열린사슬운동과 닫힌사슬운동에서 어깨 굽힘 각도에 따른 어깨안정화 근육의 근활성도 비교 (A Comparison of the Shoulder Stability Muscle Activity according to the Shoulder Flexion Angles in Open and Closed Kinetic Chain Exercise)

  • Purpose: The purpose of this study was to investigate any visible difference in shoulder stability muscle activity along the bending angle($70^{\circ}$, $90^{\circ}$, $110^{\circ}$) to perform the open kinetic chain(OKC) and closed kinetic chain(CKC) exercise. Method: This study was performed on normal 20 males subjects. Exercise was carried out in two ways OKC and CKC. Shoulder stability muscles are examined activity in the bending angles. In the OKC and CKC exercise to subjects with stretched out waist straightened, and than push forward the shoulder. Muscular activation was examined using the WEMG-8. Result: The results of this study were as follows. 1. Only in the trapezius muscle, it showed specific interaction between exercise methods and angles. When it comes to muscle activity, there was no significant difference in all muscles except for the anterior fibers of the deltoid muscle during both CKC and OKC exercise. 2. The average value of maximum muscle activity was significantly increased with increasing order of angles. But in CKC exercise, the muscle activity of infraspinatus tended to decrease with increasing angles. Conclusion: To summarize the research results, the muscle activity in upper fibers of the trapezius, anterior fibers of the deltoid, and serratus anterior was significantly different depending on the angle of the shoulder in both CKC and OKC exercise.

Effect of the Positions of Female Pro-Volleyball Players on the Stability of Shoulder Joints of the Dominant and Non-dominant Arms

  • The purpose of this study was to comparatively analyze the active stability of shoulder joints according to the frequency of overhead motions, such as serving and spiking, engaged in by female professional volleyball athletes who play different positions, and to provide the results as the basic data for developing exercise programs to prevent shoulder joint injuries. The subjects of this study were 50 Korean female professional volleyball players and positions were as follows: left and right attackers, centers, setters, and liberos. The external rotation and internal rotation muscle strength and muscle strength ratios of the dominant and non-dominant arms of all subjects were measured using Biodex. The results of this study are as follows: Frist, no significant differences were found in the internal and external rotation muscle strength of the dominant and non-dominant arms between positions. Second, for the shoulder joint muscle strength ratio of the dominant arm, by position, the setter showed significantly greater stability compared to the other positions. Third, for the shoulder joint muscle strength ratio of the non-dominant arm, by position, no significant difference in stability between positions was found. Fourth, it was found that the dominant arm had significantly greater instability of the shoulder joint than the non-dominant arm for attackers and centers, but no significant difference was found for setters and liberos. This study comparatively analyzed the muscle strength ratios of the external/internal rotations and dominant/non-dominant arms, which can determine the stability of the shoulder joints between female professional volleyball playing positions that engage in jumps and spikes using only the dominant hand and positions that do not.

Trunk Muscle Activation during Bridge Exercise with Various Shoulder Supporting Surfaces

  • PURPOSE: Bridge exercises are broadly used to develop trunk co-activation patterns that promote spine stability. This study was to analyze the trunk muscle activity during bridge exercise with various shoulder support surface(stable, sling, Swiss ball). METHODS: The subjects were 20 healthy subjects in their twenties. Subjects were performed bridge exercise on 4 different shoulder support surfaces using stable and labile instruments. 1) Bridge exercise on a stable surface. 2) Bridge exercise with their shoulder on a stable bench. 3) Bridge exercise with their shoulder on a sling. 4) Bridge exercise with their shoulder on a Swiss ball. Rectus abdominis, erector spinae, internal oblique, external oblique muscle activities were measured using electromyography. RESULTS: There were significant differences in RA, EO muscles between performing each of the 4 exercises(p<.05). RA and EO was recorded the highest activity during the bridge exercise with their shoulder on a sling. The lowest activity was recorded during conventional supine bridge on a stable surface. There were no differences found for the EO/RA and IO/RA ratio. The EO/RA and IO/RA ratio was the highest in the bridge exercise with their shoulders resting on a stable bench. CONCLUSION: These findings suggest that change of shoulder support surface during bridge exercise may be useful for enhancing the trunk stability.

Comparison of Glenohumeral Stabilization Exercise and Scapular Stabilization Exercise on Upper Extremity Stability, Alignment, Pain, Muscle Power and Range of Motion in Patients With Nonspecific Shoulder Pain

  • Background: Shoulder stabilization commonly involves two components: the glenohumeral stabilization exercise (GSE) and scapular stabilization exercise (SSE). Despite the fact that the shoulder stabilization has advantageous merit, to our knowledge, only a few studies have compared the superior of the GSE and the SSE. Objects: The purpose of this study was to assess the effects of GSE in patients with nonspecific shoulder pain. Methods: Thirty subjects with nonspecific shoulder pain were randomly divided into an experimental group and control group, each with 15 patients. The experimental group used an GSE, whereas the control group did SSE. All subjects were measured in shoulder stability, scapular symmetric alignment, pain, muscle power, and range of motion before and after the intervention. Results: GSE resulted in significantly better shoulder stability (p=.046, from $8.67{\pm}7.54$ score to $13.93{\pm}9.40$) in the experimental group compared with SSE in the control group. However, no significant effects were observed for scapular symmetric alignment including the angles of inferior scapular distance (p=.829) and inferior scapular height difference (p=.735), pain (p=.113), muscle power including shoulder flexion (p=.723) and abduction (p=.897) and range of motion including shoulder flexion (p=.853) and abduction (p=.472). Conclusion: These findings suggest that GSE may be more effective in increasing the shoulder stability than the SSE in patients with nonspecific shoulder pain, probably through a centralization effect on the shoulder mechanism.

견갑대 안정화 운동이 상지 근력에 미치는 영향 (The Effect of Neural Mobilization on the Grip Strength)

  • Purpose : The purpose of this study was to evaluate effects of shoulder girdle stability exercise on upper extremity muscle strength. Methods : Subjects were consist of 20 people who had no disorder of upper extremity from 20 to 27 years of age during 6 weeks from April 3, 2006 to May 14. Ten of all subjects exercised muscles which stabilize to shoulder girdle for 30 minutes during 6 weeks. Biodex was used to measure upper extremity muscle strength. Results : There are increasing of shoulder flexion and extension on average torque but no statistically significant difference between pre-exercise and post-exercise. Conclusion : There are no severe difference between exercise group and non-exercise group on muscle strength.

Isokinetic Shoulder Joint Characteristics by Position: Professional Korean Female Volleyball Players

  • The purpose of the study was to investigate the strength of sokinetic muscle by the position of a volleyball players. Analysis and comparison of shoulder dynamic stability will be conducted according to rotational movement of the shoulder during spiking and serving amongst the various positions. Fifty professional Korean female volleyball players (age: 20~30), all different positions - attacker (left and right), center, setter, and libero were The concentric peak torque, strength ration of the internal and external rotation of the shoulder girdle for both dominant and non-dominant arms. Firstly, there were significant differences found for the strength ratio between the setter and the other positions in the dominant arm. On a second note, there was a significant decrease in shoulder dynamic stabilization for both the attacker and center in the dominant arm. However, there were no significant differences for the setter or the libero. This study suggests that the isokinetic muscle strengths of the volleyball players are different from each other.

  • Background: This study aimed to determine the effects of pectoralis minor stretching, lower trapezius strengthening exercise with serratus anterior strengthening exercise to find an effective therapeutic exercise for alignment, shoulder range of motion, muscle imbalance of rounded shoulder patient. Methods: This study targeted on a total of 28 male subjects, aged 20s and 30s, and randomly divided the subjects into each group, with 14 subjects each. The experimental group performed pectoralis minor stretching, lower trapezius strengthening exercise, and serratus anterior strengthening exercise, while the control group performed pectoralis minor stretching and lower trapezius strengthening exercise. Results: both group showed an increase in active range of motion for shoulder flexion, a muscle activity of the serratus anterior muscle and the lower trapezius muscle, a decrease in muscle activity of the upper trapezius muscle, and a significant improvement in scapular alignment after the intervention. In the comparison between group, the experimental group showed greater increased shoulder joint flexion range of motion, serratus anterior muscle activity, and decreased upper trapezius muscle activity than the control group. Conclusion: Performing serratus anterior strengthening exercises with pectoralis minor stretching and lower trapezius strengthening exercises is effective for improving the active range of motion in shoulder joint flexion and muscle imbalance

The Effect of Exercise Using Grasping on Shoulder Muscle Activity and Muscle Thickness of Patients with Rotator Cuff Repair

  • Purpose: This study intended to mediate shoulder exercise using grasping and investigate examine how its effects on affects activities of the shoulder surrounding muscles of the shoulder and thickness of shoulder muscle to suggest effective exercise for patients with rotator cuff repair to return to daily life. Methods: This study has been done targeteding male patients more than 6 weeks after having En-masse Suture Bridge Technique as rotator cuff repair of the right shoulder, aged 40 to 55 in a medical institution located in Jeollanam-do. Experimental group I (n=15) was selected for a conducting common exercise therapy program and Experimental group II (n=15) was selected for an conducting exercise therapy program using grasping. %RVC was measured by surface electromyography and muscle thickness was measured by ultrasound before mediation to for analysisze of before and after results within group and between groups. Results: In comparison of change of %RVC, Tthere were meaningful differences in only posterior deltoid and infraspinatus in comparing a change of %RVC within experimental group I (p<0.001),. and Tthere were meaningful differences in both supraspinatus and infraspinatus in muscle thickness within experimental group I (p<0.001). In comparison of change of %RVC, Tthere were meaningful differences in anterior deltoid, posterior deltoid and infraspinatus in comparing a change of %RVC within experimental group II (p<0.05)(p<0.01) (p<0.001), and there were meaningful differences in both supraspinatus and infraspinatus in muscle thickness within experimental group II (p<0.001). In comparison of change of %RVC between groups, Tthere was a meaningful difference in only posterior deltoid in comparing a change of %RVC between groups (p<0.001). Conclusion: We found that exercise therapy using grasping has a positive effect on shoulder stability muscles such as supraspinatus muscle and infraspinatus muscle and activity of deltoid muscle.

둥근 어깨가 있는 대상자에게 골반 압박 벨트 적용을 동반한 어깨 안정화 운동의 수행이 어깨관절 주위 근육의 근활성도와 통증 및 기능에 미치는 영향 (Effect of Shoulder Stabilization Exercise with Pelvic Compression Belt Application on Muscle Activity, Pain and Function of Muscles around Shoulder Joint in Subjects with Round Shoulders)

  • Purpose : The purpose of this study is to investigate the effect of shoulder stabilization exercise accompanied by application of a pelvic compression belt on the muscle activity, pain and function of the muscles around the shoulder in subjects with round shoulders. Methods : For the study method, 28 students who were enrolled in K University with a distance of 1 cm or more between the clavicle of the peak and the outer ear path were selected through GPS 400 global postural analysis system measurement. The subjects were randomly assigned to 14 participants in the group wearing a pelvic compression belt and 14 patients in the group not wearing a pelvic compression belt. In all subjects, the muscle activities of the middle trapezius, lower trapezius, and serratus anterior muscles and the shoulder pain disorder index (SPADI) were measured. The intervention was performed 3 times a week for 4 weeks, and the applied intervention was push-up plus and modified prone cobra exercise. The muscle activities of the middle trapezius, lower trapezius, and serratus anterior muscles and SPADI score were compared using dependent t test before and after intervention. Results : In this study, both groups showed that the muscle activity of the middle trapezius, lower trapezius, and serratus anterior significantly increased after the intervention compared to before the intervention. On the other hand, SPADI showed no significant difference. Conclusion : The results of this study showed that muscle activity in the peri-shoulder joint was increased after push-up plus and modified prone cobra exercise in both groups, regardless of whether pelvic compression was applied or not. Therefore, it was found that shoulder stabilization exercise using the pelvic compression belt also contributed to the enhancement of muscle activity in the joints around the shoulder.

악성 섬유성 조직구종에서 광배근피판을 이용한 견관절 재건술 (Reconstruction of the Shoulder using Rotational Latissimus Dorsi Flap in the Malignant Fibrous Histiocytoma)

  • Introduction : The Functional muscle transfer is used to reconstruct the injuried muscle and paralysis of the shoulder. Especially transfer of the trapezius has been the treatment of choice but it has disadvantages of inadequate function and deformed contour, and instability of humeral head in case of acromion resection. We report an operation for shoulder reconstruction after wide resection of malignant fibrous histiocytoma, using rotational latissimus dorsi flap and review the operation method and clinical outcome. Materials and Methods : A patient, 53 year old, with malignant fibrous histiocytoma in the acromioclavicular joint area had been underwent wide excision, including the deltoid, clavicular head of pectoralis major, part of trapezius, lateral 1/3 of clavicle and acromion including scapular spine. The rotational latissimus dorsi flap with its neurovascular pedicle was dissected and then placed over the resected area and transfer of muscle attached at coracoid process was done to achieve stability of the humeral head. The range of motion of the shoulder and test of muscle power were evaluated for functional outcome. Total follow-up period is 2 years 11 months. Results : At last follow-up, the range of motion of the shoulder is abduction $90^{\circ}$, flexion $90^{\circ}$, internal rotation $40^{\circ}$, external rotation $50^{\circ}$ and the muscle power is 4 grade in all direction and then we obtained good functional results. There are no complications such as instability or subluxation of the humeral head and deformed contour and he is a disease-free survival state. Conclusions : The transfered latissimus dorsi flap provides adequate lever arm and stabilization and covering of the humeral head by sufficient muscle volume and width. This procedure can be useful not only for the paralysed deltoid reconstruction but also for use in reconstructive surgery after wide resection of the shoulder for malignant tumor.
