담정격(膽正格) 랄침(剌鍼)이 밤번 근무 후 간호사의 자율신경에 미치는 영향 (Effects of Sa-am Acupuncture (Damjeonggyeok) on Autonomic Nervous System in Night Nurses)

  • Objectives : The aim of the present study was to assess the effects of Sa-am acupuncture (Damjeonggyeok) on autonomic nervous system in night nurses by heart rate variability. Methods : Five night nurses were divided into two groups. We checked their HRV once after sleeping for night and 5 times after working for night, 4 times after operation. We operate Sa-am acupuncture (Damjeonggyeok) and sham acupuncture twice by turns for group A and in reverse order for group B. We checked responses of HRV change after operations. Statistical analysis was performed by using the SPSS Win. Ver. 13.0 for analysis about correlation between data, and Baysian analysis using WinBUGS(Ver. 1.4) for comparison between Sa-am acupuncture (Damjeonggyeok) and sham acupuncture. Results : The median overall decrease for difference in responses of low-frequency power in normalized units (LF (NU)) was -16.56 (-28.88, -6.446), the median overall increase for difference in responses to high-frequency power in normalized units (HF (NU)) was 16.25 (8.104, 28.6) and for the median overall decrease for difference in responses to the ratio of low- to high-frequency power (LF/HF) was -1.846 (-3.922, -0.069), 95% credibility intervals being shown in brackets. VAS of stress was correlated with LF (NU) and HF (NU), LF/HF (p<0.0l). Conclusion : Sa-am acupuncture (Damjeonggyeok) might have sympatholytic and parasympathomimetic effects.

비행안전 프로그램으로서의 생체 활성도 활용방안 연구 (A Study on the Practical Use of Human Alertness for Flight Safety Program)

  • Aircraft and the three-dimensional environment in which they operate are not user-friendly for human beings. As a result, maintaining the proficiencies necessary to safely and efficiently fly an airplane are difficult, and costly. The physiological and emotional status of the human element remains crucial in maintaining safe performance by all crew members. In the study of Hagiwara et al.(1993). they called the physiological and emotional status of the human element into the human alertness or physiological activity and stress, fatigue, circadian rhythm, alcohol. smoking, and self-medication are known the major factors that deteriorate the human alertness. Accordingly. this paper deals with the quantitative and objective performance test based on tracking error and reaction time by means of the new computer test program into which the perception-motion system of human beings is applied. Throughout this experiment using performance test, the results suggest that performance capability in state of sleep deprivation 2 hours and alcoholic 0.05~0.06% in blood were more impaired than one in a normal state, and they further showed statistically significant differences between them, which were influenced by impairment factors of body regulation and pilot's grade. We also obtained the prediction value and the 95% confidence interval of tracking error and reaction time at the normal state for the purpose of distinguishing performance capability between the normal state and the abnormal state. And it is expected that the evaluation of human alertness using performance test will be applied to the quantitative assessment of an each pilot's realistic consciousness/attention, and will lead a flight commander to the accurate decision of mission approval prior to a flight.

Midazolan과 Baclofen 투여에 의한 난치성 딸꾹질 환자의 치료 경험 -증례 보고- (Treatment of Intractable Hiccup with Midazolam and Baclofen -A case report-)

  • Hiccup is defined as an abrupt involuntary contraction of the diaphragm and intercostal muscles with sudden closure of the glottis 35 msec after onset. The term "intractable" is given to those hiccups with a duration ranging from 24 hours to more than 25 years. Short hiccup bouts are mostly associated with gastric distention or alcohol intake, resolved spontaneously or with simple remedies. In contrast, intractable hiccup is a rare but disabling condition which can induce depression, weight loss and sleep deprivation. Although the pathophysiologic mechanisms of hiccup are still poorly understood, wide variety of pathological conditions such as: brain tumor, abdominal tumor, myocardial infarction, renal failure, abdominal surgery etc., can cause intractable hiccup. A 58-year-old male who had suffered from hepatocellular cancer was consulted from medical department due to intractable hiccup. Initial treatment modalities with administrations of metoclopramide and chlorpromazine and nerve block including phrenic nerve block, cervical epidural block and glossopharyngeal nerve block were not effective. Administration of midazolam and baclofen however achieved desirable effect.

제 6기 국민건강영양조사 자료(2015)를 활용한 대사증후군 유병과 근로시간에 관한 연구 (A Study on the Relationship Between Metabolic Syndrome and the Number of Work Hours Based on Data from the 6th Korea National Health and Nutrition Examination Survey (2015))

  • There are an increasing number of reports that job related factors are associated with hypertension, sleep deprivation, obesity and diabetes. This study analyzed the relationship of working type, working hours and the prevalence of metabolic syndrome. The study's subjects were 2,777 employees (1,446 males and 1,331 females) over 19 years old and their data was from the Korea National Health and Nutrition Survey (2015). The type of work was divided into day work and shift work. The working hours were divided into less than 40 hours, 40 to 49 hours, 50 to 59 hours and 60 hours or more per week. As a result of this study, the prevalence of metabolic syndrome was 26.4% of the males and 19.1% of the females. Generally there was no relationship between metabolic syndrome and type of work (day work and shift work). As a result of analysis of working hours and metabolic syndrome, the workers who worked 40-49 hr were associated with a decreased prevalence of metabolic syndrome as compared to that of workers who worked less than 40 hr (odds ratio: 0.47, 95% confidence interval: 0.24-0.95, p<0.05). Further, positive relationships were found between the number of work hours and the prevalence metabolic syndrome for specific females who worked over 40 hr/wk. It can be concluded from the results that it is important to help workers avoid metabolic syndrome through setting appropriate work hours.

Association between presenteeism and mental health among logistic center workers

  • Background: Workers in logistics centers are always pressed for time to collect and pack products. They also participate in high-intensity manual labor in which various musculoskeletal hazards exist. In the case of logistic center labor, it is estimated that there is a high risk of presenteeism due to the above characteristics which can cause deterioration of workers' mental health. However, there is insufficient research on this topic. Methods: Workers in a logistic center were surveyed using an Internet questionnaire. The survey items included demographic characteristics, labor intensity and work-related factors, and mental health aspects such as depression and anxiety. The survey was conducted for about a month from July 26, 2021 and a total of 353 people were analyzed. Through the χ2 test and t-test, the characteristics of workers who experienced presenteeism were examined and the prevalence ratios (PRs) of depression and anxiety experiences were calculated by multivariable Poisson regression. Afterwards, stratification analysis considering gender, the type of contract, and labor intensity was implemented. Results: In the group that experienced presenteeism, the number of working days per week was higher and fixed-term workers, high labor intensity, and sleep deprivation were more common. In the multi-Poisson regression analysis conducted by adjusting the demographic characteristics, working hours, and work-related factors, the PRs of depression and anxiety were 1.98 (95% confidence interval: 1.24-3.18) and 1.81 (1.22-2.68), respectively. In particular, the p-value for interactions was significant when stratified with the type of contract. Conclusions: As a result of the study, presenteeism and mental health were associated in logistic center workers. To prevent mental health issues of logistic center workers, management of presenteeism is necessary and a prospective study is needed.

충돌시나리오 기반의 항해사 인지능력 평가시스템 개발 (Development of Collision Scenario-Based Evaluation System for the Cognitive Performance of Marine Officers)

  • 항해사의 업무수행도 감소는 해양사고 발생의 주요 원인 중의 하나로 자주 언급되고 있다. 인터뷰와 항해 기록분석을 통한 선원의 작업시간 분석에 대한 연구는 일부를 수행되었으나, 작업부하, 작업교대, 스트레스, 수면 등이 선원의 인지능력에 미치는 영향을 분석한 사례 매우 한정적이다. 항공, 철도와 같은 다른 운송 산업에서는 승무원의 근무 패턴을 관리하기 위한 피로관리 도구를 개발하고 있고 있는데, 이러한 도구들은 선원, 항해사 및 육상관리자의 효율적인 근무일정을 계획하는데 도움을 줄 수 있다. 따라서 본 연구에서는 항해사의 인지능력을 평가하기 위해, 해상 충돌상황에서의 종합적 대응능력을 평가할 수 있는 소프트웨어 프로그램을 개발하였다. 항해 전문가를 대상으로 한 실험을 통하여, 평가기준을 개발하였으며, 실습 항해사를 대상으로 하여 시스템에 대한 검증이 이루어졌다. 본 시스템은 선박운항 시 충돌사고를 회피하기 위한 항해사의 인지능력을 평가할 수 있으므로, 당직 전 혹은 승선 전에 짧은 시간 동안의 평가를 통하여 항해사의 전반적 인지능력을 평가하고, 이 결과를 항해사에게 경고메시지 형태로 전달할 수 있다.

등주식 조명과 낮은 조명의 성능 비교에 관한 연구 (A Comparative Study on Visibility Performance Between Pole Type Lightings and Low-Level Lightings)

  • 야간 도로를 주행하는 운전자의 안전을 위한 대표적인 대책으로 등주식 도로조명의 설치가 선호되고 있다. 그러나 기존 등주식 형태 도로조명의 경우 노면으로부터 일정 높이에 설치되는 특성으로 인해 광원이 운전자에게 직접적으로 강한 눈부심을 주어 운전자의 시인성을 저해하고, 도로 외의 지역에도 빛이 누설되어 에너지 효율이 떨어질 뿐 아니라 주거지역이나 동식물에 장해를 주는 빛공해가 발생하게 된다. 이러한 문제점을 해소한 낮은 위치에 설치되어 조명이 도로만을 비추는 형태의 낮은 조명이 해결책으로 제안되고 있으나, 도입 초기인 낮은 조명이 도로조명등급을 만족하여 도로조명으로서의 역할을 할 수 있는지에 대한 연구는 부족한 실정이다. 따라서 본 연구는 도로조명으로서 요구되는 최소한의 시인성능 기준을 만족함과 동시에 빛공해를 근본적으로 차단할 수 있는 구조의 낮은 조명을 개발하고 기존 등주식 조명과 시인성능 비교를 수행한 결과를 제시하였다. 비교결과, 기존 등주식 조명에 비해 평균노면휘도, 균제도, 임계치 증분 평가에서 우수한 결과를 나타냈다.
