방사선치료와 항암화학요법 등 암의 치료법이 점차 발전해감에 따라 치료후 장기간 생존하는 환자들이 많아지고 특히 소아암 치료후 장기 생존자가 증가하면서 여러가지 치료로 인한 합병증 및 문제점들이 발생하고 있다. 그중 중요한 하나가 이차암의 발생인데 본과에서 이차 악성 고형 종양 발생 환자 2예를 경험하였기에 보고하고자 한다. 한 예는 우측 슬와부에 발생한 rhab-domyosarcoma group II로 수술후 방사선치료 및 항암화학요법을 시행받았는데 3년7개월 후 방사선치료부위에서 osteosarcoma가 발생하였고 또 다른 한 예는 우측 하복부에 소장 악성 림프종이 발생하여 방사선치료 및 항암화학요법을 시행받았고 18년후에 방사선치료부위에 leimyosarcoma가 발생하였다. 문헌 고찰을 통해 소아암 치료후 이차암 발생의 위험 요인들을 고찰하였고 이차암에 대한 인식 및 세밀한 추적 조사가 필요함을 확인하였다.
Recently the construction of stations under railway lines and railway sections passing through central area of cities are increasing, calling for an urgent establishment of countermeasures against railway vibration and its subsequent second-phase noise. Of technology developed up to now, the most efficient countermeasure is the floating slab track, a track system isolated from the sub-structure by springs. Developed in this study, anti-vibration device for floating slab track (HLRM-High Load Rubber Mount) Haman station affiliated to the primary measurement was conducted in October 2012, one year after the second measurement after 2013 to be carried out in November and we want to change that. It is expected to raise awareness for the need of technology self-support and to make a meaningful contribution to mitigating vibration and noise produced by the next-generation high-speed railway.
Esophageal squamous cell carcinoma (ESCC) is the most common primary esophageal malignancy with poor prognosis. Here, due to the necessity for exploring potential therapies against ESCC, we obtained the gene expression data on ESCC from the TCGA and GEO databases. Venn diagram analysis was applied to identify common targets. The protein-protein interaction network was constructed by Cytoscape software, and the hub targets were extracted from the network via cytoHubba. The potential hub nodes as drug targets were found by pharmacophore-based virtual screening and molecular modeling, and the antitumor activity was evaluated through in vitro studies. A total of 364 differentially expressed genes (DEGs) in ESCC were identified. Pathway enrichment analyses suggested that most DEGs were mainly involved in the cell cycle. Three hub targets were retrieved, including CENPF, CCNA2 (cyclin A), and CCNB1 (cyclin B1), which were highly expressed in esophageal cancer and associated with prognosis. Moreover, amentoflavone, a promising drug candidate found by pharmacophore-based virtual screening, showed antiproliferative and proapoptotic effects and induced G1 in esophageal squamous carcinoma cells. Taken together, our findings suggested that amentoflavone could be a potential cell cycle inhibitor targeting cyclin B1, and is therefore expected to serve as a great therapeutic agent for treating esophageal squamous cell carcinoma.
The confusion of the sense of value on sex is increasing because of the rapid change in social-cultural environment. Also due to a rise in the standard of living, the age of adolescence is getting lower, and so the second sexual marks appear to primary schoolers. At this time in the aspect of the education for the whole man, it's very important for primary schoolers to acquire right knowledge and desirable attitude on sex so that can overcome psychological instability caused by physical growth, be responsible for their behaviors and lead happy lives. This study was made to find out the effects of sex education for the primary schoolers' knowledge and attitude about sex. The study was designed as simulated control group pretest-posttest design, which only pretest was practiced to control group and after sex education only posttest was practiced to experimental group. The data was collected for 18 days from March 13, 1995 to March 31, 1995. The subjects of this study were 130 six-graders in a private primary school in seoul. The control group and experimental group were composed of 65 pupils, each. Sex education consisted of lecture and discussion and the materials were this researcher's own made, 'How do I grow? and related OHP film and video. This education was practiced 40 minutes at a time, at intervals of 2-4 days' six times during 3weeks. For sex knowledge tools, 20 item questionaries on the base of related reference books and contents of this study were used. For sex attitude tools, this researcher's own made 13 item questionaries were used which were revised and complemented and laying stress on the reference books. The data was analyzed through pc-SAS program. The homogeneity test in terms of the general characteristics of experimental and control group was analyzed through $x^2$-test and t-test. And the difference in the primary schoolers' score on knowledge and attitude about sex before and after the sex education was analyzed through t-test and ANCOVA. The results of this study are as follows : 1. The result of the homogeneity test in terms of the general characteristics of the experimental and control group showed that there were significant differences in economic standard($x^2$=2.92, P=0.052) and brotherly ties($x^2$=3.78, P=0.052). 2. Hypothesis 'After sex education, the score of primary schoolers' sex knowledge will be higher than before. 'showed statistically significant difference.(t=11.99, P=.0001) Sex education was practiced under control over subjective view of economic standard and brotherly ties which showed significant difference at the homogeneity test in terms of the general characteristics between the two groups. After this education, the results of comparing the score of primary schoolers' sex knowledge also showed significant difference between the two groups. So hypothesis I was supported since primary schoolers to have education showed noticeable results in the sex knowledge score. (F=16.52, P=.0001) 3. Hypothesis 'After sex education, primary schooler's sex attitude score will be higher than before' showed statistically significant(t=5.08, P=.0001) Sex education was practiced, too, under control over subjective view of economic standard and brotherly ties which showed significant difference at the homogeneity test in terms of the general characteristics between the two groups. Bat this time the results of comparing the primary schoolers' sex attitude showed no significant difference between the two groups. So hypothesis II was rejected since for primary schoolers to have sex education showed no noticeable results in sex attitude score. (F=3.52, P=.0628) These results of the study show that sex education gives affirmative change to the primary schooler's sex knowledge, bat short periods' sex education doesn't give any change to the sex attitude. For the establishment of the pupils' desirable sex attitude, systematic and concrete sex education fit for the pupils' developing stage should be practiced over a long period of time.
PURPOSE. The purpose of this study was to compare the coronal bone level and patient satisfaction in 1-implant and 2-implant assisted mandibular overdentures. MATERIALS AND METHODS. Twenty patients who had maladaptive mandibular dentures were treated in this study. Patients were randomly divided into two groups. The first group received 1 implant (Simple line II, Implantium, South Korea) in their mandibular midline and the second group received 2 implants in their B and D regions (according to Misch's category). If the primary stability of each implant was at least 60 ISQ, ball attachment was placed and denture relined with soft liner. After 6 weeks, retentive cap incorporated with hard acrylic resin. In the 6 and 12 months recalls, periapical digital radiograph were made and visual analogue scale questionnaires were used to record patient satisfaction. The Friedman test was done for comparing the presurgical and postsurgical parameters in each group and the U-Mann Whitney test (P<.05) was done for comparison of post-treatment results between the two groups. RESULTS. All implants achieved sufficient primary stability to be immediately loaded. Patient satisfaction was high, and there were no significant differences between two groups (P>.05). In addition, mean marginal bone loss was $0.6{\pm}0.67$ mm in the first group and $0.6{\pm}0.51$ mm in the second group, after 12 month. Mean marginal bone loss showed no significant differences between two groups. CONCLUSION. This preliminary one-year result indicated that mandibular overdentures anchored to a single implant can be a safe and cost-effective method as a starting step for implant-overdenture treatment.
Medical verification of cancer diagnosis in insurance claims is a very important procedure in insurance administrations. Claims staffs are in need of medical experts' opinions about claim administration. This procedure is called medical claim review (MCR) and is composed of verification and advice. MCR verification evaluates the insured’s physical condition by medical records and compares it with product coverage. It is divided into assessment of living assurance benefit, verification of cancer, and assessment of the cause of death. Actually cancer verification of MCR is applicable to coding because the risk ratio in product development is usually coded data. There are some confusing neoplastic diseases in assessing the verification of cancer. This article reviews gastrointestinal stromal tumors (GIST) and mucosa-associated lymphoid tissue tumors (MALToma) of the stomach. The second most common group of stromal or mesenchymal neoplasms affecting the gastrointestinal tract is GIST. Nowadays there are many articles about the pathophysiology of GIST. However there are few confirmative theories except molecular cell biology of KIT mutation and some tyrosine kinase. Therefore, coding the GIST, which has previously been classified as an intermediate risk group according to NIH2001 criteria, for cancer verification of MCR is suitable for D37.1; neoplasm of uncertain or unknown behavior of digestive organs and the stomach. The gastrointestinal tract is the predominant site of extranodal non-Hodgkin's lymphomas. B-cell lymphomas of the MALT type, now called extranodal marginal zone B-cell lymphoma of MALT type in the REAL/WHO classification, are the most common primary gastric lymphomas worldwide. Its characteristics are as follows. First, it is different from traditional stomach cancers such as gastric adenocarcinoma. Second, the primary therapy of MALToma is the eradication of H. pylori by antibiotics and the remission rate is over 80%. Third, it has a different clinical course compared to traditional malignant lymphoma. Someone insisted that cancer verification is not possible for the above reasons. However, there have been findings on pathologic mechanism, and according to WHO classification, MALToma is classified into malignant B-cell lymphoma and it must be verified as malignancy in MCR.
The purpose of this study was to investigate perception of primary school teachers about operation of after-school computer class. Research questions were set up as follows: First, what type is preferred on the operation type for after-school computer class? Second, what is the perception about the adequacy of contents for after-school computer class? Third, what is the perception about instructors' expertise? Fourth, what is the perception about the effects of after-school computer class? Finally, what is the perception about facilities/ equipment and evaluation system of after-school computer class? To figure out these questions, this study conducted literature review and questionnaire survey. The results of this study are as follows: First, approximately 82% of respondents preferred to have after-school computer class operated by outsourced agency. Second, overall, the perceptions about the operation of after-school computer class were positive: The adequacy of contents, instructors' expertise, the effects, facilities/ equipment and evaluation system are seen as mostly positive. Third, its differences between school self-operated computer class and private sector computer class were insignificant. Fourth, its differences among various sizes of the school were significant. The implementaion of after-school computer class was considered as more positive in small schools. Finally, researchers suggested that it was necessary to develop an plan considering school sizes for more efficient implementation of after-school computer class.
Purpose: In this study, we apply environmental stress screening (ESS) to battle management system (BMS) of a tank and use the ESS profile based on production process data, guided by MIL-HDBK-781/344/2164. Methods: To optimize ESS Profile of the BMS of a tank, we estimate ESS model parameters (e.g., defect density, screening strength) using primary production failure reporting and corrective action system (FRACAS) data of military supply contract firm. Results: First, we collect the Primary production FRACAS data of military supply contract firm. Second, we compute curve fitting approach to find patent defect density and latent defect density using FRACAS data. Third, we solve the equation of Defect Density(patent defect density + latent defect density)($D_{IN}$) and Screening Strength(SS) Using second step data. As a result of analysis according to the order, we calculate $D_{IN}$(Temperature stress case : 74.02, Vibration stress : 10.252) and : SS(Temperature stress case : 0.4632, Vibration stress : 0.4142) and confirm the Condition II-D based on MIL-HDBK-344. According to Condition II-D, it is necessary to modify existing ESS profile through decreasing the $D_{IN}$ and increasing the SS. Conclusion: Identification of defect causes through ESS approach reduce defect densities for production. It provides feedback to a lessons-learned data base to avoid similar problems on next generation tank BMS.
Spectroscopic characteristics of river water from four major watersheds in the Ulsan area were measured to examine their potential for estimating water quality parameters. The total 176 river samples were collected from 44 sites of small streams within the watersheds during the year 2006. Spectroscopic characteristics investigated included protein-like fluorescence (FLF) intensity, fulvic-like fluorescence (FLF) intensity, terrestrial humic-like fluorescence (TLF) intensity, UV absorbance at 254 nm, and UV absorbance difference at 220 nm and 254 nm. Protein-like fluorescence intensity showed linear relationships with biochemical oxygen demand (BOD), chemical oxygen demand (COD), total phosphorous (TP) concentrations of the samples with the correlation of 0.784, 0.779, and 0.733, respectively. Due to the UV absorption characteristics of nitrate at 220 nm, UV absorbance difference at 220 nm and 254 nm was selected to represent total nitrogen (TN) concentration. Exclusion of some samples with PLF intensity higher than 5.0 improved the correlation between the UV absorbance difference and TN as demonstrated by the increase of the correlation coefficient from 0.392 to 0.784. Instead, for the samples with PLF intensity lower than 5.0, the highest correlation of TN was achieved with UV absorbance at 254 nm. The results suggest that PLF intensity could be used as the estimation index for BOD, COD, and TP concentration of river water, and as the primary screening index for the prediction of TN using UV absorbance difference. Some BOD-based water quality levels among the river water were statistically discriminated by the PLF intensity. Low p-values were obtained from the t-tests on the samples with the first level and the second level (p=0.0003) and the samples with the second and the third levels (p=0.0413). Our combined results demonstrated that the selected spectroscopic characteristics of river water could be utilized as a tool for on-site real-time monitoring and/or the primary estimation of water quality.
Purpose : The most patients with acute streptococcal pharyngitis lack of classic clinical manifestations, therefore diagnostic laboratory test such as the throat culture or a rapid antigen detection test are frequently employed in primary practices of developed countries. We'd like to know the accuracy of the throat swab culture as gold standard for diagnosis of streptococcal infection with studying the discordant and concordant rate of duplicate culture. Methods : The study included 89 normal school children (boys:50, girls:39) who were attending Uljin primary school in Uljin, Kyong Sang Buk Do on March 1996. We obtained simultaneous 2 times of throat swab from each subject, and plating and streaking on 5-7% of sheep blood agar separately. We counted the characteristic beta-hemolytic colonies after overnight incubation. Results : 1) The carrier rate of beta-hemolytic streptococci at first culture is 25.1% and second one is 29.2%. 2) Ten out of 89(11.2%) is discordant in duplicate culture. 3) Culture containing less than 50 colonies of beta-hemolytic streptococci (+2) in first culture is 70.4%, second one is 85.7%. 4) Number of colonies is less than 50 in all ten discordant children. Conclusions : The discordant rate of duplicate throat swab cullture for beta-hemolytic streptococci is 11.2%, even if the subjects are normal school children. About 5% of individuals harboring beta-hemolytic streptococci in the pharynx may be missed by a single throat culture. If we are trying to examine the patients with pharyngitis, the discordant rate will be much lower than this results.
