A Study on the Applicability of the Population-Based Health Care Model: Focusing on Social Cooperative-type Medical Clinics in a Local Area (인구집단 기반 건강관리모형의 적용가능성 검토: 한 지역의 의료복지사회적협동조합형 의료기관을 중심으로)

  • Purpose: This study was to examine whether a health care model that provides comprehensive medical services based on population groups to members of the medical cooperative is applicable as a policy alternative in terms of medical use and cost. Methods: Data were derived from National Health Insurance claim data in 2019. We compared the medical volume and expenses of patients who visited social cooperative-type medical clinics with other patients, control group who visited other clinics in a local area. Results: The average number of visit days was 25.3 days in social cooperative-type medical clinics, more than 24.2 days in the control group (p=.004). However, the average medical cost per visit was KRW 46 thousand in social cooperative-type medical clinics, which was significantly lower than KRW 51 thousand in the control group (p<.001), and the total medical cost was also KRW 16.1 billion in social cooperative-type medical clinics and KRW 16.9 billion in the control group. Conclusion: We identified that a population-based health care model might change patients' behaviors to health care services and decrease total medical cost. Further population based experiment is needed to develop alternative healthcare model.

Relationship Between Social Supports and Depression Symptoms Among the Elderlies at Care Facilities (일부 요양시설 노인들의 사회적 지지와 우울수준과의 관련성)

  • This study was undertaken to investigate the relationship between social support and depression of the elderly in care facilities. We enrolled 328 elderly people aged 65 years or older, admitted to 15 care facilities located in D metropolitan city. Data were collected through personal interviews conducted by interviewers who visited each care facility, from March 1 to May 31, 2018. Our results indicate a significantly higher distribution of depression in the group having lower social support; the score of depression was determined to be in a significant positive correlation with social support. Considering logistic regression, the adjusted odds ratio in the depressed group increased significantly by 2.7 times in the high group (Q3), 5.4 times in the middle group (Q2) and 10.1 times in the low group (Q1), which had the very high group of social support score. Multiple regression analysis revealed that the factors influencing depression include religion status, relationship with children, subjective health status, physical disabilities, smoking status, breakfast status, and social support, and the explanation power of the injected variables is 41.1%. Taken together, our findings indicate that the level of depression amongst the elderly in care facilities is significantly related to the social support available.

The Korean Social Workers' Burn-out Factors and Personal Traits in the Hospice and Palliative Care (호스피스.완화의료에 개입하는 사회복지사의 소진과 보호요인에 관한 연구)

  • Purpose: This study investigated the Korean social workers’ burn-out factors and personal traits in hospice and palliative care field, and also examined the effect and correlation between their professionality, social support and supervision. Methods: Data (N=46) from 46 social workers working at hospice and palliative care field were collected, and the data were analyzed for the inferential statistics using t-test, ANOVA, correlation and multiple regression with the SPSS 12.0 program. Results: General factors of the burn-out were age and work experience. The effect of the organization environment is greatly dependant on social support and supervision, and the burn-out were protected when workers got an emotional support from their family. For the workers with supervision, the less negative feeling, the better for the burn-out protection. Furthermore, the low burn-out was thought about when professional organization, self regulation, job vocation and autonomy were utilized. Regression analysis needed that the burn-out were protected well when individual autonomy among expertise was guaranteed. As for social support, vertical support was able to protect physical burn-out. Conclusion: The training program for social workers in hospices and palliative care field is essential to reduce and prevent the burn-out. Hospice should be more activated and a training program with up-to-date knowledge and information should be adopted.

The Effects of Young Children's Hours Spent at Kindergartens or Child Care Centers on their Language, Cognition, and Social Development: Focusing on Mediating Effects of Teacher-child Interactions (유아의 기관이용시간이 언어, 인지 및 사회성 발달에 미치는 영향: 교사-유아 상호작용의 조절효과를 중심으로)

  • The purpose of this study was to examine the effects of young children's stay hours on kindergartens or child care centers on their language, cognition, and social development with teacher-child interactions as a mediator. For this purpose, the data from the 2013 Panel Study on Korean Children were analyzed using the GLM and pattern analysis. The results of the GLM analysis were as follows. First, the main effects of the stay hours on child development were statistically significant Second, the main effects of the level of teacher-child interaction were also statistically significant on the results of child development. Third, the moderating effect of teacher-child interaction level between the stay hours and child development were also significant. The results showed that young children's stay hours effected their language, cognitive, and social development and teacher-child interactions mediated these effects.

The Effects of Self-care and Parental Monitoring on Peer Relations and Perceived Cognitive Self-Competence of School Age Children (학령기 아동의 자기보호와 부모감독에 따른 또래관계 및 인지적 자기능력 지각)

  • The objects of this study were to find out the effects of peer relations and perceived cognitive self-competence according to 1) grade, sex, and self-care, 2) parental social status and monitoring. 429 3rd- and 6th-graders (238 boys and 191 girls) were included as subjects. For measuring variables, RCP, Harter's Perceived Cognitive Self-Competence Rating Scale, and Parental Monitoring Questionnaire based on Crouter et al.(1990) were used. The data were analysed by MANOVA, t-test, and univariate ANOVA. The results were as follows.‘Social-Leadership’of peer relations was influenced by children's grade, grade $\times$ sex effects. 3-way interaction of children's variables and parental monitoring made significant differences in ‘Shy-Isolation’. Children's perceived cognitive self-competence was influenced by their grade and parent's social status. These results confirmed the significant roles of self-care and parental monitoring in children's development.

Moderating Effect of Family Resilience and Social Support on Relationship between Burden of Care for Families with Mentally Disabled and Quality of Life (정신장애인 가족의 보호부담과 삶의 질과의 관계에서 가족탄력성과 사회적 지지의 조절효과)

  • The purpose of this study is to examine the Influence of burden of care for families of the mentally disabled on the quality of life and the moderating effect of family resilience and social support. This study carried out a survey of 102 families of the mentally disabled registered in the mental health centers and social rehabilitation facilities in Seoul, Gyeonggi and Incheon areas. As a result, first, the burden of caring for families of the mentally disabled has a negative impact on the quality of life. Second, family resilience has a moderating effect on the relationship between the burden of care and the quality of life of families of the mentally disabled. The subscales of family resilience were all buffered. Finally, social support has proved to be moderating in the relationship between the burden of care and the quality of life. Based on the results of this study, it was suggested that the need for providing information for improving the resilience of the family is needed, and the institutional support to mitigate the economic burden of the mental disabled family's social support network is needed.

  • This research study was to examine the relationships of social supports and depression among long-term care center residents. Depression of long-term care center residents is the most common mental health problem confronting older adults. It is estimated that 13 percent of people aged 65 years and over have a major depressive disorder in Korean Society of Welfare for the Aged. Social support is a key ingredient in dealing with emotional distress and a critical in helping people to cope with all kinds of extreme circumstance. As the result of this research, Perceived social support was found to be related to depression. However, network size was not a predictor of depression, and high health status is accounted for a lower portion of the variance in depression., the poor health status was one of the most powerful predictors of depression, From results of this research study, for social workers who are employed institutional-based agencies.

Effect of Thought s of Suicide of Elderly that Care for a Chronic Invalid (노인의 자살생각에 영향을 미치는 생태체계요인 -중증만성질환자 돌봄 노인을 중심으로-)

  • In this study, I tried to find out the ecosystem factors which influence suicide thoughts of elderly that care for a chronic invalid. Ecosystem factors are consist of personal system, familial system and social system, tried to analysis ecosystem factors with characteristics of social population that influence suicide thoughts. For the study, I collected 274 samples from people of 55 year-old care a chronic invalid adults who reside of in Gyeonggi-do. I used factor analysis, reliability analysis, multi-regression, t-test, ANOVA with SPSS 18.0. The results are as follows. First, suicide thoughts had affected by personal characteristics and the duty of supporting selected independent variable. Second, familial system as familial characteristics and familial support had a strong influence on suicide thoughts by long-term care and deficient familial support. Third, the result of study, social support and participation of social activities apply as independent variable then every factors of social support are negative influence but participation of social activities are not influence.

A Study on Health Promotion Needs Assessment of the Rural Elderly in Korea (한국농촌노인의 건강증진관리요구에 관한 연구)

  • This study was purposed to find health promotion and care needs of the elderly in rural area of Korea. As the rural elderly are limited in accessibility to health care resources and could not immediately solve their health care needs when they need. health promotion and care services are expected to bring better and more practical solutions of their health care needs. Thus, the type of health care services to be developed in Korea rural area is discussed to have emphasis on health care service component in addition to health promoting components. Methods of this study was based on survey data analysis : total 322 persons aged older than 55 living at one 'Kun' in Korea administrative unit were interviewed by health workers working at the region and also get trained for this study data collection. The data collection interview was continued from February till May in 1996. The interview questions were modified with adjustment to Korea situation. with basis of the WHO's health promotion program components. The collected data were analyzed using SAS program for frequency, correlation, regressions. The major findings were as follows : (1) $74.8\%$ of the surveyed were sick at the survey time point. and $95.9\%$ known the diagnosis name of the disease. The most frequently complained diseases were Muscular-Skeletal diseases $(43.7\%)$. $34\%$ of those sick had never treated or discontinued therapeutic procedures. so that shown the necessity of systematic and usual health care services with health promotion program development for the elderly. (2) The percent of those who make social participation was $95.3\%$. and the activities were visiting neighbors $(70.4\%)$ and lack of qualified social activity programs. (3) $78.1\%$ of the surveyed had health counseling and education from professional health workers. Those ceased smoking and drinking were $59.6\%,\; 60.3\%$. respectively. Those had no application of therapeutic drugs or nutrion supplements was $40.7\%\;and\;94.1\%$ had regular meals. Those practiced exercises was low remarking $17.7\%$. (4) Positive health behaviors were better carried out by sick groups than by the healthier. except smoking. regular meals. and exercise. $17.5\%$ of sick group smoke more than one case of cigarettes. in contrast to $9.5\%$ of the healthier. (5) Mental health status was heathier among positive health behavior earners. Health counseling and education shown better score of mental health than those never counseled. (6) Positive health behavior practice frequency did not show significant differences when crossed by social activity participation status. (7) Health behaviors of the rural elderly people were carried out better when they had positive 'continuency in therapeutic procedure' 'health status'. 'familial relationship'. 'Health Status' of the rural olderly were explained by 'exercise'. 'drinking'. 'familial relationship'. 'activities of daily living'. Thus, health behaviors practice mutually interact with health status. In conclusion. the health promotion and care program component are recommended to include ation on the necessity of positive health promotion active social acitivities. pleasant life style, adaption into changes on the elderly, safety in residential area. community acitivity and resource utilization. etc .. in addition to the elderly's disability and sickness caring services.

Managerial Effectiveness of Integrated Delivery System in Japan (의료서비스 복합화의 경영효과 분석 : 일본의 사례)

  • This study purports to verify managerial effectiveness of the integrated delivery system(IDS) of Japanese health care institutions through comparing the managerial performance between hospital groups providing with both acute and nursing care and those with acute care only. Data on the managerial performance of 697 hospitals providing with nursing care together and 819 hospitals providing with acute care only were collected from Japanese Central Social Insurance Medical Councils 2001, 2003, 2005, and were analyzed using mean comparison test(t-test) between the two groups. The results revealed that there were significant differences between the two groups in such indicators as ratio of material cost, labor cost, depreciation rate, total margin, operating margin, average number of outpatient per day, average revenue of an inpatient per day, total amount of labor cost, gross revenue per employee, and labor productivity. However, we could not find out any consistent evidence which support the effect of integrated delivery system on the hospital managerial performance. Further discussion was made on the limitation of the study and future research agenda relevant to the topic.
