The use of alcohol is associated with the development and worsening of sleep disorder. Alcohol is generally known to have a sedative effect, but it has an arousal or sedative effect depending on the timing and drinking dose and directly affects REM sleep physiology. Alcohol acts on the central nervous system (CNS) to interfere with the sleep-wake cycle and to affect sleep-related hormone secretion. In addition, the ingestion of alcohol pre-sleep is associated with deterioration and development of sleep related breathing disorders (SBD). The increase in resistance of the upper respiratory tract and the decrease in sensitivity of the CNS respiratory center and the respiratory muscles are major mechanisms of alcohol-induced SBD, and result in snoring or apnea in healthy men or aggravating apnea in patients with OSA. Sleep-related restless leg syndrome and circadian rhythm disorders are common in alcohol use disorder patients. This review provides an assessment of scientific studies that investigated on the impact of alcohol ingestion on nocturnal sleep physiology and sleep disorders.
Sleep disorder in chronic obstructive pulmonary disease (COPD) is common and typically is associated with oxygen desaturation. The mechanisms of desaturation include hypoventilation and ventilation to perfusion mismatch. Despite the importance of sleep in patients with COPD, this topic is under-assessed in clinical practice. Impaired sleep quality is associated with more severe COPD and may contribute to worse clinical outcomes. Recent data have indicated that specific respiratory management of patients with COPD and sleep disordered breathing improves clinical outcomes. Clinicians managing patients with COPD should pay attention to and actively manage symptoms of comorbid sleep disorders. Management of sleep-related problems in COPD should particularly focus on minimizing sleep disturbance.
구호흡을 야기하는 가장 흔한 원인은 상기도의 폐쇄이다. 구호흡은 수면호흡장애의 병인론적 원인이 될 수 있으며, 어린이에서 수면호흡장애는 성장장애와 행동학적 문제를 일으킬 수 있다. 이 연구의 목적은 구호흡 어린이에서 수면호흡장애와 상기도와의 관계를 조사하고자 함에 있다. 초진 설문지에서 구호흡이 있다고 응답한 7 - 9세의 20명 남자 어린이를 대상으로 하였다. 편도평가, 구호흡 및 수면호흡장애와 관련된 설문지, 측모두부 방사선사진, 그리고 휴대용 간이수면검사를 시행하였다. 수면호흡장애에 대한 평가로 무호흡-저호흡 지수(apnea-hypopnea index, AHI), 산소불포화 지수(oxygen desaturation index, ODI)와 측모두부 방사선 사진을 통해 측정한 상기도 폭경과의 관계를 평가하였다. 상기도 부위 중 후구개 거리와 후설 거리는 무호흡-저호흡 지수와 산소불포화 지수가 커질수록 좁아지는 경향을 보였다(p = 0.002, p = 0.001). 또한, 편도의 크기와는 유의한 상관관계는 없었지만, 아데노이드 비대율의 경우 무호흡-저호흡 지수가 비정상군이 정상군에 비해 아데노이드가 비대하였다(p = 0.008). 이 연구결과를 통해 구호흡이 상기도에 영향을 미칠 수 있고 이는 수면호흡장애까지 연관될 수 있음을 확인할 수 있었다.
Stroke is a leading cause of death in most developed countries and some developing countries including South Korea. It is well known that stroke has is related in some way with several sleep disorders. At first, the onset time of stroke varies according to circadian rhythm. Early morning is the most prevalent time and late evening the least. The changes of blood pressure, catecholamine level, plasminogen activity and aggregation of platelet during sleep have been suggested as possible mechanisms. Sleep apnea (SA), a representative disorder in the field of sleep medicine, is found in more than 70% of acute stroke patients compared to 2-5% of the general population. Various sleep related breathing disorders occur after stroke and snoring is a distinct risk factor for stroke. So the relationship between stroke and SA is obvious, but the cause and effect are still not clearly known. Also, stroke may cause many sleep related problems such as insomnia, hypersomnia, parasomnia and changes in sleep architecture. Patients, family members and even medical personnel often ignore stroke-related sleep problems, being concerned only about the stroke itself. The clinical impacts of sleep problems in stroke patients may be significant not only in terms of quality of life but also as a risk factor or prognostic factor for stroke. More attention should be paid to the sleep problems of stroke patients.
Alcohol has extensive effects on sleep and daytime sleepiness. Alcohol has a sleep inducing effect and the effect of increased non-REM sleep and suppressed REM sleep during the first half portion of night sleep, but alcohol induces the effect of decreased non-REM sleep and increased light sleep and frequent awakenings and REM rebound during the second half portion of night sleep. Alcohol provokes chronobiological change such as the changes of amplitude or the phase shifts of hormones or core body temperature. The sleep disruption resulting from alcohol drinking may lead to daytime fatigue and sleepiness. The elderly are at particular in the increased risk of alcohol-related sleep disorders because they achieve higher levels of alcohol in the blood and brain than do younger adults after consuming an equivalent dose. Bedtime alcohol consumption among older adults may lead to unsteadiness if walking is attempted during the night, with increased risk of falls and injuries. Continued alcohol use for sleep induction often induces aggravation of insomnia, alcoholism or sleep related breathing disorders such as obstructive sleep apnea. Alcohol should not be used as substitution of sleep pill because of the dependence and tolerance for sleep inducing effect, and the sleep disruption produced by alcohol withdrawal.
Sleep-disordered breathing (SDB) is associated with increased cardiovascular and cerebrovascular morbidity. Epidemiological and clinic-based studies have shown that SDB is related to impaired glucose tolerance and increased insulin resistance, independent of obesity. Despite of a consistent association between SDB and impaired glucose-insulin metabolism, the mechanism underlying this relationship has not been fully elucidated. It is recognized that hypoxemia and hypercapnia that occur in SDB provoke sympathetic nervous activity and catecholamine, epinephrine and norepinephrine, and cortisol are released. Sympathetic hyperactivity and increased catecholamines can impair glucose homeostasis by increasing glycogenolysis and gluconeogenesis, which can result in increased circulating insulin levels and increased risk of insulin resistance. A prospective study is needed to investigate the causal relationship between SDB and impaired glucose-insulin metabolism in a healthy population without diabetes, hypertension and obesity as etiologic risk factors.
This study covers pattern differentiation based on Korean medical references, research trend and modern clinical applications about Sleep-Wake disorders of Diagnostic and Statistical Manual of Mental Disorders(DSM-V) published by American Psychiatric Association. Insomnia disorder is mostly caused by yin deficiency of liver-kidney or liver qi depression and main patterns are heart-kidney non-interaction, deficiency-excess complex pattern containing phlegm-heat due to qi stagnation and blood stasis. Hypersomnolence disorder is more due to yang deficiency rather than yin deficiency and it's major pattern is spleen-kidney yang deficiency. Cataplexy is main feature in narcolepsy and corresponds to depressive psychosis or fainting in terms of Korean Medicine and narcolepsy is assumed to be relevant to liver wind. Breathing-related sleep disorders are related with phlegm-fluid retention brought on spleen deficiency with dampness encumbrance. Pattern of circadian rhythm sleep-wake disorders is combined with yin deficiency of liver-kidney or liver qi depression of insomnia disorder and spleen-kidney yang deficiency or dampness-phlegm of hypersomnolence disorder. Yin deficiency with effulgent fire brought on drugs or alcohol is one of main patterns of substance/medication-induced sleep disorder and combined patterns with yin deficiency of liver-kidney and blood stasis or dampness-phlegm-heat are mostly applied clinically. This study drew major and frequently applied patterns of sleep-wake disorders based on Koran medical literature and modern clinical applications. And that can be the groundwork for the task ahead like clinical practice guideline of sleep-wake disorders containing pattern differentiation, diagnosis and prescriptions.
It has been controversial whether upper airway resistance syndrome (UARS) is a distinct syndrome or not since it was reported in 1993. The International Classification of Sleep Disorders classified UARS under obstructive sleep apnea syndrome (OSAS) in 2005. UARS can be diagnosed when the apnea-hypopnea index (AHI) is fewer than 5 events per hour, the simultaneously calculated respiratory disturbance index (RDI) is more than 5 events per hour due to abnormal non-apneic non-hypopneic respiratory events accompanying respiratory effort related arousals (RERAs), and oxygen saturation is greater than 92% at termination of an abnormal breathing event. Although esophageal pressure measurement remains the gold standard for detecting subtle breathing abnormality other than hypopnea and apnea, nasal pressure transducer has been most commonly used. RERAs include phase A2 of cyclical alternating patterns (CAPs) associated with EEG changes. Symptoms of OSAS can overlap with UARS, but chronic insomnia tends to be more common in UARS than in OSAS and clinical symptoms similar with functional somatic syndrome are also more common in UARS. In this journal, diagnostic and clinical differences between UARS and OSAS are reviewed.
연구배경 : 상기도 저항증후군 (UARS)으로 진단 받은 환자 및 이들의 수면다원검사 소견을 정상인 및 수면 무호흡증(OSA) 환자의 그것과 비교 연구함으로서 UARS의 이해에 도움을 얻고자하였다. 방법 : 임상적인 면담, 신체검사 및 식도내 음압 (Pes)의 측정을 포함하는 수면다원검사를 통하여 UARS로 진단받은 20 명의 환자와 OSA로 진단 받은 30명의 환자를 대상으로 하였고, Williams 등 (10)의 자료를 정상비교치로 이용하였다. 결과 : UARS는 OSA 보다 젊고 비만도가 낮은 환자특성을 보이고 수면다원검사시 OSA의 진단기준이 되는 RDI와 $SaO_2$가 정상에 가까운 특정을 보임을 알 수 있다. 기타 수면지표들은 정상범위에 속하거나 이상소견이 있더라도 OSA 환자의 그것보다는 경미하여 UARS가 OSA보다는 가벼운 상태의 장애임을 시사한다. 그러나 UARS 환자들의 ESS의 점수는 병적인 수준으로 낮 동안의 졸림기로 일상생활에 많은 장애를 받고 있음을 시사한다. 결론 : 본 연구결과는 UARS 환자의 특성 및 이들의 특정 적인 수면다원검사 소견을 보여줬다고 생각하며 지금까지 발표되고있는 UARS에 대한연구결과들을 지지한다. 즉 UARS는 임상적, 실험실적으로 OSA와는 다른특징이 있는 수면관련 호흡장애의 아형이다. 따라서 임상적으로 수면관련 호흡장애가 의심되는 환자의 나이가 젊고 BMI가 정상이고, 일반수면다원검사에서 RDI와 $SaO_2$ 등이 수면관련 호흡장애 진단기준에 미흡하더라도 수면중 코를 고는 등의 상기도 저항(upper airway resistance)을 시사하는 증상이 있고, 뇌파상 이유없이 자주 반복되는 전기 생리적인 각성이 있다면 UARS를 의심하여야한다. UARS 환자들이 호소하는 낮 동안의 졸림기는 OSA 환자들의 그것과 비슷하여 일상생활에 많은 고통을 주고 반복적인 흉곽내 압력의 지나친 상승은 심혈관계에 해로운 영향을 미치는 바 UARS에 대한관심이 필요하다.
연구배경: 주기성 사지운동장애(PLMD)는 여러 수면장애와 함께 공존하는 경우가 많으며, 특히 수면관련 호흡장애(SRBD)와의 관계는 밀접하다고 알려져 있다. PLMD는 반복적이고 상동증적인 사지의 움직임에 의하여 전기생리적인 각성을 일으키기 때문에 불면증과 주간 졸립증을 초래하므로 이에 대한 관심이 필요하다. 본 연구는 SRBD 환자에서 PLMD의 빈도를 조사하고, PLMD로 진단된 환자군의 특성을 조사함으로써, SRBD 환자에서 PLMD의 동반 이환에 대한 이해를 높이는데 있다. 대상 및 방법: 진단적인 수면다원 검사로 수면관련 호흡장애의 진단이 내려진 106명을 대상으로 이들의 수면기록을 후향적으로 조사하였다. 수면다원 검사의 기록과 판독은 표준적인 방법을 사용하였다. OSA 의 진단은 RDI가 10 이상인 경우 내렸고, UARS는 낮 동안에 심한 졸림증을 호소하고 수면다원 검사상 RDI가 5 이하이나 호흡노력의 증가에 이어서 짧은 각성이 반복되는 경우 내렸다. 수면중 주기성 사지운동장애의 진단은 주기성 사지운동지수가 5 이상일 때 내렸다. 자료처리는 SPSS/$PC^+$를 이용하였다. 곁 과: 평균연령은 49.5세였고, 성별은 남자 76명, 여자 30명이었다. 진단은 폐쇄성 무호흡증 78명, 상기도저항증후군 28명이었다. 전체 연구대상의 평균 수면효율은 76.3%, 평균 호흡장애 지수는 22.3/hr, 평균 혈중 산소포화도는 84.9%로 중등도의 수면중 호흡장애가 있음을 알 수 있다. 평균 주지성 사지운동 장애지수는 13.1/hr였으며 70% 이상의 환자들이 32/hr 이하에 분포하였고 개인차이가 많았다. 주기성 사지운동 장애지수가 5 이상으로 수면중 주기성 사지운동장애를 동반한다고 판단된 군이 47명으로 전체의 44.3%를 차지하였다. 수면중 주기성 사지운동장애를 동반하는 군과 동반하지 않는 군의 비교에서, 두 군간에 나이, 무호흡 지수, 성별, 1 단계 수면의 백분율에서 유의한 차이가 있었다. 결 론: 본 연구의 결과는 이전의 타 연구들보다 수면관련 호흡장애에서의 주기성 사지운동장애의 동반이환율이 더 높다. PLMD를 동반하는 군의 나이가 유의하게 더 많고, 여성에서의 동반율이 더 높고, 무호흡 지수(apnea index)가 유의하게 더 낮다. 수면중 주기성 사지운동장애는 수면관련 호흡장애와 함께 불면증과 주간 졸림증을 초래하고, 본 연구에서 뒷받침 하듯이 수면중 호흡장애 환자에서 흔히 함께 동반되기 때문에, 수면관련 호흡장애 환자의 진단 및 가료시 이에 대한 배려가 필요하다.
