Treatment of obstructive sleep apnea in children

  • Obstructive sleep apnea (OSA) in children is a frequent disease for which optimal diagnostic methods are still being defined. Treatment of OSA in children should include providing space, improving craniofacial growth, resolving all symptoms, and preventing the development of the disease in the adult years. Adenotonsillectomy (T&A) has been the treatment of choice and thought to solve young patient's OSA problem, which is not the case for most adults. Recent reports showed success rates that vary from 27.2% to 82.9%. Children snoring regularly generally have a narrow maxilla compared to children who do not snore. The impairment of nasal breathing with increased nasal resistance has a well-documented negative impact on early childhood maxilla-mandibular development, making the upper airway smaller and might lead to adult OSA. Surgery in young children should be performed as early as possible to prevent the resulting morphologic changes and neurobehavioral, cardiovascular, endocrine, and metabolic complications. Close postoperative follow-up to monitor for residual disease is equally important. As the proportion of obese children has been increasing recently, parents should be informed about the weight gain after T&A. Multidisciplinary evaluation of the anatomic abnormalities in children with OSA leads to better overall treatment outcome.

구개 및 인두편도 적출술 적응증의 변화 양상 (Changing Trends of Indications in Adenotonsillectomy)

  • Background and Objectives : Adenotonsillar hypertrophy is the most common disorder in pediatric otolaryngology, which should be suspected as a possible cause of obstructive sleep apnea syndrome (OSAS). In the past, most of the adenotonsillectomy were performed because of recurrent infection, but now OSAS is the most common indication in many centers. Materials and Method : A review of 1,945 adenotonsillectomy performed between 1990 and 1998 is presented. We classified into two categories of indication for adenotonsillectomy and analyzed changing trends of indication for adenotonsillectomy. Results : Although recurrent infection remains the predominant indication for surgery, there has been a rise in OSAS as a significant indication from 13.67% in 1990 to 24.26% in 1998. Conclusion : An increase has occurred in the percentage of adenotonsillectomy performed for OSAS due to adenotonsillar hypertrophy. This trend promises to continue as physicians become increasingly aware of the prevalence and seriousness of adenotonsillar hypertrophy as a cause of sleep apnea.

수면무호흡 환자에서의 외측 인두성형술 후 발생한 피하기종 (A Rare Case of Subcutaneous Emphysema following Lateral Pharyngoplasty for Obstructive Sleep Apnea)

  • Lateral pharyngoplasty is a surgical option for treatment of obstructive sleep apnea (OSA). Here, we present a case involving a 40-year-old healthy man who underwent surgery, including lateral pharyngoplasty and robotic tongue base resection, for OSA. There were no intraoperative or immediate postoperative complications. However, on postoperative day 3, the patient presented with swelling in the temporal and buccal areas and was diagnosed with subcutaneous emphysema, later confirmed by computed tomography. The patient was carefully monitored under conservative care and discharged without complications. Although subcutaneous emphysema following tonsillectomy is a rare complication and usually resolves with conservative management, in certain cases, it might require surgical intervention. Lateral pharyngoplasty involves tonsillectomy and additional incision along the tonsillar fossa, which makes it susceptible to pharyngeal wall defects and, consequently, subcutaneous emphysema. Additionally, lateral pharyngoplasty and robotic tongue base resection cause pain and might thus contribute to the increase in intrapharyngeal pressure, which might aggravate subcutaneous emphysema. Lateral pharyngoplasty should be performed with meticulous dissection of the superior pharyngeal constrictor muscle. Healthcare providers should be aware of these complications and, upon suspicion of the same, place the patient under close observation to prevent life-threatening situations.

  • 목 적:폐쇄성 수면무호흡증은 흔한 질환으로 심혈관계 합병증이나 주간 졸림증 때문에 점점 더 큰 관심을 끌고 있다. 수면무호흡증 진단에 야간 수면다원기록법이 표준검사로 인정되고 있으나 비교적 고가의 검사비용과 검사시 각종 감지기 등을 부착하고 인위적 환경인 검사실에서 잠을 자야 하는 불편함이 있다. 따라서 수면다원기록법을 대치할 수 있는 약식 검사들이 개발되고 있으며 그 중 대표적인 것이 손목부착형 활동기록기이다. 이 연구에서는 활동기록기가 수면무호흡증 진단에 어떤 보조적인 역할을 할 수 있는지를 수면분절의 관점에서 측정해 분석하였다. 방 법:폐쇄성 수면무호흡증이 의심되어 서울대학교병원 수면다원검사실로 의뢰된 37명의 환자들에게 야간수면다원검사와 동시에 활동기록기를 착용시켜 자료를 수집하였다. 전체 연구대상자 37명 중 폐쇄성 수면무호흡증 외에 주기성 사지 운동 등 다른 수면질환이 함께 진단된 10명의 환자를 제외한 27명의 자료를 분석하였다. 수면다원검사로 얻은 호흡장애지수(respiratory disturbance index, RDI)와 활동기록기의 자료분석으로 얻은 수면분절지수(fragmentation index, FI)를 Pearson 상관관계분석을 이용하여 비교하였다. 결 과:야간수면다원검사로 얻어진 총 수면시간, 호흡장애지수, 활동기록기로 산출된 수면분절지수는 각각 $401.37{\pm}57.83$분, $21.67{\pm}20.41$회/시간, $21.87{\pm}12.96$회/시간 이었다. 수면분절지수는 호흡장애지수와 유의한 양의 상관관계를 보였다(r=0.553 p=0.003). 결 론:수면분절지수가 호흡장애지수와 유의한 양의 상관관계를 보였다. 손목부착형 활동기록기는 수면무호흡증의 진단에 보조적인 도구로 이용될 수 있으며, 수면무호흡증의 각종 치료 후 증상 변화에 대한 평가용으로는 더 적극적인 역할을 할 수 있을 것으로 기대한다.

견봉하 관절경 수술 후 진통 작용: 모르핀과 국소마취제 혼합제와 위약의 점액낭 내 지속 주입에 대한 전향적 이중 맹검 연구 (Analgesia After Subacromial Arthroscopy: Prospective Double-blind Study of Continuous Mixed Intra-bursal Infusion with Morphine and Bupivacaine and placebo)

  • The aim of this study is to determine if intra-bursal morphine and Bupivacaine mixed infusion provided useful analgesia after subacromial arthroscopic operation. At the end of the subacromial arthroscopy, continuous intra-bursal infusion catheter was inserted. In a double-blind randomized trial, Sixty patients were allocated to one of two groups: placebo group(n=30) received continuous saline infusion. Study group received mixed 5㎖ of 0.5% Bupivacaine, 2㎎ of morphine and 0.05㎖ of 1/1000 epinephrine as bolus and mixed solution of 40㎖ of 0.5% Bupivacaine and 8㎎ of morphine as maintenance dose with continuous infusion pump(0.5㎖ hourly). In the placebo group, two patients were discontinued due to leakage and catheter coming out each. Intensity of pain was evaluated preoperatively and postoperatively for 3 days by 10 graded visual analogue scale. Night pain, pain on motion, sleep disturbance, lying on painful shoulder and amounts of intramuscular pain killer were recorded. Analgesic effect for pain was cleared at 1st and 2nd postoperative day and there was less sleep disturbance for 3 days postoperatively in study group. There was no difference in pain on motion postoperatively. In study group, less pain killers were used in the first 48 hours postoperatively. The continuous intra-bursal infusion decreased perception of pain at resting stage and reduced supplemental analgesic requirement for 2 days postoperatively.

직접구 치료에 의해 호전된 척추수술후통증증후군 환자 5례 (Case Series of 5 Failed Back Surgery Syndrome Patients Who were Treated by Direct Moxibustion Therapy)

  • Objectives : To evaluate the effectiveness of direct moxibustion in patients with failed back surgery syndrome (FBSS). Methods : 5 patients (3 females and 2 males) with FBSS underwent direct moxibustion on ouch points (阿是穴) for 10 to 15 weeks (once a week). The effectiveness of the treatment was evaluated with NRS (numerical rating scale), ODI (Oswestry disability index), BDI (Beck depression inventory) score, SF-36 (short form 36 health survey) score and PSQI (Pittsburgh sleep quality index) before and after the treatment. Results : Within observation period, average value changed from 10.0 to 4.2 in NRS of pain, from 37.0 to 20.6 in ODI, from 20.6 to 12.0 in BDI score, 42.2 to 62.6 in SF-36 score, and from 8.6 to 5.2 in PSQI. Conclusions : Direct moxibustion on FBSS patients showed more or equivalent effectiveness when compared to conventional FBSS treatment modalities.

Cone-beam CT analysis of patients with obstructive sleep apnea compared to normal controls

  • Purpose: To evaluate the upper airway dimensions of obstructive sleep apnea (OSA) and control subjects using a cone-beam computed tomography (CBCT) unit commonly applied in clinical practice in order to assess airway dimensions in the same fashion as that routinely employed in a clinical setting. Materials and Methods: This was a retrospective analysis utilizing existing CBCT scans to evaluate the dimensions of the upper airway in OSA and control subjects. The CBCT data of sixteen OSA and sixteen control subjects were compared. The average area, average volume, total volume, and total length of the upper airway were computed. Width and anterior-posterior (AP) measurements were obtained on the smallest axial slice. Results: OSA subjects had a significantly smaller average airway area, average airway volume, total airway volume, and mean airway width. OSA subjects had a significantly larger airway length measurement. The mean A-P distance was not significantly different between groups. Conclusion: OSA subjects have a smaller upper airway compared to controls with the exception of airway length. The lack of a significant difference in the mean A-P distance may indicate that patient position during imaging (upright vs. supine) can affect this measurement. Comparison of this study with a future prospective study design will allow for validation of these results.

The Beneficial and Adverse Effects of Raloxifene in Menopausal Women: A Mini Review

  • Objectives: The present mini review aimed to summarize the existing knowledge regarding the beneficial and adverse effects of raloxifene in menopausal women. Methods: This study is a review of relevant publications about the effects of raloxifene on sleep disorder, depression, venous thromboembolism, the plasma concentration of lipoprotein, breast cancer, and cognitive function among menopausal women. Results: Raloxifene showed no significant effect on depression and sleep disorder. Verbal memory improved with administration of 60 mg/day of raloxifene while a mild cognitive impairment risk reduction by 33% was observed with administration of 120 mg/day of raloxifene. Raloxifene was associated with a 50% decrease in the need for prolapse surgery. The result of a meta-analysis showed a significant decline in the plasma concentration of lipoprotein in the raloxifene group compared to placebo (standardized mean difference, -0.43; 10 trials). A network meta-analysis showed that raloxifene significantly decreased the risk of breast cancer (relative risk, 0.572; 95% confidence interval, 0.327-0.881; P = 0.01). In terms of adverse effects of raloxifene, the odds ratio (OR) was observed to be 1.54 (P = 0.006), indicating 54% increase in the risk of deep vein thrombosis (DVT) while the OR for pulmonary embolism (PE) was 1.05, suggesting a 91% increase in the risk of PE alone (P = 0.03). Conclusions: Raloxifene had no significant effect on depression and sleep disorder but decreased the concentration of lipoprotein. Raloxifene administration was associated with an increased risk of DVT and PE and a decreased risk of breast cancer and pelvic organ prolapse in postmenopausal women.

Retrospective study of changes in pharyngeal airway space and position of hyoid bone after mandibular setback surgery by cephalometric analysis

  • Background: The posterior movement of mandible was known as the main cause of the changes in the pharyngeal airway space (PAS) and the postoperative obstructive sleep apnea (OSA). The purpose of this study was to know the changes of PAS and position of hyoid bone. Methods: Lateral cephalographies of 13 patients who had undergone sagittal split ramus osteotomy (SSRO) setback surgery were taken preoperatively (T1), postoperatively within 2 months (T2), and follow-up after 6 months or more (T3). On the basis of F-H plane, diameters of nasopharynx, oropharynx, and hypopharynx were measured. The movements of the soft palate, tongue, and hyoid bone were also measured. Results: The amount of mandible setback was $7.5{\pm}3.8mm$. In the measurements of PAS, there was a statistically significant decrease of $2.8{\pm}2.5mm$ in nasopharynx (P < 0.01), and $1.7{\pm}2.4mm$ in oropharynx (P < 0.01) were observed after surgery. The hypopharynx decreased $1.0{\pm}2.1mm$ after surgery and continuously decreased $1.0{\pm}2.8mm$ at follow-up. The changes in hyoid bone position showed the posterior movement only after surgery and posteroinferior movement at follow-up. Conclusions: The PAS such as nasopharynx, oropharynx, and hypopharynx showed relatively high correlation with the amount of mandibular setback. The change of resistance in upper airway may be important for the prevention of OSA after mandibular setback surgery.

정형외과 수술 노인의 수술 전 영양상태 평가 및 관련 요인 (Preoperative Nutritional Status in Elderly Orthopedic Surgery Patients: Evaluation and Related Factors)

  • Purpose: The purpose of this study was to evaluate preoperative nutritional status in elderly patients with orthopedic surgery and identify related factors for malnutrition risk. Methods: This study enrolled 337 patient's medical record who underwent orthopedic surgery in hospital between January and December 2015. Data was collected retrospectively. Nutritional status was evaluated by using the Nutritional Risk Screening 2002. Multivariable logistic regression analysis was used to identify independent related factors for malnutrition risk. Results: Malnutrition risk developed in 58 patients (17.2%). Logistic regression analysis identified low physical activity, visual impairment, depression, sleep disorder, low serum calcium level, and low serum albumin level as related factors. Conclusion: Orthopedic surgery in elderly patients was associated with high risk of preoperative malnutrition. The results of this study suggest that evaluating the nutritional status and related factors should be done with preoperative status of elderly patients. At the same time, interventions for nutritional care should be adjusted to meet the nutritional needs of individuals and decrease the risk of malnutrition.
