Clinical Study of Combination Treatment of Oriental Medicine on Thalamic Pain Syndrome (시상증후군(Thalamic Pain Syndrome) 환자의 한방 병합치료에 관한 임상적 고찰)

  • Objectives : To evaluate the pain control effect of combination treatment of Oriental Medicine on patients who suffered from thalamic pain syndrome caused by thalamic stroke. Methods : We reviewed the medical records and brain imaging data of all patients with thalamic stroke from September 1998 to August 2000 who visited to Department of Oriental Internal Medicine, National Medical Center. We evaluated clinical features of thalamic pain syndrome, including incidence, onset interval from stroke, nature, pain distribution, and assessed the pain control effect of combination treatment by Visual Analog Scale(VAS). Results : 64 cases were selected under the inclusion criteria, and 17 patients(26.5%) with thalamic pain syndrome were identified from 64 thalamic strokes. VAS proved combination treatment effective to control pain of thalamic pain syndrome. In 12 cases(70.5%), pain onset was within the first week poststroke. The patients with allodynia were 6(35.3%). In 12 cases(70.5%), the lesion was mainly located in the posterolateral areas of thalamus. Conclusion : We conclude that combination treatment of the Oriental Medicine modalities have pain control effectiveness on thalamic pain syndrome.(t=-5.47, p=0.0001)

  • The purpose of this research is to study how the stretching exercise along with heat and electric therapy can effect on Angle of Lumbar Flexion and Reduction of Pain against the Hamstring Construction Symptom that can be found in the Chronic Low Back Patients. The target of research was 28 person selected out of the patients diagnosed as Lumbar Sprain and Lumbar Syndrome, who have positive from the active test of Hamstring construction and passive test. Their age range is 20 to 60 years old, 14 male and 14 female. As a research method, simple heat and electric therapy was applied to counter part while Hamstring Stretching Exercise including simple heat and electric therapy was applied to the experimental part. After 6 weeks treatment for each part, the results of effect on Angle of Lumbar Flexion and Pain Reduction was compared and analysed on the basis of the gravity angle meter and Visual Analogue Scale (VAS). The outcome of research is as follows: 1. Angle of Lumbar Flextion increased effectively not in the conventional counter group but the experimental group to which Hamstring Stretching was applied simultaneously. 2. remarkable effect was shown in the experimental part applied Hamstring Stretching. Consequently from the results of this study, we could find out the fact that the therapy combined preserve treatment and hamstring stretching not only produced increasing Lumbar Flexion Angle but also was more effective on reducing pain of the patients according to it than the conventional or preserve therapy.

Effects of Aroma Foot Bath on Urination Status and Stress related to Urination in Patients with Benign Prostatic Hyperplasia (BPH)

  • Purpose: The purpose of this study is to evaluate effects of aroma foot bath on urination status (Prostatic Symptom and bother score due to urinary symptoms) and stress related to urination in patients with benign prostatic hyperplasia (BPH). Methods: This study consists of pre- and post-tests in a non-equivalent group design. The 52 BPH patients selected by random sampling in G City were assigned to two groups (experimental group=26, control group=26). Data of their general characteristics, urination status and stress related to urination were collected using self-reported questionnaires. Aroma Foot Bath was performed to the experimental group for 15-20 minutes every day for a week. Descriptive statistics and independent t-test were used for the analyses in this study with the SPSS/WIN 21.0 version program. Results: Prostatic symptom and bother score due to urinary symptoms in the experimental group were improved than those in the control group (t=-7.94, p<.001). Compared to the control group, the stress related to urination in the experimental group (t=-8.78, p<.001) was significantly reduced (t=-5.21, p<.001). Conclusion: The results of this study indicate that aroma foot bath is effective in improving urination status and reducing stress related to urination in BPH patients.

The Effects of Aroma Hand Massage on Sleep and Depression in Hospitalized Elderly Patients (아로마테라피 손 마사지가 입원 노인의 수면 및 우울에 미치는 효과)

  • Objectives : To evaluate the effects of aroma hand massage on sleep disturbance and depression in hospitalized elderly patients. Methods : The data was collected using questionnaires from the subjects who were admitted to the G Medical Hospital in Daegu from 7 July to 12 July, 2008. A total of 23 subjects were selected. The subjects were applied with the aroma hand massage to both hands using a solution diluting four drops of lavender oil in jojoba oil 10cc. Aroma hand massage was given for 10 minutes, one time a day, during 5 days. Results : There was statistically significant improvement on the Korean sleep scale(t=-7.614, p=0.000) and on the Geriatric Depression Scale Short Korea Version(GDSSF-K)(t=3.444, p=0.002). Conclusions : aroma hand massage is an effective therapeutic intervention for decreased sleep disturbance and depression. Therefore, aroma therapy and hand massage by therapeutic intervention is recommended for improving psychological and physiological functions in hospitalized elderly patients.

An Integrative Review of Oncology Nursing Research in Korea: 2003-2008 (국내 종양간호연구의 분석: 2003-2008)

  • Purpose: This study was aimed to analyze the contents and trend of published research in oncology nursing during the past five years in Korea and to suggest the direction for future research. Methods: Using the key words "cancer", "cancer patients", and "tumor", 391 nursing research published from 2003 to 2008 were selected for the analysis. Results: 1) Three hundred fifty-five (90.8%) of the studies used quantitative research approach, whereas most studies were descriptive in nature. 2) The main subjects of the study were cancer patients with mixed diagnosis (36.4%) and breast cancer patients (20.0%). 3) Complementary alternative therapy (28.9%), educational program (25.6%), and psycho-social and spiritual program (16.7%) were the most frequently used nursing interventions. Most frequently measured outcome variables were psycho, socio, and spiritual factors (38.8%), symptoms (28.2%) and physiological factors (14.6%). 4) Most frequently used key words were identified as quality of life, fatigue, coping, pain, anxiety, anorexia, and depression. Conclusion: The number of published research in oncology nursing in Korea has been increased. It is suggested to conduct international studies to develop, compare, and replicate nursing interventions for patients with cancer in future. Increasing funding, establishing research infrastructure, and removing methodological challenges are warranted for better research environment.

The effect of stabilization exercise on pain-related disability, sleep disturbance, and psychological status of patients with non-specific chronic low back pain

  • Background: Sleep disturbance and depression are becoming more recognized as important symptoms among individuals with chronic low back pain. This study evaluated the effect of stabilization exercise on pain-related disability, sleep disturbance and the psychological status of patients with non-specific chronic low back pain (NSCLBP). Methods: A total of 26 patients (M/F = 17/9) with NSCLBP with a mean age of $50.0{\pm}15.5$ took part in this study. Participants were recruited from selected hospitals in Lagos state. Ethical approval was sought and obtained from the health research and ethics committee of Lagos University Teaching Hospital Idi-araba, Lagos, Nigeria. Participants performed stabilization exercise for eight weeks consecutively and were assessed for pain-related disability, anxiety, depression, and sleep disturbance using the pain-disability index, hospital anxiety and depression scale, and the insomnia severity index at baseline, the 4th week, and the 8th week, postintervention. Results: The participants studied recorded significant reduction in pain-related disability (P = 0.001). There was also improvement in the sleep quality (P = 0.001), depression level (P = 0.001), and anxiety level (P = 0.001), post intervention. Conclusions: This study revealed that stabilization exercise is very useful in the management of sleep disturbance, pain-related disability, depression, and anxiety in NSCLBP patients.

Measurement of Nutrition Counseling Effects for Diabetes Mellitus Patients (당뇨환자를 대상으로 한 영양상담의 효과측정)

  • The purpose of this study was to measure the counseling effects for diabetes mellitus patients. To measure the effects of the nutrition counseling, 40(13 men and 27 women) outpatients with diabetes mellitus were selected and they were randomly assigned to either the counseling group or non counseling group. Twenty-one counseling group patients received nutrition counseling weekly for 6 weeks and 19 patients served as non counseling group(control group) did not received counseling over same study period. The results of this study can be summerized as follows : 1) The food attitude score of the counseling group significantly increased from 61.9$\pm$15.9 prior to counseling to 87.0$\pm$7.8 after counseling (p<0.001). 2) Fasting blood glucose level and postgrandial blood glucose level also showed significant difference between two groups(p<0.01). In the counseling group, the fasting blood glucose level significantly decreased from 163.5$\pm$48.6mg/dl to 142.3$\pm$40.6mg/dl(p<0.01), and the postgrandial blood glucose level significantly decreased from 281.3$\pm$105.1mg/dl(p<0.001). 3) There was a significant difference of glycosylated hemoglobin between the two groups(p<0.005), and the glycosylated hemoglobin level of counseling group significantly decreased from 11.2$\pm$2.9% to 9.7$\pm$3.6%(p<0.005). 4) There was a significant correlation between the food attitude score difference and the glycosylated hemoglobin level difference in counseling group(p<0.05).

Anterior Lumbar Interbody Fusion with Pedicle Screw Fixation for Elderly Isthmic Spondylolisthesis

  • Objective : The surgical outcome of anterior lumbar interbody fusion[ALlF] with pedicle screw fixation for elderly isthmic spondylolisthesis was analyzed. Methods : Consecutive nineteen elderly patients [aged 65 years or more] with isthmic spondylolisthesis [Grade I or II] who underwent single level ALIF with pedicle screw fixation in 2002 were analyzed. Using clinical chart and mailed questionnaires, preoperative and postoperative Visual Analogue Scale[VAS] of back and leg pain and postopertive Macnab criteria were evaluated. Results : The mean age at the time of operation was 68.4 years [range 65 to 78 years]. Twelve patients underwent ALIF with percutaneous pedicle screw fixation. Seven patients underwent ALIF followed by posterior decompression and pedicle screw fixation. The postoperative complication rate was 10.5% [wound dehiscence in 1 patient and incisional hernia in 1 patient]. There was no postoperative major morbidity or mortality. At a mean follow-up duration of 30.7 months [range 25 to 35 months], 93.3% [14/15] of the patients showed excellent or good outcomes in terms of Macnab criteria. The mean VAS scores of back pain and leg pain significantly decreased after surgery. Conclusion : ALIF with pedicle screw fixation yielded favorable results for elderly isthmic spondylolisthesis in selected cases.

Factors Influencing the Exercise Performance of Elderly Patients with Diabetes (노인 당뇨병환자의 운동수행에 영향을 미치는 요인)

  • The purpose of this study was to identify the factors influencing the Exercise Performance of elderly patients with diabetes. The subjects were 153 elderly patients with diabetes who were selected from the public health center in Gwang ju. The data collected was analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation and stepwise multiple regression. This study found that approximately 52.9% of the subjects were exercising regularly. Exercise performance was significantly different according to education level, family income by month, and level of diabetes education. Significant factors influencing exercise performance were exercise self-efficacy, exercise social support and exercise benefits. The most powerful predictor of exercise performance was exercise self-efficacy(34.2%). This study suggests that nurses should emphasize exercise social support. and exercise benefits as well as reinforce exercise self-efficacy to improve exercise performance of the elderly patients with diabetes.

A Study on the Response Differences to the Sasang Constitution Questionnaire by Sasang Constitutions (환자용 사상체질진단 설문지의 체질별 응답차이에 관한 연구)

  • 1. Objectives This study aims to evaluate newly developing Sasang Constitution Diagnosis Questionnaire for accuracy and efficiency. 2. Methods Three hundred and twenty eight patients from 4 multi centers are involved in this study. Excluding 16 constitution-unidentified patients and 2 Taeyang-in patients, 310 patients' questionnaires are analyzed statistically by their constitutions. 3. Results Sixty one questions are statistically significant in whole 229 questions of Sasang Constitution Diagnosis Questionnaire. We have selected 30 questions from 61 questions by comparing total data from 4 multi centers with those of each center. Thirty questions are made up of 10 body shape questions, 10 appearance questions, 5 character questions and 5 symptom questions. 3. Conclusions There might be needs for revision of character and symptom questions. From results, we suggest to diagnose Sasang Constitution for screening by short-form questionnaire consisted of questions on body shapes and appearances.
