Development of a Measurement of Stress for Hospitalized Schizophrenic Patient (조현병 환자의 입원 스트레스 측정도구 개발)

  • Purpose: This study was done to develop a measurement for stress experienced by patients with schizophrenia during hospitalization. Methods: The preliminary tool was developed through in-depth interviews and a validity verification test of content. For data collection, 15 inpatients with schizophrenia were selected as participants for in-depth interviews and 195 patients admitted to one of eight psychiatric hospitals in four provinces were recruited as participants to test reliability and validity of the preliminary tool. Results: The questionnaire was developed as a four-point Likert-type scale in a self-report form with 28 items. Factor analysis showed 28 items in six factors. Factors were named 'Unjust human rights infringement', 'Futureless life', 'Alienation from other family members', 'Infringement of basic needs', 'Infringement of personal preference' and 'Inconvenience of shared living'. The six factors explained 63.5% of the total variance. Cronbach's alpha for the total items was .93 and for the factors ranged from .65 to .87. Conclusion: A tool to measure stress in patients hospitalized with schizophrenic was developed based on identified hospitalization stress experiences. Study results indicate that this tool can be used to evaluate hospitalization stress in these patients and will contribute to establishing nursing interventions for relief of hospitalization stress.

Development of Nursing Intervention Standards for Home Hospice Patients (가정호스피스 대상자를 위한 간호중재 표준서 개발)

  • Purpose: The purpose of this study is to develop a set of standardized nursing interventions and their associated nursing activities according to the NIC system to guide home care nurses in performing nursing intervention activities for hospice patients. Method: This study was a descriptive survey that first identified frequently used & specialized hospice nursing interventions based on the NIC systems and next validated a set of standardized nursing interventions and their associated nursing activities. One hundred chart records of home hospice patients were used to identify nursing interventions. Also, thirty-nine hospice specialized nurses participated twice using the Delphi technique to test the content validity of the standards. Results: Among the nursing interventions, 19 important nursing interventions and 418 associated nursing activities were selected after two rounds of Delphi technique by hospice specialized nurses. The mean content validity of the final nursing activities was 0.82. In this paper, only the 3 most frequently used nursing interventions and 3 hospice specialized nursing interventions with their associated nursing activities are presented since space is limited. Conclusion: The nursing intervention standards will be a basis for home hospice nurses to improve quality of hospice care for hospice patients.

Meta-analysis on the Effectiveness of Pulmonary Rehabilitation Program on Exercise Capacity/Tolerance and General Health Status (만성 폐쇄성 폐질환 환자를 위한 호흡재활 중재가 운동 능력 및 내구성, 일반적 건강상태에 미치는 효과에 대한 메타분석)

  • Purpose: This study was conducted to combine the effects of pulmonary rehabilitation program (PRP) on exercise capacity/tolerance and general health status of COPD patients based on the primary research results examined the effects of PRP, Method: Seventeen studies were selected by the sampling criteria established to include the studies that reported enough statistics necessary to conduct meta-analysis. Result: According to the study results, the most effective indicators for exercise capacity/tolerance were exercise time (such as cycling time or treadmill walking time) and ground walking distance within given time (6 minutes or 12 minutes), whereas effects on such indicators as VE and VO$_2$ were not statistically significant. PRP induced significant effect on patients' general health status, frequently measured by physical, psycho-emotional, and holistic indicators, the enhancement on psycho-emotional dimension resulted from PRP was more prominent than those of the other dimensions. From the results, it was noted that the place where PRP was given and the contents of PRP exercised their influence on the outcome variables. Which body part was trained was also one of the important factors that influence on the patients' perception of dyspnea during exercise as well as on exercise capacity/tolerance. Conclusion: PRP including exercise training significantly improved the exercise capacity and general health status of COPD patients.

A Study on Manifest Needs of the Patient with Hyperthyroidism (갑상선 기능항진증 환자의 욕구구조에 관한 연구)

  • The purpose of this study was to compare the needs among the patients of hyperthyroid, malignant tumor and normal adult to clarify the level of the need for the patient of hyperthyroid. We analysed relationships between their personal characteristics and their needs to contribute comprehensive nursing care for the patient of hyperthyroid. The . subjects were chosen from 60 persons who visited for general physical examination at Ewha Woman's University Medical Center., 72 patients having diagnosis with hyperthyroidism, and 72 patients admitted with malignant tumors at atomic neuclear medical center. The data was collected from September 1 to December 31, 1980, and the study population was selected by means of convenience sampling. The data was analysed by means of the standard deviation and analysis of variance. The results were as follows: 1. In the patients having hyperthyroid, male to female ratio was 1 :13.5, and incidence was peak at the age groups of 20 to 40. 2. In compare with the needs among 3 groups, there are significant relationships: the patient of cancer has shown the highest score with Achievement, Aggression and Dominance, the patient of hyperthyroid shown higher and the normal adult shown low score. .In the need related with sex, there is significant relationship: the normal adult has shown highest score, the patient having hyperthyroid shown the higher score and the patient of cancer shown the low score. 3. Tn compare with Whang's Edward personal preference schedule, the Hyperthyroidism shown relatively higher needs for aggression, dominance, exhibitionism and sex.

Risk Factors for Pediatric Inpatient Falls (아동 입원환자의 낙상위험 예측요인)

  • Purpose: The purpose of this study was to identify risk factors for pediatric inpatients falls. Methods: The study was a matched case-control design. The participants were 279 patients under the age of 6 who were admitted between January 1, 2004 and December 31, 2009. Through chart reviews, 93 pediatric patients who fell and 186 ones who did not fall were paired by gender, age, diagnosis, and length of stay. Five experts evaluated the 38 fall risk factors selected by the researchers. Results: In a general hospital, pediatric patients with secondary diagnosis, tests that need the patient to be moved, intravenous lines, hyperactivity, anxiolytics, sedatives and hypnotics, and general anesthetics showed significance for falls on adjusted-odds ratios. Conditional logistic regression analysis was performed to elucidate the factors that influence pediatric inpatient falls. The probability of falls increased with hyperactivity and general weakness. Patients who didn't have tests that required them to be moved and intravenous line had a higher risk of falls. Conclusion: These findings provide information that is relevant in developing fall risk assessment tools and prevention programs for pediatric inpatient falls.

Effects of Deep Cervical Flexors Strengthening Exercise on Cervical-Shoulder Angle, Disability index and pain and in Patients with Chronic Neck Pain (깊은목굽힘근 강화운동이 만성 목통증 환자의 통증과 목-어깨각도, 장애지수 및 통증에 미치는 영향)

  • Background: The purpose of this study on the effects of deep cervical flexors strengthening exercise to forward head posture and pain of neck in patients with chronic neck pain. Method: We selected 30 subjects among neck pain patients and carried out measurements. The subjects' forward head posture was measured including head tilt angle (HTA), neck flexion angle (NFA) and forward shoulder angle (FSA), neck disability index (NDI), numeric rating scale (NRS). The subjects underwent deep cervical flexors muscle strengthening with pressure bio-feedback device for 4 weeks. Then, after intervention, the subjects' forward head posture was measured again. It was performed Wilcoxon signed-ranks test for confirming the effect of deep cervical flexor muscles strengthening exercise. Results: As a result of comparison of measurements before and after intervention, we found that there were statistically significant improvement in the subjects' NDI, NRS and forward head posture including head tilt angle, neck flexion angle and forward shoulder angle. Conclusion: We concluded that deep cervical flexors strengthening exercise has the effect of alleviation of neck pain patients and is effective for forward head posture.

Relationship between Knowledge and Adherence to Self-management Guidelines, and Influencing Factors of Adherence for Hemodialysis Patients (혈액투석 환자의 지식과 교육 요구도, 자가-관리 방침 순응과의 일치도 및 자가-관리 방침 순응의 영향 요인 분석)

  • Purpose: This study was to examine the relationships between knowledge with same sub-areas of education needs and adherence to self-management guidelines, and to investigate the factors that influence adherence. Method: One hundred eleven patients with terminal stage of kidney failure were conveniently selected from a university hospital in Incheon. Results: The level of knowledge was high, with a mean score of 15.25. The level of adherence to self-management guidelines was low, with a mean score of 64.53. The patients' education needs were high. The significant influencing factors on adherence were gender, educational level, and two sub-areas of knowledge, 'exercise and daily life' and 'follow-up care and treatment'. Conclusion: Relationships between knowledge, education needs, and adherence should be further examined before developing and implementing intervention programs for adherence to treatment guideline in hemodialysis patients.

Effects of the Aroma Massage on Shoulder Pain, Depression, Sleep Disturbance in Hemiparesis Patients (아로마 마사지가 편마비 환자의 견통, 우울 및 수면장애에 미치는 효과)

  • Purpose: The purpose of this study is to find out the effects of the aroma massage on shoulder pain, depression, and sleep disturbance in hemiparesis patients. Method: The research design was a nonequivalent control group nonsynchronized design. The data was collected from July 1 to August 31, 2003 at D medical Center in Busan. The subjects were 37 patients that they were assigned to two groups, nineteen subjects in the experimental group and seventeen subjects in the control group. The oil made from a blend of lavendar, clarysage, bergamot at a ratio 3:1:2 used for relieving shoulder pain, depression, sleep disturbance. The instruments used for this study were Visual Analogue Scale(VAS) for shoulder pain, Zung' depression scale, 4 score's scale for sleep disturbance. The data were analyzed using SPSS and hypotheses were examined with ANCOVA. Results: Shoulder pain, depression, and sleep disturbance were significantly lower in the experimental group compared than the control group. Conclusion: The aroma massage using selected essential oils is an effective intervention for relieving shoulder pain, depression, sleep disturbance in hemiparesis patients. However differences between aroma massage and massage were not clear, further study needs to be done.

Literature Review of Cognitive Developmental Interventions on Patients with Breast Cancer undergoing Chemotherapy (항암화학요법 유방암환자의 인지기능 증진 중재에 대한 논문 분석)

  • Purpose: The purpose of this study was to analyze the trend of cognitive developmental interventions on patients with breast cancer who underwent chemotherapy. Methods: The researcher searched databases and selected 17 literatures using chemotherapy, breast cancer, cognitive function and intervention as searching key words. Searched databases included Ovid, Pubmed, Proquest and Medline and data collection period was from January 2000 to May 2010. Results: Interventions for patients with breast cancer have been continuously developed and frequent research topics included breast cancer, chemotherapy, quality of life, and cognitive function. In terms of study design, four research articles were pre-experimental study designs, one was case study, and the rest of studies were non-equivalent pretest-posttest control group design. Effects of intervention were examined in only 5 studies among 17. The interventions were not found to have direct effects in cognitive improvement. Conclusion: It is needed to figure out the mechanism of cognitive deterioration of the patients with breast cancer who underwent chemotherapy. And nursing interventions needs to be developed in order to hold up the cognitive downhill and help their cognitive rehabilitation.

Reliability and Validity of the Suffering Scale of Family of Patients with Terminal Cancer (말기 암환자 가족의 고통 측정도구의 신뢰도와 타당도 검증)

  • Purpose: The purpose of this study was to test the reliability and validity of an instrument to measure suffering of family of patients with terminal cancer. Methods: This research was a methodological study processed as follows: 1) The preliminary 32 items were developed based a conceptual framework of suffering for patients with cancer. 2) The content was validated by an expert panel (n=24) and the family of patients with terminal cancers (n=33). 3) The instrument was validated by survey (n=92). Finally, 29 items were selected in developing final version of the scale. Results: Seven factors were extracted through factor analysis: 'economical distress', 'loss of meaning in life', 'feeling of social isolation', 'emotional distress', 'burnout', 'guilty feeling', and 'physical discomfort'. These factors explained 66.94% of the total variance. Cronbach's alpha and Guttman Split-half coefficient of the 29 items were .93 and .92. respectively. Conclusion: This scale identified as a valid and reliable tool with a high degree of reliability and validity. It can be effectively utilized to assess the degree of suffering of family in palliative care settings.
