The effect of rooming-in care on the emotional stability of newborn infants (모자동실이 신생아의 정서적 안정에 미치는 영향)

  • Purpose : We aimed to examine the effect of rooming-in care on newborn infants emotional stability by comparing them with those cared for in a nursery. Methods : Forty-eight full-term newborn infants born at Cheil General Hospital between July 1 and October 31, 2007, were enrolled. Twenty-four newborn infants were roomed-in in their mothers rooms (rooming-in group), and 24 newborns were cared for in the hospital nursery (the nursery group) for the first 3 days of their lives. Those with perinatal problems that required medical treatment were excluded. By using Brazeltons neonatal behavior assessment scale, we measured irritability and self-quieting as well as the duration of crying after heel-stick puncture for the newborn metabolic screening test. Results : The rooming-in group had a higher irritability score than the nursery group ($6.8{\pm}1.7$ vs. $4.2{\pm}2.1$, P<0.001), thereby suggesting stable behavior against external irritation; the former also had a higher self-quieting activity score ($5.9{\pm}0.3$ vs. $4.5{\pm}1.8$, P=0.001), thereby suggesting that stability was reached quickly from the irritated state. Time taken to stop crying after the heel-stick puncture was significantly shorter in the rooming-in group than in the nursery group ($17{\pm}15.1$ seconds vs. $115.3{\pm}98.5$ seconds, P<0.001). Conclusion : These results show that newborn infants in the rooming-in group exhibit more stable behavior against external irritation and can be stabilized from an irritated state more quickly than infants cared for in the nursery, even after a few days of rooming-in care.

Applying Extended Theory of Planned Behavior for Lung Cancer Patients Undergone Pulmonary Resection: Effects on Self-Efficacy for Exercise, Physical Activities, Physical Function, and Quality of Life (폐절제술을 받은 폐암환자에 대한 확장된 계획행동이론의 적용: 운동 자기효능감, 신체활동, 신체기능 및 삶의 질에 미치는 효과)

  • Purpose: This study aims to examine the effects of nursing interventions based on the Extended Theory of Planned Behavior (ETPB) regarding self-efficacy for exercise (SEE), physical activity (PA), physical function (PF), and quality of life (QOL) in patients with lung cancer who have undergone pulmonary resection. Methods: This quasi-experimental study was conducted between July 2015 and June 2018 in two university-affiliated hospitals. The intervention included pre-operative patient education, goal setting (action and coping planning), and feedback (behavior intention and perceived behavioral control). The intervention group (IG) (n=51) received nursing interventions from the day before surgery to 12 months after lung resection, while the comparison group (CG) (n=36) received usual care. SEE, PA, PF (dyspnea, functional status, and 6-minute walking distance [6MWD]), and QOL were measured before surgery and at one, three, six, and 12 months after surgery. Data were analyzed using the χ2 test, Fisher's exact test, Mann-Whitney U test, t-test, and generalized estimation equations (GEE). Results: There were significant differences between the two groups regarding SEE (χ2=13.53, p=.009), PA (χ2=9.51, p=.049), functional status (χ2=10.55, p=.032), and 6MWD (χ2=15.62, p=.004). Although there were no time or group effects, the QOL mental component (Z=-2.78, p=.005) of the IG was higher than that of the CG one month after surgery. Interventions did not affect dyspnea or the QOL physical component. Conclusion: The intervention of this study was effective in improving SEE, PA, functional status, and 6MWD of lung cancer patients after lung resection. Further extended investigations that utilize ETPB are warranted to confirm these results.

Self Assembly and Formation of Bi-continuous Cubic Liquid Crystalline Phase (바이컨티니어스 큐빅상 액정의 생성과 자기조직화)

  • This study is to form the self assembly of cubic crystalline phase to penetrate into the skin epidermis. The various performance synthesized diglyceryl phytylacetate (DGPA) having hydroxyl group (-OH) and 4 methyl chains with phytyl group was carried out as an amphoteric lipid such as emulsifying power, self assembly. Emulsifying activity of DGPA was very stabilized on only 1% of small content, it could make a W/O emulsion containing high internal phase incorporated with water. Cubic liquid crystal structure with DGPA on three-phase diagram was formed, when mixed DGPA, dimethicone (2CS), and water. Through three-phase diagram forming the cubic liquid crystal area, hexagonal structure zone, and mixing water phase and hexagonal structure area, reversed micelle area were respectively certified. Its structure was proved by the SAXS (small angled x-ray scattering) analysis. As an application, formation of cubosome containing 10% of magnesium ascorbylphosphate and 5% of pyridoxine tris-hexyldecanoate was encapsulated. Occlusive effect of cubosome had above 1.7 times better than reversed micelle. From using poloxamer of dispersing agent, phase structure recovered from W/O emulsion to cubic liquid crystal phase when storage in $33^{\circ}C$ incubator. Therefore, our this study is expected to be as epidermal-dermal skin absorbers in skin care cosmetics and pharmaceuticals industries as raw materials to form a cubic crystal phase through a more in-depth research to DGPA having amphoteric lipid property.

A Study on Need Assessment in Health Promotion Programs for Developing Nursing Centers - Breast Self Examination- (간호센타 개발을 위한 건강증진 프로그램 요구사정 연구-유방자가검진 프로그램을 중심으로-)

  • The purpose of this study was to assess the needs of breast self examination education (BSEE) programs for developing nursing centers. A first, we reviewed the literature of activity and management-related factors of nursing centers: and, second. we used a questionnaire to discover the degree of knowledge, attitude, and practice on breast cancer, as well as an individual's intention to participate BSEE program. 1. Reviewing the literatures of nursing center activities. Nursing centers which were administered by a professional nurse are an ideal site for faculty and student practices. With the use of nursing models of health. professional nurses in nursing centers diagnose and treat human responses to potential and actual health problems and offer holistic, client-centered health service. In nursing centers professional nursing services include health education, health promotion, and health-related research. A nursing center is comprised of the advisory and exacutive commitee; the advisory commitee serves consultants and links community needs to the nursing center, while the director of the exacutive commitee identifies the potential resources to generate funds, support, and facilitate the activities of staffs in a nursing center. Nursing centers mobilize various financal resources for reimbursement of services from college and insurance companies, collect minimum service fees from the client, and further collect fees for providing programs to community groups, this also includes membership fees, and donations. The services provided by nursing centers focus on services related to primary prevention, health maintenance & health promotion, direct nursing care for acute & chronic diseases, and holistic care for actual and potential health problems. The client satisfaction for the services was high. Students also showed positive reponses for their clinical experiences and independent working conditions. 2. The degree of knowledge, attitudes, and practices for breast cancer. and an individual's intention to participate in the BSEE program. The subjects of this study were 308 females in K-city in the Republic of Korea. Data were collected using a self-administered questionnaire. The mean age of the respondents was 35.0 years old. Those who already participated in the BSEE were 64.9%, and those who had support and encouragement to practice BSE from significant others were 25.1 %. Clients intent to participate in the BSEE were 37.0%. The mean score of knowledge(2.4 out of 5 points) and practices(1.8 out of 5 points) for breast cancer were quite low, but the mean score of attitudes was relatively positive04.5 out of 20 point) for breast cancer. Those who already had BSEE showed significantly high scores in knowledge(t=6.48, p<0.01), attitudes (t=10.54, p<0.01). and practices(t=57.07, p<0.001) for breast cancer than those who had not participated in the BSEE. In all age groups no intention to participate in the BSEE was higher than who the intention to participate. These findings suggest some strategies should be developed to increase the awareness of breast cancer's early detection.

  • According to the increase of the proportion of aged people, the medical demand for a senile chronic disease has been increased; therefore, aged people call for a geriatric hospital for special geriatric medical service. The main purpose of this study was to analyze the general characteristics and financial status of geriatric hospitals. For the study, a questionnaire was designed and sent to the geriatric hospitals to fill out the patient statistics, number of headcount by department, etc. to find out the stability, profitability, activity and so on financial statements of the hospitals were analyzed. The major findings of this study were as belows. 1. The ratio of the medical expenses to the revenue of the geriatric hospitals is much lower than acute care hospitals. But the probability of bankruptcy is higher due to the high ratio of the liabilities therefore it is required to stabilize the financial position by donating more money. 2. Government budget for the elderly people is not enough. To support the geriatric hospitals by going subsides, government should increase the budget. 3. Portion's of the patient of the geriatric hospitals are government support patient. Since the government doesn't pay the medical charges quickly, geriatric hospitals have a serious cash flow problem. Therefore, it is required that government is to prepay the bill. 4. Since geriatric hospitals treat elderly patient and most patients are government support patients, geriatric hospitals can be said to operate under the strict. 5. When we introduce the daily medical charge, the self-liability will be reduced on approximately 50% of current. This affection will bring a huge progressing financial structure to the medical profit of the geriatric hospital, and also patient family will feel less economical burden.

  • Objectives : The aim of this study was to report the improvement of fever, myalgia, performance status, and headache after treatment with Osuyubujaijung-tang and Geopoong-san in a Soeumin with Peripheral T-cell lymphoma. Methods : We retrospectively reviewed the medical records, medical laboratory and image scans of 80-year-old male patient diagnosed as peripheral T-cell lymphoma. He couldn't conduct any conventional chemotherapy due to poor performance status, ECOG 4. Results : The symptoms of myalgia, poor performance status, and neck pain improved, and fever was reduced with Osuyubujaijung-tang. Headache was subsided with Geopoong-san. Conclusions : A patient with peripheral T-cell lymphoma suffering from fever, myalgia, poor performance status, neck pain and headache showed the improvement of symptoms with treatment of Osuyubujaijung-tang and Geopoong-san. After 4 months treatment, the patient could conduct self care, physical activity and social affairs.

  • Considering the continuous increase of teenagers' Internet use and the high rate of their participation in online communities, educators need to devote efforts to minimize the negative function of teenagers' Internet use and to guide them to experience positive aspects of Internet. Through activities in online communities, teenagers can experience various socialities and explore their self-consciousness. However, on the other hand many communities do not provide teenagers with considerate care. This may cause conflict in their sense of value and mislead them. Through this study, online learning community is designed and developed, which can reflect teenagers' various needs and connect their needs with learning experiences. The community consists of the following categories: 'about e-Tivity'(community vision, general information, list of activities, Q&A), 'activity space for teens'(event hall, data park, e-Tivity center), 'administrative space for teachers and managers'(research room, management room), and 'teen's private space'(my room, my avatar).

  • The purpose of this study was to identify the effect of psychological health and physical health on health-related quality of life(HRQoL) in Korean Echo Generation by using Korea Health Panel Data 2013. The Korea Health Panel Data 2013 were collected from February to October 2013 and 2,261 respondents were analyzed. The data were analyzed by Independent t-test, ANOVA and multiple regression using SPSS WIN 24.0 program. he mean score of HRQoL was 0.98. The effect of unmet medical needs, psychological & physical stress, unmet basic needs, anxiety about the future, depression, suicidal ideation, smoking, sleeping time, hearing problem, eating problem, restriction of activity, and self-rated health status) were significant on HRQoL. Health care providers should consider the effect of psychological and physical health when they design program for the improvement of HRQoL for Korean echo generation in community.

Perceived Social Support as Influencing Factors on Quality of Life among Cancer Patients (사회적지지가 암 환자의 삶의 질에 미치는 영향)

  • Objectives: This study was designed to understand the association between the perceived social support and the quality of life among cancer patients. Methods: Questionnaire survey was conducted from July 10 to August 14 in 2008 among inpatients and outpatients visited Gangwon Cancer Center. Survey was done for 429 patients who were diagnosed as cancer longer than two years and 232 patients responded. We used Multidimensional Scale of Perceived Social Support(MSPSS) and Euro Quality of Life-5 Dimension(EQ-5D). Results: Prevalence of problem in Quality of life among cancer patients were as follows; 28.6% in mobility, 26.0% in pain or discomfort, 24.8% in anxiety or depression, 22.7% in usual activity, and 15.7% in self-care. Cancer patients with higher social support had significantly higher Quality of Life. Also more severe stage had significantly lower Quality of Life. Conclusion: Education, income, stage, and social support seemed influential to Quality of Life among cancer patients. Programs are necessary to promote social support for Quality of Life especially for cancer patients diagnosed as higher stage. Counseling and supporting can be useful not only for patients but also for families.

  • Man pursues health as his basic right. Therefore, the government should try to preserve the right of the people's health and carry out the policy of medical treatment for that. But the system of our medical care is advantageous to the medical institutions, which produce medical goods each people buys and sells freely, more than to the maintenance and improvement of their health. That is to say, the first aim of the institution is not the healthy preservation of the people but their accumulation of riches. The medical conflicts are the social situation which is happening between those who produce medical treatment and the patients who consume it. Its behinning comes from the lack of belief by the inhuman relationship between patients and doctors. According to thelatest investigation, the patients of oriental clinics look more content than those who go to common hospitals. The reasons are as follows; fitness to one's physical constitution, the kind altitude of doctors and the view of oriental medicine toward human body. Though the content degreee is higher than western medicine, such conclusions result from the present condition the number of the patients is less. In short, the first, since the right of patients is higher and the fields make more variors and popular, the conflicts and mistakes go on increasing. The second, in their activity of treatment, the legal importance of atlention and explanatory obligation should be considered seriously so as not to break out medical mistakes. The third, in the center of technical books which are accepted by the academic world, the clinical exertion doing treatment is needful. The fourth, as the direct order of medical justification the self-determination of patients should be respected. The fifth, because the process and record of treatment become important in the time of emergency legally, the conversations and movements as well as the details of treatment must be recorded. The sixth, the academic effort about the settlement institution or the legal system is necessary.
