The Activity of Dopamine $\beta$-Hydroxylase of Central Nervous System in Genetically Epilepsy Prone Rats

  • Abnormality in the central noradrenergic system may be related to the seizure prone state in the genetically epilepsy prone rats (GEPR). The present work deals with the characterization of the deficit in noradrenergic system if susceptitibility and intensity of seizure are dependent on central noradrenregic activities by comparing the activities of dopamine $\beta$-hydroxylase (DBH) which hydroxylates dopamine into noradrenaline. DBH activities were measured in 5 areas of brain of normal rats, native GEPR and severe GEPR. The results suggest that lower DBH activities in the midbrain of GEPRs may positively be coupled to the susceptibility to seizure, whereas the same characteristics of the native or severe GEPR are not neccessarily in parallel with the intensity of seizure.

Propofol에 의한 전신마취 후 회복 시 발생한 근경련 -증례 보고- (Persistent Seizure after Propofol-Induced General Anesthesia in Recovery Room -A Case Report-)

  • There are a few case reports describing persistent seizure following propofol. A 45-year-old female underwent operation of mastoidectomy and tympanoplasty. She had no personal or family history of epilepsy. Anesthesia was induced with propofol and rocuronium, and maintained with sevoflurane-remifentanil after tracheal intubation. Any event was not noted during surgery. Seizure-like movement and shivering were developed after surgery in recovery room. Symptom was relieved by benzodiazepines, especially lorazepam. She was discharged in the 9th postoperative days without any sequelae.

  • Sandifer 증후군은 식도의 역류와 관련되어 음식섭취 후 비특이적인 경련과 자세를 취하게 되는 증후군으로서 증상이 경미할 경우 진단이 지연될 수도 있고 여러 신경학적인 질환과 근골격계의 이상과 감별을 위해 과도한 진단과정이 진행될 수도 있다. 저자들은 흡인성 폐렴과 수유 후 무호흡, 비전형적인 경련을 주소로 한 환아에서 Sandifer 증후군을 의심하고 조기에 진단하여 치료한 1례를 보고하는 바이다.

L1-norm Minimization based Sparse Approximation Method of EEG for Epileptic Seizure Detection

  • Epilepsy is one of the most prevalent neurological diseases. Electroencephalogram (EEG) signals are widely used for monitoring and diagnosis tool for epileptic seizure. Typically, a huge amount of EEG signals is needed, where they are visually examined by experienced clinicians. In this study, we propose a simple automatic seizure detection framework using intracranial EEG signals. We suggest a sparse approximation based classification (SAC) scheme by solving overdetermined system. L1-norm minimization algorithms are utilized for efficient sparse signal recovery. For evaluation of the proposed scheme, the public EEG dataset obtained by five healthy subjects and five epileptic patients is utilized. The results show that the proposed fast L1-norm minimization based SAC methods achieve the 99.5% classification accuracy which is 1% improved result than the conventional L2 norm based method with negligibly increased execution time (42msec).

MR Findings of Seizure-Related Cerebral Cortical Lesions during Periictal Period

  • Purpose: This study investigated the MRI, MR angiography (MRA) and MR perfusion findings of seizure-related cerebral cortical lesions during the periictal period. Materials and Methods: From a retrospective review of the institutional database between 2011 and 2014, a total of 21 patients were included in this study. Two radiologists assessed periictal MRI, including MRA and MR perfusion, in patients with seizure-related cortical lesions. The parameters examined include: location of cortical abnormality, multiplicity of the affected cortical region, cerebral vascular dilatation, perfusion abnormality and other parenchymal lesions. Results: All patients showed T2 hyperintense cerebral cortical lesions with accompanying diffusion restriction, either unilateral (18/21, 85.7%) or bilateral (3/21, 14.3%). Of the 21 patients enrolled, 10 (47.6%) had concurrent T2 hyperintense thalamic lesions, and 10 (47.6%) showed hippocampal involvement. Of the 17 patients (81%) who underwent MRA, 13 (76.5%) showed vascular dilatation with increased flow signal in the cerebral arteries of the affected cortical regions. On MR perfusion, all 5 patients showed cortical hyperperfusion, corresponding to the region of cortical abnormalities. Conclusion: Seizure-related cerebral cortical lesions are characterized by T2 and diffusion hyperintensities, with corresponding cerebral hyperperfusion and vascular dilatation. These findings can be helpful for making an accurate diagnosis in patients with seizure.

Effect of Vagus Nerve Stimulation in Post-Traumatic Epilepsy and Failed Epilepsy Surgery : Preliminary Report

  • Objective : Vagus nerve stimulation (VNS) has been used in epilepsy patients refractory to standard medical treatments and unsuitable candidates for resective or disconnective surgery. In this study, we investigated the efficacy of VNS to patients who had refractory result to epilepsy surgery and patients with post-traumatic epilepsy. Methods : We analyzed the effect of VNS in 11 patients who had undergone previous epilepsy surgery and patients with intractable post-traumatic epilepsy associated with brain injury. All patients underwent VNS implantation between October 2005 and December 2006. Results : We evaluated seizure frequency before and after implantation of VNS and maximum follow up period was 24 months. In the first 6 months, 11 patients showed an average of 74.3% seizure reduction. After 12 months, 10 patients showed 85.2% seizure reduction. Eighteen months after implantation, 9 patients showed 92.4% seizure reduction and 7 patients showed 97.2% seizure reduction after 24 months. Six patients were seizure-free at this time. Conclusion : We conclude that the VNS is a helpful treatment modality in patients with surgically refractory epilepsy and in patients with post-traumatic epilepsy due to severe brain injury.

What Should Be Considered to Cause the Early Post-Craniotomy Seizure : Antibiotics (Cefazolin) Irrigation

  • Objective : Post-craniotomy seizure (PCS) is reported only rarely. However, our department noted a 433% increase in PCS for a year beginning September 2010, especially after cerebrovascular surgery. Our goal was to identify the cause of our unusual outbreak of PCS. Methods : For almost one year after September 2010, cases of PCS increased significantly in our department. We analyzed 973 patients who had received a major craniotomy between January 2009 and November 2011. We included seizures that occurred only in the first 24 postoperative hours, which we defined as early PCS. After verifying the presence of PCS, we analyzed multiple seizure-provoking factors and their relation to the duration and character of seizure activity. Results : Overall PCS incidence was 7.2% (70/973). Cefazolin (2 g/L saline) was the antibiotic drug used for intraoperative irrigation in 88.4% of the operations, and no PCS occurred without intraoperative cefazolin irrigation. When analyzed by operation type, clipping surgery for unruptured aneurysms was the most frequently associated with PCS (80%). Using logistic regression, only 2 g cefazolin intraoperative irrigation (p=0.024) and unruptured aneurysm clipping surgery (p<0.001) were associated with early PCS. The seizure rate of unruptured aneurysm clipping surgery using 2 g cefazolin intraoperative irrigation was 32.9%. Conclusion : Intraoperative cefazolin irrigation must be avoided in patients undergoing craniotomy, especially for clipping of unruptured aneurysms, because of the increased risk of early PCS.

급성 엔도설판 중독의 임상적 고찰 (Clinical Analysis of Acute Endosulfan Poisoning: Single Center Experience)

  • Purpose: Acute endosulfan poisoning is rare but causes significant morbidity and mortality. The aim of our study is to describe complications and features of seizure and determine factors associated with mortality in acute endosulfan poisoning. Methods: Twenty-eight adult patients with acute endosulfan poisoning admitted to our emergency department during a 15-year period were studied retrospectively. The clinical features of seizure, use of antiepileptic drugs during seizure, and hospital courses were evaluated. Clinical factors between survived group and non-survived group were compared for identification of factors associated with mortality. Results: Of the 28 patients with endosulfan poisoning, 4 patients (14.3%) died and 15 (53.6%) patients developed generalized tonic-clonic seizure. Thirteen patients (46.4%) and 5 patients (17.9%) progressed to status epilepticus (SE) and refractory status epilepticus (RSE), respectively. SE and RSE were associated with mortality. Almost all significant complications including shock, acute renal failure, hepatic toxicity, rhabdomyolysis, and cardiac injury developed in SE and RSE patients. Conclusion: SE and RSE were important contributors to death in endosulfan poisoning. Emergency physicians treating endosulfan poisoning should make an effort not to progress seizure following endosulfan poisoning to SE and RSE using a rapid and aggressive antiepileptic drug.

Kainic acid로 유도된 생쥐의 간질 발작에 대한 천마 메탄올 추출물의 항경련 효과 연구 (Anti-convulsant Effects of Methanol Extract of Gastrodia Elata on Kainic Acid-induced Epilepsy Mouse Model)

  • Kainic acid (KA) is a excitatory agonist causing epileptic seizure and excitotoxicity in the hippocampus. Gastrodia Elata (GE) is known to have anti-convulsant and anti-oxidant effects. This study was investigated a possible role of GE in suppressing epileptic seizure using KA-induced epilepsy mouse model. Eight-week-old male C57BL/6 mice were administrated GE (50 or 500 mg/kg) once a day for 5 days, and then injected KA (30 mg/kg) intraperitoneally. Behavioral changes in mice by KA were evaluated for 90 minutes immediately after the KA administration. Six hours after the KA administration, their brains were harvested and the expressions of glutamate decarboxylase 67 (GAD-67) and K+-Cl- cotransporter 2 (KCC2) in the hippocampus of the mice were measured by immunohistochemistry.GE delayed the onset of epileptic seizure after KA administration, suppressed the severity of the seizure and decreased the number of severe seizures dose dependently. Moreover, GAD-67 and KCC2 expressions in the cornu ammonis (CA) 1 and CA3 of 500 mg/kg GE administrated mice were significantly increased compared to those in KA-treated mice.GAD-67 and KCC2 play an important role in regulating GABAergic system. Our results suggest that GE has anti-convulsant effect against KA-induced epileptic seizure through enhancing GABAergic system.

  • Objectives : LI11 has been known to suppress epileptic seizure. Using an mouse epilepsy model, we investigated whether acupuncture stimulation at LI11 can suppress kainic acid(KA)-induced epileptic seizure and apoptosis in the mouse hippocampus. Methods : Eight-week-old male C57/BL6 mice(20~25 g) were given acupuncture at LI11 or ST36 once a day for 3 days. After the last acupuncture stimulations, KA(30 mg/kg) was injected intraperitoneally and the degree of seizure was observed for 90 minutes. Twenty-four hours after KA administration, mice were sacrificed and the neural cell death, astrocyte activation and caspase-3 expression in their hippocampi were investigated. Results : Acupuncture stimulation at LI11 suppressed KA-induced epileptic seizure, neuronal cell death, astrocyte activation and caspase-3 expression. Conclusions : Acupuncture stimulation at LI11 decreases the KA-induced epileptic seizure and protects hippocampal cell death via regulating astrocyte activation and caspase-3 expression.
