변형된 Neviaser 술식을 이용한 견봉쇄골관절 탈구의 치료 (The Treatment of the Acromioclavicular Dislocation using the Modified Neviaser Method)

  • Purpose: To describe a technique of the modified Neviaser method and to evaluate the clinical outcome of the technique in the treatment of the complete acromioclavicular joint dislocation, Materials and Method: We evaluated 20 patients who were treated by a modified Neviaser method from June 1996 to January 1999, They were followed up for a minimum of two years. The operative technique includes fixation of the acromioclavicular joint, repair of coracoclavicular ligament and transfer of the anterolateral band of coracoacromial ligament. Results: The 20 patients were evaluated clinically using Kang's criteria. The satisfactory results obtained in 85%. The mean coracoclavicular interval ratio was decreased from 2.22 preoperatively to 1.07 postoperatively. At the time of two year follow-up, the mean ratio was 1.20. There was no definite complication such as re-dislocation of the joint. Conclusion: In above type 3 acromioclavicular dislocation, the modified Neviaser method provided the advantage of strong and stable fixation with a low complication rate. Therefore, it is thought to be one of the useful operative technique.

  • 견봉-쇄골 관절 손상은 흔히 견관절 상외측에서의 직접적인 외상으로 발생한다. Rockwood 제4형 손상은 상대적으로 드물게 생기며, 다발성 외상 환자의 경우 타 손상으로 쉽게 잘못 진단되어지거나 간과되어지는 경우가 있다. 제 4형 손상 환자의 잘못된 치료를 피하기 위해서는 견봉-쇄골 관절의 세심한 이학적 검사와 적절한 방사선학적 검사가 필요하다. 이에 저자들은 다발성 늑골 골절이 동반된 제 4형 견봉-쇄골 관절 손상을 초기에 진단하지 못하고 간과한 2예를 변형 Weaver-Dunn 재건술로 치료하였기에 문헌 고찰과 함께 보고하고자 한다.

  • Background: Arm swing plays a role in gait by accommodating forward movement through trunk balance. This study evaluates the biomechanical characteristics of arm swing during gait. Methods: The study performed computational musculoskeletal modeling based on motion tracking in 15 participants without musculoskeletal or gait disorder. A three-dimensional (3D) motion tracking system using three Azure Kinect (Microsoft) modules was used to obtain information in the 3D location of shoulder and elbow joints. Computational modeling using AnyBody Modeling System was performed to calculate the joint moment and range of motion (ROM) during arm swing. Results: Mean ROM of the dominant elbow was 29.7°±10.2° and 14.2°±3.2° in flexion-extension and pronation-supination, respectively. Mean joint moment of the dominant elbow was 56.4±12.7 Nm, 25.6±5.2 Nm, and 19.8±4.6 Nm in flexion-extension, rotation, and abduction-adduction, respectively. Conclusions: The elbow bears the load created by gravity and muscle contracture in dynamic arm swing movement.

상완골 불안정성 외과적 경부 이분 골절의 경피적 핀 삽입 고정술 (Percutaneous Pinning in Unstable Two-parts Fracture of Surgical Neck in Humerus)

  • Purpose : The aim of this study is the assessment of the clinical outcomes after percutanous pinning of unstable two-parts fracture of surgical neck in humerus. Materials and Methods: This study was based on thirteen cases of non-comminuted unstable surgical neck fracture of humerus among 19 cases, which followed-up more than one year. Follow-up averaged 29 months. We treated with percutaneous pinning techniques and assessed clinical outcomes. Functional evaluation was performed using the standard method of research committee of American Shoulder and Elbow Surgeons(ASES). Results: Last follow-up ROM of shoulder joint were 142 degrees of forward elevation, 57 degrees of external rotation, 72 degrees of external rotation in 90 degrees abduction, and T8 of internal rotation. Pain scale was l(range : 0∼3). ASES scores was 86.2(range : 63.3~98.3). Patient satisfaction based on ASES were excellent in 6 cases, good in 5 cases, fair in 1 case, poor in 1 case. A case of fair result was caused by limitation of motion in shoulder joint and poor case was paraplegia patient after traffic accident. Conclusion : Percutaneous pinning is recommended for non-comminuted unstable fracture of surgical neck in humerus.

견갑하근에 발생한 이소성 골화 -증례보고- (A Heterotopic Ossification in the Subscapularis Muscle - Case Report -)

  • We report the case of a heterotopic ossification in the subscapularis muscle. A 30-year-old woman was diagnosed as a heterotopic ossification in the right subscapularis muscle. X-rays, a CT scan and a MRI showed bone-like lesions in the muscle. The location of the ossification was both inferior and anterior to the joint. The mass nearly bridged between the proximal humerus and the inferior portion of the coracoid process. Symptoms did not respond to rest, NSAIDs for 3 months and to stretching exercises treatment for 6 months. Excision and biopsy was performed through deltopectoral approach. Disodium Etidronate was administered during the postoperative period. She regained normal range of motion postoperatively and improved in the shoulder pain after performing strengthening exercises with Therabands. We think that a cause of limitation of the shoulder was impingement between the mass and the coracoid process, and that another cause was adhesion between the mass and the joint capsule. We conclude that excision and postoperative rehabilitation exercises are good methods for a heterotopic ossification in the subscapularis muscle for those that are nonresponsive to nonoperative treatments.

Clinical outcomes of bending versus non-bending of the plate hook in acromioclavicular joint dislocation

  • Background: We aimed to assess the effect of plate hook bending in treatment of acromioclavicular (AC) dislocation by analyzing clinical and radiological results according to the angle of the plate hook (APH). Methods: This was a retrospective, observational, case-control study including 76 patients with acute AC joint dislocation that were divided into two groups according to treatment with bent or unbent plate hook. The visual analog scale (VAS), the American Shoulder and Elbow Surgeons (ASES) shoulder score, and range of motion (ROM) were evaluated as clinical outcomes. Comparative coracoclavicular distance (CCD) was measured to evaluate radiological outcomes. Results: While the VAS and ASES of the bending group at 4 months after surgery were significantly higher (p=0.021 and p=0.019), the VAS and ASES of the bending group at other periods and ROM of the bending group showed no significant difference. The initial CCD decreased from 183.2%±25.4% to 114.3%±18.9% at the final follow-up in the bending group and decreased from 188.2%±34.4% to 119.1%±16.7% in the non-bending group, with no statistical difference (p=0.613). The changes between the initial and post-metal removal CCD were 60.2%±11.2% and 57.3%±10.4%, respectively, with no statistical difference (p=0.241). The non-bending group showed greater subacromial osteolysis (odds ratio, 3.87). Pearson's coefficients for the correlation between APH and VAS at 4 months after surgery and for that between APH and ASES at 4 months after surgery were 0.74 and -0.63 (p=0.027 and p=0.032), respectively. Conclusions: The APH was associated with improved postoperative pain and clinical outcomes before implant removal and with reduced complications; therefore, plate hook bending is more useful clinically during plate implantation.

한국 프로야구 선수의 상지 수술 분포 연구 (The Study of Distribution of Upper Extremity Surgery for Korean Baseball Player)

  • Background: The objective of this research is to provide base line data on Korea Baseball rehabilitation treatment by presenting the distribution of upper extremity surgeries according the surgical sites of Korean professional baseball players. Methods: The targets of this research are 561 players from nine Korean baseball clubs. The number of players that have undergone surgery was investigated. In other words, the frequency of shoulder joint surgeries, elbow joint surgeries, and wrist and hand surgeries per each baseball club was investigated. Results: The number of the players who have had surgeries was 151, but among the players, there were some who have had repeated surgeries for which the total number of surgery experience was 162. The result showed that elbow joint surgeries were the most frequently enacted, followed by shoulder joint surgeries, and wrist and hand surgeries. From the nine clubs, Samsung was the club with the highest frequency of surgeries. Conclusions: This research shows that Korean professional baseball players need special training centers with systematic rehabilitation programs. Another conclusion suggests that urgent need for rehabilitation program of upper arm which particularly focused on elbow joint is existing. On the basis of this research, we expect more studies on the specific rehabilitation programs which is categorized by injury areas.
