Long-term Follow-up of Limited T3 Symathicotomy in Palmar Hyperhidrosis (수장부 다한증에 제한적 교감신경절간 절단술의 장기 고찰)

  • Background: Conventional thoracoscopic sympathectomy or sympathicotomy is an effective method in treating localized hyperhidrosis; however, this may result in a postoperatively compensatory hyperhidrosis or facial anhidrosis in the treatment of palmar hyperhidrosis. We modified the conventional sympathicotomy by limiting the extent of nerve transection (limited T3 sympathicotomy) since May 1998. However, there are many reports of a good short-term outcome of limited T3 sympathicotomy. Therefore, we reviewed long-term follow-up of limited T3 sympathicotomy based on outcomes analysis using a questionnaire. Methods: Fifty four patients with palmar hyperhidrosis underwent a limited T3 sympathicotomy between May 1998 and March 1999 and had a complete follow-up over two years using a questionnaire (the mean follow-up was 2.6 years). The patients' postoperative satisfaction was determined by their subjective responses to the questionnaires; the degree of compensatory hyperhidrosis, the effects on foot hyperhidrosis, gustatory hyperhidrosis and facial dryness, and recurrence, and patient's satisfaction. Results: Of the total, 87% of patients had a compensatory hyperhidrosis and 3.7% of them were disabled. 31.5% of patients showed improvement in foot hyperhidrosis, while 68.5% of patients demonstrated no change or got worse. 31.5% of patients had gustatory hyperhidrosis and facial dryness and 22.2% of patients showed a mild palmar hyperhidrosis. The postoperative patients' satisfaction was significantly in 96.3% of patients. Conclusions: The limited T3 sympathicotomy is a highly effective treatment of palmar hyperhidrosis and has a low rate of postoperative compensatory hyperhidrosis, gustatory hyperhidrosis, and facial dryness.

Factors Influencing on Medical Service Satisfaction of Patients Utilizing a Day-Case Surgery Center in Korea (국내 당일수술센터 이용환자의 의료서비스 만족도에 영향을 미치는 요인)

  • This study was attempted to identify the factors relating to the medical service satisfaction on the patients utilizing a day-case surgery center in Korea. A questionnaire survey was conducted to patients were utilizing a day-surgery center, independently operated by one general hospital from January 13 to February 13, 2009. The results of this study can be summarized as follows. First, in the characteristic related with health and medical situations, day-surgery patients had little experience with surgery or hospitalizing and much interest in their health. Secondly, there were significant differences in regard to the quality of medical service for surgery waiting time and surgery time(p<0.05). Thirdly, multiple regression analysis revealed that age and level of education affected patients perception of day surgery(p<0.05). In conclusion, in order to increase the rate of day-surgery, it is necessary to give information about the advantages of day-surgery center and preoperative education.

Evaluation on the Outdoor Space in Farmhouse (농가 옥외공간의 평가 - 옥외공간의 구성요소를 중심으로 -)

  • The purpose of this study is to clarify the components for the improvement of outdoor spaces in farmyards and to suggest guidelines for the planning of these spaces. The objects of this study were the general farmyards located around farmhouses which had government support for the construction of on-site gardens. The results were as follows: 1. Although over 50% of the general farmyards did not have gardens, most gardens of farmyards (90% and over) featured trees and/or flowers. This demonstrated that the residents of the farmyards thought positively about green spaces. 2. With the establishment of a garden, the self-image of the household improved along with the physical environment. Thus, the construction of the garden brought about psychologically and physically positive results. 3. Regarding the degree of satisfaction with green space and shade, the rate of farmyards with gardens was higher than those without. With more government support for developing such green spaces, satisfaction with them could increase. The same result was obtained regarding the degree of satisfaction towards visual harmony with nearby farmyards, indicating that neighboring farm households want to have positive relationships with one another. 4. The frequency of use of living space increased through the construction of gardens in farmyards, in spite of decreased working space. %is aspect could be used as data in the planning of gardens in farmyards. 5. The perceived necessity of gardens in the general farmyards and desire to construct new gardens rose. This preference was high in gardens with abundant greenery and a lot of rest facilities. This study suggests those reasons result from a poor farm economy. With government support, general farmyards without gardens could construct gardens. And we might infer that the residents' desire to construct a garden in general famyards had been influenced by a neighbor with a garden.

The Prognostic Factors Influencing on the Therapeutic Effect of Percutaneous Vertebroplasty in Treating Osteoporotic Vertebral Compression Fractures

  • Objective : This retrospective study of 215 patients with 383 symptomatic osteoporotic vertebral compression fractures (VCFs) treated by percutaneous vertebroplasty (PVP), was performed to evaluate the clinical outcomes, and to analyze the various clinical factors affecting these results. Methods : The authors assessed the clinical outcome under the criteria such as the pain improvement, activity, requirement of analgesics, and the patient's satisfaction, and determined the relation to various peri- and intra-operative factors, and postoperative imaging findings. Results : The outcome was determined as 84.2% in relief of pain, 72.0% in change in activity, 65.7% in analgesics use, and 84.7% of satisfaction rate. More severe focal back pain, high uptake bone scan, and the lower mean T-score were related to the better pain relief following PVP. The longer the duration between fracture and PVP, the less severe focal back pain, low uptake bone scan, and leakage of PMMA into the paravertebral space were related to the less improvement in activity. Female and low uptake bone scan showed a correlation with more analgesic use. The longer the duration between fracture and PVP, low uptake bone scan, and the higher the mean T-score were correlated with the less the patients satisfaction. Conclusion : Our study suggests that PVP may be more effective in the acute phase of VCFs, more severe focal pain, and far advanced osteoporosis on BMD. Leakage of PMMA into the paravertebral spcae also could be affecting the surgical results.

Parents' perception on middle school foodservice using environment friendly agricultural products (EFAP) in Masan area (마산시 중학교 학부모의 친환경 농산물 학교급식에 대한 인식 조사)

  • The purpose of this study was to investigate parent's perception of a middle school foodservice in Masan city, Korea that uses environmentally friendly agricultural products (EFAP). From May 29, 2009 to July 11, 2009, questionnaires were distributed to 350 parents of middle schools located in Masan city. A total of 295 responded and 276 (response rate, 78.9%) were used for the final analysis after excluding improperly completed questionnaires. First, 47.8% of the parents responded that they had participated in the school foodservice operation (receiving food, attending foodservice-related events, working as a meal service aid, evaluating suppliers). Receiving food (53.7%) accounted for the greatest proportion in the participating area. Second, most subjects (83.0%) responded that they were aware of the use of EFAP. Additionally, 96.4% of the subject approved expanding the use of EFAP for all items. Third, their degree of interest in the school foodservice was 3.86, and satisfaction with the school foodservice was 3.94 based on a 5-point Likert scale. The overall satisfaction on the quality of EFAP used in the school foodservice was 3.68 based on a 5-point Likert scale. Fourth, awareness that the school used EFAP in the foodservice by the parents who participated in school foodservice operations was higher than that of parents who did not participate. Satisfaction (4.10) with the school foodservice by parents who participated in school foodservice operations was significantly higher than that (3.79) of parents who did not participate (p < 0.001). In conclusion, parent's participation in school foodservice operations should increase to improve the quality of the school foodservice and improve communication between parents and the school.

  • The aim of this study was to recognize guardians' desire and awareness on sedation, and to guide guardians' satisfaction and cooperation. Followings are the conclusion : 1. Information supply about conscious sedation were restrictive and insufficient. 2. Both first-visit guardians and guardians of sedated patients were interested in decrease of dental fear. 3. Both first-visit guardians and guardians of sedated patients were satisfied with psychological improvement better than behavior improvement. Therefore, decrease of patients'ear and anxiety was essential. 4. Both first-visit guardians and guardians of sedated patients didn't recognize accurate safety of conscious sedation.

  • This study takes a holistic approach to understanding the diffusion of IPTV services by combining the adoption-diffusion model and the use-diffusion model of innovation. IPTV service, a leading Digital converged application coupling media content with telecom, has been recently launched commercially in Korea. We created a structural model of adoption-diffusion, using the perceived ease-of-use and usefulness of TAM(Technology Acceptance Model) as mediating variables, and a structural model of use-diffusion, with the rate of use and the variety of use as mediating variables. To empirically analyze these models, non-users of IPTV were surveyed using the adoption-diffusion model to identity factors influencing their intention to subscribe to the service. Meanwhile, users of IPTV were surveyed using the use-diffusion model to determine the factors that influence their satisfaction with the service and their intention to fe-use it. Under the adoption-diffusion model, we found that trialability, household innovativeness and perceived risk were the determinants of user satisfaction with IPTV, and perceived ease-of-use, the mediating factors. Under the use-diffusion model, complementarity and communication were shown to be the determinants of users' satisfaction with IPTV, and variety of use, the mediating factor. We also found that consumers' intention to re-use IPTV was strongly influenced by its relative advantage and perceived risk.

Factors That Affect Family Planning of Fertile Women in Volta Region in Ghana (가나 볼타지역 가임여성의 가족계획 실천에 영향을 미치는 요인)

  • Background: Family planning is widely practiced today to resolve the over-growing population and overcome obstacles that thwart socio-economic growth. While Ghana was the first country in Africa to implement family planning program, its birthrate is still twice as much as world average due to weak infrastructure and strategic plans to enforce the policy. Thus, there is a need to objectively verify the factors that affect family planning of fertile women of Ghana. Methods: Total of 630 self-administered questionnaires were distributed from April 8 to 17 of 2013 to collect data. Six-hundred eighteen questionnaires were analyzed, excluding the 12 incomplete questionnaires. Collected data were analyzed using PASW SPSS ver. 18, and logistic regression analysis was performed to verify the factors that affect practice of family planning. Results: Satisfaction with health and medical facilities, experience with family planning education and awareness of birth control methods significantly affected practice of family planning. Based on analysis using odds ratio, enforcement rate of family planning increased by 4.574 times when the subjects were satisfied with health and medical facilities, by 3.920 times when received family planning education, and by 3.284 times when they were aware of birth control methods. Conclusion: By adopting family planning education program, government should be able to change fertile women's perception of family planning. A strategic plan is necessary in order to increase access to medical facilities, improve service satisfaction, and induce women to enforce family planning voluntarily.

A Study on the Selection Attributes regarding American Beef by Importance-Performance Analysis -Focused on the University Students in Seoul and Gyeonggi Area- (미국산 쇠고기 선택속성의 중요도-만족도 분석 - 서울.수도권 대학생을 중심으로 -)

  • The purpose of the study is to examine the importance and performance degrees between Selection Attributes of American beef by IPA(Importance-Performance Analysis) for university students in Seoul and Gyeonggi Area. A total of 590 questionnaires were distributed, and 481 questionnaires were used in the study (81.5% of response rate). According to the IPA results of 12 selection attributes for American beef, the selection attributes with relatively low satisfaction but high importance(II quadrant) were 'country of origin', 'safety', 'cleanliness and hygiene'. The factor to be improved relatively through to the results of IPA was consumer confidence factor ('country of origin', 'safety', 'cleanliness and hygiene') and it was confirmed that university students had a low satisfaction in large degrees compared to the significance of American beef. Therefore, recovery of consumer confidence of American beef is urgent as well as the need to increase satisfaction of consumers. From now on, based on these results, it is considered that many efforts are necessary to increase the trust of consumers by planting awareness that American beef is safe by setting institutional conditions to vitalize purchase of American beef. Moreover, it is considered that these efforts would be significant for the vitalization of restaurants with beef as their main menu, and vitalization of consumption of Korean beef as well as American beef henceforth.

Wearing Comfort Evaluation of a Summer Flight Suit to Improve Ventilation (통기성 향상을 위한 하계비행복 설계 및 착용쾌적성 평가)

  • This study verified the effect of summer flight ventilation developed in a previous study based on wearing comfort evaluation. Seven healthy males in their twenties volunteered for this experiment conducted in aclimatic chamber. The experiment consisted of three consecutive periods of rest (20 minutes), running on a treadmill (10 minutes) and recovery (20 minutes). A comparative evaluation was conducted on the general flight suit which had no ventilation holes and summer flight suit that use subjective satisfaction measures and objective measures. The subjective satisfaction was evaluated according to the criteria of temperature sensation, wet sensation, thermal comfort and fatigue sensation. The objective satisfaction was measured by skin temperature, microclimate (temperature and humidity), sweat rate and thermography. The comparative wearing evaluation identified the summer flight suit decreased the temperature between skin and suit by $0.42^{\circ}C$ (upper arm), $0.9^{\circ}C$ (calf) and the skin temperature by $0.3^{\circ}C$ (shoulder), $0.4^{\circ}C$ (upper arm), $0.5^{\circ}C$ (calf) as compared to the general flight suit. The humidity inside the summer flight suit decreased at head (7.73%), shoulder (5.86%), upper arm (5.26%), and calf (8.73%) compared to the one inside the general flight suit. Thermography showed that the air flowed through ventilation holes (neck and armpit). The design of ventilation holes applied to the summer flight suit can be applicable to overall clothing that requires thermal comfort such as dust-free garments, mechanical clothing and combat uniforms.
