  • Recently, the number of obese people have been increasing due to westernized diets and increased sedentary activity. In particular, obese adolescent have been newly recognized as having significant health problems. This study focuses on determining the gender difference of obese adolescents through implementation of a structured questionnaire on general characteristics, attitudes toward dieting, eating habits and emotional depression due to obesity. Data collection was carried out by 291 students in attendance at 3 high schools in Pusan from 11. 2. 1998 to 11. 30. 1998. The targets were 148 boys and 143 girls who were judged as obese in the school's physical check-up. The test of reliability of this research was calculated by chronbach's α. Data analysis was managed by computer and statistics by SPSS/WIN. The results of the survey are as follows: 1. For the questionnaire regarding attitudes toward dieting which include 28 items, 10 out of 28 items (about 36%)showed gender difference. 2. For the questionnaire regarding eating habits which include 17 items, 5 out of 17 items (about 30%)showed gender difference. 3.For the questionnaire regarding emotional depression about obesity, 30.6% of boys and 27.3% girls answered but no statistical significance. The results of this survey indicated that this study will be important as a good guideline for treating obese adolescents. It is hoped that much more research on gender difference in obese adolescents will be implemented in the future. In addition, the development of effective treatments for obesity should be based on the gender of the adolescent.

  • The quality including in situ degradability in the rumen of Holstein of the orchardgrass silage added with apple pomace was investigated in this study. The amount of apple pomace added in different treatments were 0, 20, 40 and 60% respectively. With higher amount of addition of apple pomace to orchardgrass, ADF, NDF and crude ash contents decreased significantly (p<0.05). Crude protein contents in the silages (11.8- 12.9%) were similar to that of 100% orchardgrass silage. Moisture contents increased according to the higher proportion of apple pomace in the silages. On the contrary to moisture content, pH was lower in 40-60% addition of apple pomace (3.7-3.9) than that of 100% orchardgrass silage (4.7). However the contents of lactic acid (1.7-2.5%), acetic acid (1.3- 1.7%) and total organic acid (2.9-4.2%) significantly increased according to higher levels of addition of apple pomace compared to the respective values of 100% orchardgrass silage (1.1%, 0.6% 1.7%). In siru disappearance rates of dry matter and NDF in the rumen were significantly higher at the stages of incubation after 24h in 40-60% addition of apple pomace than in 100% orchardgrass silage. No statistical differences were observed with quickly degradable fraction (a) and slowly degradable fraction (b) in the disappearance rates of dry matter and NDF. However, fractional rate of disappearance (c) and effective degradability (ED, k=0.08) for dry matter and NDF were significantly higher in 20-60% addition of apple pomace as 0.0076-0.0079 and 0.0099-0.0130. and 39.3-41.7% and 18.4- 20.6% respectively than the respective values of 0.0054 and 0.0064, and 36.8 and 16.5% of 100% orchardgrass silage. (Key words : Orchardgrass silage. Apple pomace. Lactic acid, In Situ Degradability. Effective degradability)

  • Objective : This study aimed to compare the patients' satisfactions with local Korean Medicine(KM) clinics and networked KM clinics, and to find out which factors had an effect on it. Method : Six local KM clinics and six networked KM clinics were selected, and patients' questionnaires on the satisfaction with KM clinic use were administered using exit poll method. The questionnaires were developed to measure satisfactions with facility, doctor and staffs, and services. 240 patients completed the questionnaires, and the data were statistically analyzed. Results : There was more diversity in patients' diseases among networked KM clinics users, however, treatments provided were not differentiated clinically between two types of KM clinics. Patients' satisfaction scores for local KM clinics were significantly higher than those for networked KM clinics in the aspects of doctors/staffs (9.35 vs 8.92, p<.01) and services (8.76 vs 8.42, p<.05). 90.0% of local KM clinic users expressed their willingness to recommend the clinics they used to others, while 75.0% of networked KM clinics users did. For both the local and networked clinics, 'KM doctors' kindness', and 'process and waiting time' were statistical factors affecting patients' satisfaction. Conclusion : The patients' satisfactions with general KM clinics were higher than those with networked KM clinics. Networked KM clinics need to develop services to meet the diverse medical demands which were created by them selves.

  • The "ShikLyoChanYo", written in 1460 by JunSoonYi (全循義), master court doctor in JoSeon (朝鮮) Dynasty, is the very first specialty publication of Korean dietary treatment existing today. Both Chinese and Korean scholars have assumed that this book had been lost long time ago. In November 2003, however, a Korean philologist found a version of the book, Yangyang (襄陽, a district name in Korea), and this book has attracted a lot of interest of Korean traditional medical science and agricultural science since then. This paper is to dissert the document of food therapy from the book with profound document study and statistical analysis in the fields of traditional Chinese medicine and traditional Korean medicine on dietetics. It completes the study of the application of all the dietetic treatments according to symptoms of diseases and all the plants and medication applied to cure chronic conditions that are clinically examined for the purpose of food therapy. A general survey on sundry records related to this food therapy of the "ShikLyoChanYo" has been done to make this dissertation and it carried out a statistic analysis of all the dietetic mixing technique of all plants and medication. Among other symptoms of illnesses from the book, there are 7 frequently addressed ailments chosen from the aspect of food therapy - a stroke, a disease diagnosed by thirst, a serious cough, an ache resulting from numbness, a disease relating to stomach, blurry vision and weak hearing, and a drinking related disease. This part is to discuss these illnesses and how to cure them with food based on its characteristics and rules of application.

  • The records for the tuberculosis patients who discharged from the health center during 1982 in Kangwha county were analized in order to study the characteristics and the patterns of treatments for the pulmonary tuberculosis patients with the history of previous treatment before registration and also the relationship between the previos history of treatment and the outcome at the time of discharge from the health center. The major findings are as follows. 1. Those who have a history of previous treatment were 58, 22.5% of those 258 patients who were studied. 2. There is no difference in sex however the rate of previous history of treatment was higher among middle age group (20-59) than young and old age group (under 19 or over 60). 3. The rate of previous treatment was rather higher in those lived in remote area from Eup. 4. As for the reationship with occupation, students and civil servants who easily exposured to the public relations of government's tuberculosis control program experienced lower previous treatment before registration than farmers or unemployed. 5. A total of 62.1% were previously treated less than 6 months, 29.3% between 6 and 12 months, and only 8.6% more than 12 months before registration to health center. 6. The most common used anti-tuberculosis drugs were isoniazid and ethambutol but only 13.8% used government-standardized precription and 69.0% used secondary drugs from its beginning. 7. There was no statistical difference between the previous history of treatment and the outcome at the time of discharge from the health center. However the longer the duration of treatment before registration was the lower the cure rate at health center was.

  • The current study was undertaken to determine the effects of the ratios of dietary fat to carbohydrate and energy restriction on insulin sensitivity in the growing rats. Male rats weighting 80-90g were fed experimental diets for two weeks. Rats were killed and epiddymal adipose tissue were removed and sliced. Explants of adipose tissues were incubated for 2 hours in KRB(Krebs-Ringer bicarbonate) buffer containing various concentrations of human insulin and [U-14C]glucose. Insulin sensitivity was determined as glucose conversion to total lipids (lipogenesis) during 2 hr incubation. Exp't I : Effects of Ratios of Fat to Carbohydrate on Insulin Sensitivity. Eighteen male rats were fed 3 diets for 2 weeks. Diet 1 was low fat-high carbohydrate (4% soybean oil and 66.5% cornstarch) ; diet 2, medium fat-medium sarbohydrate(12% soybean oil and 58.5% cornstarch) ; diet 3, high fat-low carbohydrate (20% soybean oil and 50.5% cornstarch). Insulin sensitivity was higher in the order of LF-HC, MF-HC and HF-LC diet groups (p<0.05), i.e, lipogenesis was higher at all insuline concentration in the explants from rats fed LF-HC diet. However, thers was no significant difference in body weight gain and epididymal adipose tissue weight among treatments. Exp't II ; Effects of Energy Restriction on Insulin Sensitivity. Twelve rats were grouped into ad libitum feeding and restricted feeding(70% of ad libitum). The experimental diet was medium fat-medium carbohydrat diet as used in the Exp't I. Restricted feeding group tended to show higher insulin sensitivity compared to ad libitum group. However, there was no statistical difference between two groups. As expected, body weight gain and epididymal adipose tissue were higher in the ad libitum group. In summary, the resutls of the current study showed that the epididymal adipose tissue taken from the rats fed low fat-high carbohydrate diet showed higher insulin sensitivity compared to those fed high fat-low carbohydrate, and that resticted feeding tended to elevate insulin sensitivity in these tissues.

  • Objective : To evaluate the incidence of postsurgical sensory complications in patients with scalp masses and classify the locations of them from a surgical standpoint according to anatomical considerations. Methods : A total of 121 patients who underwent surgery for scalp mass were included in this study. The authors reviewed medical records and preoperative radiologic images. We investigated the complications related to sensory changes after procedure. Enrolled patients have been divided into three groups. Group A included patients with tumors above the superior nuchal line (SNL), Group B with tumors within the trapezius muscle area and patients who had tumors on the lateral trapezius muscle area were assigned to Group C. We compared the incidence related to postoperative sensory complications and summarized their additional treatments for these with clinical outcome. Results : There were 12 patients (10%) with sensory complications related on the mass excision site (Group A : 1 patient, Group B : 2 patients, Group C : 9 patients). Six patients were affected with lesser occipital nerve (LON), 2 patients on greater occipital nerve (GON) and 4 patients on GON and LON. Over 6 months after surgery, two of the twelve patients with sensory complications did not have complete recovered pain in spite of proper medications and local chemical neurolysis with 1.0% lidocaine and dexamethasone. Conclusion : Occipital neuropathy should be considered as a complication related excision of scalp mass. The sensory complications are more frequent in Group C because of the anatomical characteristics of the occipital nerves and there were no statistical difference for other variables.

  • Background/Aims: Gastric varices (GVs) are a major cause of upper gastrointestinal bleeding in patients with liver cirrhosis. The current treatments of choice are balloon-occluded retrograde transvenous obliteration (BRTO) and the placement of a transjugular intrahepatic portosystemic shunt (TIPS). We aimed to compare the efficacy and outcomes of these two methods for the management of GV bleeding. Methods: This retrospective study included consecutive patients who received BRTO (n=157) or TIPS (n=19) to control GV bleeding from January 2005 to December 2014 at a single tertiary hospital in Korea. The overall survival (OS), immediate bleeding control rate, rebleeding rate and complication rate were compared between patients in the BRTO and TIPS groups. Results: Patients in the BRTO group showed higher immediate bleeding control rates (p=0.059, odds ratio [OR]=4.72) and lower cumulative rebleeding rates (logrank p=0.060) than those in the TIPS group, although the difference failed to reach statistical significance. There were no significant differences in the rates of complications, including pleural effusion, aggravation of esophageal varices, portal hypertensive gastropathy, and portosystemic encephalopathy, although the rate of the progression of ascites was significantly higher in the BRTO group (p=0.02, OR=7.93). After adjusting for several confounding factors using a multivariate Cox analysis, the BRTO group had a significantly longer OS (adjusted hazard ratio [aHR]=0.44, p=0.01) and a longer rebleeding-free survival (aHR=0.34, p=0.001) than the TIPS group. Conclusions: BRTO provides better bleeding control, rebleeding-free survival, and OS than TIPS for patients with GV bleeding.

  • Objectives: This study conducted a retrospective review to compare the effectiveness of herbal medicine, while confirming the effectiveness of Korean medicine (KM) in traffic accidents (TA). Methods: We investigated the medical records of TA patients who were hospitalized at Muuido Korean medicine hospital from January 2016 to June 2018 retrospectively. Medical records included general characteristics (Gender, age), TA & treatment-related information (Collision type, chief complaint, hospitalization period, and herbal medicine prescription), and clinical scales (Five-scale improvement, numerical rating scale (NRS)). Statistical analysis was performed for data distribution and effectiveness comparison. Results: 755 TA patients showed 0.78:1 gender ratio. 20-50s years old, rear collision, pain in the cervical and lumbar spine, and hospitalization period within 14 days were the majority. KM improved 83.18% of 755 TA patients' symptoms and ROM over 'improvement' level, which indicated 30-70% improvement. With common KM treatments including acupuncture, cupping, heat therapy, and manipulation therapy, TA patients received various herbal medicines and herbal medicine for breaking severe blood stasis (Tongdo-san) showed improvement in both five-scale improvement and NRS change at the same time. Conclusions: We could confirm the effectiveness of KM, particularly herbal medicine in TA. It would be necessary to develop new herbal medicines such as Tongdo-san to treat TA-related symptoms.
