 자기관리간의 관계 (Relations of Stroke Literacy, Health Literacy, Stroke Knowledge, and Self-Management among Middle-Aged and Older Adults)

  • 본 연구는 중노년기 성인의 뇌졸중 문해력, 건강정보 문해력, 뇌졸중 지식과 자기관리의 정도를 비교하고 그 관계를 확인함으로써 뇌졸중 발병률이 높은 중노년기 성인의 뇌졸중 예방을 위한 자기관리의 유의한 영향요인을 파악하고자 수행되었다. 2019년 12월 5일부터 2020년 3월 31일까지 198명을 대상으로 인구사회학적 특성, 뇌졸중 문해력, 건강정도 문해력, 뇌졸중 지식 및 자기관리에 관한 자료를 체계적인 설문지를 통해 수집되었다. 자료분석은 독립표본 t 검정, 일원배치 분산분석, Pearson 상관계수, 다중회귀분석을 사용하여 기술 통계량에 대해 SPSS/WIN 23.0 프로그램을 사용하였다. 중노년기 성인의 자기관리에 영향을 미치는 요인은 연령(β=.190. p=.015), 흡연을 할 때(β=-.226, p=.001), 주관적 건강상태(β=.297, p<.001)와 뇌졸중 지식(β=-.168, p=.014)으로, 이들의 설명력은 약 20.3%였다. 본 연구 결과를 볼 때, 중노년기 성인의 자기관리를 향상시키기 위한 간호 중재를 개발할 때 주관적 건강상태 및 뇌졸중 관련 지식을 고려해야 함을 알 수 있다. 또한, 지역사회에 거주 중인 중노년기 성인들의 뇌졸중 지식과 자기관리 사이의 매개변수 탐색이 필요하며, 뇌졸중 예방에 대한 체계적이고 지속적인 교육의 중요성이 강조된다.

Dietary patterns and cardio-cerebrovascular disease in a Chinese population

  • BACKGROUND/OBJECTIVES: Dietary pattern and its association with cardio-cerebrovascular disease have not been studied in Baoji city by now. This study was aimed to identify the dietary patterns among Chinese adults in Baoji, and explore the association between these dietary patterns and cardio-cerebrovascular disease. SUBJECTS/METHODS: A total of 4,968 participants were included in this study at 12 counties. With multistage stratified random sampling and semi quantitative food frequency questionnaire, the prevalence of cardio-cerebrovascular disease and dietary intake were investigated in 2013. We used factor analysis to establish dietary patterns. RESULTS: A total of 4,968 participants over 15 years old were included in this study. Five dietary patterns were identified in Baoji: protein, balanced, beans, prudent, and traditional patterns. The protein dietary pattern mainly included animal and plant proteins and was negatively associated with hypertension as well as stroke. The balanced pattern included carbohydrates, protein, and fat and was negatively associated with hypertension as well as stroke. The beans pattern was mainly beans and beans products and was negatively associated with hypertension. The prudent pattern only included staple foods and pickled vegetables and was positively associated with hypertension as well as coronary heart disease. The traditional pattern was representative of local Baoji traditional recipes and was positively associated with hypertension. CONCLUSIONS: The protein, balanced, and beans dietary patterns showed many protective effects on cardio-cerebrovascular disease. Based on these results, Baoji city residents should be encouraged to choose protein, balanced, and beans dietary patterns and abandon prudent and traditional patterns to prevent incidence of hypertension, coronary heart disease, and stroke.

고혈압 전 단계 성인여성의 뇌졸중 위험과 뇌졸중 지식 (Stroke Risk and Knowledge in Women with Prehypertension)

  • Purpose: This study was undertaken to identify stroke risk with risk factors and knowledge of stork in women with prehypertension. Methods: A total of 46 pre-hypertensive women in urban area aged 44.8 (SD 11.4) yr old in average were selected by a convenient sampling. Demographic data, risk factors, and knowledge of stroke were assessed through face to face interview. Stroke risk scores were calculated based on points given to age, systolic blood pressure, smoking, DM, and heart disease in Framingham stroke risk score. Data were analyzed by SPSS WIN 12.0 using descriptive statistics, two sample t-test, and Pearson's correlation coefficient. Results: The mean systolic blood pressure of participants was 133.11 mmHg, and HDL cholesterol was lower than the recommended level, although rest of physiologic risk factors were within normal. Of the participants, 82% did not do regular physical activity and 75% did not control fat intake persistently. Mean stroke risk points and knowledge scores were 4.48 and 29.15, respectively, which were significantly different between women with and without persistent controlling fat intake. Conclusion: Results indicate stroke prevention intervention for prehypertensive women should address comprehensively modifying lifestyle as well as physiologic factors, and information focusing on stroke risk factors and warning symptoms.

일부 대학생들의 성인건강 교육의 효과에 관한 연구 (A Study on the Educational Effectiveness of Chronic Diseases Among University Students)

  • Korea has been recently reducing the quality of life as well as rising medical cost because of the increase of chronic diseases. But we can prevent those chronic diseases through the improvement of environment or life style. We evaluated the educational effectiveness of chronic diseases(hypertention, diabetes, cancer, stroke and other chronic diseases) designed to increase the knowledge, attitude and practice of chronic diseases among university students. Between August 1994 and November 1994, we implemented chronic diseases prevention instruction in intervention students; unmatched control students were selected in same university. We conducted pre- and post-intervention surveys both intervention and control students with self-reported questionnaires(50 items). We assigned score(0-4 points) to items and conducted a analysis of covariance(ANCOVA) with sex, grade and economic status as the covariate, using the SAS PC computer statistical package. And we culculated odds ratio with safety scores between intervention and control students. The results of this study were followed. 1. In demographic characteristics of subjects both pre- and post intervention, we found no significant differences in intervention and control students at religion, father's education, mother's education, mother's occupation and type of residence(p>0.05), but we found significant differences at sex(p<0.001), grade(p<0.001) and economic status(p<0.05). 2. The sex, grade and economic status-adjusted mean prevention knowledge scores for diabetes and stroke increased from the pre- to post-survey in the intervention students(p<0.001), but control students did not increased(p>0.05). As odds ratios in knowledge were below 1, the knowledge of intervention students were higher than control students. 3. The attitudes for general adult health increased from the pre- to post-survey in the intervention students(p<0.05), but control students did not increased(p>0.05). As odds ratios in attitudes were approximately 1, we can not say effectiveness in intervention students 4. The pratices for cancer and stroke increased from the pre- to post-survey in both the intervention and control students(p<0.001). Also odds ratio of hypertention was 0.91, and that of stroke was 1.14. 5. Health related behaviors did not increased from the pre- to post- survey in both the intervention and control students(p>0.05). But odds ratio of drinking was 0.76 and that of body weight was 1.21. 6. Health status did not increased from the pre- to post- survey in both the intervention and control students(p>0.05). As odds ratio of health status was 1.09, prevention education was not effect in intervention students We would like to recommend as follows; 1. University students must learn about prevention of chronic diseases. Because the knowledge of invetervention students was higher than that of control students. 2. The prevention education of chronic diseases should be taught from primary school. 3. Adult health education for university students must be practiced continuously. Education period(l5 weeks) in this study was not complete. 4. The evaluation of chronic diseases was conducted real measurement(such as BP check) as well as self reported-survey. 5. Educational materials(video tape, pamphlet) related the prevention of chronic diseases should be developed at national level. And we must easely use those materials. 6. The prevention education of chronic diseases should be made through mass media as well as school education.

중풍환자에 대한 일차 한의임상진료 가이드라인 (Clinical Practice Guideline of Korean Medicine for Stroke : Preliminary Guideline and Recommendation)

  • The aim of this new statement is to provide comprehensive and timely evidence-based recommendations on stroke management for clinical practitioners. Many countries are already well engaged in developing and releasing their own clinical practice guidelines, whereas Korean Medicine (KM) is still beginning. It will take time and effort to develop evidence-based guidelines and recommendations of KM or other traditional medicine because they are weak in the area of scientific evidence. The clinical practice guideline of Korean medicine for stroke was formulated through extensive review of published literature and consensus meeting of Korean medicine specialists. This project was supported by a grant of the Oriental Medicine R&D Project, the Ministry of Health and Welfare. Referring to guidelines developed in other countries, the experts in the subject tried to organize and develop guidelines and recommendations adequate for domestic medical circumstances. In December, 2008, a multi-disciplinary team called the Evidence Based Clinical Practice Guidelines Development Group (EBCPGsDG) for Stroke was organized. The writing committee was comprised of experts in internal medicine, acupuncture, rehabilitation, and Sasang constitution. Outside specialists and associated panels were invited for consultation. The scope of the guideline encompasses acupuncture, moxibustion and herbal medicine (including Korean medicine, traditional Chinese medicine, Kampo medicine) as interventions for stroke patients. It includes statements about ischemic stroke (I63), stroke not specified as hemorrhage or infarction (I64), and sequelae of cerebrovascular disease (I69) according to the International Classification of Disease (ICD). The committee subdivided the description of herbal medications into acute stroke management, subacute stroke management, post-stroke management, and secondary prevention of stroke. Guidelines on the practice of acupuncture and moxibustion were described in order for acute stroke management, subacute stroke management, chronic stroke management, and post-stroke rehabilitation. Clinicians who are working in the field of stroke care can adopt this guideline for their practice.
