기관지 성형술 (Bronchoplastic Procedures)

  • Bronchoplastic procedure has been considered as an appropriate surgery for traumatic bronchial disruption and occasionaly for primary bronchial tumors or tuberculosis because it can bring preservation of pulmonary tissue for patients without compromising the chance for cure. Nowadays bronchoplastic procedure is also applicable for the selected cases of bronchogenic carcinomas with favorable long term survival, when compared to standard pneumonectomy.Eighteen bronchoplastic procedures were performed with or without pulmonary resection at Department of Thoracic and Cardiovascular Surgery, Catholic University Medical College, between 1990 and 1994. The patients were 11 men and 7 wemen with average age of 57 years [range, 19 to 71 years . Tumor comprised 56% of the lesions, including 6 squamous cell carcinoma [33% , 2 bronchial adenoma [11% , 1 leiomyoma and 1 metastatic osteogenic sarcoma. Cicatrical stenosis secondary to endobronchial tuberculosis and traumatic disruption occurred in 6 [33% and 1 patient respectively.Applied bronchoplastic procedures were as follows ; sleeve lobectomy, 8 cases [right upper : 6, left upper : 1, right middle : 1 : bronchial segmental resection without pulmonary resection, 2 cases : sleeve bi-lobectomy, 1 cases :patch dilating bronchoplasty with or without concomitant lobectomy in 7. There was no perioperative mortality. Morbidity in 4 patients included 1 transient recurrent laryngeal nerve palsy, 1 unstability of bronchial patch resulting atelectasis of afftected lung and 2 bronchial stenosis of anastomotic site.Throughout our experiences, we feel strongly that bronchoplastic procedure is a safe and effective surgical method preserving normal pulmonary tissue below affected bronchus for the wide range of various bronchial lesion including selected cases of bronchogenic carcinoma with acceptable complication and mortality.

  • Pelvic bone fracture with hemodynamic instability is fatal and the mortality rate can range up to 40%. Despite the big advances in the treatment of massive bleeding and hemorrhagic shock, the mortality associated with hemodynamically unstable pelvic bone fractures remains high. The gold standard of treatment for pelvic bone fracture with hemodynamic instability has not yet been determined and is an issue of main discussion among many doctors. Retroperitoneal packing is not yet wide spread in Korea, but is a good modality for managing of massive bleeding from pelvic bone fractures when an angiography suite or an expert surgeon is not available. A vacuum-assisted closure (VAC) system can also be applied with retroperitoneal packing in the manner of damage control surgery and open abdomen surgery. We present the case of a 51-year-old male who had a pelvic bone fracture with massive bleeding. We performed retroperitoneal gauze packing with a VAC system for the first time. The postoperative vital signs of patient were immediately stable, the massive bleeding was easily and quickly controlled, and the amount of transfusion of blood components was reduced.

중환자실 환자의 건강결과 예측을 위한 중증도 평가도구의 정확도 비교분석 (Comparative Analysis of the Accuracy of Severity Scoring Systems for the Prediction of Healthcare Outcomes of Intensive Care Unit Patients)

  • Purpose: The purpose of this study was to compare the applicability of the Charlson Comorbidity Index (CCI) and Acute Physiology, Age, Chronic Health Evaluation III (APACHE III) to the prediction of the healthcare outcomes of intensive care unit (ICU) patients. Methods: This research was performed with 136 adult patients (age>18 years) who were admitted to the ICU between May and June 2012. Data were measured using the CCI score with a comorbidity index of 19 and the APACHE III score on the standard of the worst result with vital signs and laboratory results. Discrimination was evaluated using receiver operating characteristic (ROC) curves and area under an ROC curve (AUC). Calibration was performed using logistic regression. Results: The overall mortality was 25.7%. The mean CCI and APACHE III scores for survivors were found to be significantly lower than those of non-survivors. The AUC was 0.835 for the APACHE III score and remained high, at 0.688, for the CCI score. The rate of concordance according to the CCI and the APACHE III score was 69.1%. Conclusion: The route of admission, days in ICU, CCI, and APACHE III score are associated with an increased mortality risk in ICU patients.

Analysis of Basic Emergency Obstetric and Neonatal Care (PONED) Services in Baso Public Health Center, Indonesia: Recommendations from Qualitative Interviews

  • One of the main indicators of the health status of a country is the Maternal Mortality Rate (MMR). In order to improve the MMR in Indonesia, the government has made a number of primary health centers (PHC) capable of providing basic obstetric and neonatal emergency care (PONED) services. The aim of this research is to learn how well PONED services have been implemented at the Baso PHC in Agam Regency as of 2015. We used a qualitative approach, with in-depth interviews and observations from June-August 2015. Ten respondents participated in the in-depth interviews, and observations were made about the infrastructure. The validity of the data is based on the standard of credibility, transferability, dependability, and confirmability of the content of the interviews. The analysis shows that there are still some obstacles in PONED PHC implementation, including the lack of human resources at the PHC and insufficient facilities, funding, and oversight, which causes many cases that should be handled by a PONED team to be referred to the hospital. Suggestions are provided to help improve policies and ultimately patient care.

Replacement of Soyflakes with Cottonseed Meal in Diets of Angora Rabbits

  • This experiment was conducted with 30 adult wool producing Angora rabbits of either sex, to evaluate the effect of replacing soyflakes (SF), on equal protein basis, with low gossypol containing cottonseed meal (CSM) either alone or in combination with lysine and methionine; on the biological performance, total wool yield, wool yield per shearing, wool characteristics and mortality. Three experimental mash diets were prepared by incorporating SF (6%) as standard / control protein source $(T_1)$ and CSM (9%) as test protein source ($T_2$ and $T_3$). In $T_3$, amino acids-lysine and methionine (0.1 % each) were added. Animals were given the experimental diets about $150g{\cdot}day^{-1}{\cdot}head^{-1}$, for a period of 225 d or three shearing, and ad libitum Kudzu-vines. No significant effect of $T_2$ or $T_3$, on the body weight gain, total wool yield, wool yield per shearing and wool characteristics, was observed compared to $T_1$. However, the digestibility of dry matter, crude fibers, ether extract, acid detergent fibers, neutral detergent fibers, cellulose and hemicellulose was significantly (p<0.05) depressed in CSM based diets. Mortality of about 20% was recorded in $T_2$ and $T_3$, but not related to the addition of CSM or gossypol toxicity. More studies are needed to standardize the safe level of CSM, duration of safe feeding of CSM, and level of amino acids supplementation in CSM based diets.

Dilemmas of Oral Cancer Screening: An Update

  • Oral cancer is a global health burden with high mortality and morbidity. Advances in treatment have failed to improve the relatively poor survival rate due to late-stage diagnosis. Early detection and screening have been shown to be effective in reducing mortality and morbidity of most common cancers. Several studies have evaluated the effectiveness of oral cancer screening programs but clear results were not obtained. This narrative commentary aimed to give a critical insight into the dilemma of oral cancer screening and to suggest recommendations for future trends. Conventional oral examination still constitutes the gold standard screening tool for potentially malignant oral lesions and cancer. Interestingly, the findings of the most lasting (15-year) randomized controlled trial on oral cancer screening using visual examination (Kerala) supported the introduction of a screening program in high-risk individuals. Several screening adjuncts exist but are still not at the introduction stage. Further research to find an appropriate adjunct reliable tool for oral cancer screening is needed. In conclusion, oral cancer fulfills most of the essential principles of cancer screening but still many points need to be clarified. Therefore, there is a striking need to establish a global consortium on oral cancer screening that will oversee research and provide recommendations for health authorities at regular intervals.

중복판막치환에서 이오네스큐 단고형 판막의 내구성 (Durability ofthe low-profile Ionescu-Shiley valve in mitral and aortic position)

  • Improved clinical performance was expected from the introduction of the low-profile model of the Ionescu-Shiley pericardial valve. The long-term clinical results were assessed on the consecutive 47 patients who underwent MVR + AVR with this valve between 1984 and 1988. Three patients died within 30 days of surgery[operative mortality, 6.4%], and 44 early survivors were followed up for a total of 203.8 patient-years [Mean + SD, 4.63 + 1.47 years]. One died during the follow-up with a linearized late mortality of 0.491%/patient~year[pt-yr]. None experienced thromboembolism. Bleeding and endocardiris were seen in each single patient with the incidences of complication of 0.491%/pt-yr respectively. The linearized rate of primary tissue failure [PTF] was 0.491%/pt-yr. The actuarial survival and rate of freedom from PTF were 97.6 _+ 2.4% and 92.6 +7.1% at 7 years of follow-up.These results are favorably comparable with the ones seen in the patients of MVR + AVR with the standard profile lonescu-Shiley valve in all respects except the higher mean age of the low-profile group. Although the clinical performance was compatible with other major reports, the durability of the valve remains to be proved with the prolonged follow-up.

Termiticidal Activity and Chemical Components of Bamboo Vinegar against Subterranean Termites under Different Pyrolysis Temperatures

  • In this study, the chemical components and termiticidal activities of bamboo vinegar against subterranean termite were evaluated. Bamboo vinegar used in this study were produced from Mayan (Gigantochloa robusta Kurz.), Balcoa (Bambusa balcooa Roxb.), and Taiwan (Dendrocalamus latiflorus Munro). It was analyzed by gravimetric methods. Rubberwood (Hevea brasiliensis) was soaked for 24 h and tested against Coptotermes curvignathus Holmgren according to the Indonesian National Standard (SNI 7207-2014). The bamboo vinegar demonstrated antitermite activities against subterranean termite. Vinegar of 400℃ recorded higher total phenol and acid than that of 300℃. Results from this study suggest that the phenolic and acidic component of the bamboo vinegar contributed to 100% mortality rate of the termite, some wood weight loss, and a decrease in the termite feeding rate. These results strongly recommend that bamboo vinegar from D. latiflorus Munro is a potential environmentally friendly preservative.

BENZENE AND LEUKEMIA An Epidemiologic Risk Assessment

  • To assess quantitatively the association between benzene exposure and leukemia, we examined the mortality rate of a cohort with occupational exposure to benzene. Cumulative exposure for each cohort member was estimated from historical air-sampling data and, when no sampling data existed, from interpolation on the basis of existing data. The overall standardized mortality ratio (a measure of relative risk multiplied by 100) for leukemia was 337 (95 percent confidence interval, 154 to 641), and that for multiple myeloma was 409 (95 percent confidence interval, 110 to 1047). With stratification according to levels of cumulative exposure, the standardized mortality ratios for leukemia increased from 109 to 322, 1186, and 6637 with increases in cumulative benzene exposure from less than 40 parts per million-years (ppm-years), to 40 to 199, 200 to 399, and 400 or more. respectively. A cumulative benzene exposure of 400 ppm years is equivalent to a mean annual exposure of 10 ppm over a 40-year working lifetime; 10 ppm is the currently enforceable standard in the United States for occupational exposure to benzene. To examine the shape of the exposure-response relation, we performed a conditional logistic-regression analysis, in which 10 controls were matched to each cohort member with leukemia. From this model, it can be calculated that protection from benzene induced leukemia would increase exponentially with any reduction in the permissible exposure limit.

옹진군 어장관리해역 해제 및 변경 타당성 평가 (Feasibility of Changing or Canceling Designated Mariculture Management Areas in Ongjin-gun, Korea)

  • Some of the mariculture grounds near Ongjin-gun, Korea, were designated as mariculture management areas in 2007. Areas are so designated when the environmental quality of the mariculture ground deteriorates or there is an outbreak of hypoxia or harmful red tide that kills mariculture organisms. We surveyed the water and sediment quality and examined the mortality of mariculture organisms in the Ongjin-gun mariculture area. In a survey conducted in 2016, the water quality was better than the environmental quality standards for mariculture grounds, excepts for dissolved inorganic nitrogen, and the sediment quality was good. However, there was still mortality of mariculture organisms in some of the designated management areas. The areas that met the environmental quality standards should be delisted and the areas in which we observed mortality should be classified as management areas. This will enable the sustainable development of aquaculture and preserve healthy mariculture grounds.
