Objective : The purpose of this study was to determine the effect of bone cement augmentation of pedicular screwing in severe osteoporotic spondylolisthetic patients. Methods : Twenty patients with spondylolisthesis (8 : spondylolytic spondylolisthesis 12 : degenerative spondylolisthesis) who had undergone pedicular screwing and interbody fusion for osteoporotic lumbar spine (T-score on bone mineral density<-3.0) from 2002 to 2005 were reviewed. Mean age was 62.3 years with 3 male and 17 female patients. Average follow-up period was 14 months. Average T-score on bone mineral density (BMD) was -3.62. After decompression of neural elements, about 6cc of polymethylmethacrylate (PMMA) was injected into the each vertebral body through transpedicular route. All patients underwent one level interbody fusion and pedicular screw fixation. Clinical outcome was assessed using Oswestry Disability Index (ODI) on the last clinical follow-up. In addition, a modified MacNab's grading criteria was used to objectively assess patient's outcome postoperatively. Radiographic analysis of sagittal contour was assessed preoperatively, immediately postoperatively, and at final follow-up including fusion rate. Results : Eighteen of 20 patients were graded as excellent or good according to the modified MacNab's criteria. An significant improvement of ODI was achieved in both groups. Mean sagittal angle at the preoperative state, postoperative state and at the last follow-up state was $11.0^{\circ},\;20.1^{\circ}$ and $18.3^{\circ}$, respectively, with mean sagittal angle correction gain $7.3^{\circ}$. Firm fusion was achieved in all patients. There were one compression fracture above the fused segment after 6 months follow-up and one case of seroma. But there were no postoperative complications related to bone cement leakage and pedicular screwings such as screw pullout or screw cut-up. Conclusion : Bone cement augmentation of pedicular screwing can be an effective procedure for osteoporotic lumbar spine in spondylolisthetic patients.

Since the conventional image segmentation methods using mathematical morphology tend to yield over-segmented results, they normally need postprocess which merges small regions to obtain larger ones. To solve this over-segmentation problem without postprocess had to increase size of structuring element used marker extraction. As size of structuring element is very large, edge of region segments incorrectly. Therefore, this paper selectively applies structuring element of mathematical morphology to improve performance of image segmentation and classifies input image into texture region, edge region and simple region using averaged local variance and image gradient. Proposed image segmentation method removes the cause for over-segmentation of image as selectively applies size of structuring element to each region. Simulation results show that proposed method correctly segment for pixel region of similar luminance value and more correctly search texture region and edge region than conventional methods.

Superior mesenteric artery (SMA) syndrome is a rare disorder caused by extrinsic compression of the third portion of the duodenum by the SMA. The operative treatment of choice is bypassing the obstructed duodenal segment by duodenojejunostomy. We report one case of SMA syndrome treated by laparoscopic duodenojejunostomy and followed up by 3D-reconstructive CT scan. A fifteen-year-old boy with intermittent vomiting and weight loss was admitted. Ultrasonography showed narrowing of the distance between the SMA and aorta. Hypotonic duodenography showed dilatation of duodenal third portion and barium stasis. On 3Dreconstructive CT scan, the angle between SMA and aorta was $37^{\circ}$. The postoperative course was uneventful. Three months later, he had gained 3 kg of weight and the angle between SMA and aorta increased to $38-39^{\circ}$ on 3D reconstructive CT scan. Laparoscopic duodenojejunostomy for bypassing the obstructive duodenum in SMA syndrome is a feasible and safe method.
Multi-Vertical Loop Arch Wire(MVLAW) is a kind of appliance for uprighting the mesially inclined posterior teeth axes simultaneously. In this study MVLAW was classified as 3 types by modifing the vertical loop design and named type A, B and C. Each MVLAW was fabricated from .017'x.025' TMA wire and preactivated at the distal end of the open vertical loop with 10 degree tip-back bend(type B has an electric welding stop at the distal end of each loop and type C has no electric welding stop). Type A MVLAW was preactivated at the apex of each open vertical loop with 10 degree tip-back bend(the electric welding stop of type A is positionod at the mesial side of each loop). The aim of the present study was to identify when and which MVLAW is more effective to correct the buccal segment axes simultaneously. The Photoelastic overview of the upper and lower right quadrant showed that stress concentrations were observed in its photoelastic model. The obtained results were as follows : 1. Higher level compression can be seen clearly at the distal curvature of the lower 1st and 2nd molar when A type MVLAW was applied without short class m elastic, but mild compression cannot be seen at the distal curvature of lower anterior teeth using the class m elastic. 2. Higher concentration was presented at the mesial curvature from the lower 1st premolar to the 2nd molar than the anterior teeth when B type MVLAW without short class III elastic was applied, but using the short class III elastic, higher concentration of compression was presented in the anterior teeth area. 3. Areas of higher compression and tension were not observed at the mesial and distal curvature of the entire 1ower teeth except lower central and lateral incisors in C type MVLAW without short class III elastic, but using the short class III elastic, higher concentration was seen at the mesial curvature of the lower 1st premolar and 1ower anterior teeth.

The material and geometrical nonlinear finite elment analysis of UHPFRC 50M composite box girder was carried out. Constitute law in tension and compressive region of UHPFRC and HPC were modeled based on specimen test. The accuracy of nonlinear FEM analysis was verified by the experimental result of UHPFRC 50M composite girder. The UHPFRC 50M segmental composite box girder which has 1.5% steel fiber of volume fraction, 135MPa compressive strength and 18MPa tensile strength was tested. The post-tensioned UHPFRC composite girder consisted of three segment UHPFRC U-girder and High Strength Concrete reinforced slab. The parts of UHPFRC girder were modeled by 8nodes hexahedron elements and reinforcement bars and tendons were built by 2nodes linear elements by Midas FEA software. The constitutive laws of concrete materials were selected Multi-linear model both of tension and compression function under total strain crack model, which was included in classifying of smeared crack model. The nonlinearity of reinforcement elements and tendon was simulated by Von Mises criteria. The nonlinear static analysis was applied by incremental-iteration method with convergence criteria of Newton-Raphson. The validation of numerical analysis was verified by comparison with experimental result and numerical analysis result of load-deflection response, neutral axis coordinate change, and cracking pattern of girder. The load-deflection response was fitted very well with comparison to the experimental result. The finite element analysis is seen to satisfactorily predict flexural behavioral responses of post-tensioned, reinforced UHPFRC composite box girder.
Owing to needs of biomechanical comprehension and analysis to obtain various medical treatment designs which are related with the spine in order to cure and diagnose LBP patients, the FE modeling and nonlinear analysis of lumbosacrum including a partial ilium and iliolumbar ligaments, were carried out. First, we investigated whether the geometrical configuration of vertebrae displayed by DICOM slice files is regular and normal condition. After constructing spinal vertebrae including a partial ilium, a sacrum and five lumbars (from L1 to L5)with anatomical shape reconstructed using softwares such as image modeler and CAD modeler, we added iliolumbar ligaments, lumbar ligaments, discs and facet joints, etc.. And also, we assigned material property and discretized the model using proper finite element types, thus it was completely modeled through the above procedure. For the verification of each segment, average sagittal ROM, average coronal ROM and average transversal ROM under various loading conditions(${\pm}10Nm$), average vertical displacement under compression(400N), ALL(Anterior Longitudinal Ligament) and PLL(Posterior Longitudinal Ligament) force at L12 level, strains of seven ligaments on sagittal plane at L45 level and maximal strain of disc fibers according to various loading conditions at L45 level, etc., they were compared with experimental results. For the verification of multilevel-lumbosacrum spine including partial ilium and iliolumbar ligaments, the cases with and without iliolumbar ligaments were compared with ROM of experiment. The results were obtained from analysis of the verified FE model as follows: I) Iliolumbar ligaments played a stabilizing role as mainly posterior iliolumbar ligaments under flexion and as both posterior and anterior iliolumbar ligaments of one side under lateral bending. 2) The iliolumbar ligaments decreased total ROM of 1-8% in total model according to various motion conditions, which changed facet contact forces of L5S level by approximately 0.8-1.4 times and disc forces of L5S level by approximately 0.8-1.5 times more than casewithout ilioligaments, under various loading conditions. 3) The force of lower discs such as L45 and L5S was bigger than upper discs under flexion, left and right bending and left and right twisting, except extension. 4) It was predicted that strains of posterior ligaments among iliolumbar ligaments would produce the maximum 16% under flexion and the maximum 10% under twisting. 5) It's expected that this present model applies to the development and design of artificial disc, since it was comparatively in agreement with the experimental datum.

Recently, the use of signal compression methods to improve the efficiency of wireless networks have increased. In particular, the MPC system was used in the pitch extraction method and the excitation source of voiced and unvoiced to reduce the bit rate. In general, the MPC system using an excitation source of voiced and unvoiced would result in a distortion of the synthesis speech waveform in the case of voiced and unvoiced consonants in a frame. This is caused by normalization of the synthesis speech waveform in the process of restoring the multi-pulses of the representation segment. This paper presents an ACFBD-MPC (Amplitude Compensation Frequency Band Division-Multi Pulse Coding) using amplitude compensation in a multi-pulses each pitch interval and specific frequency to reduce the distortion of the synthesis speech waveform. The experiments were performed with 16 sentences of male and female voices. The voice signal was A/D converted to 10kHz 12bit. In addition, the ACFBD-MPC system was realized and the SNR of the ACFBD-MPC estimated in the coding condition of 8kbps. As a result, the SNR of ACFBD-MPC was 13.6dB for the female voice and 14.2dB for the male voice. The ACFBD-MPC improved the male and female voice by 1 dB and 0.9 dB, respectively, compared to the traditional MPC. This method is expected to be used for cellular telephones and smartphones using the excitation source with a low bit rate.
Pulmonary gangrene is a rare complication of severe pulmonary infection in which a pulmonary segment or lobe is sloughed. It is a part of a spectrum of disease in which lung tissue is devitalized(such as necrotizing pneumonia, pulmonary abscess), but apart from them, pulmonary gangrene has more extensive area of necrosis and thrombosis of large vessels plays a prominent role in the pathogenesis. We experienced a case of pulmonary gangrene in 71 year old female obstructive pneumonia patient with non-small cell lung carcinoma. She complained high fever, chill and despite treatment with antibiotics, pneumonia progressed to empyema. At that time chest radiograph showed a large cavity including sloughed lung tissue, freely moving to dependent position at both lateral decubitus view. RML and RLL were resected and compression of pulmonary vessels by enlarged lymph nodes was observed. Defervescence was obtained immediate postoperative period and the patient was discharged after infection control with antibiotics, chest tube drainage. The perivascular lymph nodes dissected during lobectomy were proved to be reactive hyperplasias. We speculated that the carcinoma caused obstructive pneumonia, in turn, resulted in reactive hyperplasia of the draining lymph nodes surrounding the large vessels and finally the lung tissues supplied by them necrotized and sloughed.

This paper proposes an effective watermarking approach for copyright protection of fire video images. The proposed watermarking approach efficiently utilizes the inherent characteristics of fire data with respect to color and texture by using a gray level co-occurrence matrix (GLCM) and fuzzy c-means (FCM) clustering. GLCM is used to generate a texture feature dataset by computing energy and homogeneity properties for each candidate fire image block. FCM is used to segment color of the fire image and to select fire texture blocks for embedding watermarks. Each selected block is then decomposed into a one-level wavelet structure with four subbands [LL, LH, HL, HH] using a discrete wavelet transform (DWT), and LH subband coefficients with a gain factor are selected for embedding watermark, where the visibility of the image does not affect. Experimental results show that the proposed watermarking approach achieves about 48 dB of high peak-signal-to-noise ratio (PSNR) and 1.6 to 2.0 of low M-singular value decomposition (M-SVD) values. In addition, the proposed approach outperforms conventional image watermarking approach in terms of normalized correlation (NC) values against several image processing attacks including noise addition, filtering, cropping, and JPEG compression.

The volume of shells, a prominent form of marine waste, is steadily increasing each year. However, a significant portion of these shells is either discarded or left near coastlines, posing environmental and social concerns. Utilizing shells as a substitute for traditional aggregates presents a potential solution, especially considering the diminishing availability of natural aggregates. This approach could effectively reduce transportation logistics costs, thereby promoting resource recycling. In this study, we explore the feasibility of employing wasted shell aggregates in 3D concrete printing technology for marine structures. Despite the advantages, it is observed that 3D printing concrete with wasted shells as aggregates results in lower strength compared to ordinary concrete, attributed to pores at the interface of shells and cement paste. Microstructure characterization becomes essential for evaluating mechanical properties. We conduct an analysis of the mechanical properties and microstructure of 3D printing concrete specimens incorporating wasted shells. Additionally, a mix design is proposed, taking into account flowability, extrudability, and buildability. To assess mechanical properties, compression and bonding strength specimens are fabricated using a 3D printer, and subsequent strength tests are conducted. Microstructure characteristics are analyzed through scanning electron microscope tests, providing high-resolution images. A histogram-based segmentation method is applied to segment pores, and porosity is compared based on the type of wasted shell. Pore characteristics are quantified using a probability function, establishing a correlation between the mechanical properties and microstructure characteristics of the specimens according to the type of wasted shell.
