Effects of Self-directed Practice through Writing Self-Reflective Journal on Basic Nursing Performance ability, Confidence in performance, Learning self-efficacy and Practice satisfaction (자아성찰 자율실습이 기본간호 수행능력 및 수행자신감, 학업적 자기효능감, 실습만족도에 미치는 효과)

  • This study conducted non-equality control post-design similar experimental study in order to investigate the effect of self-directed practice through self-reflection by using videos on the basic nursing performance ability, confidence in performance, learning self-efficacy, and practice satisfaction. The experimental group was 57 students and the control group was 56 students in the Department of Nursing in G region. The data was collected from April 1, 2017 to June 14, 2017. For analyzing data, the SPSS 22.0 program was used and the homogeneity test was analyzed with ${\chi}^2$ test, t-test, and Fisher's exact test. Also, independent-test was used. According to the result, all of the experimental groups including the basic nursing performance ability, confidence in performance, learning self-efficacy(t=15.304, p<.001), and practice satisfaction(t=15.304, p<.001) were improved and showed significant differences compared to the control group. As a result, it is predicted that the practice method has a positive effect on participating in self-study and it contributes to active participation by increasing learning motivation of learners. Thus, it is suggested to be applied to various practical training to improve basic nursing skills.

The Effect of Self-Efficacy and Teacher's Image on Student Teachers' Practicum Anxiety (예비유아교사의 교육실습불안에 대한 자기효능감과 교사이미지의 영향)

  • The purpose of this study is to investigate early childhood student teachers' practicum anxiety on the basis of the teacher's image and self-efficacy during their practicum. The participants of this study were 499 student teachers who were enrolled in the department of early childhood education in a college or a university and had completed the practicum in Daejeon metropolitan city or the Chungcheong area. The instruments for this study were the Teacher's Image Scale, Practicum Anxiety Scale, Self-efficacy Scale, and a questionnaire for socio-demographic variables. The collected data were analyzed by using a t -test, F test, Pearson's partial correlation, and multiple regression with SPSS software (version 20.0). The results were as follows: First, the student teachers expressed higher practicum anxiety in a provincial area than in metropolitan city, and their self-efficacy was higher in public kindergarten than in private kindergarten. Further, the highest level of the teacher's image was obtained in the case of student teachers who enrolled in a 2- or 3-year college program and completed the practicum in the metropolitan city. Second, there was a significant negative correlation between the sum of student teachers' practicum anxiety and their professional skills of teacher's image as well as significant correlations between the sum of student teachers' practicum anxiety and their self-efficacy, except self-control efficacy. Finally, the student teachers' practicum anxiety was found to be 17.5%-24.4% and was explained the most by the confidence of self-efficacy.

Effects of the Lasater's Clinical Rubric of Debriefing in Advanced Cardiovascular Life Support Training (전문심폐소생술 교육에 Lasater의 임상판단 루브릭을 적용한 디브리핑의 효과)

  • The purpose of this study was to test the effects of the Lasater's debriefing on critical thinking disposition, problem- solving skills and clinical judgement of nursing students. The study design was a nonequivalent control group non synchronized design. The subjects were 60 students for 12 groups. They were randomly assigned to the experimental group (n=30) or control group (n=30). Critical thinking and problem-solving skills were measured using a self-administered questionnaire. Clinical judgement was measured by a Lasater's clinical judgement rubric. The Results of the study was : After Lasater's debriefing, the level of students' critical thinking, problem-solving skills and clinical judgment in experimental group were higher than the level of control group. Through this experiment the rubric of Lasater debriefing is an effective tool for clinical judgment, therefore we know that the future of simulation training is needed to use the Lasater's clinical judgement rubric of debriefing in simulation training.

Effectiveness and Retention of Repeated Simulation-based Basic Life Support Training for Nursing Students (간호학생 대상 시뮬레이션기반 기본소생술 반복교육의 효과와 지속성)

  • Propose: This study was to investigate the educational effect and retention of repeated simulation-based basic life support (BLS) training for nursing students. Methods: A comparison group design with pretest and posttest was used. A total of 35 nursing students (18 for the experimental group, 17 for the control group) participated in the study. A repeated simulationbased BLS training program which include a lecture, skills training, and two repeated sessions of simulation practice and debriefing was provided twice for experimental group. Knowledge, self-efficacy, and skill performance of cardiopulmonary resuscitation (CPR) were measured three times: at baseline, week 2, and week 6. Descriptive analysis, repeated measures ANOVA, and t-test were used for data analyses. Results: Knowledge, self-efficacy and skill performance of CPR were not significantly changed by group assignment, by the time, and interaction of group by time. Effectiveness of intervention was not maintained until Week 6. Conclusion: The results suggest that the timing of repeat education, total training time, and students' mastery of CPR performance should be considered when developing simulation-based programs to improve and maintain students' CPR knowledge, self-efficacy, and skill performance.

The art of diabetes care: guidelines for a holistic approach to human and social factors

  • A holistic approach to diabetes considers patient preferences, emotional health, living conditions, and other contextual factors, in addition to medication selection. Human and social factors influence treatment adherence and clinical outcomes. Social issues, cost of care, out-of-pocket expenses, pill burden (number and frequency), and injectable drugs such as insulin, can affect adherence. Clinicians can ask about these contextual factors when discussing treatment options with patients. Patients' emotional health can also affect diabetes self-care. Social stressors such as family issues may impair self-care behaviors. Diabetes can also lead to emotional stress. Diabetes distress correlates with worse glycemic control and lower overall well-being. Patient-centered communication can build the foundation of a trusting relationship with the clinician. Respect for patient preferences and fears can build trust. Relevant communication skills include asking open-ended questions, expressing empathy, active listening, and exploring the patient's perspective. Glycemic goals must be personalized based on frailty, the risk of hypoglycemia, and healthy life expectancy. Lifestyle counseling requires a nonjudgmental approach and tactfulness. The art of diabetes care rests on clinicians perceiving a patient's emotional state. Tailoring the level of advice and diabetes targets based on a patient's personal and contextual factors requires mindfulness by clinicians.

Categorization of Competencies and Description of Job Informations for Dietitians in Employee Foodservice by Wdorker-oriented Job Analysis Methodology (직영 및 위탁 사업체 급식소 영양사에 대한 작업자 지향적 직무분석(제 2 보) : 요인분석에 의한 직무수행 특성 영역 및 직무정보 기술)

  • The job characteristics and competencies of 285 dietitians in employee foodservices were investigated by worker-oriented job analysis methodology. The purposes of this study were to : a) determine the knowledge, abilities, skills and other characteristics(KASO) needed for dietitians in two types of employee foodservice groups(self-operated vs. contracted), b) categorize KASO items by factor analysis and c) provide job information and job descriptions for dietitians in employee foodservices. The job analysis questionnaire was consisted of questions concerning the 134 KASO items. The questionnaire was mailed to 250 dietitians who are members of The Korean Dietetic Association Practice Group in self-operated foodservices(group A) and 250 dietitians who are employed in contracted foodservice companies(group B). Completed questionnaires were received from 285 dietitians(121 in self-operated, 164 in contracted) ; with response rate of 57%. Statistical data analysis was completed using the SPSS/win and the SAS/win packages. The results of this study can be summarized as follows. 1) Through factor analysis, the 134 KASO items were regrouped into 6 categories :‘primary dietetic tasks concerning menu management and administrative work regarding merchants’, ‘primary dietetic tasks concerning procurement and purchasing of food and supplies and meal production and service control’, ‘primary dietetic tasks concerning facility, sanitation and safety control’, ‘secondary dietetic tasks concerning nutrition education and research’, ‘secondary dietetic tasks concerning foodservice operation management’and‘human attributes’. 2) Analysis of the total scores(average scores of degree of frequency, importance and entry requirements of KASO items within 6 categories), group A showed high priorities placed on‘human attributes’, ‘procurement and purchasing of food and supplies and meal production and service control’, ‘menu management and administrative work regarding merchants’, ‘whereas group B showed high priorities placed on’‘human attributes’, ‘foodservice operation management’. Effects of Action Oriented Breast Self-examination(BSE) Education on Knowledge, Self-efficacy, and Performance Competence in Nursing Students (행위중심적 유방자가검진 교육이 간호학생의 지식, 자기효능감 및 수행능력에 미치는 효과)

  • Purpose: This study was undertaken to test effects of action oriented BSE education on knowledge, selfefficacy, and performance competence in nursing students. Methods: A non-equivalent pre-posttest of time serial quasi-experimental design was used to prevent diffusion of the experiment. Out of 53 nursing students selected by convenient sampling in the third grade at a nursing college, 27 students were randomly assigned to the control group and were first given a traditional lecture on BSE. For the experimental group of 26 students action oriented BSE education consisted of lectures, demonstrations with breast models, and BSE skill practice by micro teaching. Results: There was no significant difference in general and BSE-related characteristics between the two groups. Scores of the experimental group in knowledge, self-efficacy, and performance after the intervention were significantly higher than those of the control group. Conclusions: Results showed that action- oriented BSE education was effective in increasing knowledge, self-efficacy and performance competence in nursing students. Education for students should incorporate strategies reinforcing confidence by practicing actual BSE skills as well as acquiring knowledge of BSE, so that students can become competent and practical cancer prevention educators.

Effects of Formative Feedback Practice on Practice satisfaction, Learning motivation and Academic Self efficacy (자율실습에서 형성적 피드백이 간호대학생의 실습만족도, 학습동기 및 학업적 자기효능감에 미치는 효과)

  • This study was a quasi-experimental design with a non-equivalent pre-post test that verified the effect of formative feedback practice on practice satisfaction, learning motivation and academic self efficacy for nursing students. 37 were assigned to an intervention group and 41 to a control group. Formative feedback practice was applied to intervention group and peer review smartphone videos was applied to control group. Data were analyzed using the SPSS/WIN 22.0 program. There was no significant difference in learning motivation between the groups, but there were a significant difference in practice satisfaction(t=-2.79, p=.007) and academic self efficacy(t=2.30, p=.024) between the pre-post scores of the experimental group. This is meaningful in that it is more effective to provide formative feedback practice than to provide peer review smartphone videos. Therefore, it is necessary to consider formative feedback practice for the acquisition of core fundamental nursing skills of nursing students.

  • Background : Diabetes mellitus is chronic condition which needs patients' understanding of disease status and effort for disease control. We investigated whether the patients' estimation for blood glucose control was reliable or not comparing with their HbA1C value, Method: Among people who visited one health promotion center in Seoul from 1995 to 2003,those who answered as having diabetes mellitus on self reported questionnaire were identified. Subsequent question was whether his or her diabetes was well controlled or not. We compared these replies with their HbA1C values. When their HbA1C values were below7% with answering not well controlled, we analyzed if there was any variable which can predict the discrepancy. And the same analyze we did for the opposite situation. Result: A total of 1193 patients were eligible. Those who answered as well controlled formed32.9% of the group with HbA1C values above 7%. In the cases with well controlled though significantly associated with this discrepancy, Conclusion: Considerable proportion of diabetic patients' estimation for diabetic control status was not matched with glycemic control guideline. For successful management of diabetes mellitus, more efforts for patient education and communication skills are essential.

  • Purpose: The purposes of this study were to examine the effects of the Cooperative Clinical Nursing Education Program on the competency and satisfaction of nursing students in clinical practice. Method: The research design was a quasi-experimental pretest-posttest design with a convenience sample of 107 nursing students who were at one College of Nursing located in Seoul. Data were collected using a self-administered questionnaire. Results: Nursing students participated in the program reported higher nursing skills and total satisfaction than control group after completing education program. Conclusion: Cooperative clinical nursing education program for nursing students had positive effects on nursing skills and satisfaction of clinical practice.
