In this study, the authors aimed to give careful consideration to the circulation of bioenergy (Qi) by using cardiorespiratory massage technique. Finding enlightenment in harmony with nature thousands of years ago, our ancestors created Qi using natural breathing and tried to achieve good health and long life by controling the up and down movement. Experiencing the importance of the abdomen during the hungry times, also, grandmothers cured diseases by rubbing the abdomen, which is the ocean of life with their soft, soothing touch of hand. Naval breathing therapy was formed, based on natural breathing, emotional recognition of grandmothers' soft, soothing touch, and the Theory of Unified Reason and Energy that is the key of Oriental medicine. Natural breathing and Yakson massage are kinds of naturopathy to change body and mind, cure diseases naturally, and maintain the improvement in physical functions by taking care of the abdomen through the creation of Qi and the up and down movement and keeping dynamic balance between the natural world and the inside and outside of human body. It is anticipated that this study could contribute to the practical spread for an active application of naval breathing therapy and be broadly used in scientific clinical researches.
The centrifuge model test is usually used for two-dimensional deformation and instability study of the soil slopes. As a typical loose slope, the municipal solid waste (MSW) landfill is easy to slide with large deformation, under high water levels or large earthquakes. A series of centrifuge model tests of landfill slide induced by rising water level and earthquake were carried out. The particle image velocimetry (PIV), laser displacement transducer (LDT) and marker tracer (MT) methods were used to measure the deformation of the landfill under different centrifugal accelerations, water levels and earthquake magnitudes. The PIV method realized the observation of continuous deformation of the landfill model, and its results were consistent with those by LDT, which had higher precision than the MT method. The deformation of the landfill was mainly vertically downward and increased linearly with the rising centrifugal acceleration. When the water level rose, the horizontal deformation of the landfill developed gradually due to the seepage, and a global slide surface formed when the critical water level was reached. The seismic deformation of the landfill was mainly vertical at a low water level, but significant horizontal deformation occurred under a high water level. The results of the tests and analyses verified the applicability of PIV in the two-dimensional deformation measurement in the centrifuge model tests of the MSW landfill, and provide an important basis for revealing the instability mechanism of landfills under extreme hydraulic and seismic conditions.
For Special Concentrically Braced Frame (SCBF), it is common that the damage concentrates at a certain story instead of spreading over all stories. Once the damage occurs, the soft-story mechanism is likely to take place and possibly to result in the failure of the whole system with more damage accumulation. In this study, we use a strongback column which is an additional structural component extending along the height of the building, to redistribute the excessive deformation of SCBF and activate more structural members to dissipate energy and thus avoid damage concentration and improve the seismic performance of SCBF. We tested one-third-scaled, three-story, double-story X SCBF specimens with static cyclic loading procedure. Three specimens, namely S73, S42 and S0, which represent different combinations of stiffness and strength factors ${\alpha}$ and ${\beta}$ for the strongback columns, were designed based on results of numerical simulations. Specimens S73 and S42 were the specimens with the strongback columns, and S0 is the specimen without the strongback column. Test results show that the deformation distribution of Specimen S73 is more uniform and more brace members in three stories perform nonlinearly. Comparing Drift Concentration Factor (DCF), we can observe 29% and 11% improvement in Specimen S73 and S42, respectively. This improvement increases the nonlinear demand of the third-story braces and reduces that of the first-story braces where the demand used to be excessive, and, therefore, postpones the rupture of the first-story braces and enhances the ductility and energy dissipation capacity of the whole SCBF system.
Purpose This study was to develop and study 'Diagnostic Index for the Ordinary Symptom of Soyangin'. Methods The main diagnosis and evaluation index symptoms of Soyangin were extracted based on 『Donguisusebowon』 and Clinical Practice Guidelines for Sasang Constitutional Medicine Symptomatology; CPG for soyangin. The selected ordinary symptoms were divided into 4 categories such as Exterior-Cold ordinary symptom (表寒素證), Exterior-Disease unfavorable's ordinary symptom (表病逆證素證), Interior-Heat ordinary symptom (裏熱素證), Interior-Disease unfavorable's ordinary symptom (裏病逆證素證) of Soyangin and translated into Korean. And the inclusion, importance and translation validity of major diagnosis and assessment indicators of Soyangin were surveyed online by a group of Sasang constitutional experts. Results & Conclusions 29 ordinary symptoms and pathological mechanism were selected from references and translated into 51 Korean symptoms. After inclusion yes or no survey by expert group, 45 symptoms were selected. The importance survey showed that 'Have a soft stool' of Exterior-Cold ordinary symptom indication, '(If you're stressed, if you're not in good health) Have a soft stool or diarrhea' of Exterior-Disease unfavorable's ordinary symptom, 'The feces harden easily' of Interior-Heat ordinary symptom and '(If you're stressed, if you're not in good health) Have an upset stomach' of Interior-Disease unfavorable's ordinary symptom were the most important. Finally, [Diagnostic Index for the Ordinary Symptom of Soyangin] was completed after survey of translation validity, reflection of individual opinions by the expert committee, and application of weighted value calculated from the importance survey.
In order to explore the stable anchoring conditions of coal side under the mining disturbance of soft section coal pillar in Wangcun Coal Mine of Chenghe Mining Area, the distribution model of the anchoring support pressure at the coal pillar side was established, using the strain-softening characteristics of the coal to study the distribution law of anchoring coal side support pressure. The analytical solution for the reinforcement anchorage stress in the coal pillar side was derived with the inelastic state mechanical model. The results show that the deformation angle of the roadway side and roof increases with the roof subsidence due to the mining influence at the adjacent working face, the plastic deformation zone extends to the depth of the coal side, and the increase of anchorage stress can effectively control the roof subsidence and further deterioration of plastic zone. The roadway height and the peak support pressure have a certain influence on the anchorage stress, the required anchorage stress of the coal side rises with the roadway height and the peak support pressure. The required anchorage stress of the coal pillar side decreases as the cohesion between the coal seam and the roof and floor and the anchor length increases. Then, applied the research result to Wangcun coal mine in Chenghe mining area, the design of anchor cable reinforcement support was proposed for the section of coal pillars side that has been anchored and deformed, which achieved great results and effectively controlled the convergence and deformation of the side, providing a safety guarantee for the roadway excavation and mining.

The ship collision analysis of steel pile group as protection system of bridge in navigable waterways was performed to analyze the structural characteristics of protective structure during ship collision. The analysis encompassed finite element modeling of ship and pile, modeling of material non-linearity, hard impact analysis, displacement-based analysis and soft impact analysis for collision scenarios. Through the analysis of hard impact with a rigid wall, impact load for each collision type of ship bow was estimated. In the displacement-based analysis the estimate of energy which protection system can absorb within its maximum horizontal clearance so as to secure bridge pier from vessel contact during collision was performed. Soft impact analysis for various collision scenarios was conducted and the collision behaviors of vessel and pile-supported protection system were reviewed for the design of protection system. The understanding of the energy dissipation mechanism of pile supported structure and colliding vessel would give us the optimized design of protective structure.
Purpose: This study investigated the epidemiology, management, outcomes, and postoperative disabilities of degloving soft tissue injuries (DSTIs) treated at a tertiary care center in northern India. Methods: A prospective study of patients with DSTIs was conducted over 15 months. The type of degloving injury, the mechanism of injury, and any associated injuries were analyzed using the World Health Organization Disability Assessment Schedule (WHODAS) 2.0 along with the management, outcomes, and disabilities at a 3-month follow-up. Results: Among 75 patients with DSTIs, the average age was 27.5 years, 80.0% were male, and 76.0% had been injured in traffic accidents. The majority (93.3%) were open degloving injuries. Lower limbs were affected most often (62.7%), followed by upper limbs (32.0%). Fractures were the most commonly associated injuries (72.0%). Most patients required more than two procedures, including secondary debridement (41.3%), split skin grafting (80.0%), flap coverage (12.0%), or vacuum-assisted closure (24.0%), while five patients underwent conservative management for closed degloving injuries. Postoperative complications included surgical site infections (14.7%) and skin necrosis (10.7%). Two patients died due to septic shock and multiple organ dysfunction syndrome. The mean length of hospital stay was 11.5±8.1 days, with injuries affecting the lower limbs and perineum requiring longer hospital stays. The mean WHODAS 2.0 disability score at 3 months was 19. Most patients had mild disabilities. Time away from work depended largely upon the site and severity of the injury. Approximately 75% of patients resumed their previous job or study, 14% changed their job, and 8% stopped working completely due to residual disability. Conclusions: DSTIs are common injuries in trauma and management is challenging. Although open DSTI are clinically evident at secondary survey, closed degloving injuries may be missed in the primary survey, necessitating a high index of suspicion, thorough clinical examination, and protocol-based management. Primary preventive strategies (e.g., road safety protocols, preplacement training, and proper protective equipment in industries) are also needed to reduce the incidence of these injuries.

The present study has attempted to look into the mechanism of ras-induced carcinogenesis in a human epithelial cell system. Human epithelial cells immortalized with Ad12-SV40 hybrid virus were used to assess carcinogenic potential of the ras-oncogene. Cells transfected with pSV2-ras showed characteristics of cellular transformation. The transformation parameters such as cell density, soft-agar colony formation, and cell aggregation were significantly increased in the cells expressing ras oncoprotein. In addition, the duration required for the appearance of foci was shortened in the ras-transfected cells. Consistent with other reports, our results demonstrated an evidence that the ras-oncogene induced the cellular transformation of human epithelial cell system. When a high concentration of glucocorticoid was added into the media, transformation process was accelerated. It is speculated that glucocorticoid may provide an advantageous environment for the proliferation of the transformed cells. The induction of the intracellular free calcium concentrations following agonist treatment was significantly lower in the transformed cells than in the control cells. These effects were more manifested in the presence of extracellular cacium, indicating that the transformation process may alter the influx pathway of extracellular calcium. The induction of $IP_3$ following agonist treatment was also lower in the transformed cells than in the control cells. Thus, it is suggested that phospholipase C-coupled pathway was down-regulated in the process of the ras-induced transformation. While the levels of $TGF-{\beta}_1$ and PAI-2 mRNAs were decreased, the level of fibronectin mRNA was increased. The results indicate that mechanism of the ras-induced transformation may be associated with the altered expressions of growth regulatory factors. The present study demonstrates an evidence that the ras-induced cellular transformation may be associated with alteration of signal transduction and growth regulatory factors. The study will contribute to improve the understanding of molecular mechanism of epithelium-derived cancers including oral cancer.

Velopharyngeal closure is a sphincter mechanism between the activities of the soft palate, lateral pharyngeal wall and the posterior pharyngeal wall, which divides the oral and nasal cavity. It participates in physiological activities such as swallowing, breathing and speech. It is called a velopharyngeal dysfunction when this mechanism malfunctions. The causes of this dysfunction are defects in (1) length, function, posture of the soft palate, (2) depth and width of the nasopharynx and (3) activity of the posterior and lateral pharyngeal wall. The purposes of this study are to analyze the nasopharynx of cleft palate patients using cephalometry and to evaluate the degree of hypernasality using nasometry to find its relationship with velopharyngeal dysfunction. The following results were obtained : 1. In cephalometry, there were significant differences in soft palate length, soft palate thickness, nasopharyngeal depth, nasopharyngeal area, and adequate ratio between two groups. 2. In nasometry, there were significant differences between two groups in vowel /o/ and sentences including oral consonants. 3. In cleft palate patients, though no general correlation was found between Anatomic VPI and nasalance scores, vowel /i/ and sentences including oral consonants were slightly correlated. In conclusion, cephalometry and nasometer results were significantly different between the two groups. Though in the cleft palate group, Anatomic VPI and nasalance scores, which are indices for velopharyngeal closure, excluding the vowel /i/ and sentences including oral consonants show generally no significance.

Failure to use effective methods of reduction, fixation and immobilization may lead to osteomyelitis with the exposed necrotic bone, as the overzealous use of transosseous wires & plates that devascularizes bone segments in the compound comminuted fractures of mandible. Once osteomyelitis secondary to fractures has become established, intermaxillary fixation should be instituted as early as possible. Fixation enhances patient comfort and hinders ingress of microorganisms and debris by movement of bone fragments. Teeth and foreign materials that are in the line of fracture should be removed and initial debridement performed at the earliest possible time. Grossly necrotic bone should be excised as early as possible ; no attempt should be made to create soft tissue flaps to achieve closure over exposed bone. The key to treatment of chronic osteomyelitis of the mandible is adequate and prolonged soft tissue drainage. If good soft tissue drainage is provided over a long period, sequestration of infected bone followed by regeneration or fibrous tissue replacement will occur so that appearance and function are not seriously altered. Localization and sequestration of infected mandible are far better performed by natural mechanism of homeostasis than by cutting across involved bone with a cosmetic or functional defect. As natural host defenses and conservative therapy begin to be effective, the process may become chronic, inflammation regresses, granulation tissue is formed, and new blood vessels cause lysis of bone, thus separating fragments of necrotic bone(sequestra) from viable bone. The sequestra may be isolated by a bed of granulation tissue, encased in a sheath of new bone(involucrum), and removed easily with pincettes. This is a case report of the long-term conservative drainage care in osteomyelitis associated with mandibular fractures.
