Comparison of knowledge, cognition and practice level on oral health care of the clients visiting dental clinic in Pusan area (부산지역 일부 치과의원 내원환자의 구강건강 지식 및 인식과 실천수준 비교)

  • Objectives : To propose the data for prevention of oral health problems through assessment on the knowledge, cognition and practice about oral health care of the clients who visit dental clinics. Methods : The subjects were total 400 clients from the four dental clinics in Busan city. Their knowledge, cognition and performances about oral healthcare collected with questionnaire survey from February to March, 2011. The rate and mean compared by chi-square test, t-test, and ANOVA. Results : Mean level of their knowledge, recognition and practice were 72.5, 80.2, and 65.6, respectively. Theses levels were higher in women (p<0.01), in the group of higher interested (p<0.001), immediately treated (p<0.001), correct tooth brushing (p<0.05), toothbrushing educated (p<0.001), take regular scaling (p<0.001), use assistant hygiene items (p<0.001), instead no differences by age and education level. And, the rate of correct toothbrushing was higher as 1.24 (1.03~1.50) times in the group with higher knowledge level, the rate of immediate treatment and regular scaling were higher as 1.35 (1.07~1.70) times, and 2.26 (1.41~3.64) times in the group with higher recognition level, respectively. Conclusions : These results demonstrate that insufficient knowledge and recognition of oral health care are related to a lack of its practice. Though their primary goal of the visits was treatment, the clients' attention needed to raise the comprehensive knowledge and recognition levels for their oral health promotion, especially reach to the performance level rather than simple knowledge acquisition. And more, to achieve the efficient oral health promotion, the importance of early treatment and strengthened efforts for prevention should be emphasized.

Effects of adjunctive daily phototherapy on chronic periodontitis: a randomized single-blind controlled trial

  • Purpose: The purpose of this randomized single-blind controlled trial was to elucidate the clinical and antimicrobial effects of daily phototherapy (PT) as an adjunct to scaling and root planing (SRP) in patients with chronic periodontitis. Methods: The study was conducted from December 2013 to May 2014 at Ewha Womans University Mokdong Hospital, Seoul, Korea. Forty-one patients with mild to moderate chronic periodontitis were randomly divided into two therapeutic groups in a 1:1 ratio: SRP+PT and SRP (control) groups. All participants underwent full-mouth SRP. PT was performed thrice a day for a month by using electric toothbrushes with embedded light-emitting diodes. Plaque index, gingival index, probing pocket depth (PPD), clinical attachment level (CAL), and bleeding on probing were assessed before (baseline) and four weeks after (follow-up) the treatment. Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Prevotella intermedia, Fusobacterium nucleatum, Parvimonas micra, Campylobacter rectus, Eikenella corrodens, Streptococcus mutans, and Streptococcus sobrinus levels were detected by a real-time polymerase chain reaction at the same points in time. Results: The clinical parameters improved in both the groups. At the follow-up assessment, PPD was significantly decreased in the SRP+PT group (P=0.00). Further, PPD and CAL showed significantly greater changes in the SRP+PT group than in the SRP group (PPD, P=0.03; CAL, P=0.04). P. gingivalis and T. forsythia levels decreased in this group, but no significant intergroup differences were noted. Conclusions: Adjunctive PT seems to have clinical benefits, but evidence of its antimicrobial effects is not sufficient. Long-term studies are necessary to develop the most effective PT protocol and compare the effectiveness of PT with and without exogenous photosensitizers.

Reliability and Validity on Measurement Instrument for Health Status Assessment in Occupational Workers (직장인들의 건강수준 평가를 위한 측정도구의 신뢰도와 타당도 분석)

  • In order to test scaling assumption, and to assess the validity, reliability, and acceptability of the Short form 36(SF-36) health survey questionnaire, we conducted a survey. Samples were 296 workers who had been employed in small sized companies. All scale passed for item internal consistency(100% sucess rate) and item discriminant validity(100% success .ate). Reliability coefficients were ranged from the lowest 0.51 to the highest of 0.85. For 87.5% of the total workers, inconsistent responses were not observed. Only 3.0% of the total workers failed two or more checks. Factor analysis was performed using principal axis factor method and quartimax rotation. In this survey, the SF-36 retained available psychometric properties even when used in a generally healthy worker group. But further study with some consideration to develope health status measurement is expected : first, the definition of health status should be rationalized. Second, the measurement of outcome is an important consideration in evaluations of quality of care. But ambiguities hinder understanding of this important topic. Third, internal consistency should be interpreted with caution as an indication reliability because it ignores potentially important sources of variation that can occur over time.

A Durability Assessment on Complex Deterioration of Concrete with Ground Granulated Blast-Furnace Slag Replacement (복합열화 환경하에서의 고로슬래그미분말 사용 콘크리트의 내구성능 평가)

  • This paper presents the experimental results of frost durability characteristics including freezing-thawing and de-icing salt scaling of the concrete for gutter of the road and marine structure. Mixtures were proportioned with the three level of water-binder ratio(W/B) and three binder compositions corresponding to Type I cement with 0%, 30% and 50% GGBS(Ground granulated blast furnace slag) replacement. Also, two different solutions of calcium chloride were used to evaluate their effect on the frost durability resistance. Specially, in case of complex of freezing and thawing with salt and carbonation, the deterioration of concrete surface is evaluated. Test results showed that the BFS30 and BFS50 mixture exhibited higher durability and lower mass loss values than those made with OPC mix and the use of GGBS can be used effectively in terms of economy and frost durability of the concrete to be in complex deterioration. Therefore, the resistance to complex deterioration with freezing-thawing was strongly influenced by the strength and the type of cement.

Awareness and educational needs on preventive dental treatment among oral health workers (구강보건인력의 예방치과진료에 대한 인식 및 교육요구도)

  • Objectives: The objective of this study is to provide basic data needed in developing an educational program designed to upgrade capacity and awareness of preventive dental treatment among oral health workers, by analyzing levels of awareness of preventive dental treatment and educational needs among dentists and dental hygienists. Methods: The collected data was analyzed with SPSS program ver. 19.0. The data was under t-test. Results: The frequency level of giving preventive dental treatment to patients among dentists and dental hygienists is below mid-point, 3 on the 5-point Likert scale. In terms of frequency level per item, scaling & polishing was ranked the highest, followed by periodontal maintenance, tooth-brushing instruction, and prescription and instruction of oral care product in descending order. On the questions asking how important preventive dental care they perceive to be, both dentists and dental hygienists perceived it to be highly important. When they were asked to rank those items by the importance of education, they considered periodontal maintenance as the most important one, followed by individual education of oral health, incremental oral health care, scaling& polishing, toothbrushing instruction, and prescription and instruction of oral care product. Respondents pointed out problems in running a preventive dental treatment program as follows: overwork, lack of dedicated workforce, un-fixed costs, and lack of necessary equipment. When they were asked to point out items needed to run such a program, the largest number of respondents indicated dedicated workforce placement, followed by improving awareness of the customer, and improving awareness of the dental workers. Conclusions: In order to effectively run a preventive dental treatment program, it is necessary for oral health workers to clearly understand the concept of it. It is also necessary to develop and operate an education program on preventive dental treatment targeting oral health professionals.

The Impact of Environmental Health Factors on Extreme-heat Vulnerability Assessment in a Metropolitan City (환경보건적 요소가 도시 내 폭염 취약성 평가 결과에 미치는 영향 분석)

  • Objectives: This analysis seeks to evaluate the impact of environmental health factors (EHF; e.g. hospital beds per capita, employees of medical institutions) on extreme-heat vulnerability assessment in Busan Metropolitan City during 2006-2010. Methods: According to the vulnerability concept suggested by the Intergovernmental Panel on Climate Change (IPCC), extreme-heat vulnerability is comprised of the categories of Exposure, Sensitivity, and Adaptive Capacity (including EHF). The indexes of the Exposure and Sensitivity categories indicate positive effects, while the Adaptive capacity index indicates a negative effect on extreme-heat vulnerability. Variables of each category were standardized by the re-scaling method, and then each regional relative vulnerability was computed with the vulnerability index calculation formula. Results: The extreme-heat vulnerability index (EVI) excepting EHF was much higher in urban areas than in suburban areas within the metropolitan area. When EHF was considered, the difference in the EVI between the two areas was reduced due to the increase of the Adaptive capacity index in urban areas. The low EVI in suburban areas was induced by a dominant effect of natural environmental factors (e.g. green area) within the Adaptive capacity category. Conclusions: To reduce the vulnerability to extreme heat in urban areas, which were more frequently exposed to extreme heat than others areas, public health and natural environments need to be improved in sensitive areas.

Environmental Risk Assessment of Watermelon Grafted onto Transgenic Rootstock Resistant to Cucumber Green Mottle Mosaic Virus (CGMMV) on Non-Target Insects in Conventional Agro-Ecosystem

  • We investigated the impact of watermelon grafted onto Cucumber Green Mottle Mosaic Virus (CGMMV)-resistant transgenic watermelon rootstock on insects as non-target organisms in a greenhouse in 2005. We quantitatively collected insect assemblages living on leaves and flowers, and we used sticky traps to collect alate insects. We compared the patterns of insect assemblages and community composition, cotton aphid (Aphis gossypii Glover) on watermelon leaves and western flower thrip (Frankliniella occidentalis Trybom) on watermelon male flowers, between CGMMV-resistant transgenic watermelon (TR) and non-transgenic watermelon (nTR). Non-parametric multidimensional scaling (NMS) ordination verified that insect assemblages on leaves and sticky traps were different between TR and nTR (P<0.05). The insect assemblages on male flowers were not statistically significant. Multi-response permutation procedures proofed our results from NMS results (P>0.05). Conclusively, TR watermelons appear to have some adverse effects on the population of cotton aphids on leaves and sticky traps, but watermelon male flowers do not show an adverse effect. Further research is required to assess the effect of TR on the aphid and western flower thrip. Life table experiments might support the specific reason for the adverse effects from leaf assemblages. Assessment of non-target impacts is an essential part of the risk assessment of non-target insects for the impact of transgenic organisms.

Assessment of Counting Efficiency of a Whole Body Counter by Human Body Size and Standing Position Using Monte Carlo Method (몬테카를로 방법론을 이용한 측정 대상의 인체 크기와 측정 위치에 따른 전신계수기 계수효율 평가)

  • For the case of radiation emergency, it is required to assess internal contamination of the public, including children as well as adults. The objective of the present study was to assess counting efficiency of a whole body counter by human body size and standing position of the measurement person. In this study, the FASTSCAN whole body counter used at National Radiation Emergency Medical Center of Korean Institute of Radiological and Medical Science was simulated by a radiation transport computer code. The simulation results of the counting efficiencies agreed well with measurements within the 2% of discrepancy for 4-year child and 5% for adults. The standing positions of the people were adjusted by body size to find the consistent trend of the counting efficiencies by human body size. Body size scaling factors of the whole body counter were derived to consider human body size and improve the measurement accuracy. The counting efficiency assessment methodology in this study can be successively used to improve the measurement accuracy when using a whole body counter for the case of radiation emergency.

Regional Categorization of Gyeonggi Province for Fine Dust Management (경기도 지역 미세먼지 관리를 위한 권역 범주화 연구)

  • The similarity of hourly PM10 and PM2.5 concentration profiles of the atmospheric monitoring stations in Gyeonggi-do was evaluated through the multilateral analysis between stations. The existing category for most stations in the regions shows relatively low Pearson correlation values of 0.68 and 0.7 for PM10 and PM2.5 on average respectively, and some monitoring stations revealed high relationships over 0.8 to other regions. Since the current regions are mainly categorized by cluster analysis based on the number of occurrence of high concentration events and geological factors, it is necessary to reclassify them by concentration characteristics for precise fine dust management. In accordance, multi-dimensional scaling being able to visualize could categorize the regions based on regional emission contribution rate and hourly fine dust concentration. As a result of the current analysis, PM10 and PM2.5 could be reclassified into five regions and fourregions, respectively.

Assessment of ECMWF's seasonal weather forecasting skill and Its applicability across South Korean catchments (ECMWF 계절 기상 전망 기술의 정확성 및 국내 유역단위 적용성 평가)

  • Due to the growing concern over forecasting extreme weather events such as droughts caused by climate change, there has been a rising interest in seasonal meteorological forecasts that offer ensemble predictions for the upcoming seven months. Nonetheless, limited research has been conducted in South Korea, particularly in assessing their effectiveness at the catchment-scale. In this study, we assessed the accuracy of ECMWF's seasonal forecasts (including precipitation, temperature, and evapotranspiration) for the period of 2011 to 2020. We focused on 12 multi-purpose reservoir catchments and compared the forecasts to climatology data. Continuous Ranked Probability Skill Score method is adopted to assess the forecast skill, and the linear scaling method was applied to evaluate its impact. The results showed that while the seasonal meteorological forecasts have similar skill to climatology for one month ahead, the skill decreased significantly as the forecast lead time increased. Compared to the climatology, better results were obtained in the Wet season than the Dry season. In particular, during the Wet seasons of the dry years (2015, 2017), the seasonal meteorological forecasts showed the highest skill for all lead times.
