Surgical implications of anatomical variation in anterolateral thigh flaps for the reconstruction of oral and maxillofacial soft tissue defects: focus on perforators and pedicles

  • Objectives: To gain information on anatomical variation in anterolateral thigh (ALT) flaps in a series of clinical cases, with special focus on perforators and pedicles, for potential use in reconstruction of oral and maxillofacial soft tissue defects. Materials and Methods: Eight patients who underwent microvascular reconstructive surgery with ALT free flaps after ablative surgery for oral cancer were included. The number of perforators included in cutaneous flaps, location of perforators (septocutaneous or musculocutaneous), and the course of vascular pedicles were intraoperatively investigated. Results: Four cases with a single perforator and four cases with multiple perforators were included in the ALT flap designed along the line from anterior superior iliac spine to patella. Three cases had perforators running the septum between the vastus lateralis and rectus femoris muscle (septocutaneous type), and five cases had perforators running in the vastus lateralis muscle (musculocutaneous type). Regarding the course of vascular pedicles, five cases were derived from the descending branch of the lateral circumflex femoral artery (type I), and three cases were from the transverse branch (type II). Conclusion: Anatomical variation affecting the distribution of perforators and the course of pedicles might prevent use of an ALT free flap in various reconstruction cases. However, these issues can be overcome with an understanding of anatomical variation and meticulous surgical dissection. ALT free flaps are considered reliable options for reconstruction of soft tissue defects of the oral and maxillofacial area.

  • This study will explore the reenactment of visual special effects used in TV dramas. The subjects of the study are images that are reproduced in visual special effects through and . Accordingly, we looked at the changes in the way TV dramas were produced according to the times, and looked at the changes in the market. Also, we looked at cases of visual special effects of Korean dramas, and looked at expressions according to the degree of completion of visual special effects. For the purpose of the analysis of the research targets, the images of reproducibility and the flow through reality were analyzed. In the case of , the period setting is realistic, but visual special effects were used to maximize the actor's safety and expression, and in the case of , the visual effects were used to maximize the aesthetic of the period background. In addition, it can be seen that the visual special effects were used for the effect of PPL on the export of images. This shows that the emphasis of reality and image montage techniques are being used to pursue hyperrealism. It is now possible to actively express and use the beauty of TV dramas rather than technical limitations. In addition, the pursuit of realism can actively express the changing times of digital age. This is an active representation of the camera's angle, lighting, and perspective that coincides with the background. The environment of video production is crucial for realistic expressions. The study examined various visual directions and applications. In TV images, we looked at reproduction, which can make a natural period of time by combining. As a follow-up study, we are going to study the changes in the new quadratic image based on the present image representation.

Clinical analysis of completion pneumonectomy; report of 11 cases (Completion pneumonectomy의 임상적 고찰;11례 임상보고)

  • Among elevn consecutive cases having undergone Completion Pueumonectomy[CP]between 1958 and Aug. 1993 at the Dep. of Thoracic & Cardiovascular Surgery in National Medical Center. The patient`s mean age was 43 years[range 28 yrs, to 68yrs.],& they consisted with 10 males and 1 female. The indications for CP were benign diseases in 9 cases & 2 cases of lung cancer. The mean interval between the first operation & CP was 62.3 months[from 17 days to 288 months]. The several special intraoperative procedures such as intrapericardial pulmonary vesselsdivision & suture ligation, reinforcement of bronchial stumpmargin, & applied the Fibrin glue & hemostatics. The mean intra operative bleeding was 3582ml.[1500ml. to 6500ml.] The post orerative complication were developed in 5 cases[45.5%] they were empyema with BPF in 2 cases, empyema in 2 cases, & 1 case of repiratoy insufficiency which leading to death. We concluded that the C P noted high morbidity & mortality compared with ordinary first pulmonary resectional surgery. But, it will be a challenge to improved the morbidity because of increasing trend of completion pneumonectomy in a furture time.

Biceps Rerouting Technique(Modification of Clancy) for Posterolateral Rotatory Instability (대퇴이두건 전환술(Clancy 변형 술식)을 이용한 후외측 회전 불안정성의 재건)

  • Introduction : This study compared the clinical results with biceps rerouting fer the isolated posterolateral instability (PLI) and for the PLI combined with PCL injuries. Methods : 21 cases of isolated PLI (group I) and 25 cases of PLI combined with PCL rupture were included in the study. The PLI was reconstructed by modified biceps femoris rerouting technique with PCL reconstructions performed prior to the PLI correction in cases of combined injury The clinical results were reviewed and analyzed. Results : Pre-operatively positive reverse pivot shift test turned negative in 43 cases post-operatively. Increased preoperative external rotation thigh foot angle (ERTFA) showed significant differences between the two groups and all fell within normal limits post-operatively At a mean follow-up of 40.3 months, the average Lysholm knee score and. The Hospital for Special Surgery Knee Ligament Score for group I and group II revealed above 90 points without statistically significant difference between the groups. 3 cases of tenodesis failure developed and re-operation was performed. Discussion and Conclusion : The advantages of modified Clancy technique include reduced surgical damages to the iliotibial band and fixation of the biceps tendon at the isometric position. The modified biceps rerouting technique is recommended for the reconstruction of both isolated and combined PLI except in patients with severe damages at the attachment of biceps tendon.

A New Complexity Analysis of the SymMerge Algorithm (SymMerge 알고리즘의 새로운 복잡도 분석)

  • The SymMerge algorithm is an efficient merging algorithm for input sequences u and v of sizes $\left|u \right|=m$ and $\left|v \right|=n$, $m{\leq}n$. We consider complexity analysis for SymMerge algorithm regarding to the required number of comparisons. The focus of the previous complexity analysis was on finding the values of upper bounds, i.e. showing the asymptotical optimality. In this paper, in a different way from the previous complexity analysis, we show that the overall required number of comparisons for two representative special cases "symmetric case" and "maximum spanning case" can be calculated exactly i.e. the least upper bounds regarding to the required number of comparisons are calculated. Symmerge requires exactly $m\;log\frac{n}{m}+4m-logm-3$ comparisons for symmetric case of sizes $m=2^k,\;n=2^l,l{\geq}k$ of input sequences and exactly $\frac{1}{2}m^2+(m+1)logm-\frac{3}{2}m+2$ comparisons for maximum spanning case of sizes $m=2^k,n=2^m-m$ of input sequences. Additionally we show that the complexity of the Symmerge algorithm regarding to the overall required number of comparisons for these special cases can be defined by recurrence relations.

Analysis of Experiences of Child Care and Education Teachers and Special Education Teachers On Implementation of the 2019 Revised Nuri Curriculum at Inclusive Settings: Based on Qualitative Interviews (「2019 개정 누리과정」실행에서 장애통합어린이집 일반교사와 특수교사의 경험 분석: 질적 면담을 중심으로)

  • Objective: The purpose of this study was to investigate the implementation experiences of the 「2019 revised Nuri curriculum」 of early childhood education teachers and special education teachers at inclusive child care and education centers. Methods: Individual interviews with 14 teachers at seven inclusive settings in Seoul were conducted. The collected data were transcribed, and six categories and 42 sub-themes in early childhood education teachers and six categories and 37 sub-themes in special education teachers were derived through qualitative-comparative content analysis. Results: The teacher in-service education conducted prior to the implementation of the 「2019 revised Nuri curriculum」 gave substantial aid in the applied practices of cases. The perceptions of teachers related to the 「2019 revised Nuri curriculum」 were heightened. The inclusive settings had been changed according to the principles of play, case, and children's interest. As the 「2019 revised Nuri curriculum」 progressed, children also changed. The implementation of the 「2019 revised Nuri program」 allowed the positive and the negative experiences to coexist at the inclusive settings. For the successful implementation of the 「2019 revised Nuri curriculum」 , calls for the improvement of the child care and education center and national level must be stressed. Conclusion/Implications: Based on the results, discussions and recommendations were presented to solve the difficulties.

  • In this paper, we use the auxiliary principle technique to suggest and analyze a class of predictor-corrector methods for solving noncoercive mixed variational inequalities. The convergence of the proposed method requires only the partially relaxed strongly monotonicity. which is even weaker than the co-coercivity. As special cases, we obtain a number of new and known results for classical variational inequalities.

  • The purpose of this paper is to determine conditions under which equicontinuity of the family of iterates {f$^{n}$ } of a continuous function that maps the circle S$^{1}$ into itself does occur. We shall see that equicontinuity of the family of iterates {f$^{n}$ } occurs only under special cases. Actually, we will show that this happens only for rotations when degree of the function is 1, and for involutions when degree of the function is -1.

  • In this paper, we suggest and analyze some new classes of three step iterative algorithms for solving generalized quasivariational inclusions by using the properties of proximal maps. Our results include the Ishikawa, Mann iterations for solving variational inclusions(inequalities) as special cases. The results obtained in this paper represent an improvement and significant refinement of previously known results [3, 5-8, 10, 14-18].

  • A time series model with Laplacian (double-exponential) marginal distribution, NLAR(2), was proposed by Dewald and Lewis (1985). The special cases of NLAR(2) process and their properties are considered. Extensions to the NLAR(p) is discussed. It is shown that the NLAR(1) satisfies the strong-mixing conditions, hence the model-free prediction interval using the sample quantiles can be obtained.
