금연 자기효능감 증진 프로그램이 흡연 급성관상동맥증후군 환자에게 미치는 효과 (Effects of the Smoking Cessation Self-efficacy Improvement Program on Smoking Patients after Acute Coronary Syndrome)

  • Purpose: The purpose of this study was to examine the effects of the program to improve on smoking patients' after acute coronary syndrome (ACS) smoking cessation rate, smoking cessation related self-efficacy, carbone monoxide (CO), nicotine dependence, and pain sense. Methods: This study used a non-equivalent control group pretest-posttest design. The participants of this study were 60 ACS patients: experimental group (30), control group (30), who received percutaneous coronary intervention. The study lasted from Aug 16, 2016 to Jan 13, 2017. Smoking cessation rate, smoking cessation related self-efficacy, CO, nicotine dependence, and pain sense were measured using the structured questionnaires and CO monitor Results: An examination of the effects of this program revealed the experimental group to have a significant increase in smoking cessation rate, smoking cessation related self-efficacy than the control group, and a significant decrease in CO, nicotine dependence and pain sense than in the control group. Conclusion: The findings indicate that this smoking cessation self-efficacy improvement program is effective for hospitalized patients after ACS.

한국 청소년의 흡연경험 여부에 따른 구강건강행태, 정신건강에 관한 연구 (The study of oral health-related behaviors and mental health, according to smoking experience in korean adolescents)

  • This study surveyed 57,303 teenagers using the 15th (2019) online Juvenile Health Behavior survey data to understand the relationship between oral health behavior, mental health, and smoking. The results of analyzing the factors affecting smoking experience are as follows: The smoking rate was higher in men (p<0.001), in a lower academic ability (p<0.001), and in moderate economic status was moderate (p<0.001). The smoking rate was higher in 2 or less brushing frequency (p<0.001), in no brushing before going to bed (p<0.001), in no brushing aftger lunch (p<0.001), in no experience of oral health education (p<0.001), and when the subjests felt unhealty in oral health (p<0.001). The smoking rate was significantly higher in those who experienced a lot of stress (p<0.001) and feelings of depression (p<0.001). The odds ratio for smoking was 1.44 when not brushing teeth before bedtime, 0.76 in experience of oral health education, and 1.29 in unhealthy subjective oral health. The odds ratio for smoking was 0.91 in no stress and 1.85 in depression. Since oral behavior and mental health affect smoking, intervening with these factors is necessary to quit smoking.

Cigarette Smoking among Adolescents aged 13-15 in Viet Nam and Correlates of Current Cigarette Smoking: Results from GYTS 2014 Data

  • The aim of this paper is to report the rate of current and ever cigarette smoking and explore correlates of current cigarette smoking among adolescents aged 13-15 in Viet Nam. This analysis was derived from GYTS survey, which comprised of 3,430 adolescents aged 13-15, conducted in 2014 in 13 cities and provinces of Viet Nam. We calculated the weighted rates of current and ever cigarette smoking and reported patterns of smoking behavior. We also performed logistic regression to explore correlates of current cigarette smoking behavior. The weighted rate of ever cigarette smoking was 9.5% (95% confidence interval (CI): 8.5 %-10.5%), in which the weighted rate among males (15.4%; 95% CI: 13.6%-17.0%) was higher than that among females (4.2%; 95% CI: 3.3%-5.1%). The weighted rate of current cigarette smoking was relatively low at 2.5% (95%CI: 2.0%-3.0%) with higher weighted rate among males (4.9%; 95% CI: 3.8%-5.9%) compared to the corresponding figure among females (0.2%; 95% CI: 0.0 %-0.5%). Current cigarette smoking was significantly higher among males than females, in students aged 15 versus 13 years old, and in students who had several or all close friends smoking and students with daily observation of smoking at school. For greater smoking reduction outcomes, we recommend that tobacco interventions for adolescents should consider targeting more male students at older ages, establish stricter adherence to school-based banning of cigarette smoking, engage both smoking and nonsmoking adolescents and empower adolescents to resist peer smoking influence as well as changing their norms or beliefs towards smoking benefits.

Timely Interventions can Increase Smoking Cessation Rate in Men with Ischemic Stroke

  • Purpose: Smoking cessation is strongly recommended for every smoker after ischemic stroke, but many patients fail to quit smoking. An improved smoking cessation rate has been reported with intensive behavioral therapy during hospitalization and supportive contact after discharge. The aim of this study was to demonstrate the usefulness of the timely interventions for smoking cessation in men with acute ischemic stroke. Methods: Patients who participated in the timely interventions strategy (TI group) were compared with those who received conventional counseling (CC group). In the TI group, a certified nurse provided comprehensive education during admission and additional counseling after discharge. Outcome was measured by point smoking success rate and sustained smoking cessation rate for 12 months. Results: Participants, 157 men (86 of the TI group and 71 of the CC group), were enrolled. Mean age was $58.25{\pm}11.23$ years and mean initial National Institutes of Health Stroke Scale score was $4.68{\pm}5.46$. The TI group showed a higher point smoking success rate compared with the CC group (p=.003). Multiple logistic regression analysis showed that the TI group was 2.96-fold (95% CI, 1.43~6.13) more likely to sustain smoking cessation for 12 months than the CC group. Conclusion: Findings indicate that multiple interventions initiated during hospital stay and regular follow-up after discharge are more effective than conventional smoking cessation counseling in men with acute ischemic stroke.

Impact of Admission Diagnosis on the Smoking Cessation Rate: A Brief Report From a Multi-centre Inpatient Smoking Cessation Programme in Singapore

  • Objectives: Few studies have been published regarding the relevance of the admission diagnosis to the smoking cessation rate. We studied smoking cessation rates in relation to admission diagnoses in our inpatient smoking cessation programmes. Methods: This retrospective study included all patients recruited into our inpatient smoking cessation programmes at 2 institutions in Singapore between June 2008 and December 2016. Patients were given individualized intensive counselling and were followed up via phone interviews for up to 6-month to assess their smoking status. Multivariable logistic regression was used to analyse potential associations between admission diagnoses and 6-month abstinence. Results: A total of 7194 patients were included in this study. The mean age was 54.1 years, and 93.2% were male. In total, 1778 patients (24.7%) were abstinent at the 6-month follow-up call. Patients who quit smoking tended to be of Chinese ethnicity, have initiated smoking at a later age, be better educated, and have lower Fagerström Test of Nicotine Dependence scores. After adjusting for these factors, patients with a cardiovascular admission diagnosis had a significantly higher probability of quitting tobacco use than patients with a respiratory or other diagnosis. Conclusions: In patients acutely admitted to the hospital, a diagnosis of cardiovascular disease was associated with the highest quit rate. Smoking cessation interventions need to be incorporated into all cardiovascular disease treatment pathways to leverage the patient's motivation and to improve the quit rate. In addition, patients in groups with lower quit rates may benefit from more intensive programmes to increase the rate of successful cessation.

Acute Effects of Tobacco and Non-tobacco Cigarette Smoking on the Blood Pressure and heart Beat Rate

  • Smoking of tobacco cigarettes is associated with a rise in blood pressure together with an increase in heart beat rate. This study examined the acute effect of tobacco and non-tobacco cigarette smoking on the blood pressure and heart beat rate by randomized crossover study involved 39 volunteers. In the results, systolic blood pressure and heart beat rate changes after smoking were significantly different in non-tobacco cigarette smoking group from in tobacco cigarette smoking group.

보건소 금연클리닉 이용자의 금연성공 관련요인 (Factors relating to Success to Quit Smoking among Citizens Receiving Smoking-Cessation Clinics Services in a Public Health Center)

  • 이 연구는 일개광역시 보건소 금연클리닉 서비스 이용자의 금연관련 요인을 찾고자 수행되었다. 연구 대상자는 2007년 일개광역시 보건소 금연클리닉에 등록된 이용자 2,125명이었다. 빈도분석, 카이제곱검장, 다중로지스틱 회귀분석 등이 이용되었다. 6개월 금연 성공률은 39.8%였다. 남성, 65세 이상에서 금연률이 높았다. 고혈압, 주 2회 이상 음주자, 규칙적인 운동자, 10ppm 이상의 일산화탄소량 일수록 금연성공률이 유의하게 높았다. 다른 독립변수는 금연성공률과 통계학적으로 유의한 결과를 보이지 않았다. 로지스틱회귀분석 결과, 연령, 사용된 니코틴패치 수, 총 의뢰횟수 등이 금연성공과 유의한 기여요인으로 도출되었다. 특히 총 의뢰횟수가 가장 높은 오즈비를 보였다. 규칙적인 의뢰가 보건소 금연클리닉 프로그램에서 금연성공의 가장 효과적인 중재가 될 수 있음을 확인하였다.

관상동맥조영술을 받은 허혈성 심장질환자의 금연 행위에 영향을 미치는 요인 (Factors Influencing Smoking Cessation Behavior in Patients with Ischemic Heart Disease Following Coronary Angiography)

  • Purpose: The purpose of this study was to elucidate the relationship between readiness to quit smoking and smoking cessation, and identify factors associated with smoking cessation in smokers with ischemic heart disease post-coronary angiography (CAG). Methods: This descriptive study was conducted between December 1, 2020 and May 14, 2021 at a P hospital, Busan. A total of 164 subjects completed the questionnaire including general characteristics, coronary artery disease characteristics, readiness to quit smoking, and smoking cession behavior during hospitalization and 4 weeks after discharge. Results: The success rate of smoking cessation after coronary angiography in patients with ischemic heart disease was 49.4% (n = 81). In addition, 48.2% of smokers (n = 83) attempted smoking cessation after CAG, while 39.8% showed changes in smoking behavior. The readiness to quit smoking was significantly associated with smoking cessation (OR= 2.23, p< .005). Conclusion: Readiness to quit smoking was identified as an important factor associated with smoking cessation in patients with cardiovascular disease in this study. In order to increase the smoking cessation rate, it is necessary to strengthen the readiness to quit smoking, followed by a tailored program for smoking cessation in patients with ischemic heart disease.

일개 대학병원에서 치료중인 만성질환자의 금연, 절주 및 운동 실천 정도 (The status of quit-smoking, abstinence and exercise of patients with chronic disease in a hospital)

  • Background: Many chronic diseases are associated with the lifestyle such as smoking, alcohol drinking and exercise. Attention is increasingly paid on the effect of exercise for the management of chronic disease these days. Objective: The purpose of this study was to evaluate the status of the smoking habit, alcohol drinking and physical exercise of the patients with chronic diseases. Methods: Total 793 persons(normal:422, chronic disease patients:371) in a hospital were questioned regarding the habits of smoking and alcohol drinking, and a practice of physical exercise in 1999. The patients with chronic illness were divided into three groups(Group I included the patients of hypertension and diabetes mellitus. Group II includes respiratory disease and cancer. Group III included liver disease). The rate of exercise, the rate of smoking cessation and the rate of abstinence were assessed. In logistic regression analysis with each independent variables in each disease groups (I, II and III, odds ratio for the presence of disease was controlled for age and education. Results: The rate of exercise, the rate of smoking cessation and the rate of abstinence was 31.5%, 27.3% and 9.5% in the patients with chronic disease, respectively. In control group, those were 31.5%, 21% and 2.1%, respectively. In logistic regression analysis with exercise as independent variable, odds ratios(95% CI for age and education were significantly high, with smoking cessation, odds ratio for age was high and with abstinence, odds ratio for disease with high in all disease groups. Conclusion: It was suggested that an effort for proper changes of lifestyle related to disease such as smoking, alcohol drinking and exercise should be intervened in the patients with chronic diseases.

National Trends in Smoking Cessation Medication Prescriptions for Smokers With Chronic Obstructive Pulmonary Disease in the United States, 2007-2012

  • Objectives: Smoking cessation decreases morbidity and mortality due to chronic obstructive pulmonary disease (COPD). Pharmacotherapy for smoking cessation is highly effective. However, the optimal prescription rate of smoking cessation medications among smokers with COPD has not been systemically studied. The purpose of this study was to estimate the national prescription rates of smoking cessation medications among smokers with COPD and to examine any disparities therein. Methods: We conducted a retrospective study using National Ambulatory Medical Care Survey data from 2007 to 2012. We estimated the national prescription rate for any smoking cessation medication (varenicline, bupropion, and nicotine replacement therapy) each year. Multiple survey logistic regression was performed to characterize the effects of demographic variables and comorbidities on prescriptions. Results: The average prescription rate of any smoking cessation medication over 5 years was 3.64%. The prescription rate declined each year, except for a slight increase in 2012: 9.91% in 2007, 4.47% in 2008, 2.42% in 2009, 1.88% in 2010, 1.46% in 2011, and 3.67% in 2012. Hispanic race and depression were associated with higher prescription rates (odds ratio [OR], 5.15; 95% confidence interval [CI], 1.59 to 16.67 and OR, 2.64; 95% CI, 1.26 to 5.51, respectively). There were no significant differences according to insurance, location of the physician, or other comorbidities. The high OR among Hispanic population and those with depression was driven by the high prescription rate of bupropion. Conclusions: The prescription rate of smoking cessation medications among smokers with COPD remained low throughout the study period. Further studies are necessary to identify barriers and to develop strategies to overcome them.
