The Influence of Video-Assisted Thoracic Surgery on Hospital Course of Spontaneous Pneumothorax (비디오 흉강경 수술법이 자연기흉의 치료과정에 미치는 영향)

  • Video-assisted thoracic surgery(VATS) is emerging as a viable alternative to thoracotomy when surgical treatment of spontaneous pneumothorax is required. 20 patients with spontaneous pneumothorax underwent bullectomy between July 1995 and May 1996. The patients were divided into two groups : Control group ; the patients who received with mid-axillary approach(n=10), Experimental group ; the patients who received with VATS (n=10). The results were as follows ; 1. The total sex distribution was male predominance (male:female=17:3). Mean age of control group was 29.6$\pm$9.8 years and experimental group was 27.2$\pm$11.9 years. 2. The mean period of postoperative chest tube indwelling duration and hospital stay were 3.3$\pm$0.8 days and 7.9$\pm$1.2 days in control group and 2.1$\pm$0.9 days and 5.2$\pm$3.1 days in experimental group(p=0.005 and p=0.02). 3. The mean time of operation, vital signs and arterial blood gas analysis did not showed any statistical differences between the groups. 4. Percent recovery of tidal volume and forced vital capacity were significantly improved in experimental group comparing with control group (p<0.05). 5. The patients undergoing VATS experienced significantly less postoperative pain and limitation of motion. In conclusion, VATS is safe and offers the potential benefits of shorter postoperative hospital stays and less pain with cosmetic benefits.

A Study on Time Allocation in Transition to Old Age -Focusing on the Patterns of Time Allocation among People aged 45 or more- (생애과정 전환기의 생활시간 배분에 관한 연구 -중.고령자의 생활시간 비교를 중심으로-)

  • This study examined how middle-aged and elderly persons allocated their time in overall perspective and whether the patterns were varied by age, socio-demographic factors, and the interaction effect between them. This study analyzed the 2009 Time Use Survey Data (17,096 time-diaries of people aged 45 or more) of the National Statistical Office using tobit regression model. The main results are as follows. First, middle-aged and elderly people gradually decreased paid work times. Second, the patterns of time allocation were varied by age, gender, education level, marital status and household type(Agricultural/non-agricultural). Third, there were interaction effects between age and the socio-demographic factors on paid work time and leisure time. Highly educated or urban persons were likely to have less time in paid work less than other groups with increasing age. And females were likely to have less time in domestic labor and care work than males with increasing age. But they had more time in social and economic productive activities than males. Based on these findings, this study suggests comprehensive approach to analyze the time use of elderly beyond economic working time or leisure time. To establish effective ageing society policy, it is necessary to consider the time allocation of elderly which divided into the social stratification.

  • Purpose: This study was to analyze the trends and content of all the research (171 research papers) published in the Journal of Fundamentals of Nursing from 1994, when it began to 2000 using an objective tool developed by the researches, and to identify the direction for the future research in Fundamentals of Nursing. Method: Research published in the Journal of Fundamentals of Nursing from 1994 were reviewed. Result: Most of research (94.2%) was non-degree based and funded research was very limited. The average number of researchers per research study was 2.2 and collaborative research has been gradually increasing. The total number of key words was 331 and the average was 2.6 per research. The major key words were not different from other nursing departments and included home care nursing (10 times), health promotion (8 times), self-care (7 times). self-efficacy (7 times), and homo-dialysis (6 times). Eighty seven percent of the research was quantitative research. and there were very few qualitative studies. Considering theory level, it was found that 40% were factor related research For study design, non-experimental studies were most frequent (66%), and the rate, especially of surveys, has remarkably decreased. Selection of subjects by convenience sampling, was most frequent and there were very few studies that provided the rationale for the calculation of sample size. The major subjects of study were patients (44.8%) with various diagnoses. The subjects usually gave oral consent to take part in that study. Giving information (46.9%) and exercise (26.5%) were common nursing interventions, and physiological indices (16.5%). vital signs (10.3%), physical functioning (8.2%), level of knowledge/skill (7.2%) and level of activity (6.2%) were frequently measured as outcome. Variables questionnaires were the major approach used to collect data, and 57.8% of the research provided the Cronbach alpha to guarantee internal consistency of study instruments. Data were analyzed with computerized statistical packages using, ANOVA (42.0%), T test(39.5%), and chi-square test. For the last seven years, nursing research in Fundamentals of Nursing has gradually improved in both quantify and quality. Conclusion: It was difficult to find any uniqueness or difference compared to other departments of nursing. In fact, because the history of the Journal of Fundamentals of Nursing is rather short, we can expect that there will be further improvement in qualify and content in the future.

The Effect of Lactose and Calcium on the Acute Lead Poisoning in Rats (白鼠에서 乳糖과 칼슘이 急性 납중독에 미치는 영향)

  • This study was performed to investigate the effect of lactose in 4 different concentrations against the protective effect of calcium on the acute lead poisoning in rats after 4 weeks treatment. In this animal experiment, 70 albino male weanling rats (50-70g of body weight) of Sprague-Dawley strain were used. Lead was dissolved in the distilled water and intubated at the dose of 400mg lead (as acetate)/ kg of body weight/day. Calcium and lactose were administered in drinking water ad libiturn dissolved with the solution of 0.7% calcium gluconate mixed with 40, 80, 160 and 320mM lacotse respectively. The results obtained were summarized as follows: 1. The rate of body weight gain in all treated groups turned out to be lower than that in the control group during 4 weeks treatment. The slow-down of body weight gain was the most significantly observed in the group treated with lead only ( p < 0.05). 2. The relative spleen weight in lead only treated group was significantly lower than that of lead + calcium, lead + calcium + 80mM lactose treated group ( p < 0.05). 3. The value of RBC, WBC, Hb and Hct showed a decreasing tendency in the group treated with lead only ( p < 0.05), however, a significant decrease was not observed in the group treated with lead + calcium. On the other hand, the protective effect of calcium was deteriorated in the group treated with lead + calcium + lactose. 4. The activity of $\delta$-aminolevulinic acid dehydratase ($\delta$-ALAD activity) showed the same tendency as No. 2. 5. The lead concentration in the blood (PbB) showed an increasing tendency and the interrelation among the different groups was also identical with No. 2. 6. With a statistical approach, it was found out that the activity of $\delta$-ALAD and the lead concentration in the blood show a relation of inverse proportion(r=-0.7301). The diagram was interpreted with the logarithmic equation InY = 5.5357-0.0251X (X:PbB, Y:$\delta$-ALAD activity). 7. In the histopathological findings of the kidney, the protective effect of calcium was observed. However, the protective effect of calcium was restricted in the group treated with lead + calcium + lactose. As a conclusion, the intensity of the acute ingested lead poisoning was obviously reduced by calcium, however, the protective effect of calcium was deteriorated in proportion with the concentration of the lactose to be administered. On the other hand, it was also noted that the deterioration was lightly restrained in the group treated with the physiological concentration of 80mM lactose than the results shown in the groups treated with lactose of other concentrations.

Meta-analysis on Methane Mitigating Properties of Saponin-rich Sources in the Rumen: Influence of Addition Levels and Plant Sources

  • Saponins have been considered as promising natural substances for mitigating methane emissions from ruminants. However, studies reported that addition of saponin-rich sources often arrived at contrasting results, i.e. either it decreased methane or it did not. The aim of the present study was to assess ruminal methane emissions through a meta-analytical approach of integrating related studies from published papers which described various levels of different saponin-rich sources being added to ruminant feed. A database was constructed from published literature reporting the addition of saponin-rich sources at various levels and then monitoring ruminal methane emissions in vitro. Accordingly, levels of saponin-rich source additions as well as different saponin sources were specified in the database. Apart from methane, other related rumen fermentation parameters were also included in the database, i.e. organic matter digestibility, gas production, pH, ammonia concentration, short-chain fatty acid profiles and protozoal count. A total of 23 studies comprised of 89 data points met the inclusion criteria. The data obtained were subsequently subjected to a statistical meta-analysis based on mixed model methodology. Accordingly, different studies were treated as random effects whereas levels of saponin-rich source additions or different saponin sources were considered as fixed effects. Model statistics used were p-value and root mean square error. Results showed that an addition of increasing levels of a saponin-rich source decreased methane emission per unit of substrate incubated as well as per unit of total gas produced (p<0.05). There was a decrease in acetate proportion (linear pattern; p<0.001) and an increase in propionate proportion (linear pattern; p<0.001) with increasing levels of saponin. Log protozoal count decreased (p<0.05) at higher saponin levels. Comparing between different saponin-rich sources, all saponin sources, i.e. quillaja, tea and yucca saponins produced less methane per unit of total gas than that of control (p<0.05). Although numerically the order of effectiveness of saponin-rich sources in mitigating methane was yucca>tea>quillaja, statistically they did not differ each other. It can be concluded that methane mitigating properties of saponins in the rumen are level- and source-dependent.

Manikin Model Study on Reproducibility and Accuracy of Maxillofacial Measurements Determined by Stereocamera: Comparative Study of Direct Anthropometry, Digitizer and Stereophotogrammetery (스테레오 카메라를 이용한 안면부 측정의 재현성과 정확도에 대한 마네킨을 이용한 연구: 직접 인체계측, Digitizer, Stereophotogrammetry의 비교 연구)

  • Purpose: Recently, a three dimensional approach to hard and soft tissues of the maxillofacial area has been widely used. This study was to evaluate the reproducibility and accuracy of a stereocamera compared to actual measurement methods using a digital caliper and digitizer. Methods: The stereoscopies of 7 head dummies with different sizes and shapes were obtained using a Di3D system (Dimensional Imaging, Glasgow, UK) after marking reference points on facial areas. From the obtained stereoscopy, 10 measurements representing the width, height and depth of each of the facial sections of the dummy were measured twice using a three dimensional reverse engineering software program (RapidForm$^{TM}$ 2006, Inus, Seoul, Korea). The x, y, and z coordinates of each of the three dimensional measurements were obtained and distances between two points were calculated. All procedures were repeated twice. The actual measurement method was performed twice, directly on dummies, using a digital caliper and values were compared with the previously determined values. Results: The results were as follows. In the ANOVA analysis, there were no significant statistical differences among the three measurement methods. In the Bonferroni analysis, with adjustments applied for multiple comparisons, there was no difference between actual measurement methods using a digitizer and a digital caliper. However, there was some difference between using a stereocamera and actual measurement methods using a digitizer and a digital caliper in values of $Ex_{Rt}-Ex_{Lt}$, $En_{Rt}-En_{Lt}$, $Ala_{Rt}-Ala_{Lt}$, $Ch_{Rt}-Ch_{Lt}$, G-Pg', $Ala_{Rt}$-Prn, $Ala_{Rt}$-Prn. The mean value for technical error in measurement (TEM) in Di3D (0.98 mm) was slightly higher than for a digital caliper (0.17 mm) and a digitizer (0.30 mm). In an intraclass correlation coefficient (ICC) there were no significant differences among the three measurement methods, but the Di3D system with the stereocamera showed relatively lower reproducibility compared to actual measurement methods using a digitizer and a digital caliper. Conclusion: These results indicate that some complementary measures may be needed to improve accuracy and reproducibility in the Di3D system with stereocamera.

Development and Application of Robust Decision Making Technique Considering Uncertainty of Climatic Change Scenarios (기후변화 시나리오의 불확실성을 고려하기위한 로버스트 의사결정 기법의 개발 및 적용)

  • Climate change is expected to worsen the depletion of streamflow in urban watershed. In this study, we therefore considered the treated wastewater (TWW) use as an adaptation strategy and devised a framework to identify prioritized areas for TWW use. An integrated framework that includes hydrological factors as well as social and environmental components were employed to determine the criteria for decision making. Fuzzy theory was employed to consider the uncertainties in the climate change scenarios and the weights of the performance value. All alternatives were evaluated using the fuzzy TOPSIS method. In addition, statistical method and decision making methods under complete uncertainty were used for robust decision making. As a result, ranking the alternatives using the fuzzy TOPSIS method and robust approach such as maximin, maximax, Hurwicz and equal likelihood criterion mitigated the level of uncertainty and ambiguity in each alternative. The finding of this study can be helpful in prioritizing water resource management projects considering various climate change scenarios.

Development of Java/VRML-based 3D GIS's Framework and Its Prototype Model (Java/VRML기반 3차원 GIS의 기본 구조와 프로토타입 모델 개발)

  • Recently, 3D GIS based on 3D geo-processing methodology and Internet environment are emerging issues in GIS fields. To design and implement 3D GIS, the strategic linkage of Java and VRML is first regarded: 3D feature format definition in the passion of conventional GIS including aspatial attributes, 3B feature indexing, 3D analytical operators such as selection, buffering, and Near, Metric operation such as distance measurement and statistical description, and 3D visualization. In 3D feature format definition, the following aspects are implemented: spatial information for 3D primitives extended from 2D primitives, multimedia data, object texture or color of VRML specification. DXF-format GIS layers with additional attributes are converted to 3D feature format and imported into this system. While, 3D analytical operators are realized in the form of 3D buffering with respect to user-defined point, line, polygon, and 3D objects, and 3D Near functions; furthermore, 'Lantern operator' is newly introduced in this 3D GIS. Because this system is implemented by Java applet, any client with Java-enable browser including VRML browser plug-in can utilize the new style of 3D GIS function in the virtual space. Conclusively, we present prototype of WWW-based 3D GIS, and this approach will be contribute to development of core modules on the stage of concept establishment and of real application model in future.

The Effect of Corporate Social Responsibility on Corporate Image and Corporate Performance (기업의 사회적 책임활동이 기업 이미지 형성과 기업 성과에 미치는 영향에 관한 연구: 공유가치창출 인지정도에 따른 차이비교)

  • Purpose - Recently, although corporate social responsibility activities have been increasing in size, they do not have to achieve qualitative improvements and can be passive and cost consuming. Therefore, companies should make quantitative as well as qualitative improvements in their efforts in corporate social responsibility activities. In this study, the classification of social responsibility activities in a variety of studies was analyzed through a more specific path than in previous studies. Corporate behavior image, social behavior image, and corporate contributions image were analyzed through a more detailed analysis of performance. This study suggests that more detailed and concentrated social responsibility activities be pursued by forming companies. Research design, data, and methodology - The purpose of study is to gauge the corporate need for a more intensive, specific area of CSR activities. For this purpose, the sample of consumers that were targeted for CSR activities, recognized as 261 persons, have been investigated. Through a theoretical discussion on previous research, nine hypotheses were established on corporate image, the influence of corporate performance on CSR, and the CSV regulation effect. In order to test the hypothesis, a survey was conducted on 261 male and female consumers who were targeted for CSR, being persons in their 20s to 40s. PASW Statistics 18.0 and AMOS 18.0 were used for statistical analysis. Results - Corporate behavior image was formed through legal responsibility activities and economic responsibility activities. In addition to economic responsibilities, ethical responsibilities and environmental responsibilities were confirmed to have influence on social behavior image. Corporate social responsibility and philanthropic responsibility were confirmed to have influence on economic contribution image. Corporate image has positive effects on brand attitude, corporate reputation, and corporate competition. In addition, when CSV awareness is high, consumers perceive corporate image only through economic responsibility. However, when CSV awareness is low, economic responsibility as well as legal responsibility through charitable activities form the corporate image that influences the brand attitude and corporate reputation, as well as corporate competitiveness. It would appear that the area of corporate social responsibility needs more intensive management for corporate image and corporate competitive advantage. Conclusion - First, the findings of this study show that each CSR activity has a different effect on corporate image and thus, the corporate image influences corporate performance in distinct ways, depending on the CSR activity. This implies that reactive strategies should be tailored to the required image. Second, there is a difference in CSV awareness between groups. When the CSV awareness is low, we can confirm that legal responsibility activities have an especially significant effect on corporate image, implying that corporations should pursue their economic objectives within legal regulations and need to invest significant time and effort for this. This study has limited generalization potential because the result of the model fit has insufficient reference value. In future research, we need to approach various dimensions of corporate performance.

A Study on Consumer's Evaluations on and Preferences for Formative images of Products (제품디자인을 위한 조형 이미지의 평가와 선호에 관한 연구)

  • Today's world has been changing form post-industrial society to information society, and criteria of human value, also, has been changing from physical to intellectual and sensible. Based on this trend, intellectual and sensible value of a product is becoming more important in comsumer's market than physical value. The environment of product design has been rapidly changing. Users and consumers pursuit sensible satisfaction through their lives based on images of products. As the environment in which a product is used has been more important, companies have placed more emphasis on consumers and users, and are trying to satisfy their consumers by developing products with emphasis on their design, as well as to develop human technology. In ever-changing environment, a question is if sensible design can be developed in product design. Companies could upgrade their competitive power and brand image only if they could develope designs which could satisfy consumers through sensible design approach with a base on systematic research on h~man sensibility. In this context, the purpose of this study was to: investigate consumer's evaluations on and preferences for formative images of products according to the types of human sensibility. This research study placed emphasis on quantitative analysis of consumers' emotional needs to find out consumer's evaluations on and preferences for formative images of products. The researcher reviewed related literature on human sensibility and design, as well as G-Kansei and product design. Data were collected through a questionnaire and analyzed through a statistical program, and highly effective design strategy was investigated through analysis of data and interpretation of the results.
