Atherectomy in Peripheral Artery Disease: Current and Future (말초동맥질환에서의 죽종절제술: 현재와 미래)

  • Atherectomy has become a promising treatment option for peripheral artery disease caused by diabetes mellitus or end-stage renal disease. Atherectomy refers to the removal of atheromatous tissue by mechanical method, resulting in an enlarged lumen of the treated blood vessel. Based on this method, the term is limited to the percutaneous minimally invasive approach, and there are currently two types of atherectomy devices available in Korea. The increased prevalence of atherectomy has led to the concept of "vascular preparation" and a new treatment concept of "leave nothing behind." Various studies have proven the safety and effectiveness of atherectomy; however, there are some limitations. We need to remain focused on patient selection and subsequent large-scale research.

Fenton Oxidation of Landfill Leachate by $Fe^0,\;Fe^{2+},\;Fe^{3+}\;/\;H_2O_2$ Systems ($Fe^0$, $Fe^{2+}$, $Fe^{3+}$ / $H_2O_2$ 시스템을 이용한 침출수의 Fenton 산화반응)

  • This investigation aimed at selecting the optimum catalyst and reaction conditions used in Fenton oxidation for landfill leachate treatment and was carried out at ambient temperature using a lab-scale experiment. The investigation led to the following results: 1) The optimum pH and dose for each iron catalyst were as follows: $Fe^{2+}\;=\;1,200\;mg/L$, $H_2O_2\;=\;1,200\;mg/L$, initial pH=3.0; $Fe^{3+}\;=\;1,200\;mg/L$, $H_2O_2\;=\;1,500\;mg/L$, initial pH=4.5; $Fe^0\;=\;1,200\;mg/L$, $H_2O_2\;=\;900\;mg/L$, initial pH=4.0, respectively. 2) The progress of Fenton oxidation could be instrumentally monitored by measuring redox potential evolution during leachate oxidation, thus, indicating the possibility of an on-line process monitoring. 3) A simple acid-base titration of Fenton-treated leachate proved that a relevant fraction of by- products formed during the treatment was made of acidic compounds in the optimum reaction condition for each catalyst used, thus demonstrating that the higher the extent of Fenton oxidation the greater was the amount of acids formed. 4) With the aim of selecting the optimum catalyst among $Fe^0$, $Fe^{2+}$ and $Fe^{3+}$, removal efficiency of each parameter in the optimum reaction conditions was considered. Although $Fe^{3+}$ was higher than other catalysts($Fe^0$, $Fe^{2+}$) in removal efficiency, $Fe^0$ was a optimum catalyst with a view of cost effectiveness.

Remediation Process by using Lime and Calcium Carbonate for Heavy Metal Contaminated Groundwater Originated from Landfills (소석회$(Ca(OH)_2)$와 탄산칼슘$(CaCO_3)$을 이용한 매립장 주변 중금속 오염 지하수 정화)

  • Coagulation and precipitation process by using lime$(Ca(OH)_2)$ and calcium carbonate $(CaCO_3)$ were applied to remove heavy metals from groundwater in laboratory scale. From results of batch tests, by the addition of $0.3\;wt.\%$ lime, more than $90\%$ of As and Mn were removed and $70-80\%$ of Cd and Zn were removed by using $0.5\;wt.\%$ of lime. Removal efficiency of Pb almost reached $100\%$ with only $0.1\;wt.\%$ of calcium carbonate and more than $93\%$ of Cd were removed by the addition of $0.1\;wt.\%$of calcium carbonate. Pilot scale column experiments were performed to remove heavy metals in the separation process of precipitated Hoc to supernatant after the coagulation/ precipitation. For lime as a coagulant, more than $99\%$of As were removed from artificial groundwater and removal efficiencies of Cd, Mn, and Zn were over $80\%$. By using calcium carbonate, more than $95\%$ of Cd and Pb were removed in column experiment. Fe and Mn contaminated groundwater taken from a real landfill site, Ulsan was used for the column experiment and more than $99\%$ of Fe and Mn were removed by the addition of $1\;wt.\%$ lime in column experiment, suggesting that the coagulation/precipitation process by using lime and calcium carbonate have a great possibility to remove heavy metals from contaminated groundwater.

Pilot-scale Applications of a Well-type Reactive Barrier using Autotrophic Sulfur-oxidizers for Nitrate Removal (독립영양 황탈질 미생물을 이용한 관정형 반응벽체의 현장적용성 연구)

  • The applicability of a well-type autotrophic sulfur-oxidizing reactive barrier (L $\times$ W $\times$ D = $3m\;{\times}\;4\;m\;{\times}\;2\;m$) as a long-term treatment option for nitrate removal in groundwater was evaluated. Pilot-scale (L $\times$ W $\times$ D = $8m\;{\times}\;4\;m\;{\times}\;2\;m$) flow-tank experiments were conducted to examine remedial efficacy of the well-type reactive barrier. A total of 80 kg sulfur granules as an electron donor and Thiobacillus denitrificans as an active bacterial species were prepared. Thiobacillus denitrificans was successfully colonized on the surface of the sulfur granules and the microflora transformed nitrate with removal efficiency of ~12% (0.07 mM) for 11 days, ~24% (1.3 mM) for 18 days, ~45% (2.4 mM) for 32 days, and ~52% (2.8 mM) for 60 days. Sulfur granules attached to Thiobacillus denitrificans were used to construct the well-type reactive barrier comprising three discrete barriers installed at 1-m interval downstream. Average initial nitrate concentrations were 181 mg/L for the first 28 days and 281 mg/L for the next 14 days. For the 181 mg/L (2.9 mM) plume, nitrate concentrations decreased by ~2% (0.06 mM), ~9% (0.27 mM), and ~15% (0.44 mM) after $1^{st}$, $2^{nd}$, and $3^{rd}$ barriers, respectively. For the 281 mg/L (4.5 mM) plume, nitrate concentrations decreased by ~1% (0.02 mM), ~6% (0.27 mM), and ~8% (0.37 mM) after $1^{st}$, $2^{nd}$, and $3^{rd}$ barriers, respectively. Nitrate plume was flowed through the flow-tank for 49 days by supplying $1.24\;m^3/d$ of nitrate solution. During nitrate treatment, flow velocity (0.44 m/d), pH (6.7 to 8.3), and DO (0.9~2.8 mg/L) showed little variations. Incomplete destruction of nitrate plume was attributed to the lack of retention time, rarely transverse dispersion, and inhibiting the activity of denitrification enzymes caused by relatively high DO concentrations. For field applications, it should be considered increments of retention time, modification of well placements, and intrinsic DO concentration.

The Importance of Video Fluoroscopy Swallowing Study for Nasogastric Tube Removal in Rehabilitation Patients (재활치료환자의 비위관(nasogastric tube)제거에 따른 비디오 투시연하검사(VFSS)의 중요성 평가)

  • Acute phase patients who are unconscious and are suffering from cerebral infarction, cranial nerve disorders, or cerebral apoplexy are susceptible to aspiration pneumonia due to dysphagia. In these cases, a nasogastric tube is inserted to supply nutrients. Although bedside screening tests are administered during recovery after rehabilitation, clinical examinations may not be able to ascertain asymptomatic aspiration. Therefore, a video fluoroscopy swallowing study (VFSS) was performed in 10 patients with dysphagia after rehabilitation therapy; these patients had nasogastric tubes inserted, and a rehabilitation specialist assessed the degree of swallowing based on the patients' diet and posture. If aspiration or swallowing difficulties were observed, dysphagia rehabilitation therapy was administered. The patients were reassessed approximately 30-50 days after administration of therapy, based on the patients' condition. If aspiration is not observed, the nasogastric tube was removed. A functional dysphagia scale was used to analyze the VFSS images, and the scores were statistically calculated. The mean score of patients with nasogastric tubes was $49.79{\pm}9.431$, thereby indicating aspiration risk, whereas the group without nasogastric tubes showed a mean score of $11.20{\pm}1.932$, which indicated low risk of aspiration. These results demonstrated that a significantly low score was associated with nasogastric tube removal. Mann-Whitney's test was performed to assess the significance of both the groups, and the results were statistically significant with a P value <0.001. In conclusion, VFSS can effectively assess the movements and structural abnormalities in the oral cavity, pharynx, and esophagus. It can also be used to determine the aspiration status and ascertain the appropriate diet or swallowing posture for the patient. Therefore, VFSS can potentially be used as a reliable standard test to assess swallowing in order to determine nasogastric tube removal.

A Study on Process Optimization for CSOs Application of Horizontal Flow Filtration Technology (수평흐름식 여과기술의 CSOs 적용을 위한 공정 최적화 연구)

  • The management of Combined Sewer Overflows(CSOs) and Separated Sewer Overflows(SSOs) discharge directly to the effluent system in an untreated state, which occurs when the facility capacity is exceeded due to heavy rain, has become an important issue in recent years as the heavy rain becomes a regular phenomenon. Despite the continuous development of filtration technology, targeting densely populated urban areas, CSOs are rarely applied. Therefore, this study was carried out to optimize the process to apply CSOs in a pilot-scale horizontal flow filtration system with a rope-type synthetic fiber. The research was carried out in two steps: a preliminary study using artificial samples and a field study using sewage. In the preliminary study using an artificial sample, head loss of the filter media itself was analyzed to be approximately 1.1cm, and the head loss was increased by approximately 0.1cm as the linear velocity was increased by 10m/hr. In addition, the SS removal efficiency was stable at 81.4%, the filtration duration was maintained for more than 6 hours, and the average recovery rate of 98% was obtained by air backwashing only. In the on-site evaluation using sewage, the filtration duration was approximately 2 hours and the average removal efficiency of 83.9% was obtained when belt screen (over 450 mesh) was applied as a pre-treatment process to prevent the premature clogging of filter media. To apply the filtration process to CSOs and SSOs, it was concluded that the combination with the pre-treatment process was important to reinforce the hydraulic dimension for the stable maintain of operation period, rather than efficiency. Compared to the dry season, the quality of incoming sewage was lower in the rainy season, which was attributed to the characteristics of the drainage area with higher sanitary sewerage. In addition, the difference in removal efficiency according to the influent quality of the wet season and dry season was small.

A Prediction Model for Removal of Non-point Source Pollutant Considering Clogging Effect of Sand Filter Layers for Rainwater Recycling (빗물 재활용을 위한 모래 정화층의 폐색특성을 고려한 비점오염원 제거 예측 모델 연구)

  • An artificial rainwater reservoir installed in urban areas for recycling rainwater is an eco-friendly facility for reducing storm water effluence. However, in order to recycle the rainwater directly, the artificial rainwater reservoir requires an auxiliary system that can remove non-point source pollutants included in the initial rainfall of urban area. Therefore, the conventional soil filtration technology is adopted to capture non-point source pollutants in an economical and efficient way in the purification system of artificial rainwater reservoirs. In order to satisfy such a demand, clogging characteristics of the sand filter layers with different grain-size distributions were studied with real non-point source pollutants. For this, a series of lab-scale chamber tests were conducted to make a prediction model for removal of non-point source pollutants, based on the clogging theory. The laboratory chamber experiments were carried out by permeating two types of artificially contaminated water through five different types of sand filter layers with different grain-size distributions. The two artificial contaminated waters were made by fine marine-clay particles and real non-point source pollutants collected from motorcar roads of Seoul, Korea. In the laboratory chamber experiments, the concentrations of the artificial contaminated water were measured in terms of TSS (Total Suspended Solids) and COD (Chemical Oxygen Demand) and compared with each other to evaluate the performance of sand filter layers. In addition, the accumulated weight of pollutant particles clogged in the sand filter layers was estimated. This paper suggests a prediction model for removal of non-point source pollutants with theoretical consideration of the physical characteristics such as the grain-size distribution and composition, and change in the hydraulic conductivity and porosity of sand filter layers. The lumped parameter ${\theta}$ related with the clogging property was estimated by comparing the accumulated weight of pollutant particles obtained from the laboratory chamber experiments and calculated from the prediction model based on the clogging theory. It is found that the lumped parameter ${\theta}$ has a significant influence on the amount of the pollutant particles clogged in the pores of sand filter layers. In conclusion, according to the clogging prediction model, a double-sand-filter layer consisting of two separate layers: the upper sand-filter layer with the effective particle size of 1.49 mm and the lower sand-filter layer with the effective particle size of 0.93 mm, is proposed as the optimum system for removing non-point source pollutants in the field-sized artificial rainwater reservoir.

Study on the Combination of In-situ Chemical Oxidation Method by using Hydrogen Peroxide with the Air-sparging Method for Diesel Contaminated Soil and Groundwater (과산화수소를 이용한 현장원위치 화학적 산화법과 공기분사법(Air-sparging)을 연계한 디젤 오염 토양/지하수 동시 정화 실내 실험 연구)

  • Laboratory scale experiments were performed to investigate the removal efficiency of the in-situ chemical oxidation method and the air-sparging method for diesel contaminated soil and groundwater. Two kinds of diesel contaminated soils (TPH concentration : 2,401 mg/kg and 9,551 mg/kg) and groundwater sampled at Busan railroad station were used for the experiments. For batch experiments of chemical oxidation by using 50% hydrogen peroxide solution, TPH concentration of soil decreased to 18% and 15% of initial TPH concentration. For continuous column experiments, more than 70% of initial TPH in soil was removed by using soil flushing with 20% hydrogen peroxide solution, suggesting that most of diesel in soil reacted with hydrogen peroxide and degraded into $CO_2$ or $H_2O$ gases. Batch experiment for the air-sparging method with artificially contaminated groundwater (TPH concentration : 810 mg/L) was performed to evaluate the removal efficiency of the air-sparging method and TPH concentration of groundwater decreased to lower than 5 mg/L (waste water discharge tolerance limit) within 72 hours of air-sparging. For box experiment with diesel contaminated real soil and groundwater, the removal efficiency of air-sparging was very low because of the residual diesel phase existed in soil medium, suggesting that the air-sparging method should be applied to remediate groundwater after the free phase of diesel in soil medium was removed. For the last time, the in-situ box experiment for a unit process mixed the chemical oxidation process with the air-sparging process was performed to remove diesel from soil and groundwater at a time. Soil flushing with 20% hydrogen peroxide solution was applied to diesel contaminated soils in box, and subsequently contaminated groundwater was purified by the air-sparging method. With 23 L of 20% hydrogen peroxide solution and 2,160 L of air-sparging, TPH concentration of soil decreased from 9,551 mg/kg to 390 mg/kg and TPH concentration of groundwater reduced to lower than 5 mg/L. Results suggested that the combination process of the in-situ hydrogen peroxide flushing and the air-sparging has a great possibility to simultaneously remediate fuel contaminated soil and groundwater.

Treatment of Animal Wastewater Using Woodchip Trickling Filter System and Physical and Microbial Characteristics of Wood Chip Media (목편살수여상조를 이용한 축산뇨오수 처리와 목편여재의 물성 및 부착미생물 특성)

  • Trickling filter has been extensively studied for the domestic wastewater treatment especially for the small scale plants in rural area. The purpose of this research is to survey the physical and microbial characteristics of wood chip media and the removal efficiency of animal wastewater using wood chip trickling filter system. The trickling filtration system comprises a filtration bed packed with wood chip media having a particle dia. of 5~7cm. The method comprises natural air from the bottom of the bed. The system also comprises a control mechanism including a time a constant discharge pump for controlling supply of the wastewater into the bed. The following conclusions were obtained from the results of this research. 1. The specific surface area of wood chip was 0.4123 $m^2$/g, pore volume was 0.0947 $cm^3$/g, density was 0.49 g/$cm^3$. It has forms of parallelogram and oblong which have numerous small pore space. This wood chip has been good condition for microorganism's habitat, having very larger specific surface area by complex the three dimension structure of cellulose at wood's major ingredients. 2. The total counts of in attached aerobic microbes were ranged from $10^6$ to $10^8$ CFU/g, and anaerobes microbial numbers were from $10^4$ to $10^7$. The aerobic microbial numbers appeared to be much more than those of anaerobic microbial numbers. 3. The average efficiency of $BOD_5$ and CODcr were 74.5% and 51.5%, respectively. The removal efficiency of T-N and T-P were 61.4%, 56.2%, respectively. But SS removal levels remain 19.3%.

Current status and future of insect smart factory farm using ICT technology (ICT기술을 활용한 곤충스마트팩토리팜의 현황과 미래)

  • In the insect industry, as the scope of application of insects is expanded from pet insects and natural enemies to feed, edible and medicinal insects, the demand for quality control of insect raw materials is increasing, and interest in securing the safety of insect products is increasing. In the process of expanding the industrial scale, controlling the temperature and humidity and air quality in the insect breeding room and preventing the spread of pathogens and other pollutants are important success factors. It requires a controlled environment under the operating system. European commercial insect breeding facilities have attracted considerable investor interest, and insect companies are building large-scale production facilities, which became possible after the EU approved the use of insect protein as feedstock for fish farming in July 2017. Other fields, such as food and medicine, have also accelerated the application of cutting-edge technology. In the future, the global insect industry will purchase eggs or small larvae from suppliers and a system that focuses on the larval fattening, i.e., production raw material, until the insects mature, and a system that handles the entire production process from egg laying, harvesting, and initial pre-treatment of larvae., increasingly subdivided into large-scale production systems that cover all stages of insect larvae production and further processing steps such as milling, fat removal and protein or fat fractionation. In Korea, research and development of insect smart factory farms using artificial intelligence and ICT is accelerating, so insects can be used as carbon-free materials in secondary industries such as natural plastics or natural molding materials as well as existing feed and food. A Korean-style customized breeding system for shortening the breeding period or enhancing functionality is expected to be developed soon.
