Polypharmacy is increasing owing to an increase in the elderly population and multimorbidities associated with the increased risk of administration of potentially inappropriate medications (PIMs). The negative effects of polypharmacy on various health conditions and aspects, such as fall, fracture, mortality, cognitive function, and dementia, have been reported. The management of excess and inappropriate polypharmacy through proper interventions and local or national guidelines has been highlighted. The purpose of polypharmacy management is to appropriately prescribe medicines that are essential to treat diseases in patients and to avoid inappropriate polypharmacy, such as interactive or duplicate medicines under prescription and PIMs for specific diseases. Community pharmacists in Australia, the EU, USA, and Japan are collaborating with prescribers to review medications to ensure that the patients can be prescribed appropriate medications. The service cost is reimbursed by public or private insurers. A study in the United States has shown that even with medication review costs, the overall medication cost has reduced. In Korea, various projects such as Drug Utilization Review service and safe use of medicines have been conducted; however, no national guidelines or management measures have been established. It is necessary to implement a national long-term plan on polypharmacy management. Furthermore, a phased implementation plan is required. Shortly, active medication review services and education programs for healthcare professionals with the support of the government should be considered in Korea with reference to other countries in order to raise awareness of seriousness and risks of inappropriate polypharmacy.
This study was uncertaken to find an effective management modality for patients with hypertension. Patients 60years or older were selected for this purpose from a general hospital and a welfare center in Pusan and from a public health clinic in Cheju Island. During the month of October in 1984. the management modality for hypertension. the incurred medical expenses. and knowledge about the hypertension were analyzed and compared. The following results were obtained: 1. The a erage total medical expense per year was highest at the general hospital followed by the public health clinic and the welfare center. 2. It was found that the patients chose the public health clinic for its low medical cost. the welfare center for its proximity as well as for its low medical cost. and the general hospital for its quality medical services Also. the study has shown that the prescription of drugs was the major service provided by the public health clinic whereas the blood pressure measurement. the prescription of drugs. and family planning were the primary service offered by the welfare center. Drug prescription and blood pressure measurement were two of the most important services by the general hospital. 3. It was shown that the knowledge of hypertension was highnest at the welfare center whereas the management of hypertension was highest at the public health clinic. Based on above results and considering the chronic nature. it is believed that family awareness of the illness was one of the most important aspects for effective management of the hypertension for the elderly patients. In addition. the health care clinics at every level should come forward to execition of the educational programs and home nursing schomes for the effective management of the hypertension patients.
Background: The number of colorectal cancer (CRC) patients in Taiwan has increased in recent years; therefore, the effective dissemination of information related to symptom care has become especially important. Previous studies indicated that the physical and psychological status of cancer patients can be effectively improved by telephone counseling services (TCS). Thus, determining the most effective means of establishing a TCS to support the clinical practice of oncology has become a crucial goal for nursing. The purposes of this study were to analysis the content of the TCS for CRC and explore stratification of the TCS. Materials and Methods: The study design was retrospective. A total of 850 calls were made to CRC patients in the cancer center of Southern Taiwan during the period of January 2007- December 2011. A structure questionnaire was adopted to analysis satisfaction. Results: Responses provided by the TCS included information regarding nutrition, side effects resulting from chemotherapy and pain. Moreover, 28.7% of CRC patients needed advanced treatment. More than 90% satisfaction with all aspects of the calls was found. Conclusions: The TCS coulkd be shown to provide an effective means by which to expand the reach of nursing care to different times, places and patients, allowing for greater cost efficiency and more rapid service.

As internet traffic increases, the problem that it should be efficiently accepted in to the existing voice network in the pending problem importantly to the existing communication corporations. The feature of next generation exchange network is made up of the form of integration network that connect data traffic(internet service. Etc) with the existing voice network and it can be showed with very diverse aspects according to the constitution time of network or the characteristics of business. The progress strategy that develop the existing circuit based communication network into packet-based communication network can be divided into two in a large scale according to the application position These are VoDSL application method(Technology that packetize access network) and softswitch technology application method(after packetizing relay network, packetizing that into the access network). In this paper, we will deduce the desirable technology that can construct packet-based next generation exchange networks in the structure of the existing communication network environment. We will perform the research on a device to offer the necessary core technique VoSDL service with realistic resolutions primarily.
Real-time streaming services over the Internet have increased with the explosive growth of the various mobile platforms, with a focus on smart phones, and the demand for them is growing. In addition, the bandwidth occupied by the streaming services over the Internet had already surpassed 50% in 2010. Because of the shortage of network bandwidth for multimedia services traffic, restrictions on quality and capacity will become more and more serious. CCN is a future Internet architecture that improves how existing host-based Internet architecture handles content-oriented structure, but it is designed for the transmission of general contents and is not suitable for transmitting real-time streaming contents. In this paper, we focus on the inefficient aspects of CCN and propose a hierarchical grade-based scheme for real-time service for a more efficient environment in real-time streaming services. Experiments have shown better performance in terms of bandwidth, network load, and reliability.

The principle objective of this study is to evaluate the fatigue behavior of structural steel components of highway bridges subjected to service stresses. The main aspects of this investigation are; 1) a measurement and statistical analysis of service stress cycles observed in highway bridge. 2) fatigue tests under equivalent constant-amplitude(CA) loading and simulated variable-amplitude(VA) loading 3) a evaluation of the fatigue behavior under VA-loading by eqivalent root mean cube (RMC) stress range. Theoretically, the RMC model is adequate in evaluation of fatigue behavior under VA-loading, because the regression coefficient (m) of crack growth rate is 3 approximately. The result of fatigue test shows that the RMC model is fitter than the current RMS model in fatigue evaluation under VA-loading. The interaction effects and sequence effects under VA-loading affect little fatigue life of structural components. As the transition rate of stress ranges is higher, the crack growth rate is higher.

The purpose of this study is to analyze service quality factors that affect user satisfaction of Open Access Korea(OAK) services and to draw strategic assignments for activating open access activities in Korea through a user satisfaction survey. User satisfaction indexes were developed to measure all aspects of the OAK services. The quality index, satisfaction index, and performance index were identified in this survey. According to the survey findings, the levels of social quality and social satisfaction are relatively high, this shows the positive evaluation and expectation of researchers for OAK services. However, relatively low level is identified in the area of service process quality, and many opinions about OA contents shortage in Korea represent the importance of the national level contents development strategy. Sustainable policy support, publishing of open access journals, management of institutional repository, OA governance system, expansion of researchers' participation, construction of global collaboration system are suggested as major implications to promote open access activities in Korea.

Serological surveillance programs in animal populations are becoming increasingly important to estimate prevalence of a specific disease and subsequently to document disease-free status in a region or a country. For these purposes, the programs need to be based on both theoretical and economical aspects from the designing phase. From Aujeszky's disease (AD)-eradication program point of view, group of animals (aggregates, herds) not individual animal is the more important sampling unit of concern. In this study the authors therefore attempted to compute an appropriate sample size tailored to a current surveillance program against AD, assuming that the goal of this program is either herd-level prevalence estimation or documentation of AD-freedom. For prevalence estimation, assuming a finite population with imperfect sensitivity (Se) and specificity (Sp) of ELISA kit for AD diagnosis, the number of herds present, expected herd prevalence, and desired accuracy for a certain level of confidence, sample size was estimated at herd-level in the first stage and individual animal-level in the second stage. A two-stage sampling design was used to calculate a sample size to indicate AD-freedom. In this instance, the computation was based on the possible detection of a predetermined prevalence at a certain herd-level Se and Sp. This study indicated that the sample size varied with predetermined confidence, tolerance, Se and Sp at herd- and animal-level, and within- and among-herd prevalence. In general, smaller sample size was required to estimate AD prevalence than to document of AD-freedom. Compared to individual-based samples, two-stage sampling strategy requires a larger sample size to show disease-freedom. Statistical considerations including herd-level test characteristics when designing surveillance program also are further discussed.
Several detailed aspects of medical care services during one year (Aug 1, $1982{\sim}$July 31, 1983) were compared between two community health sub-centers; one (A) was served by a local clinician, the other (B) by a trainee of preventive medicine. The results were analysed as follows: 1. Total spells of medical services during the same period in 'A' sub-center was 4,087 and that in 'B' 2,347. Medicaid visitors in 'A' was 1,051 (25.7% of the total), that in 'B' was 250 (10.7%). Significant difference was observed. 2. Average number of visits for the same case in 'A' was 2.0, that in 'B' was 1.4. Average duration of service by day for the same case in 'A' was 3.9, that in 'B' 3.0. 3. Average cost of service for a case in 'A' was 6,770 won, that in 'B' 4,230 won. 4. Difference in age distribution of visitors in the two sub-centers was insignificant. $0{\sim}9$ year group was 38.5% of the total. Difference in distribution by sex (between two sub-centers) was minimal. Male utilisers more than female by ca. 10%. 5. Concerning distribution of visitors by month, utilisation in July, August and September was the highest; the sum of them was 32.7% of the total in 'A' and 32.9% in 'B'. 6. Distribution by diagnoses in 'A' in decreasing order of frequency was Respiratory system diseases (26.1%), Digestive system diseases (23.0%), Diseases of skin and subcutaneous tissue (17.4%), Accidents and poisoning (11.5%) and Diseases of musculoskeletal system and connective tissue (5.4%). That in 'B' was Respiratory system diseases (39.0%), Digestive system diseases (23.0%), Diseases of skin and subcutaneous tissue (14.2%), Accidents and poisoning (10.9%) and Infection and parasitic diseases (9.1%). 7. The 10 most frequent diagnoses comprised 68.5% of the total in 'A' and 65.5% in 'B' 8. Utilisation by accessibility to each sub-center was studied. It is shown that the nearer the distance or the easier the accessibility, the more visits to services. The distinct pattern is manifested in the utilisation in 'A'-Myun.
Bull-boxing in South Korea is a form of competitions in which bulls measure their strength with each other in the form of pushing by putting their heads together. Bull-boxing has been admitted after being designated as a play culture by the government because it does not cause great injuries to animals' bodies and has strong positive aspects such as contributing to livestock farmers' leisure activities, encouragement of animal husbandry, and communities' economic development. However, bull-boxing sometimes causes damage to the head and horns due to the heavy bodies. According to the results of examinations of these damage cases, damage that caused irregular shapes of the base of horns was identified in 31.5% of fighting bulls. The damage to horns is thought to have been caused by the great forces of fighting bulls repeatedly imposed to each other's horns during bull-boxing that caused minute fractures leading to damage to the blood vessels inside the horns resulting in the blocking of delivery of nutrients to the horns causing the deformation of the horns into abnormal shapes. Since bulls' bodies are injured during bull-boxing although the injuries are small as bull-boxing is mainly conducted by pushing, animal protection groups regard bull-boxing as cruelty to animals and request to stop bull-boxing. The present study aims to develop animal welfare type headgears that can protect bulls' horns and heads in order to protect fighting bulls during bull-boxing in terms of animal protection and welfare.
