Development of a Digital Otoscope-Stethoscope Healthcare Platform for Telemedicine (비대면 원격진단을 위한 디지털 검이경 청진기 헬스케어 플랫폼 개발)

  • We developed a device that integrates digital otoscope and stethoscope for telemedicine. The integrated device was utilized for the collection of tympanic membrane images and cardiac auscultation data. Data accumulated on the platform server can support real-time diagnosis of heart and eardrum diseases using artificial intelligence. Public data from Kaggle were used for deep learning. After comparing with various deep learning models, the MobileNetV2 model showed superior performance in analyzing tympanic membrane data, and the VGG16 model excelled in analyzing cardiac data. The classification algorithm achieved an accuracy of 89.9% for eardrums data and 100% for heart sound data. These results demonstrate the possibility of diagnosing diseases without the limitations of time and space by using this platform.

  • A comparative study was made of the valvular sounds produced by normal prosthetic valves with thrombosed prosthetic valves. Comparisons of the closing sound were made for the power frequency spectra associated with individual valves. We used periodogram approach to obtain the spectral characteristics of the valve. Spectral analysis system was tested in mock circulatory system by comparing normal valves with those produced by the same valves but having simulated thrombosis at the hinge of the valve. The heart sounds was recorded from two patients having normal mechanical valve and thrombosed mechanical valve. The estimated spectrum of the thrombosed mechanical valve displayed lower apparent peak frequency than that of the normal valve. The results showed that frequency spectra gave information pertinent to the valve malfunction. Sound spectral analysis is simple and alternative diagnostic tool for early detection of prosthetic valve mal function.

  • The purpose of this article is to discuss that ajaeng sanjo music playing (ASMP) can change a person's life. Ajaeng is often called a Korean cello and is the largest and lowest-pitched instrument in the string instrument family in Korean music. From far away it has a deep, low sound. Its low sound adds to its comfort and peace. Listening to a low tone music can help you calm down and heart-easing. As soon as he listened ajaeng sanjo music he felt that low sound is mildly under his patronage. He felt much more interesting energy and vitality about our minds and bodies are in ajaeng sanjo music. He could immerse himself in playing ajaeng sanjo music (ASMP) for several years. The Korean music, tune is one of expression, meaning it is inseparable from the feeling. Many studies show that low tone music not only activates brain power, but soothes minds. Music always stood by him during difficult times. In a word, ASMP leads him to change of life and music is the language of emotion to him.

  • Purpose: The purpose of this study was to compare the result of one-way ANOVA with that of cross-correlation time series analysis in order to evaluate physiologic responses of premature infants to human voices. Methods: Four premature infants born prior to 32 weeks gestational age were included in the study. The Gould 4000TA Recording System recorded the preterm infant's heart and respiratory rate while they were listening to a pre-recorded voice recording. Each infant listened to both male and female voices (1 min each) at each testing session. Results: The results of both one-wayANOVA and cross-correlation time series analysis using heart and respiratory rate data were not consistent in some of premature infants. A cross-correlation time series analysis revealed that the responses of premature infant to vocal stimulation occurred at a varying number of seconds after the stimulus was presented and lasted for over 20-30 sec. Conclusion: The results indicate that a time series analysis can provide more detailed information on the rapidly changing physiologic status of premature infant to the auditory stimulus. In addition, the results provide an insight into an auditory responsitivity of premature infants to a naturally occurring sound, the human voice, in the neonatal intensive care unit.

  • 본 연구에서는 생환공간에서의 음향감성을 평가하기 위하여 긍정감성을 유발하는 음환경으로서 '명상음악'과 부정감성을 유발하는 음환경으로서 '헬리콥터소음'과 '마루가 삐거덕거리는 소음'을 제시하였을 때 행동활성화체계(BAS)와 행동억제체계(BIS)의 민감성이 서로 다른 집단들을 대상으로 전전두엽의(PFC) 비대칭성과 심장박동율변동성(HRV)을 분석하였다. 연구결과, 명상음악을 청취시에는 안정상태에 비하여 좌측전두엽이 더 활성화되고 HRV의 LF/HF는 감소된 반면에 소음환경에서는 우측전두엽이 더 활성화되고 LF/HF는 더 증가하였다.

Ebstein`S Anomaly: A Case Report of Plication and Tricuspid Valve Replacement (Ebstein 심기형 수술 1례[Plication 및 삼첨판막 이식예])

  • A 8 year old male was admitted to the Department of Thoracic Surgery, Korea University Hospital on June 22, 1978. The chief complaints were cyanosis and exertional dyspnea since at birth. EKG shows BVH and dextrocardia, phonocardiogram revealed the accentuation of second heart sound in aortic area. Echocardiogram from the left ventricle to the base of the heart, there is a discontinuity between the ventricular septum and the anterior aortic margin with a large aortic root & aortic overriding. His cardiac catheterization data and cardiac angiogram shows situs inversus totalis, dextrocardia, right aortic arch, large ventricular septal defect etc., and finally diagnosed Truncus Arteriosus. Edwards type IV with retrograde aortogram and selective bronchial angiogram. This is the first operative case reported as Rastelli operation for Truncus Arteriosus type IV in the literatures in Korea. Authors have experienced I case of Truncus Arteriosus, Edward type IV and Rastelli operation with Dacron Arterial Conduit Graft under cardiopulmonary bypass on July 3, 1978. The procedures were as follows; 2] Cardiopulmonary bypass: Origin of bronchial arteries excised from descending aorta bilaterally; defects in aorta closed. 2] Horizontal incision made high in right ventricle. 2] Ventricular septal defect [Kirklin type I+II] closed with Teflon patch. 4] Bifurcated dacron arterial graft with pericardial monocusp sutured to the bilateral pulmonary arteries. [Diameter 9 mm: Length 7 cm]. 5] Proximal end of the conduit graft anastomosed to right ventricle. [Diameter 19 mm: Length 5 cm]..Total perfusion time was 220 min. The result of operation was poor due to anastomotic leakage and increased pulmonary vascular resistance resulting acute right heart failure. The patient was died on the operation table. Literatures were briefly reviewed.

Matching Pursuit based on Frequency Characteristics of Heart Sounds for Phonocardiogram Analysis (심음 분석을 위한 주파수 특성에 기반한 매칭퍼슈잇 방법)

  • Although heart auscultation is an important tool for the diagnosis of heart disease, it is seen that the human ear is poorly suited for cardiac auscultation such as the paradoxical split of S2, the split of S2, and the summation of S2 and S3 gallop. In this paper, the analysis-synthesis method that is suitable for the characteristics of PCG is proposed to classify the heart sound. In analyzing the PCG according to matching pursuit method and synthesizing the PCG in each bandwidth based on the frequency characteristics, it sorts normal hearts with abnormal hearts that is difficult to classify as cardiac auscultation. The result shows that it qualifies the appearance of split in S2 and the paradoxical split of S2 and also can estimate the delay between the two components.

Heart Sound-Based Cardiac Disorder Classifiers Using an SVM to Combine HMM and Murmur Scores (SVM을 이용하여 HMM과 심잡음 점수를 결합한 심음 기반 심장질환 분류기)

  • In this paper, we propose a new cardiac disorder classification method using an support vector machine (SVM) to combine hidden Markov model (HMM) and murmur existence information. Using cepstral features and the HMM Viterbi algorithm, we segment input heart sound signals into HMM states for each cardiac disorder model and compute log-likelihood (score) for every state in the model. To exploit the temporal position characteristics of murmur signals, we divide the input signals into two subbands and compute murmur probability of every subband of each frame, and obtain the murmur score for each state by using the state segmentation information obtained from the Viterbi algorithm. With an input vector containing the HMM state scores and the murmur scores for all cardiac disorder models, SVM finally decides the cardiac disorder category. In cardiac disorder classification experimental results, the proposed method shows the relatively improvement rate of 20.4 % compared to the HMM-based classifier with the conventional cepstral features.

Telemedicine Conference System for Realtime Transfer of Heart Sound (실시간 심음 전송을 위한 원격 의료상담시스템)

  • Counselling between a patient and a doctor is crucial in telemedicine. In order for the doctor to examine the patient accurately, it needs an auscultation, at least. Currently, some video conference systems are implemented but it is hard to use them in the case of an cardiac disorder, because the patients suffering from cardiac disorder cannot be examined by a stethoscope over Internet. To solve this problem, the remote counselling service has to support real time transmission of the heart sound of the patient. In this paper, we present a remote counselling system with stethoscope. We also design and implement the system in order for health monitor to connect the patient with his attending physician for the environment of u-healthcare service. The proposed system supports a mobility for doctor and patient by exchanging IP addresses at an user authentication protocol. The system implemented by this paper can be used for cardiac patients in remote clinical setting in the future.
