Prevalence of Depression in Breast Cancer Survivors: a Systematic Review of Observational Studies

  • Background: Depression is common in breast cancer patients. The aim of this paper was to make a systematic review of its prevalence and associated factors oin breast cancer survivors. Materials and Methods: An extensive systematic electronic review (PUBMED, CINAHL, PsyINFO and Ovid) and handsearch were carried out to retrieve published articles up to November 2012, using Depression OR Dysthymia AND (Cancer OR Tumor OR Neoplasms as the keywords. Information about the design of the studies, measuring scale, characteristics of the participants, prevalence of depression and its associated factors from the included studies were extracted and summarized. Results: We identified 32 eligible studies that recruited 10,826 breast cancer survivors. Most were cross-sectional or prospective designed. The most frequent instrument used to screen depression was the Center for Epidemiological Studies for Depression (CES-D, n=11 studies) followed by the Beck Depression Inventory (BDI, n=6 studies) and the Hospital Anxiety and Depression Scale (HADS, n=6 studies). CES-D returned about similar prevalence of depression (median=22%, range=13-56%) with BDI (median=22%, range=17-48%) but higher than HADS (median=10%, range=1-22%). Depression was associated with several socio-demographic variables, cancer-related factors, treatment-related factors, subject psychological factors, lifestyle factors, social support and quality of life. Conclusions: Breast cancer survivors are at risk for depression so that detection of associated factors is important in clinical practice.

Properties of the Twenty-seven Pulses in DongUiBoGam Based on the Eight Important Pulses (팔요맥을 중심으로 살펴본 『동의보감』 27맥 속성 연구)

  • Objectives : Pulse diagnosis is considered particularly important among several methods of diagnosis in DongUiBoGam. In spite of its importance, numerous and various pulse descriptions made it difficult to learn and practice pulse diagnosis. In this article, we tried to analyze properties of the twenty-seven pulses from pulse diagnosis cases from DongUiBoGam to enable the practical understanding of pulse diagnosis. Methods : We constituted the four axis according to the eight important pulses. And we analyzed properties of the twenty-seven pulses through the relationship between the four pairs of important pulses and the twenty-seven pulses. To quantify the relevances of important pulses to the twenty-seven pulses, we used the term frequency-inverse document frequency(TF-IDF) method. Results : We could elicit properties of the twenty-seven pulses according to the four axis. Also, we reexamined the categorization of the seven exterior pulses / the eight interior pulses and the similar pulses from DongUiBoGam with the analysis results. Conclusions : We could understand properties of the twenty-seven pulses more specifically with the eight important pulses. And we also could see the relationship among the twenty-seven pulses on each axis. However, the limitation arising from insufficient number of pulse diagnosis cases in this research requires further research with more sources such as other traditional medical records or clinical records in the present time.

A Genotypical Analysis of Korean REMCs and Generation of Base Line Data for the Analysis and Evaluation for Future (REMCs) Designs Using Space Syntax

  • Purpose: The purpose of this paper is to analyze the spatial configurations of a sample of Korean regional emergency medical centers (REMCs) to explore its underlying genotypes and thus produce a base line data for the analysis and evaluations of future REMCs designs using space syntax theory. Methods: Space syntax analysis was used as a major tool for the analysis and exploration of Genotype. The measures of Integration(overall integration with exterior and without exterior as well as the integration of individual clinical spaces for each center), base difference factor (DF) and Space link ratio were calculated for a sample of seven Korean REMCs. Results: The result shows a strikingly similar pattern of Syntactic measures across the sample, the mean integration of sample ranges from 0.82-0.99 with exterior (while considering the exterior space as a root) and 0.81-1.01 without exterior (considering the connections of interior spaces only with no outside connection). The base difference factor (DF) of the sample varies from 0.60-0.81 with exterior and from 0.59-0.82 without exterior. Case number-1 was identified as non-genotype with differing order of Syntactic values. Although the genotype had different forms, layouts and even sizes, these results cannot be explained by Phenotypical comparisons. Implications: This study will contribute to the configurational analysis and evaluation of existing and future Korean REMCs design and practice of emergency healthcare delivery system in Korea.

Comparative Study About Academic Thoughts of Xu Lingtai and Yoshimasu Todo (II) - Analysis of the Cause of Similarities and Differences in their Academic Thoughts - (서영태(徐靈胎)와 길익동동(吉益東洞)의 학술사상 비교 연구 (II) - 학술 사상이 같고 다른 원인에 대한 분석 -)

  • In the 18th century, Xu Lingtai (徐靈胎) and Yoshimasu Todo (吉益東洞) were medical revolutionaries. They emphasized researches about synthesis of formulae, efficacy of medication and observation and then classification of clinical phenomena, so they assumed a modern scientific character. But, there were clear differences between their academic thoughts. In this paper, we examine the causes of difference in three fields, i.e. traditional culture, viewpoints of talented people and academic personality. The first, difference was due to traditional culture. Chinese medicine has a long history and heavy traditional culture. Yin-Yang (陰陽) theory, Five Phase(五行) theory, Viscera and Bowels (臟腑) theory and Meridian and Collateral (經絡) theory stemmed from everyday practice, and Chinese people learn these theories from experience and observation. From the standpoint of Chinese people, particularly scholarly doctors [儒醫] such as Xu Lingtai, it was easy to debate medical theories. In contrast, Japanese traditional culture didn't have as long a history as China. Thus as a necessity, it was harder to disseminate traditional Chinese medicine theories in Japan. Yoshimasu Todo simplified it by cutting out the superfluous traditional Chinese medicine theory, so at that time it must have been shocking to the Japanese medical world's trends. The second, difference was due to viewpoints of talented experts. From the standpoint of Xu Lingtai, above all, medicine is just a learning, only a kind of technique, even more not a means of living. Xu Lingtai was concerned with the appearance of very talented experts such as 'great man' (偉人), and 'exceptional man' (奇士) who carried out medical research. Instead of cultivating a few talented people, Yoshimasu Todo tried to produce a large number of clinicians quickly who could treat ordinary people. The third was due to personality difference. As Xu Lingtai threw away Confucianism and studied medicine in his youth, although he had a critical attitude, he was always mild-mannered. Yoshimasu Todo always had a clearly critical and rebellious nature. Personality influenced their literary spirit and learning style, so although both advocated reactionism, the academic thought of Xu Lingtai was reformative and mild, while that of Yoshimasu Todo was revolutionary and fierce. Xu Lingtai and Yoshimasu Todo had considerably similar research domains and academic thought, so it is proper for them both to serve as examples for making a comparative study of medical history in China and Japan in 18th century.

An On-line Survey on the Perception and Usage of Korean Medicine Doctors about Pattern Identification of Eight Principles (국내 한의사의 팔강변증에 관한 인식 및 활용현황 조사)

  • In order to increase the clinical value of an identification of patterns according to the eight principles (IPEP) in Korean medicine practice, The research on the Clinical Practice Guideline (CPG) of IPEP should comprehend the situation of clinical usage of IPEP practiced by Korean medicine doctors at first. Google survey form were emailed to Korean Medicine doctors registered in the Association of Korean Medicine on 04/15/2021 and the survey was closed at 04/22/2021. Data of 505 answered cases were analyzed by Frequency analysis, Chi-Square analysis, correlation analysis for understanding differences by groups. Out of 505 respondents, 57.6% have answered that they are using IPEP. It means that 42.4% of KM doctors don't use in the medical practice reversely in spite of fundamental diagnostic theory. The 64.7% respondents of no using IPEP presented their opinion about the theoretical problem that it is difficult to use because the concept of IPEP is ambiguous. And next, the 52.1% of the respondents expressed that there is no objective tools to measure and record the IPEP evidences in actual implementation. And 49.6% of the respondents also suggested that it is hard to trust and use IPEP similar to the previous comment. Even about 50% of the respondents are carrying out diagnosis and treatment using IPEP, it showed that there were several unsolved problems such as lack of understanding and practical tools or objective indicators for diagnosis of IPEP. Through the above results, the concept, usage, measurement requirements with indices and discriminant logic of IPEP were manifested as the main hopes of attending members of Korean medicine in the survey, so the IPEP CPG should make clear about these difficult but necessary assignment in the near future.

Conflict resolution and political tasks on the usage of beauty care devices by beauty artists (미용업종사자의 미용기기 사용에 대한 분쟁해결과 정책적 과제)

  • In contemporary society interest in and consumption of beauty treatment are increasing, raising interest in health and beauty. However, beauty-related laws are becoming factors of hindrance of beauty development. Currently the Public Health Control Act plays a basic role in the beauty art business in Korea, However the contents are in discord with international laws and its definition is not clear. Therefore it is causing conflicts of different occupations and job associations which are similar to art business. Especially, because neither definitions nor policies on beauty care devices exist in the Public Health Control Act, beauty care devices using in foreign countries cannot be used in Korea due to classification as medical devices. Under this circumstance, therefore, beauty care device uses by beauty artists violate the law. The government has tried to solve these irrational regulations. Recently, the Small and Medium Business Administration announced 'the improvement plan of small business and young founders site regulation for public economy recovery' in a ministerial meeting on December 28, 2016. Regulations on policy preparation for skincare devices were inclusive in this announcement. It is the question whether the regulations will be executed or not. Even though beauty industrial competitiveness was presented in the 18th Presidential Council on National Competitiveness in 2009, it was not practiced. The proposal bills for beauty law improvement have been put forth several times since 2000 including an improvement plan for regulating beauty care devices. However, so far there have been no improvements. The damage on the regulation classifying beauty devices as medical devices is not only restricted to skincare. This develops beauty devices and the beauty industry which imports and exports beauty devices. When beauty devices are exported, complicated procedures are unavoidable and when beauty devices are imported, irrational problems like reregistration procedures and costs occur. The reason why an improvement plan has not gone into practice is the resistance of the dermatologists' association. Dermatologists tend to stand positively against harming public health by saying that beauty devices used by beauty artists cause people to suffer side effects. In contrast, anyone who has a licence to use beauty devices is able to use them in foreign countries. It is not only infringement of one's right as a beauty artist but also people's right to receive beauty care services. With this reason, Korean's current law under which beauty devices are ruled as medical devices should be revised with accordance to domestic surroundings. Therefore in order to advance and globalize the beauty industry, the support and cooperation of the Korean government and relevant associations is needed to legislate and revise the beauty devices laws. The relevant associations abandon regional self-centeredness and cooperate to define ranges, size and management of beauty devices for safe use. If no collaboration exists, an arbitration agency should be established to solve the problem.

Verification of Shielding Materials for Customized Block on Metal 3D Printing (금속 3D 프린팅을 통한 맞춤형 차폐블록 제작에 사용되는 차폐 재료 검증)

  • As 3D printing technology is used in the medical field, interest in metal materials is increasing. The Department of Radiation Oncology uses a shielding block to shield the patient's normal tissue from unnecessary exposure during electron beam therapy. However, problems such as handling of heavy metal materials such as lead and cadmium, reproducibility according to skill level and uncertainty of arrangement have been reported. In this study, candidate materials that can be used for metal 3D printing are selected, and the physical properties and radiation dose of each material are analyzed to develop a customized shielding block that can be used in electron beam therapy. As candidate materials, aluminum alloy (d = 2.68 g/cm3), titanium alloy (d = 4.42 g/cm3), and cobalt chromium alloy (d = 8.3 g/cm3) were selected. The thickness of the 95% shielding rate point was derived using the Monte Carlo Simulation with the irradiation surface and 6, 9, 12, and 16 energies. As a result of the simulation, among the metal 3D printing materials, cobalt chromium alloy (d = 8.3 g/cm3) was similar to the existing shielding block (d = 9.4 g/cm3) in shielding thickness for each energy. In a follow-on study, it is necessary to evaluate the usefulness in clinical practice using customized shielding blocks made by metal 3D printing and to verify experiments through various radiation treatment plan conditions.

A study of quality of working life to dental hygienist's (치과위생사의 근로생활의 질(QWL)에 관한 연구)

  • Objectives : Dental hygienist's work satisfaction and stress affect the overall quality of work life(QWL). Therefore, this research is intended to suggest fundamental data to improve QWL by finding out characteristics of each work satisfaction and stress element. To this end, a total of 327 dental hygienists working at general hospitals, university hospitals, dental hospitals and dental clinics across Seoul, Gyeonggi and Incheon were surveyed. Results of survey are as follows. Methods : The collected data were analyzed by using an SPSS 12.0 statistical program, obtaining the following results. The collected data conducted a questionnaire survey for 327 dental hygienists who work at the hospitals, university hospitals, dental hospitals, and dental clinics located at Seoul, Gyeonggi-do, and Incheon district from January until March, 2009, and drew the conclusions as follows. Result : 1. Demographic characteristics, income from 1.5 to 1.99 million were the whole lot, more than 2 million to less than 1.5 million was similar. Marital status Married Unmarried higher than the atheist religion, Christianity, Catholicism, Buddhism, and other, respectively. Classification by level of education in the college graduate, university graduate, graduate diploma, respectively. 2. Are working in a job-related characteristics of dentistry, dental hospital, general and university hospital, respectively. The making in position, Mount, contractor, responsible, senior, was an intern in the order. The five-day workweek whether working at night and is not going to care whether the conduct was similar. Classification of working hours and 8 hours, 8 hours, 8 hours or less orderly, and total of less than 1-3 years of clinical experience, 5 years, less than one year, less than 3-5 years, respectively. 3. There comes out a significant difference according to age, income, position, gross clinical experience, and whether to put night shift into practice in job stability in terms of the quality subsequent to general characteristics(p<.05). 4. There comes out a significant difference according to marital status, one's place of work, position, whether to put a five-day workweek into practice in work environment and benefits package in terms of the quality subsequent to general characteristics (p<.05). 5. There comes out a significant difference according to age, marital status, income, position, and gross clinical experience in education & training and benefits packages in terms of the quality subsequent to general characteristics(p<.05). 6. There comes out a significant difference according to whether to put night medical treatment into practice in social usefulness in terms of the quality subsequent to general characteristics(p<.05). 7. There comes out a significant difference according to marital status, income, one's place of work, gross clinical experience, work hours, and whether to put a five-day workweek into practice in leisure activity in terms of the quality subsequent to general characteristics(p<.05). 8. There comes out a significant difference according to income, one's place of work, and position in wage level in terms of the quality subsequent to general characteristics(p<.05). 9. There was no significant difference in all items related to human relations and free communication in terms of the quality subsequent to general characteristics(p>.05). Conclusions : It is necessary to analyze factors related to work satisfaction and stress in order to improve dental hygienist's quality of work life. Hospitals must support them systematically and institutionally and related organizations must conduct practical research.

A Study on Principle of Kigong mentioned in the lecture on Truth of ${\ll}$Samilshingo${\gg}$(三一神誥) and its Interrelationship with Oriental Medicine (${\ll}$삼일신고(三一神誥).진리훈(眞理訓)${\gg}$에 나타난 氣功原理(氣功原理) 및 한의학(韓醫學)과 의 상관성(相關性)에 관(關)한 연구(硏究))

  • Jigam(止感), Josik(調息), Kumchock(禁觸), from the lecture on Truth of ${\ll}Samilshingo{\gg}$(三一神誥) treat three elements of Kigong, regulation of mental activties, regulation of breathing and adjustment of posture and there are some similar mentions with the view of human body based on oriental medicine like those Samjin(三眞), Sammang(三妄), Samdo(三途) and Sippalkyoung(十八境) ect. Thus as a result of comparison and observation about the interrelationships between principle of Kigong in the lecture on Truth of ${\ll}Samilshingo{\gg}$(三一神誥) and oriental medicine. I have conclusion as follows. 1. According to the lecture on Truth, the components of human body are the one and only Samjin(三眞), Sammang(三妄) and Samdo(三途) resulted from facing Samjin(三眞) to Sammang(三妄) and Sippalkyoung(十八境). This fact presents the principle of human change. 2. the principle of Kigong mentioned in the lecture on Truth shows the original Ilshin(一神) on the basis of Samjin(三眞), Sammang(三妄), Samdo(三途) and Sippalkyoung(十八境). This makes common, unity and sound Sim(心), Ki(氣) and Shin(身), Sammang(三妄) through Jigam(止感), Josik(調息) and Kumchock(禁觸), be versed, intellected and guaranteed Sung(性), Myoung(命) and Jung(精), Samjin(三眞) and emit Kyun(見), Mun(聞), Ji(知) and haeng(行), Sadaeshingi(四大神機) and finally all these are harmonized into Duk(德), Hye(慧) and Ryuk(力), Samdae(三大) which is the entity of God. 3. Samsipyookjongmyowhasang(三十六種妙化相) is an ascetic practice done after a chulin(哲人) deduce Ji(止), Jo(調) and Kum(禁), Sambup(三法) on the basis of the lecture on Truth. So I suppose it correlates nature's six elements. Kong(空)(Chun(天)). Yol(熱)(Wha(火)). Jin(震(Jeon(電)), Seup(濕)(Shoo(水)), Han(寒)(Poong(風)) and Ko(固)(Ji(地)), human's Samjin(三眞), Sung(性), Myoung(命) and Jung(精) and Sammang(三妄), Sim(心), Ki(氣) and Shin(身) and makes clear the principle of discipline. 4. In comparison with Samjin(三眞) and Sammang(三妄) says from the lecture on Truth and the Three Essential Elements of the body construction(三寶) from oriental medicine, Samjin(三眞) and Sammang(三妄) as factors of human body in the concept of practical knowledge. That is the one and only Samjin(三眞) in terms of the Three Essential Elements of the body construction(三寶) is considered a structural principle for every single person and Sammang(三妄) is considered a functional form for each individual. And it can be Sung(性)+Sim(心)=Spirit(神), Myoung(命)+Ki(氣)=Vital Force(氣) and Jung(精)+Shin(身)=True Essence(精). 5. In comparison with Sippalkyoung(十八境) of Samdo(三途) from the lecture on Truth and three medical causes of disease, Gamdoyookkyoung(感途六境) is similar with endopathic cause caused by Naesangchiljung(內傷七情), Sikdoyukkyoung(息途六境) is similar with exopathic cause by six climatic conditions in excess as pathogenic factors(六淫) and Yoegi(?氣) and Chokdoyukkyoung(觸途六境) is similar with non-endo-exopathogenic causes by diet imbalance, fatigue, intemperance in sexual life and trauma etc. 6. In the lecture on Truth, the Chulin(哲人) who discipline Sambup(三法), Sangchul(上哲), Choongchul(中哲) and Hachul(下哲) can be compared with Kigong expert(health preserving expert) such as the Spiritual Men(眞人), the Sapients(至人), the Sages(聖人) and the Men of Exellent Virtue(寶人) in the Sang Gu Tian Zhen Lun of the Huang Ti Nei Ching Su Wen(素問 上古天眞論) and then Sangchul(上哲) is the Spiritual Men(眞人), Choongchul(中哲) is the Sapients(至人) and Hachul(下哲) is the Sages(聖人) while the men of Exellent Virtue(賢人) is inferior to Chulin(哲人) when he goes to extremes.

Prophylactic effect of aqueous extract of Sesamum indicum seeds on ethanol-induced toxicity in male rats

  • The liver is vulnerable to alcohol-related injury because it is the primary site of alcohol metabolism. Additionally, a number of potentially dangerous by-products are generated as alcohol is broken down in the liver. However, dietary supplements may prevent or relieve some of alcohol's deleterious effects. Therefore, this study was conducted to evaluate the prophylactic effect of aqueous extract of Sesamum indicum (SI) on ethanol induced toxicity in rats. Male Wistar albino rats were divided into control, ethanol, pre-treatment, simultaneous and post-treatment groups. In the prophylactic experiment, Sesamum indicum, (200 mg/kg body weight) was administered by oral gavage for 28 days; two hours before, simultaneously with or two hours after ethanol exposure. Toxicity was induced by administering 45% ethanol (4.8 g/kg bw) by oral gavage. Lipid peroxidation (TBARS) and reduced glutathione (GSH) levels and catalase (CAT), glutathione peroxidase (GPx), superoxide dismutase (SOD) and gluthathione-S-transferase (GST) activities were then determined in the liver, serum triglyceride (TG) levels, alanine aminotransferase (ALT) and aspartate aminotransferase (AST) activities were monitored and histological examination was carried out. The results revealed that ethanol administration led to significant elevation of TBARS level while depleting in the level of GSH as well as CAT, GPx, SOD and GST activities. Similarly, TG level and ALT and AST activities were elevated. The SI pre-treated group significantly inhibited TBARS, restored GSH level, enhanced CAT, GPx, SOD and GST activities and significantly decreased the elevated level of serum TG, ALT and AST activities. SI treatment (simultaneously with ethanol) exhibited similar effects to those of the SI pre-treated groups, while the SI post-treated group did not show the same protection as the Pre-treated group. S. indicum possesses antioxidant and hepatoprotective properties, that eliminate the deleterious effects of toxic metabolites of ethanol.
