Acromioclavicular (AC) joint dislocations account for about 9% of shoulder injuries. Among them, acute high-grade injury following high-energy trauma accounts for a large proportion of patients requiring surgical treatment. However, there is no gold standard procedure for operative treatment of acute high-grade AC joint injury, and several different procedures have been used for this purpose in clinical practice. This review article summarizes the most recent and relevant surgical options for acute high-grade AC joint dislocation patients and the outcomes of each treatment type.

The authors present a case of transient postoperative inferior subluxation of the shoulder after arthroscopic surgical stabilization for recurrent anterior dislocation. The patient was a 61-year-old woman with myasthenia gravis (MG). The first anterior shoulder dislocation occurred because of a fall to the ground. Despite a successful closed reduction, two more dislocations occurred in 3 weeks. Magnetic resonance imaging revealed an anterior labroligamentous periosteal sleeve avulsion (ALPSA) lesion, an engaging Hill-Sachs lesion, and large tears of the supraspinatus and infraspinatus tendons. The patient underwent arthroscopic rotator cuff repair and ALPSA repair with a remplissage procedure. Intraoperatively, no tendency for instability was found; however, a widened glenohumeral joint space and inferior subluxation of the humeral head without functional compromise was observed on the day after surgery and disappeared spontaneously on radiographs 2 weeks later. To the authors' knowledge, this is the first report documenting the occurrence of transient postoperative inferior subluxation of the shoulder in a patient with MG.

Purpose: The purpose of this study was to identify the effects of cancer-overcome BeHaS exercise program on shoulder joint function, stress, body image and self-esteem in women who have had surgery for breast cancer. Method: A non-equivalent control group pre-post test design with an experimental group (n=25) and a control group (n=25) was used. The experimental group participated in the program once a week for eight weeks. Data were analyzed using descriptive statistics and Chi square and t-test with the SPSS Win 17.0. Results: There were significantly increased in shoulder joint function (p=.012), body image (p=.001), and self-esteem (p=.013), and significantly decreased in stress (p=.003). Conclusion: The results suggest that breast cancer-overcome BeHaS exercise program had beneficial effects on shoulder joint function, body image, self-esteem and stress in patients who have had surgery for breast cancer.

결핵은 새로운 항결핵제의 개발, 생활 여건의 향상, 예방 대책의 발달로 유병률이 적어진 질환으로 알려지고 있으나, 최근 예방 접종률의 저하와 항결핵제에 내성을 보이는 균주가 출연하는 등 다시 증가하는 추세이며 또한, 견관절 질환에 대한 관심이 높아지며 일차 치료로써 다양한 견관절내 주사 치료가 시행되면서 화농성 견관절염 역시 증가하는 추세이다. 이에 저자들은 견관절의 결핵과 화농성 관절염이 동반 감염된 증례를 경험하여 만족할 만한 결과를 얻었기에, 관련 문헌과 함께 보고하는 바이다.

Radiographic osteolysis after total shoulder arthroplasty (TSA) remains a challenging clinical entity, as it may not initially manifest clinically apparent symptoms but can lead to clinically important complications, such as aseptic loosening. A thorough consideration of medical history and physical examination is essential to rule out other causes of symptomatic TSA-namely, periprosthetic joint infection-as symptoms often progress to vague pain or discomfort due to subtle component loosening. Once confirmed, nonoperative treatment of osteolysis should first be pursued given the potential to avoid surgery-associated risks. If needed, the current surgical options include glenoid polyethylene revision and conversion to reverse shoulder arthroplasty. The current article provides a comprehensive review of the evaluation and management of osteolysis after TSA through an evidence-based discussion of current concepts.

Despite its importance for the understanding of joint kinematics in vivo, there has been few studies about shoulder joints. The purpose of this study is to analyze the glenohumeral joint during internal and external rotation at 90 degrees of abduction using in vivo noninvasive motion analysis system. MRI was performed for the following seven positions from maximum internal rotation to maximum external rotation at intervals of 30 degrees. We used 3D-gradient echo sequencing (TR: 12 ms, TE: 5.8 ms, 0.8 mm-slice thickness). Our method is based on matching three-dimensional MR images by the similarity of the image intensity. We analyzed the in vivo three-dimensional motions of the glenohumeral and scapulothoracic joint during this motion. In scapla plane, the mean rotation angle of the glenohumeral join was 105.5 degrees ($SD{\pm}39.0^{\circ}$). The mean rotation angle of the scapulothracic joint was 27.5 degrees ($SD\;{\pm}\;7.7^{\circ}$). The contribution ratio is almost 3.8:1 of glenohumeral and scapulothracic joint respectively.

Typically, massive rotator cuff tears have stiff and retracted tendon with poor muscle quality, in such cases orthopaedic surgeons are confronted with big challenging to restore the cuff to its native footprint. Furthermore, even with some restoration of the footprint, it is related with a high re-tear rate due to less tension free repair and less tendon coverage. In this tough circumstance, the partial repair has yielded satisfactory outcomes at relatively short follow-up by re-creating the transverse force couple of the rotator cuff. Through this partial repair, the massive rotator cuff tear can be converted to the "functional rotator cuff tear" and provide improvement in pain and functional outcomes in patient's shoulder.

견봉-쇄골 관절 낭종은 비교적 드물게 발생되나, 대부분의 경우 만성적인 회전근 개 질환과 퇴행성 견봉-쇄골관절염에서 동반된다. 이러한 병변에 대한 임상적 해석, 치료적 접근 및 치료 후 합병증을 최소화하기 위해서는 견관절 질환과의 병태생리학적 기전을 이해하는 것이 필수적이라 할 수 있다. 저자들은 단순 흡입술 후 재발되는 견봉-쇄골 낭종에 대한 추가적인 단순 절제술 이후 재발성 무균성의 누공이 합병된 증례와 함께 초음파 검사상 누공을 가진 낭종성 병변과 동반된 광범위한 회전근 개의 파열을 진단하였던 예를 경험하였기에 보고하고자 하며, 문헌고찰을 통하여 견봉-쇄골 관절 낭종의 발생 기전을 고려한 임상적 접근 및 치료적 방법들을 재조명하고자 한다.

Arthroscopy of the stiff elbow joint is a technically difficult procedure because of the decreased joint space of the elbow joint. even to experienced surgeons. Problems encountered include limited access of instrument to the intra-articular ,pace and an increased risk of cartilage injury in the contracted elbow joint. This study describes a novel transarticular approach for elbow arthroscopy that allow, the safe and effective creation of the proximal medial and lateral portals.

Infection on sternoclavicular joint after electrical burn is rare. No case was reported previously in Korea. Even though the disease is rare, we treated the case successfully with intravenous antibiotics followed by curettage and drainage. Successful treatment was achieved in the case of infection on sternoclavicular joint after electrical burn.
