Shortened Problem Gambling Severity Index for Telephone Surveys (전화조사를 위한 단축형 한국판 문제 도박 지표의 개발)

  • This paper discusses how we developed an index for measuring problem gambling severity and determined the cutoff score for identifying addicts. This index uses fewer items than the current index and will be particularly useful for telephone surveys. We selected four items among the nine Korean Problem Gambling Severity Index (K-PGSI) items based on our statistical analysis of a survey of 501 gamblers. We applied this four-item index to a survey of 1,584 adults. The analysis of this survey proved the validity of the reduced index. The sensitivity and specificity of this index was 1.00 and 0.99 respectively, showing again the usefulness of this scale.

Percentile Approach of Drought Severity Classification in Evaporative Stress Index for South Korea (Evaporative Stress Index (ESI)의 국내 가뭄 심도 분류 기준 제시)

  • Drought is considered as a devastating hazard that causes serious agricultural, ecological and socio-economic impacts worldwide. Fundamentally, the drought can be defined as temporarily different levels of inadequate precipitation, soil moisture, and water supply relative to the long-term average conditions. From no unified definition of droughts, droughts have been divided into different severity level, i.e., moderate drought, severe drought, extreme drought and exceptional drought. The drought severity classification defined the ranges for each indicator for each dryness level. Because the ranges of the various indicators often don't coincide, the final drought category tends to be based on what the majority of the indicators show and on local observations. Evaporative Stress Index (ESI), a satellite-based drought index using the ratio of potential and actual evaporation, is being used as a index of the droughts occurring rapidly in a short period of time from studies showing a more sensitive and fast response to drought compared to Standardized Precipitation Index (SPI), and Palmer Drought Severity Index (PDSI). However, ESI is difficult to provide an objective drought assessment because it does not have clear drought severity classification criteria. In this study, U.S. Drought Monitor (USDM), the standard for drought determination used in the United States, was applied to ESI, and the Percentile method was used to classify drought categories by severity. Regarding the actual 2017 drought event in South Korea, we compare the spatial distribution of drought area and understand the USDM-based ESI by comparing the results of Standardized Groundwater level Index (SGI) and drought impact information. These results demonstrated that the USDM-based ESI could be an effective tool to provide objective drought conditions to inform management decisions for drought policy.

Peak drought index analysis of cheongmicheon watershed using meteorological and hydrological drought index (기상학적 및 수문학적 가뭄지수를 이용한 청미천 유역의 첨두가뭄지수 분석)

  • This study analyzed the peak drought severity and drought duration of the Cheongmicheon watershed from 1985 to 2015 to assess the lag time of peak drought severity between several drought indices. Standardized Precipitation Index (SPI) based on precipitation and Standardized Precipitation Evapotranspiration Index (SPEI) based on precipitation and evapotranspiration were applied as meteorological drought indices. Streamflow Drought Index (SDI) based on runoff data was applied as hydrological drought index. In case of SDI, we used Soil and Water Assessment Tool (SWAT) model for simulation of daily runoff data. As a result, the time of peak drought severity of SDI occurred after the occurrence of SPI and SPEI. The lag time for the peak drought severity, on average, between SDI and SPI was 0.59 months while SDI and SPEI was 0.79 months. As compared with SDI, the maximum delay was 2 months for both SPI and SPEI. This study results also shows that even though the rainfall events were able to cope with meteorological droughts, they were not always available to solve the hydrological droughts in the same time.

Balthazar Computed Tomographic Severity Index Application for Experimental Acute Necrotizing Pancreatitis in Dogs (개의 실험적 급성 괴사성 췌장염 평가에서 Balthazar Computed Tomographic Severity Index의 적용)

  • Computed tomography (CT) is considered as gold standard in evaluating pancreatitis in human, but there have been only a few studies in veterinary field. Balthazar CT severity index (CTSI) used to assess the severity of acute necrotizing pancreatitis in human could be applicable to dogs, because the severity of acute pancreatitis depends on the area of pancreatic necrosis in dogs more than in human. In this study, 25 adult, clinically healthy beagle dogs were used. CT examinations was performed in normal pancreas, positive control group and acute necrotizing pancreatitis induced by autologous bile injection. Balthazar CTSI was applied to canine acute necrotizing pancreatitis in contrastenhanced CT image and compared with the result of histopathologic examination. The Hounsfield unit (HU) of normal canine pancreas was $52.44{\pm}4.58$ and the density was significantly decreased in acute necrotizing pancreatitis (P < .05). In contrast-enhanced CT examination, pancreatic density was decreased significantly and this area was compatible to pancreatic necrosis. Balthazar CTSI showed positive correlation with histopathologic evaluation with a sensitivity of 100% and a specificity of 88.89%. Balthazar CTSI can be applied to evaluate the severity of acute necrotizing pancreatitis in dogs.

Development of a Tool to Identify Severity of Illness Index in High Risk Newborns (고위험 신생아의 중증도 측정도구 개발)

  • Purpose: The purpose of this study was to develop a tool to assess the severity of illness in high risk newborns. Method: The research design was a methodological study. The tool was developed in 4 stages: first, preliminary items were developed based on a questionnaire about the severity of illness index that was given to 8 health professionals in Neonatal Intensity Care Units (NICU) second, a panel of specialists reduced the preliminary items using 3 validity tests; third, final items were selected from the results of a pre-test. Finally, from July 2005 to May 2006, reliability and validity were tested with a sample of 160 high risk newborns admitted to the NICU. Results: The final tool to identify the severity of illness index in high risk newborns consisted 39 items and Cronbach's alpha coefficient for internal consistency was .922. Using factor analysis, 4 factors were extracted and these factors explained 54.451% of the total variance. Conclusion: The instrument for assessing the severity of illness in high risk newborns developed in this study was identified as a tool with a high degree of reliability and validity. In this sense, this tool can be effectively utilized for assessing and implementing care for high risk newborns.

  • 본 논문은 전압 이벤트 현상 중 순간전압강하(Sag) 현상에 초점을 맞추었다. Sag 현상의 심각한 정도를 표현하는 심각도(Voltage Sag Severity) 지수는 동일 지속시간에 대한 임계치와의 비로 표현하였다. 제안하는 확장된 심각도(Expanded Severity) 지수는 sag현상의 분포에 따른 일시반복성의 정보를 표현하였다. 기존의 임계치를 표현하는 ITIC curve를 기반으로 된 심각도와 sag 현상이 발생하는 지속시간-전압 그래프의 분포를 fuzzy clustering을 통하여 medoid를 측정하고, medoid의 심각도와 실제 임계치에 근접한 sag 지점의 심각도를 계산하여 비교하였다. 확장된 심각도 지수는 심각도가 높은 현상들과의 연계성을 나타내는 지수로 심각한 정도의 수치 정보 이외에 일시적인 현상인지 지속 반복적인 현상인지를 0과 1사이의 수치로 표현하였고, 실험을 통하여 입증하였다.

Drought analysis of Cheongmicheon watershed using meteorological, agricultural and hydrological drought indices (기상학적, 농업학적, 수문학적 가뭄지수를 이용한 청미천 유역의 가뭄 분석)

  • This study assessed drought of Cheongmicheon watershed from 1985 to 2015 according to duration. In order to quantify drought, we used meteorological and hydrological drought index. Standardized Precipitation Index(SPI) based on precipitation and Standardized Precipitation Evapotranspiration Index(SPEI) based on precipitation and evapotranspiration were applied as meteorological drought index. Palmer Drought Severity Index(PDSI) and Stream Drought Index(SDI) based on simulation of Soil and Water Assessment Tool(SWAT) model were applied as agricultural and hydrological drought index. As a result, in case average of extreme and averaged drought, 2014 and 2015 have the most vulnerable in all drought indices. Variation of drought showed different trend with regard to analysis of frequency. Also, the extreme and averaged drought have high correlation between drought indices excluding between PDSIs. However, each drought index showed different occurrence year and severity of drought Therefore, drought indices with various characteristics were used to analysis drought.

Severity Measurement Methods and Comparing Hospital Death Rates for Coronary Artery Bypass Graft Surgery (관상동맥우회술의 중증도 측정과 병원 사망률 비교에 관한 연구)

  • Objective : Health insurers and policy makers are increasingly examining the hospital mortality rate as an indicator of hospital quality and performance. To be meaningful, a risk-adjustment of the death rates must be implemented. This study reviewed 5 severity measurement methods and applied them to the same data set to determine whether judgments regarding the severity-adjusted hospital mortality rates were sensitive to the specific severity measure. Methods : The medical records of 584 patients who underwent coronary artery bypass graft surgery in 6 general hospitals during 1996 and 1997 were reviewed by trained nurses. The MedisGroups, Disease Staging, Computerized Severity Index, APACHE III and KDRG were used to quantify severity of the patients. The predictive probability of death was calculated for each patient in the sample from a multivariate logistic regression model including the severity score, age and sex to evaluate the hospitals' performance, the ratio of the observed number of deaths to the expected number for each hospital was calculated. Results : The overall in-hospital mortality rate was 7.0%, ranging from 2.7% to 15.7% depending on the particular hospital. After the severity adjustment, the mortality rates for each hospital showed little difference according to the severity measure. The 5 severity measurement methods varied in their statistical performance. All had a higher c statistic and $R^2$ than the model containing only age and sex. There was a little difference in the relative hospital performance evaluation by the severity measure. Conclusion : These results suggest that judgments regarding a hospital's performance based on severity adjusted mortality can be sensitive to the severity measurement method. Although the 5 severity measures regarding hospital performance concurred, more often than would be expected by chance, the assessment of an individual hospital mortality rates varied by the different severity measurement method used.

A Clinical Report of Two Patients with Insomnia by Korean Traditional Treatment and Progressive Muscle Relaxation (한방치료와 점진적 근육이완법으로 호전된 불면 환자 치험 2례)

  • Objectives : This study was designed to report the effect of Korean traditional treatment and progressive muscle relaxation for insomnia. Methods : We treated two patients scored more than 15 point at insomnia severity index by korean traditional treatment and progressive muscle relaxation. Results : After treatment, sleep duration and sleep quality was improved. Insomnia Severity Index, Pittsburgh Sleep Quality Index score were decreased. Conclusions : We may conclude that korean traditional treatment and progressive muscle relaxation is effective in patients with insomnia.

Application of Meteorological Drought Indices for North Korea (북한지역에 대한 기상학적 가뭄지수의 적용)

  • North Korea is one of the vulnerable countries facing the threat of a drought, so that it is unavoidable to experience fatal damage when drought is occurred, and it is necessary to improve the drought response capability of water resources systems. However, it is still difficult to find research efforts for drought characteristics and drought management in North Korea. This study is to quantify drought duration and magnitude and to analyze drought characteristics in North Korea. In order to quantitatively identify historical drought conditions and to evaluate their variability, drought indices are commonly used. In this study, drought indices including dry-day index, deciles of normal precipitation, Phillips drought index, standardized precipitation index and Palmer drought severity index are calculated and compared monthly using the weather data for the twenty one meteorological stations in North Korea. The indices compared with the drought damage records that have reported from 1990 to present to understand how the indices can explain the drought. A comparative study was also conducted to evaluate the relative severity of the significant droughts occurred during 2000 and 2001 which were reported as the worst drought in North Korea. Drought indices calculated from this study demonstrated that those can be the effective tools in quantitatively evaluating drought severity and measures of drought. Thus it is recommended the distributed trend of drought be considered when the plan or measures for drought in North Korea are established.
