The relationship of sperm DNA integrity with serum vitamin levels (folate and cobalamin) and food consumption in infertile men

  • Objective: The aim of this study was to investigate the relationships of serum folate (vitamin B9), cobalamin (vitamin B12) levels and diet with semen parameters (semen standard parameters [SSP] and DNA fragmentation index [DFI]) in infertile men. Methods: Sperm samples were assessed for SSP and DFI (using the sperm chromatin dispersion test). Serum vitamin concentrations were measured with an immuno-electrochemiluminescence assay, and men completed a semi-quantitative food frequency questionnaire (FFQ). Results: Serum folate levels were positively correlated with sperm progressive motility and DFI. A comparison of SSP between two groups of patients according to serum folate concentration (B9 <4.840 ng/mL and B9 ≥4.840 ng/mL) showed significantly higher sperm concentration and sperm progressive motility in the latter group. However, there was no difference between these groups regarding DFI. Interestingly, serum folate levels were significantly higher in patients with a high DFI (using the cut-offs of 30% or 18%). FFQ data showed that the consumption of fruits and egg yolk correlated positively with sperm concentration and sperm motility, respectively. Conclusion: Serum folate levels showed significant associations with sperm concentration and sperm progressive motility. However, the positive association of serum folate with DFI raises the need for careful prescription of folate supplements.

A Change of Serum Folate and Vitamin $B_{12}$ Concentrations of Maternal and Umbilical Cord Blood during Pregnancy (임신 중 모체와 신생아 제대 혈청의 엽산과 비타민 $B_{12}$ 농도 변화)

  • Folate and Vitamin $B_{12}$ are essential nutrients important during pregnancy. This study was conducted to evaluate the folate and vitamin $B_{12}$ nutritional status of Korean pregnant women and to investigate the relationship between maternal­umbilical cord serum folate and vitamin B12 levels and pregnancy outcomes. Dietary intakes of the pregnant women were estimated by 24 hour-recall (3 times). Serum folate and vitamin B12 levels in maternal blood and umbilical cord of 27 pregnant women at 1'st-, 2'nd-, 3'rd-trimester and delivery were measured by RIA (radioimmuno assay), respectively. Means of folate and vitamin $B_{12}$ intake were $283.53\pm58.01{\mu}g/day\;and\; 2.99\pm1.32mg/day$, respectively. Maternal mean serum folate levels of the trimester and delivery were $9.75\pm3.60ng/ml,\;10.46\pm4.63ng/ml,\;10.71\pm4.14ng/ml\;and\;15.05\pm7.04ng/ml$. Those maternal levels were significantly lower than that of umbilical cord blood $(23.99\pm9.42ng/ml)$. Serum vitamin $B_{12}$ levels of maternal trimester and delivery were $479.07\pm137.56 pg/ml,\;310.96\pm137.56pg/ml,\;308.22\pm74.65pg/ml,\;and\;295.67\pm93.36pg/ml$, which were significantly lower than those of umbilical cord blood $(500.13\pm185.60ng/ml)$. This finding indicates that the uptake of folate and vitamin $B_{12}$ in the fetus may be due to an active placental transport mechanism. Maternal serum level correlated positively with those of umbilical cord blood, showing that folate and vitamin $B_{12}$ concentration of umbilical cord blood might be affected by maternal status. There was no significant correlation between the serum folate levels in maternal-umbilical cord blood and the pregnancy outcomes. However, maternal vitamin $B_{12}$ level at l'st trimester was significant positive correlation between the gestational age except for birth weight and weight gain.

Vitamin B Status and Serum Homocysteine Levels in Infertile Women (불임 여성의 비타민 B 영양 상태 및 혈청 호모시스테인 수준)

  • Adequate vitamin B$_2$ vitamin B$_{6}$, folate and vitamin B$_{12}$ nutrition is known to be important for reproductive function in women of childbearing age. The purpose of this study was to evaluate serum vitamin B$_2$ vitamin B$_{6}$, folate and vitamin B$_{12}$ status and serum homocysteine levels in 115 women aged 33.2$\pm$4.0 years, who had been diagnosed with infertility, and 49 women aged 34.5$\pm$3.8 years having at least one born child. Total vitamin B$_2$ and vitamin B$_{6}$ intakes in infertile women were significantly lower than those in control. Serum vitamin B$_2$ vitamin B$_{6}$, folate and vitamin B$_{12}$ concentrations were significantly lower in infertile women than those in control and serum homocysteine levels were significantly higher in infertile women than those in control. Thirteen percent in infertile women and zero percent in control were assessed as hyperhomocysteinemic and there was a significant difference in the prevalence of hyperhomocysteinemia between infertile women and control. 41% infertile women were assessed as folate deficiency. Serum folate concentrations was negatively correlated with serum homocysteine of the infertile women and control. Total vitamin B$_2$ intakes was negatively correlated with serum homocysteine of the infertile women and control. Total vitamin B$_{6}$, folate intakes were negatively correlated with homocysteine of infertile women only. In conclusion, infertile women are needed to intake more B vitamins intakes. Comparison of Serum Homocysteine, Folate and Vitamin B12 Level in Korean Schizophrenics (한국 정신분열병 환자에서의 혈중 Homocysteine, 엽산, Vitamin B12 농도 비교연구)

  • Objective:There have been a kind of transmethylation theory that high homocysteine serum concentration affects schizophrenia by neurotoxic mechanism and clinical reports that some schizophrenic patients with high homocysteine were improved by high folate ingestion. This study was done to confirm previous research results and find the clinical characteristics of schizophrenia showing high serum homocysteine and low folate. Method:We compared the serum levels of homocysteine, folate and vitamin B12 level between 234 schizophrenic patients(male 99, female 135) group and 234 normal controls(male 99, female 135) group. The subjects of two groups were age and sex matched. The evaluated clinical characteristics items were sex, age, onset of disease, hereditary loading, disease course, hallucination and subtype of schizophrenia. Results:1) Homocysteine level of the schizophrenia group was significantly higher than the normal control group and folate level of the schizophrenia group was significantly lower than the normal control group. Homocysteine level was more negatively correlated with folate level in the schizophrenia group than the normal control group. 2) The percentage of high homocysteine(above 12.46umol/L;90 percentile of normal control) was 33.8% of schizophrenia patients and 51.5% of male schizophrenia. The percentage of low folate(below 3.8nM/L;bottom tertile of normal control) was 66.2% of schizophrenia. 3) In low folate group and not-low folate group, schizophrenia showed significantly higher homocysteine level than normal control. Especially, low folate schizophrenia group showed significantly higher homocysteine level than low folate normal control group. Conclusions:Some schizophrenia patients with high serum homocysteine may be genetic defector and having low folate serum level. In that case, folate ingestion could be a good management for clinical improvement.

  • The purpose of this study was to evaluate whether the MTHFR variants, folate and vitamin $B_{12}$ deficiencies increase the risk of hyperhomocysteinaemia and adverse pregnancy outcome such as short gestational age or reduced birth weight. Healthy pregnant women (n=136; 24-28 gestational weeks; 20-40 years old), who visited Ewha Womans University Hospital for prenatal care, participated in this study. At the time of delivery, trained nurses recorded the pregnancy outcome from medical chart. We determined maternal MTHFR polymorphisms (C to T subsitution at nucleotide 677) and measured serum homocyteine, vitamin $B_{12}$, and folate concentrations. We compared serum homocysteine level by MTHFR genotype, serum folate and serum vitamin B12 levels using ANOVA. To evaluate the association between serum homocysteine level and pregnancy outcome, we compared the gestational age and birth weight by serum homocysteine levels using multiple regression analysis, adjusting for other potential predictors. Mean level of serum homocysteine was highest among pregnant women of the MTHFR variants with low levels of serum folate and vitamin $B_{12}$. Regarding association with birth outcome, we found the relationship between homocysteine levels and increased gestational age (p=0.03) and reduced birth outcome (p>0.05). Our data demonstrates that serum level of folate and vitamin $B_{12}$ among pregnant women affects significantly serum homocysteine levels, and the genetic polymorphism of MTHFR modulates the relationship between them. However, we did not have conclusive evidence of association between high homocysteine level and adverse pregnancy outcome such as preterm or low birth weight.

Folate Deficiency and FHIT Hypermethylation and HPV 16 Infection Promote Cervical Cancerization

  • Fragile histidine triad (FHIT) is a suppressor gene related to cervical cancer through CpG island hypermethylation. Folate is a water-soluble B-vitamin and an important cofactor in one-carbon metabolism. It may play an essential role in cervical lesions through effects on DNA methylation. The purpose of this study was to observe effects of folate and FHIT methylation and HPV 16 on cervical cancer progression. In this study, DNA methylation of FHIT, serum folate level and HPV16 status were measured using methylation-specific polymerase chain reaction (MSP), radioimmunoassay (RIA) and polymerase chain reaction (PCR), respectively, in 310 women with a diagnosis of normal cervix (NC, n=109), cervical intraepithelial neoplasia (CIN, n=101) and squamous cell carcinoma of the cervix (SCC, n=101). There were significant differences in HPV16 status (${\chi}^2=36.64$, P<0.001), CpG island methylation of FHIT (${\chi}^2=71.31$, P<0.001) and serum folate level (F=4.57, P=0.011) across the cervical histologic groups. Interaction analysis showed that the ORs only with FHIT methylation (OR=11.47) or only with HPV 16 positive (OR=4.63) or with serum folate level lower than 3.19ng/ml (OR=1.68) in SCC group were all higher than the control status of HPV 16 negative and FHIT unmethylation and serum folate level more than 3.19ng/ml (OR=1). The ORs only with HPV 16 positive (OR=2.58) or with serum folate level lower than 3.19ng/ml (OR=1.28) in CIN group were all higher than the control status, but the OR only with FHIT methylation (OR=0.53) in CIN group was lower than the control status. HPV 16 positivity was associated with a 7.60-fold increased risk of SCC with folate deficiency and with a 1.84-fold increased risk of CIN. The patients with FHIT methylation and folate deficiency or with FHIT methylation and HPV 16 positive were SCC or CIN, and the patients with HPV 16 positive and FHIT methylation and folate deficiency were all SCC. In conclusion, HPV 16 infection, FHIT methylation and folate deficiency might promote cervical cancer progression. This suggests that FHIT may be an effective target for prevention and treatment of cervical cancer.

Clinical Significances of Serum Vitamin $B_{12}$, Folate and Eerritin Levels in Patients with Malignant Tumors (악성종양에서 혈청 비타민 $B_{12}$, 엽산 및 Ferritin의 진단적 의의)

  • In order to evaluate the clinical significances of the serum vitamin $B_{12}$, folate and ferritin levels in patients with malignant tumors, the levels were measured in 10 normal contol subjects, 70 patients with malignant tumors, 7 patients with liver cirrhosis and 25 patients with other benign diseases. The results are as follows: 1) In normal control subjects, mean serum values for vitamin $B_{12}$, folate and ferritin level were $588.80{\pm}131.58pg/ml,\;5.59{\pm}1.52ng/ml\;and\;89.22{\pm}42.78ng/ml$ retrospectively 2) There was no significant difference in serum levels between patients with benign diseases and normal control subjects. 3) The serum vitamin $B_{12}$ and ferritin levels in patients with liver cirrhosis were significantly higher than in normal control, and the serum folate levels in these patients were lower than in normal control subjects. 4) The serum vitamin $B_{12}$ and ferritin levels in patients with malignant tumors were significantly higher than in normal control subjects, and the serum folate levels in these patients were significantly lower than in normal control subjects. The above results suggest that the serum vitamin $B_{12}$ and ferritin may be useful as tumor markers in patients with malignant tumors.

Maternal Folate Status and Its Influencing Factors in Early Pregnancy (임신초기 모체의 엽산영양상태와 동 영향인자)

  • Folate nutrition in early pregnancy is crucial in order to prevent neural tube defects (NTDs) in the fetus and maternal folate deficiency in late pregnancy If the influencing factors on maternal folate status are identified we may be able to detect the women at most risk of folate deficiency. This study intends to determine folate intakes, assess the levels of serum folate, erythrocyte folate, and plasma homocysteine, and reveal theinfluencing factors on maternal folate status in early pregnancy. A total of 151 healthy women in their first trimester volunteered for this study. The average length of gestation period was 9.1$\pm$ 2.3 weeks and seventy subjects were primipara. They consumed 1599$\pm$589 ㎉/day of energy and 230.8$\pm$145.2 $\mu\textrm{g}$/day of folate. This represented 72.5% and 46.2% of the Korean RDA respectively for pregnant women in the first half of Pregnancy. Results show that they consume less folate and energy due to morning sickness. Morning sickness correlated negatively with the intakes of energy, folate, ana Kimchi also. Their levels of serum folate, erythrocyte folate, and plasma homocysteine were 5.5$\pm$1.9 ng/mL, 266.6$\pm$75.0 ng/mL, and 7.0$\pm$1.8 $\mu$mol/L, respectively. Results indicate that 7.8% were deficient and 60.3% were borderline deficient in serum folate, 4.3% were deficient in erythrocyte folate, however, all had normal levels of plasma homocysteine. Results indicate that the folate status was not poor Contributing factors influencing serum folate concentrations ($R^2$= 0.724, p = 0.0001) were self-reported health status (+), folate intake (+), age (-), length of gestation (-), and homocysteine levels (-). Factors influencing erythrocyte folate concentrations ($R^2$ = 0.570, p = 0.0029) were the infant birth height of the last pregnancy (+), energy intake (+), age (-), plasma homocysteine concentration (-), and education level (-). Factors influencing plasma homocysteine concentrations ($R^2$= 0.450, p = 0.0051) were income level (+), prepregnancy weight (+), serum folate concentration (-), and the infant birth weight of the last pregnancy (-). These results indicate that pregnant women are likely to have a folate deficiency if they are in poor health status, having a history of delivering small infant and low energy and/or folate intake, and/or are older. And folate status is likely to decline as pregnancy progresses.

Effects of Folic Acid and Ascorbate Supplementation on Plasma Homocysteine and Oxidative Stress in Patients with Type 2 Diabetes Mellitus (제2형 당뇨병 환자에게 엽산과 아스코르브산 보충이 혈장 호모시스테인 농도와 산화 스트레스에 미치는 영향)

  • In patients with type 2 diabetes, oxidative stress could be increased by their metabolic changes. Elevated plasma homocysteine is considered as one of markers of enhanced oxidative stress. Due to oxidative stress, some complications like cardiovascular or renal diseases may develop in type 2 diabetes patients. Plasma homocysteine concentration may be increased if folate status were inadequate. Protective effects against oxidative stress may be diminished if the status of anti-oxidative nutrient as vitamin C was poor. It is, therefore, important to maintain adequate status of folate and vitamin C in type 2 diabetes patients. Thus, this study was performed to determine the effects of supplementation of folate and/or ascorbate on blood glycated hemoglobin ($HbA_{1c}$) level, serum concentrations of homocysteine and cholesterol, plasma oxidized low density-lipoprotein (LDL), concentration and plasma glutathione peroxidase (GSH-Px) activity in the patients with type 2 diabetes. A total of 92 type 2 diabetes patients participated voluntarily with written consents. They were divided into one of the four experimental groups; Control (C), Folate-supplemented (F), Ascorbate-supplemented (A), and Folate plus ascorbate-supplemented (FA). The subjects in C were taken placebo, those in F were supplemented 1 mg of folate, those in A received 1,000 mg of ascorbate, and those in FA were given 1 mg of folate plus 1,000 mg of ascorbate daily for 4 weeks. Supplementation of folate or ascorbate resulted to increase serum folate level or plasma ascorbate concentration apparently, respectively. Folate supplementation not ascorbate seemed to decrease plasma concentrations of homocysteine and oxidized LDL and reduce plasma GSH-Px activity. There might not be synergic effect of the supplementation of folate plus ascorbate. The results indicate that oxidative stress in the patients with type 2 diabetes may lower mainly by folate supplementation.

Folate Status in Pregnant and Lactating Women (임신기와 수유기의 엽산 영양)

  • During pregnancy and lactation, folate status is important because folate requirements increase during the periods as well as maternal folate status influences on pregnancy outcome and human milk folate; especially folate deficiency around periconceptional period may induce neural tube defects(NTDs) of fetus. There have been a plenty of evidences that maternal folate status deteriorates during pregnancy of fetus. There have been a plenty of evidences that maternal folate status deteriorates during pregnancy and lactation if folate needed is not sufficiently provided. The Public health Service of the United States recommends all child-bearing is not sufficiently provided. The Public Health Service of the United States recommends all child-bearing women to intake 0.4mg of folate daily, and the Food and Drug Administration the folate status of child-bearing women and to reduce the rate of occurrence of NTDs. Many authors have insisted that the current recommended dietary allowances of folate for Americans are too low to maintain good folate status. There are little data about Korean folate status including pregant and lactating women. A couple of reports indicated that the folate intakes of Korean pregant and lactating women are below the Korean RDAs of folate and serum folate levels of them are subnormal. The authors pregnant and lactating women. Therefore, it is worth to review the assessment methods of folate status of pregnant and lactating women, folate RDAs for them, the relationships between maternal folate status and pregnancy outcome as well as human milk folate, the methods to increase folate intake, and the problems of large dose of folic acid supplementatiion.
