만성 폐쇄성 폐질환 환자의 기능적 용량 설명모형 (An Explanatory Model on Functional Capacity in Patients with Chronic Obstructive Pulmonary Disease)

  • Purpose: This study was conducted to develop and test an explanatory model on functional capacity in patients with chronic obstructive pulmonary disease using path analysis. Methods: Data were collected from 149 chronic obstructive pulmonary disease patients using 6-minute walk test, measurement of oxygen saturation, pulmonary function test, and self-reported questionnaires from June to October, 2005. The collected data were analyzed using SPSS/WIN 12.0 program and AMOS/WIN 4.0 program. Results: The overall fitness indices of modified model were good($x^2$ = 14.324, p = .281 GFI = .981, RMSEA = .006, AGFI = .944, NFI = .927, NNFI = .999, CFI = .999, PNFI = .613, $x^2$/df = 1.194). Functional capacity was influenced directly by age(${\beta}$ = -.304, p = .000), dyspnea(${\beta}$ = -.278, p = .000), self-efficacy(${\beta}$ = .240, p = .000), social support(${\beta}$ = .175, p = .004), pulmonary function(${\beta}$ = .169, p = .008), and oxygen saturation(${\beta}$ = .099, p = .048). These variables explained 39.3% in functional capacity. Conclusion: The findings of this study suggest that comprehensive nursing interventions should focus on decreasing dyspnea and increasing self-efficacy, social support, and oxygen saturation. In this perspective, pulmonary rehabilitation would be an effective strategy for improving functional capacity in patients with chronic obstructive pulmonary disease.

노년전기 여성의 악력 수준에 따른 낙상 두려움의 영향 요인 (Factors Influencing Fear of Falling among Women in the Early Phase of Old Age by Assessing Level of Hand Grip Strength)

  • Purpose: This study compared factors related to fear of falling among women in the early phase of old age based on hand grip strength. Methods: Researchers obtained data of 1,071 women who had participated in the 4th Korean Longitudinal Study of Aging. Data were analyzed using multiple regression analysis. Results: In both groups, fear of falling was positively correlated with falling experience within two years, osteoarthritis-related leg pain, depression, and instrumental daily life abilities; however, self-rated health was negatively correlated. In the low hand grip strength group, self-rated health, osteoarthritis-related leg pain, and hand grip strength accounted for 21.3% of the variance regarding fear of falling. In the normal hand grip strength group, age, self-rated health, fall experience within two years, osteoarthritis-related leg pain, and depression significantly accounted for 16.6% of the variance regarding fear of falling. Conclusion: In applying interventions to prevent falling in women in the early phase of old age, health care workers must verify if women have low hand grip strength and determine whether to focus on fitness improvement through exercise interventions or consider psychological interventions for depression.

간호사의 조직내 대인관계 역량에 관한 구조모형 (The Structural Modeling for Nurses' Interpersonal Competence within an Organization)

  • Purpose: This study was designed to identify a path model that anticipates the interpersonal competence of nurses by anticipating factors that explain interpersonal competence within an organization, and analyzing the effects of these factors. A hypothetical model was formulated based on a literature review of interpersonal competence. For the study, influential factors were divided into two variables: exogenous variables including communication style, coaching leadership, and social support, and endogenous variables of self-efficacy, job satisfaction, and organizational commitment. Methods: The sample included 202 hospital nurses. Data were collected through questionnaires and analyzed using SPSS and AMOS. Results: The overall fitness was good ($x^2=74.707$, p<.001), d.f=19, $x^2/df=3.932$, GFI=.940, AGFI=.826, RMR=.009). Social support, self-efficacy, horizontal communication and organizational commitment directly affected the interpersonal competence of the nurses, and informal type of communication, horizontal, upward communication and coaching leadership indirectly effected the interpersonal competence of the nurses. Horizontal communication, social support, self-efficacy, and organizational commitment explained 46.5% of the variance in interpersonal competence of nurses. Conclusion: Based on the study results, nurses in hospitals need informal types of communication, horizontal, upward communication and coaching leadership to increase their interpersonal competence.

자기-상대방 상호의존모형 분석을 적용한 임신부부의 건강증진행위에 관한 연구 (Health Promoting Behavior in Pregnant Couples: Actor-Partner Interdependence Model Analysis)

  • Purpose: This study aimed to identify the actor and partner effects of self-efficacy, marital adjustment, and social support on the health promoting behavior of Korean pregnant couples. Methods: Participants were 132 couples who met the eligibility criteria. Data were collected from June to November, 2016 at a community health center. The Actor-Partner Interdependence Model was used for analyzing the actor and partner effects of self-efficacy, marital adjustment, and social support on health promoting behavior. Results: The fitness indices for the model were GFI=0.90, NFI=0.92, CFI=0.91, TLI=0.90, and RMSEA=0.04, which satisfied the criteria. Self-efficacy had actor and partner effect on health promoting behavior of wives, but had only actor effect of on health promoting behavior of husbands. Marital adjustment showed actor and partner effect on the health promoting behavior of pregnant couples. Social support only had an actor effect on the health promoting behavior of wives. And, marital adjustment and social support had a mutual effect. Conclusion: This study indicates that the partner involvement is needed to develop health promotion programs for pregnant couples.

자기결정성이론 기반 관상동맥중재술 환자의 건강행위 이행 구조모형 (A Structural Model of Health Behavior Compliance in Patients with Percutaneous Coronary Intervention based on Self-Determination Theory)

  • Purpose: The purpose of this study was to construct and test a structural equation model of health behavior compliance among patients with percutaneous coronary intervention based on self-determination theory. Methods: A total of 227 participants who received follow-up care after percutaneous coronary intervention were recruited. A structured questionnaire was used to assess health providers' autonomous support, basic psychological needs, autonomous motivation, controlled motivation, type D personality, and health behavior compliance. Collected data were analyzed using SPSS 21.0 and AMOS 21.0 program. Results: The final hypothetical model showed a good fitness with data: GFI=.94, RMSEA=.07, CFI=.96, NFI=.92, TLI=.94. The results revealed that autonomous support of health care providers, basic psychological needs, and autonomous motivation, and D-type personality accounted for 51.8% of health behavior compliance. Conclusion: The findings of this study indicate that enhanced autonomous support of health care providers is essential to promote patients' basic psychological needs and autonomous motivation. This leads to maximized compliance to the health behaviors among patients who underwent percutaneous coronary intervention. We recommend that health care institutions establish various measures to foster the special environments in which health care providers can actively provide and utilize autonomous support for their patients.

융복합 시대에서 대학생의 성격강점(인간애) 및 자기통제력과 이타행동의 관계: 도덕적 고양의 매개효과 (The Relationship Between College Student's Character Strengths(Humanity), Self-Control, and Altruistic Behavior in the Times of Convergence: The Mediating Effect of Moral Elevation)

  • 본 연구의 목적은 융복합 시대에서 대학생의 성격강점(인간애) 및 자기통제력과 이타행동의 관계에서 도덕적 고양의 매개효과를 규명하기 위한 것이었다. 이를 위해 청주에 소재한 C대학교 재학생 647명을 대상으로 네 가지 측정도구인 성격강점(인간애), 자기통제력, 도덕적 고양, 이타행동 검사를 실시하였다. 수집된 자료를 가지고 SPSS 프로그램과 AMOS을 통한 확인적 요인분석과 구조방정식 모형 분석을 실시하였으며 최종 모델의 적합도 지수는 CFI=.937, TLI=.917, RMSEA=.077로 나타났다. 본 연구의 주된 연구결과는 첫째, 성격강점(인간애)와 자기통제력은 이타행동에 모두 유의한 영향을 미치는 것으로 나타났다. 둘째, 구조방정식 모형 분석을 실시한 결과 도덕적 고양은 성격강점(인간애), 자기통제력과 이타행동 관계에서 매개효과가 있는 것을 확인하였다.

20~30대 여성의 수영경력과 신체적 자아개념에 따른 수영복 선택기준과 구매만족도 (Selection Criteria and Swimsuit Purchase Satisfaction of Female Consumers According to Swimming Experiences and Physical Self-concepts)

  • This study analyzed the relationship among swimming experiences, swimsuit selection criteria, swimsuit purchase satisfaction level, and the physical self-concept of female consumers. This study was based on a descriptive survey method using a questionnaire. The survey was conducted from June 15 through July 20, 2012, and the sample consisted of 330 female consumers in their 20s and 30s residing in the Seoul and Gyeonggi area. Factor analysis and Cronbach's ${\alpha}$ coefficients, ANOVA, Duncan's Test, and multiple regression analysis were employed for the data analysis. The results revealed that individual self-concepts on health, sports competence, and fitness were influenced by swimming experiences. There was a tendency for those with a longer period of swimming experience to have a higher level of brand consideration as a swimsuit selection criterion; in addition, they showed a higher satisfaction level with swimsuits. Individual physical self-concept influenced the consideration given to each swimsuit selection criterion as well as swimsuit purchase satisfaction level. The findings of the study reflect the possibility of utilizing swimming experiences as a criterion for swimsuit market segmentation. Attention to the quality of swimsuits as well as to the physical self-concept of consumers are required for marketing activities.

노년기 주관적 건강상태와 우울 간의 종단적 상호인과관계: 자기회귀교차지연모형의 검증 (The Longitudinal Reciprocal Relationship between Self-rated Health Status and Depression in the Elderly : Testing the Autoregressive Cross-lagged Model)

  • 본 연구는 노년기 주관적 건강상태와 우울 간의 종단적 상호인과관계를 밝히는 데 있다. 이를 위해 전국단위의 조사자료인 고령화연구패널조사 제5차연도 자료부터, 제6차연도, 제7차연도 자료까지 추적 조사에 모두 응답한 65세 이상 3,363명을 대상으로 자기회귀교차지연모형을 검증하였다. 주요 분석결과는 다음과 같다. 첫째, 이전 시점의 주관적 건강상태는 이후 시점의 주관적 건강상태에 정(+)적으로 유의한 영향을 미쳤다. 둘째, 이전 시점의 우울은 이후 시점의 우울에 정(+)적으로 유의한 영향을 미치는 것으로 나타났다. 셋째, 조사대상 기간 동안 이전 시점의 주관적 건강상태는 이후 시점의 우울에 부(-)적으로 유의한 영향을 미치는 것으로 나타났으나, 그러나 이전 시점의 우울은 이후 시점의 주관적 건강상태에 유의미한 영향을 미치지 않았다. 연구결과에 기초해 노년기 초기 기초체력 증진을 위한 체계적인 정책의 필요성과 건강증진 프로그램의 확대 시행 및 통합적 우울 관리 프로그램 시행이 필요함을 제안하였다.

간호대학생의 주관적 행복감 경로모형 분석 (Validation of a Path Model for Subjective Happiness of Nursing Students)

  • 본 연구는 간호대학생의 자아존중감, 사회적지지, 자기효능감, 전공만족도 및 주관적 행복감의 관계에 대한 경로모형을 검증하고자 시도되었다. 간호대학생 416명을 대상으로 자가보고형 설문지를 이용하여 2017년 3월 10일부터 4월 10일까지 자료수집 하였다. 자료분석방법은 PASW 21.0 프로그램과 Amos 18.0 프로그램을 이용하였다. 연구결과는 다음과 같다. 첫째, 간호대학생의 자아존중감, 사회적지지, 자기효능감, 전공만족도 및 주관적 행복감 간의 상관관계가 나타났다. 둘째, 간호대학생의 주관적 행복감에 대해 자아존중감, 사회적지지, 자기효능감, 전공만족도가 직접효과가 있었으며, 자아존중감, 사회적지지, 자기 효능감은 주관적 행복감에 간접효과가 있었다. 셋째, 간호대학생의 전공만족도에 대해 자아존중감, 사회적지지, 자기 효능감이 직접효과가 있었으며, 자아존중감, 사회적지지는 주관적 행복감에 간접효과가 있었다. 간호대학생의 자기효능감에 대해 주관적 행복감에 직접효과가 있었다. 이러한 결과에서 간호대학생의 주관적 행복감에 영향을 미치는 자아존중감, 사회적지지, 자기효능감, 전공만족도 등의 요인을 고려한 행복감 증진프로그램을 개발해야 한다.

장기복무 직업군인의 긍정성과 가족의사소통이 조직몰입에 미치는 영향과 자기효능감의 매개효과 (The Effects of Positivity and Family Communication on Organizational Commitment and the Mediating Effects of Self-Efficacy between Them among Long-Term Professional Soldiers)

  • 본 연구는 직업군인의 긍정성과 가족의사소통이 조직몰입에 미치는 영향을 확인하고 이들의 관계에서 자기효능감의 매개효과를 알아보는 것이 목적이다. 이를 위해 2015년 국방부의 승인을 거친 직업군인들에 대한 연구에 대해 육 해 공 해병대 부대의 협조를 구하여 장기복무 직업군인 241명에게 설문조사를 거쳤으며 SPSS 21.0과 AMOS 21.0을 사용하여 분석하였다. 연구결과는 첫째, 장기복무 직업군인의 조직몰입은 긍정성, 가족의사소통 그리고 자기효능감과 모두 정적 상관관계를 가지는 것으로 나타났다. 둘째, 조직몰입에 영향을 미치는 변인을 모두 고려하여 분석한 결과, 측정모형과 구조모형의 적합도는 양호한 것으로 나타났으며 직업군인의 조직몰입에 대해 긍정성과 가족의사소통은 긍정적인 영향을 미치며 이들의 관계에서 자기효능감은 매개효과를 가지는 것으로 나타났다. 이와 같은 연구결과를 토대로 장기복무 직업군인의 조직몰입을 위한 효과적인 함의를 제시하고자 한다.
