Health Impairment and Concentration of Hippuric Acid in Urine of Workers Exposed to Toluene (톨루엔 폭로근로자의 요중 마뇨산 농도 및 건강장해)

  • The relationships between subjective symptoms of toluene exposed workers and concentration of their urinary excretion of hippuric acid were investigated. The exposed groups of 146 workers exposed to toluene and the control groups of 47 workers have never been exposed to toluene in Ulsan area were selected and studied. Hippuric acid was measured by HPLC, and counts of blood cells and liver function test were also performed. The mean value of urinary hippuric acid concentration of the control group was 0.322($\pm$0.267) g/L, while that of the exposed group was 1.260($\pm$0.395) g/L. As the concentration of hippuric acid had statistical proximity in $0.1\%$ level, WBC, GOT, and GPT didn't have any proximity(P>0.05). The exposed group showed lower level of leucocyte counts 6522.40(1710.3) than the control group 6891.50(1483.7). The exposed group showed higher level of GOT(25.75), GPT(27.09) than GOT(23.75), GPT(25.21) of the control group. Dried skin was the highest complained symptom of toluene exposed workers, the second strained eye, the third poor auditory function, and the fourth was headache.

Developing Telephone Guidelines for Symptom Management of Cancer for Outpatients with Cancer after Chemotherapy (항암화학요법 후 퇴원한 암 환자의 증상관리를 위한 전화상담 가이드라인 개발)

  • Purpose: This study was done to provide oncology nurses systemic guidelines designed to perform telephone counseling in outpatient settings treating patients with chemotherapy. Methods: Symptoms which are frequently questioned were identified through case reports recorded by 4 clinical nurse specialists from January 4 to December 31. 2010. Fifteen clinical experts and two hemato-oncologists reviewed the developed draft. Their opinions were synthesized to arrive at consensus on the ideal guidelines. This process confirmed content validity. The two hemato-oncologists also verified the face validity. Results: The six symptoms which were most frequently asked about were identified through 4,644 case reports and concerned pain, fever, skin alteration, nausea/vomiting, stomatitis and diarrhea. Evidence-based telephone triage guidelines for the 6 major symptoms in patients receiving chemotherapy were developed. Conclusion: These guidelines will help oncology nurses from novice to expert incorporate evidence-based telenursing into their practice.

The Effects of Neurofeedback Training on Physical, Psychoemotional Stress Response and Self-Regulation for Late Adolescence: A Non-Randomized Trial (뉴로피드백 훈련이 후기청소년의 신체적, 정서심리적 스트레스 반응과 자기조절에 미치는 효과: 비무작위 연구)

  • Purpose: The aim of this study was to analyze the effects of neurofeedback training for reducing stress and enhancing self-regulation in late adolescence to identify the possibility of use for nursing intervention. Methods: A nonequivalent control group pre-post quasi-experimental design was used. Participants were 78 late adolescents assigned to the experimental group (n=39) that received the neurofeedback training and the control group (n=39). Data were collected on heart rate variability (HRV) and skin conductance level (SCL) to assess stress-biomarker response. The questionnaire contained 164 items from: Positive and Negative Affect Schedule (PANAS), Symptom Checklist-90-Revised (SCL-90-R) and Self-regulatory Ability scale. The neurofeedback training was based on the general adaptation syndrome and body-mind medicine. The intervention was conducted in a total of 10 sessions for 30 minutes per session with high-beta, theta and sensory motor rhythm training on scalp at central zero. Results: There were significant difference in standard deviation of normal to normal interval (p=.036) in HRV and SCL (p=.029) of stress-biomarker response between the two groups. Negative affect (p=.036) in PANAS and obsessive compulsive (p=.023) and depression (p<.001) in SCL-90-R were statistically significant. Self-regulation mode (p=.004) in self-regulation ability scale showed a significant difference between the two groups. Conclusion: The results indicated that the neurofeedback training is effective in stress-biomarkers, psychoemotional stress response and self-regulation. Therefore, neurofeedback training using neuroscientific approach based on brain-mind-body model can be used as an effective nursing intervention for late adolescents in clinics and communities for effective stress responses.

Evaluation of the Anti-inflammatory and Immunomodulatory Effects of BSASM Using in vitro Experiments (시험관내에서 천연물제제 BSASM의 항염증 및 면역억제 효능 평가)

  • For effective management of atopic dermatitis, it is important to introduce a therapeutic agent although having the fewest side effects, has the greatest anti- inflammatory effect. In the course of screening anti-inflammatory agents, we obtained BSASM composed of several plant extracts. This study was designed to investigate anti-inflammatory and immunomodulatory effects of BSASM. As a first step, $NF-{\kappa}B$ luciferase reporter assay was performed to know the involvement of BSASM in the production of proinflammatory cytokines because $NF-{\kappa}B$ element has been known to play a major role in expression of cytokine genes such as interleukin-8 (IL-8) or tumor necrosis $factor-{\alpha}\;(TNF-{\alpha})$. LPS (lipolysaccharide)-induced $NF-{\kappa}B$ activation was inhibited by BSASM. In addition, we found the fact that BSASM inhibits LPS-induced produced production of IL-8 and $TNF-{\alpha}$ proinflammatory cytokines, indicating BSASM has anti-inflammatory effect. In interleukin-2 (IL-2) luciferase reporter assay in Jurkat T cells, BSASM reduced PHA (Phytohemagglutinin)-induced IL-2 luciferase activity, suggesting the possibility that BSASM might also have an immunomodulatory function in T cell-mediated immune response. Based on these results, we suggest the possibility that BSASM can be introduced to improve symptom of immune-related skin diseases, namely, atopic dermatitis.

Topics for Evidence-Based Clinical Nursing Practice Guidelines in Korea (국내 근거기반 임상간호실무지침의 주제 선정)

  • Purpose: This study was to identify topics for evidence-based clinical nursing practice guidelines in Korea. Methods: Data were collected from 330 staff nurses from 10 general hospitals and 53 nurses in charge of nursing education in 110 hospitals with over 500 beds. Using open questions, the nurses identified activities which could not be verified, which lacked consistency among nurses, clinical units and/or hospitals, which were not based on the up-to-date knowledge and which needed reform. The data were analysed by content analysis using a qualitative methodology. Results: Collected data consisted of 1882 clinical topics, which were classified into 50 topics, 207 mid-categories, and 456 sub-categories. The most frequent topics in order of frequency were medications, central line management, intravenous injections, urinary catheterization, perioperative nursing care, skin tests, pressure ulcer care, blood transfusions, laboratory examination-culture, respiratory care which were performed routinely in clinical setting by staff nurses. Conclusion: The research findings indicate the urgent need to develop evidence-based clinical nursing practice guidelines related to these research findings. Further research is needed to identify topics related to health promotion, and symptom/management of health problem.

A Case Report of Chin Area Folliculitis Improved by Administration of Gyejigadaehwang-tang (계지가대황탕(桂枝加大黃湯) 투여 이후 호전된 턱 주위 모낭염 치험 1례)

  • Objectives : This study aims to report a case of chin area folliculitis improved by oral administration of Gyejigadaehwang-tang. Methods : The patient of this study visited our hospital with purulent lesions accompanied by erythema, papules and pustules around the chin area that have recurred over 10 years. Based on the patient's tendency of chills and constipation, Gyejigadaehwang-tang was prescribed. Results : About a month later, most of the skin symptoms improved (Total Folliculitis Symptom score 11→2), and only brown marks and mild erythema due to pigmentation after inflammation, were observed. Symptoms related to constipation also improved from 13 to 4 points. In the follow-up process after the treatment, It was confirmed that the symptoms of folliculitis did not recur. Conclusions : After taking Gyejigadaehwang-tang for a month, purulent lesions generally improved. Symptoms related to constipation also diminished, allowing regular bowel movements once a day.

Reliability of Quantifying Maximal Mouth Opening and Lateral Mandibular Shift in Individuals With and Without Temporomandibular Disorder Using Three-dimensional Ultrasound-based Motion Analysis

  • Background: Although magnetic resonance imaging is accurate, it is expensive to measure the movement of temporomandibular joint. The three-dimensional (3D) motion analysis system is an inexpensive measurement tool. Objects: This study examined the reliability of quantifying the mouth opening and lateral mandibular shift and differences between individuals with and without temporomandibular disorder (TMD) using the hygienic method of surface markers on the skin with 3D ultrasound-based motion analysis. Methods: This study included 24 subjects (12 with and 12 without TMD). Temporomandibular joint motion during mouth opening was recorded using two surface markers with 3D ultrasound-based motion analysis. An intraclass correlation coefficient [ICC (3,k)] was used to confirm the intrarater reliability of quantifying kinematic temporomandibular joint motion, and an independent t-test was used to evaluate differences in maximal mouth opening and lateral mandibular shift between the two groups. Results: Assessment of mouth opening and lateral mandibular shift showed excellent test-retest reliability with low standard error of measurement. The lateral mandibular shift and opening-lateral mandibular shift ratio were significantly increased in the TMD group during maximum mouth opening (p<.05). However, no significant difference in maximal mouth opening was observed between the groups with and without TMD (p>.05). Conclusion: This hygienic and simple surface marker method can be used to quantify the mouth opening and lateral mandibular shift at the end-range of mouth opening. The TMD group showed an increased lateral mandibular shift movement at the end-range of mouth opening. The lateral mandibular shift movement can be regarded as a symptom in the diagnosis and treatment of TMD.

A Case Report of Facial Dermatitis Herpetiformis with Allergy Diseases Improved by Administration of Mahaengeuigam-tang (마행의감탕(麻杏薏甘湯)으로 호전된 알레르기 질환을 동반한 안면부 포진상 피부염 치험 1례)

  • This study aims to report a case of facial dermatitis herpetiformis with allergy diseases improved by administration of Mahaengeuigam-tang (Maxingyigan-tang). Multiple blisters appeared on the patient's face, Erythema, papules, and scales were also observed on the neck, back, and knee areas of the patient. The patient has had cough, sputum, and dyspnea symptoms since she was 5-6 months old. After diagnosing the patient's condition as dermatitis herpetiformis, atopic dermatitis, and bronchial asthma through history taking and symptom observation, Mahaengeuigam-tang (Maxingyigan-tang) was prescribed for about 30 days. As a result of evaluation through our own criteria, dermatitis herpetiformis symptoms improved from 18 to 2 points. Atopic dermatitis and asthma symptoms also decreased from 12 to 0 and 20 to 3 points, respectively. Since this study has proven the efficacy of Mahaengeuigam-tang (Maxingyigan-tang) for dermatitis herpetiformis patient, this prescription may be fully utilized as the treatment for other inflammatory skin diseases.

The Slough of Cicadidae Periostracum Ameliorated Lichenification by Inhibiting Interleukin (IL)-22/Janus Kinase (JAK) 1/Signal Transducer and Activator of Transcription (STAT) 3 Pathway in Atopic Dermatitis

  • It is known that animal-origin medicine could be one of effective treatment to remedy atopic dermatitis (AD) by controlling the cytokines. Cicadidae Periostracum (CP), the slough of Cryptotympana pustulata, has been frequently used for treating AD and skin affliction in traditional Korean Medicine. This study is aimed at investigating the ameliorating effects of CP on AD and its potential mechanism. The dinitrochlorobenzene sensitized mice were treated with CP for 2 weeks. The various biomarkers and the dermatitis scores presented that CP treatment can induce the visual and biological improvements of AD model. Pruritus, the most serious symptom of AD, which can cause repeated scratching behaviors and finally lead to lichenification, was reduced with CP treatment by regulating the inflammatory reactions. In addition, CP treatment diminished the number of mast cells that are known for causing inflammatory reactions. Moreover, it is proven that CP can decline secretion of interleukin-22, which means CP treatment has anti-inflammatory effects. CP treatment can correct the imbalance of helper T (Th)1 and Th2, downregulating thymic stromal lymphopoietin that leads to decrease of mRNA level of inflammatory cytokines. The crucial role of CP treatment is controlling of the Janus kinase 1/signal transducer and activator of transcription 3 pathway. In addition, CP treatment has the inhibitory effects on kallikrein related peptidase (KLK) 5 and KLK7. Taken together, CP treatment can ameliorate most symptoms and problems caused by AD disease, improving the AD patients' life quality.

Management of Steroid Rebound Phenomenon in Patients with Eczema (습진 환자의 스테로이드제 반동 현상 관리 및 치험 1례)

  • Objectives : We would like to report what symptoms appear in patients experiencing steroid rebound phenomenon and how to manage and treat them at Korean medicine hospital. Methods : A 20-year-old woman who had been using topical and oral steroids for more than 6 weeks presented to the hospital. She hospitalized for 5 weeks and stopped using steroid. The treatment applied was acupuncture, wet dressing therapy and herbal medicine including Yangdokbackho-tang, Sunbangpaedok-tang and Danguieumja. We assessed symptoms through Inflammation Index and Chronicization Index. Results : Right after she stopped using steroid, symptoms of skin got worse gradually. However, after 5 weeks of treatment, all symptoms except itching and dryness disappeared. When symptom was severe, Inflammation Index and Chronicization Index was 12 and 9 points and after the treatment they were 2 and 4 points. Conclusions : This study shows various symptoms in patients with topical steroid addiction and how to manage at Korean medicine hospital.
