The Effect of Induced Weight Bearing Method Using a Cane on Weight Bearing Distribution in Children with Spastic Hemiplegic Cerebral Palsy

  • Purpose: This study was undertaken to determine whether the position of cane use affects the distribution of weight-bearing on both feet of children with hemiplegic cerebral palsy in a standing posture. Methods: Twenty participants with cerebral palsy were recruited as volunteers for this study. Using the Zebris FDM-System, weight-bearing distribution according to the method of using a cane was measured under three conditions in randomized order: (1) standing unaided (no cane); (2) standing with the affected side using the cane; and (3) standing with the non-affected side using the cane. The cane was matched by measuring length-from-floor to the greater trochanter of the subject, and was placed 15 cm outward from the little toe on the supporting side. Results: Evaluating the method of using a cane under the three conditions, we determined that pressure of the foot on the affected side was higher in the order: standing with affected side using cane > standing unaided (no cane) > standing with non-affected side using cane (p<0.05). In the post-hoc analysis, a significant difference was observed between (i) standing unaided (no cane) and standing with the affected side using cane, and (ii) standing with affected side using cane and standing with non-affected side using cane (p<0.05). Conclusion: This study suggests that induced weight-bearing methods using a cane on the affected side could increase the weight-bearing capacity on the affected side in children with spastic hemiplegic cerebral palsy, which will have a positive effect on reducing asymmetry weight support.

A Comparison Study on the Change in Lumbar Lordosis When Standing, Sitting on a Chair, and Sitting on the Floor in Normal Individuals

  • Objective : To compare radiographic analysis on the sagittal lumbar curve when standing, sitting on a chair, and sitting on the floor. Methods : Thirty asymptomatic volunteers without a history of spinal pathology were recruited. The study population comprised 11 women and 19 men with a mean age of 29.8 years. An independent observer assessed whole lumbar lordosis (WL) and segmental lordosis (SL) between L1 and S1 using the Cobb's angle on lateral radiographs of the lumbar spine obtained from normal individuals when standing, sitting on a chair, and sitting on the floor. WL and SL at each segment were compared for each position. Results : WL when sitting on the floor was reduced by 72.9% than the average of that in the standing position. Of the total decrease in WL, 78% occurred between L4 to S1. There were significant decreases in SL at all lumbar spinal levels, except L1-2, when sitting on the floor as compared to when standing and sitting on a chair. Changes in WL between the positions when sitting on a chair and when sitting on the floor were mostly contributed by the loss of SL at the L4-5 and L5-S1 levels. Conclusion : When sitting on the floor, WL is relatively low; this is mostly because of decreasing lordosis at the L4-5 and L5-S1 levels. In the case of lower lumbar fusion, hyperflexion is expected at the adjacent segment when sitting on the floor. To avoid this, sitting with a lordotic lumbar curve is important. Surgeons should remember to create sufficient lordosis when performing lower lumbar fusion surgery in patients with an oriental life style.

정상파와 초고주파 집속 빔을 이용한 음향집게시스템의 개발에 관한 연구 (A Study on Development of Acoustic Tweezer System Using Standing Waves and Very High Frequency Focused Beams)

  • 정상파와 초고주파 집속 빔을 이용한 초음파 음향집게의 개발 가능성을 타진하는 것을 목적으로, 수중에 놓인 미세입자의 위치를 제어할 수 있는 시스템을 구축하였다. 그 시스템은 입자를 평면 정상파의 방사력에 의해 1차적으로 포획하여 집속 빔의 초점 부근으로 이동시킨 다음, 초고주파 집속 빔의 방사력을 이용하여 2차적으로 포획하는 것이다. 구축된 시스템에 의해 구형 미세 입자의 위치제어 실험을 행한 결과, 정상파 음장에서는 음압의 마디에 입자가 잘 포획되었으며, 주파수의 조정에 의해 그 입자를 임의 위치로 이등조작이 가능하였다. 그리고 단일의 초고주파 집속 빔에 의해서는 음원으로부터 멀어지는 방향으로만 구동력이 작용하나, 2개의 대항하는 집속 빔을 사용할 경우 그 중심 부근에 포획됨을 알았다.

  • Purpose: The purpose of this study was to measure changes in the H-reflex and V wave under loading conditions (e.g. prone and standing position) and to investigate whether postural change would affect the H-reflex and V wave in post stroke hemiplegic patients. Methods: Thirty persons with hemiplegia resulting from stroke (20 males, 10 females) participated in this study. Electromyography (EMG) was used to electrically stimulate and record the soleus H-reflexes and V waves under various loading conditions. The normality of the distribution of each variable (H latency, $H_{max}/M_{max}$ ratio, $V_{max}/M_{max}$ ratio) was tested using the Kolmogorov-Smirnov test. The means of normally distributed continuous data were assessed by independent t-test (${\alpha}$=0.05). Results: There were statistically significant differences in $H_{max}/M_{max}$ ratio (p<0.01), $V_{max}/M_{max}$ ratio (p<0.01), H latency (p<0.01) among the prone and standing position. Conclusion: We found that the H-reflex and V wave in standing position was more active to weight bearing load than prone position.

  • The increasing number of crimes in rapidly changing modern society is enhancing people's desire for safety. As of 2008, 2,900 private security businesses with 133,000 employees operated in the private security industry which emerged in response to growing demands from society and people. Of the employees, bodyguards (hereinafter referred to as "private security guard") accounted for about 10% or 13,000. Most private security guards were suffering from various occupational diseases. Especially as they needed to guard their clients many hours a day and worked in a standing position for a long time, private security guards often complained of low back pain. Under the pain, they were hardly expected to perform their tasks efficiently. There are several causes of low back pain. The most prevalent cause is muscle weakness and imbalance around low back. Especially because private security work often requires security guards to maintain a standing position for a long time, many of them are suffering from low back pain. This study pursued the following purposes. First, it tried to identify the pathogenesis of low back pain caused by muscle weakness and imbalance around low back. Second, it tried to provide private security guards, who can hardly have personal time at work, with an easy method to prevent and manage low back pain any time by researching an effective therapy for low back pain caused by muscle weakness and imbalance around low back.

Development and feasibility test of sit-to-stand transfer assistive device

  • Background: The purpose of this study was to develop a sit-to-stand transfer assistive device, and to conduct a feasibility test. Design: A technical note. Methods and results: In this study, we developed a sit-to-stand transfer assistive device for the elderly and the disabled who have difficulty standing up independently from sitting positions. The sit-to-stand transfer assistive device allows the user to transfer the weight from a sitting position to a standing position while shifting the weight forward by grabbing and pulling a support stand. Ten healthy adults participated in the feasibility test of the device. Each participant used the developed sit-to-stand transfer assistive device and investigated supplementation through a brief interview. As a result of the feasibility test, the opinion was that the device could assist the sit-to-stand transfer to some extent. There were opinions that it needed a function to adjust the height of the knee protective plate in the sitting position according to the user's physical characteristics. Because of the inconvenience of operating the lock device for fixing the position and adjusting inclination, there was an opinion that the improvement for a locking device is needed. There were opinions that it would be better to reduce the size of the device due to its inconvenience of portability. Conclusion: In this study, we developed the sit-to-stand transfer assistive device for the elderly and the disabled who have difficulty standing up independently from sitting positions. In addition, it is considered that the upgrade of the device is necessary for the future since there are supplementary opinions on some points.

인라인 스케이트 운동이 정적균형에 미치는 영향 (The Effects of the Inline-Skating Exercise on the Static Balance)

  • The purpose of this study was to investigate the effects of Inline-skating exercise on the static balance. Total 10 subjects with common were recruited from university for student in Kwangyang, and the Inline-skating exercise was conducted 3times at a week during 2 weeks. Balance test was measured by using Good Balance with overall index, NSEO(normal standing when eye open), NSEC(normal standing when eye close), OLREO(one leg right standing when eye open), OLREC(one leg right standing when eye close), OLLEO(one leg left standing when eye open), OLLEC(one leg left standing There was statistically significant difference of the balance ability of the left-right only between the before and the after exercise in the case of NSEO position. however, the statistically difference was not found in terms of the Mean X speed, Mean Y speed, Velocity moment between the before and the after exercise in the case of NSEC position. There was statistically significant difference of the balance ability of the Mean X speed, Mean Y speed between the before and the after exercise in the case of OLLEO and OLLEC position, however, the statistically difference was not found in terms of the Velocity moment between the before and the after exercise in this case. There was statistically significant difference of the balance ability of all the terms between the before and the after exercise in the case of OLREO. however, the statistically difference was not found in terms of the Mean Y speed only between the before and the after exercise in the case of OLREC. In this case, the statistically difference was found in terms of Velocity moment between the before and the after exercise. This study showed that Inline-skating exercise was effected on the improvement of the balance ability in normal peoples.

앉은 자세와 선 자세에서의 인체 관절 동작의 지각 불편도 Ranking (Ranking of Perceived Joints Discomfort in Sitting and Standing Postures)

  • The purpose of this study is to measure a perceived joint discomfort in the seated and standing position, and to provide ranking systems of perceived joint discomfort. Nineteen mole subjects with no history of musculo-skeletal disorders participated in the experiment. Their physical characteristics were: age $-25.4{\pm}2.7$years, stature $-171.9{\pm}6.0cm$, and body weight $-67.1{\pm}7.0kg$. The results showed that the perceived joint discomforts were different depending upon the joints involved in motion and their movement directions (degree of freedom of motions), which implied that the human body motions and their degrees of freedom should be classified into several distinct classes that need to be assigned different weights of postural stress. Therefore, three ranking systems based on the perceived joint discomforts were suggested, which were classified by the degree of freedom of motions and joints, by only degree of freedom motions, and by joints involved in motion, respectively. In the seated position, the hip movement was the most stressful, the bock was the second, and the shoulder was the third. Likewise, in the standing postures, the hip was the most, the bock was the second, and the ankle was the third. It was expected that these joint motion ranking systems could be used by practitioners of health and safety to improve the comfort of working postures in industry.

Effects of Stimulation Conditions and Waveforms on Muscle Contractile Characteristics

  • This study was designed to apply the stimulation system developed in our laboratory to investigate how the stimulation conditions affect the muscle contractile characteristics in the isometric condition as well as during the FES standing/walking. Four paraplegic and ten healthy subjects participated in this study, and their knee extensors were voluntary contracted or electrically stimulated to measure the muscle force and the fatigue index for different waveforms of the pulse train. We also investigated different combinations of the electrode positions during standing/walking. It was confirmed that continuous and high-frequency stimulation causes faster fatigue than intermittent and low-frequency stimulation. Fatigue resistance was higher around the optimal muscle length than at a stretched position in healthy subjects, whereas the opposite was observed in paralyzed subjects. The paired t-test results with the level of significance at 0.01 indicated that the sinusoidal waveform generated the largest torque among the four typical waveforms. Although statistically not very significant, the sinusoidal waveform also generated, in general, the highest fatigue resistance at an intensity level below the supramaximal stimulation. One of the paraplegic subject who participated in the standing/walking program can now stand up for 1 minute and 50 seconds with the knee extensors, and walk for about 5 minutes at the speed of 12m/sec.

Delayed Clinical Signs Associated with Cerebellar Lesion in Holstein Calf Infected with Bovine Viral Diarrhea Virus

  • A 5-month-old Holstein downer calf was presented, and bovine viral diarrhea virus was detected in the feces and the cerebrospinal fluid. Combined treatment of recombinant bovine somatotropin (rBST) administration and standing rehabilitation had been performed for two months, and the calf could maintain the standing position for a while. However, the symptoms were not improved, and the calf died due to ruminal tympany and aspiration pneumonia. Megaesophagus, abomasal ulcer, and a normal-sized but softened cerebellum were observed in clinical necropsy. Improvement of symptoms was confirmed by rBST, but was not reached the complete recovery of gait and standing position.
