  • Objectives : The purpose of this study is to review and summarize about occupational medicine and to applicate the occupational medicine to oriental medicine. Methods : We investigated several books, articles and related internet sites to know occupational medicine's present conditions in Korea and other countries, and researched the plan on oriental medical participation in the industrial health. Results and Conclusion : The conclusions are as follows, 1. Recent years, industrialization has changed working environment and health condition in workplace, so industrial health service has became important part. 2. Occupational medicine has the purpose of protection. prevention, fitness, health management, medical treatment. rehabilitation, and primary medical service. The range of occupational medicine is enlarged mental, social good health and social, economical production capacity recently. 3. After research each country's occupational medicine, we can know the purpose is same, but the system and work contents are different one another. 4. Oriental medical participation in the industrial health has various benefits, satisfaction of workers, efficiency of cost, and increasing rate of medical treatment through the western and oriental medical cooperation.

A case report of the effectiveness of endometriosis. (자궁내막증 환자의 치험 1례)

  • Purpose: The purpose of this study was to report the clinical effectiveness of Traditional Korean Treatment on infertility patients with endometriosis. Methods: The patient in this case was 32 years-old female. The chief complains were lower abdominal pain, dysmenorrhea, infertility with endometriosis. She was treated by Traditional Korean Medicine, acupuncture and retention enema therapy. So the symptom of she complained was improved. Results: After the retetion enema therapy and Traditional Korean Medicine, the lower abdominal pain and dysmenorrhea of endometriosis was improved. And she was pregnant and delivered. Conclusion: This case study shows that the retetion enema and Traditional korean therapy were effective in treating endometriosis and fertility. So we make more studis about this disease, it will bring the high satisfaction of patient and the improvement of treatment rate.

The Effect of Footbaths on Sleep and Fatigue in Older Korean Adults (족욕요법이 한국노인의 수면과 피로에 미치는 효과)

  • Purpose: To determine the type and degree of effect that a hot footbath has on sleep quality and fatigue level in older Korean adults. Methods: A non-equivalent control group quasi-experimental design was used. Fifty participants from a long-term care facility in Kwangju, South Korea, were randomly selected and assigned to two groups: experimental group (27 participants) and control group (23 participants). The participants in the experimental group received hot footbaths in a temperature-controlled water tub of $42^{\circ}C$. They soaked their feet up to 20cm above the ankles for 30 minutes before going to bed for 3 nights. Actigraphy was used to measure their sleep patterns. The data were analyzed using the SAS program. Results: After the intervention, the total sleep satisfaction rate of the participants increased, while sleep latency and fatigue decreased significantly in the experimental group compared to the control group. Conclusion: The study results suggest that hot footbaths are beneficial for older Korean adults in enhancing sleep quality as well as reducing fatigue. Therefore, hot footbaths are recommended as a nursing intervention to improve sleep quality and to reduce fatigue in older Korean adults.

Development and Effects of a Comprehensive Web-Based Nursing Process for Nursing Students (간호학생을 위한 웹 기반 간호과정 프로그램의 개발 및 만족도 평가)

  • Purpose: This study was conducted to develop a comprehensive web-based nursing process program to assist nursing students and to evaluate the effects of the program. Methods: The system provides nursing students with guidelines based on NNN (NANDA, NOC, NIC) when the nursing students conduct a series of nursing procedures on diagnosis-outcome-intervention for a specific symptom. It also maintains and manages nursing processes actually conducted by students for clinical practices, and provides an environment where the patient information and nursing process can be converted into a formatted document for printing. This web-based program was used to surgical patients from April to June 2011. Results: The overall satisfaction rate was 3.64. The item with the highest score was, 'Do you think a web-based nursing process program is necessary?' (3.87), followed by 'Do you want to use this program when you become a nurse and implement the nursing process?' (3.33). Conclusion: These results indicate that implementation of web-based programs needs to be continued as an effective tool, but more research is needed on the best way to implement web-based programs in various clinical setting.

Evaluation of Government Assisted Visiting Nursing Services of Health Center in 2000 (2000년 보건소 공공근로 방문간호사업 평가)

  • This study is a fact-finding research to understand the status of visiting nursing services operated by health centers in Korea and it aims to provide basic information for policy development on operation and management of visiting nursing services in health centers. Method: This study investigates the results of visiting nursing services in 242 health centers from Jan. 10 through Dec. 30, 2000, where 3,106 visiting nurses were employed by the public work program. Result: In 2000, 129,401 new household as service recipients was identified and that was 0.9% of Koreas total households (15,137,000), and 5.8% of low income households (2,242,000). The highest high risk group was dementia patients(aver. 55.2/1,000 person). Average number of households visited by visiting nurse were 4.5 households per day and the first-visited houses per visiting nurse were 1.1 households per day. The re-visiting rate was 71.3%. Total 4,059,130 service items were provided and assessment ranked the highest with 33.7%. The satisfaction level of clients on the nurses was an average of 3.17 points in the scale of 4 and the nursing service was a 2.60 points in a scale of 3. Conclusion: Visiting nursing service should continue to provide comprehensive healthcare services in cost-effective ways while cooperating with others

Presenteeism and Absenteeism According to Health Problems on Nurses (간호사의 건강문제에 따른 프리젠티즘과 결근율의 관계)

  • Purpose: The purpose of this study was to examine the presenteeism and absenteeism in relation to the health problems of nurses. Method: Informed consent was provided before study. Data was collected from January 7 to March 6, 2008 by Stanford Presenteeism Scale-13. Data was collected from 3 hospitals in Seoul Gyeonggi area. The response rate of the questionnaires was 96.2%. Incomplete data was analyzed using SPSS program. Results: Shoulder, back and neck pain were the highest reported health problems and primary health Problem of nurses. Impaired presenteeism was ranked in this order: depression or anxiety, cold, and uterus or ovary disorder. Absenteeism was ranked in this order: uterus or ovary disorder, cold, and dermatological problem. Impaired presenteeism was significantly increasing according to the number of health problems. But Absenteeism was not. Presenteeism was significantly different by age, educational level, religion, whether or not the worker lived with their family, marital status, whether or not the worker has children, time spent in the workforce, experience of turnover, shift work, night work, and level of satisfaction with salary. But absenteeism was not significantly different given these criteria. Conclusions: Health problems of nurses can negatively effect the quality of nursing service and productivity at hospital. Therefore the hospital administrator should control the presenteeism and absenteeism through the management of health problems of nurses.

Lower Extremity Amputations for the Diabetic Foot Complication (당뇨병성 족부 합병증에 따른 하지 절단술)

  • Purpose: To evaluate the clinical manifestations, the patterns and the functional outcome of the amputations due to the diabetic foot complications. Materials and Methods: Fifty patients (50 feet) of diabetic foot amputations were followed for more than 1 year. The mean age was 62.5 years, and the mean follow-up period was 46 months. Retrospective analysis was performed using chart review and interview with the patients. The outcome was assessed with modified AOFAS scale. Results: The diabetic foot lesions were infection in 45 feet, gangrene in 35 feet and ulcer in 15 feet. Toe amputation was most commonly performed procedure (23 cases) followed by below knee and ray amputation. Postoperative modified AOFAS score was average 51.5 points, and 94% were satisfied with outcome. Minor amputations showed better outcome than the major amputations. Conclusion: Overall postoperative functional outcome was encouraging with high patient satisfaction rate (94%). Better outcome was obtained with the minor amputations.

The Work Environment and Wearing Conditions of Industrial Protective Clothing in Shipbuilding Workshops (조선업 작업장의 작업환경 및 산업용 보호복의 착의실태)

  • This study examined the work environment and wearing conditions of industrial protective clothing in shipbuilding workshops. It also investigated the relationship between the wearing sensation of industrial protective clothing and overall comfort, according to work process. In addition, the work posture according to work process was evaluated based on ergonomic factors. The wearing rate of industrial protective clothing was 73.3%, 66.7%, and 60.1% for workers engaged in welding, grinding, and painting, respectively. The harmful work environment factors, listed from most harmful to least harmful, were found to be high temperature pyrogens, noxious fumes, organic solvents, UV rays, and heavy dust. The aspect of wearing performance of industrial protective clothing that was most related to user dissatisfaction was poor sweat absorbency. In terms of the correlation between the overall comfort and the wearing sensation of industrial protective clothing, the satisfaction was low shown in orders of physiological comfort, sensual comfort, and movement comfort.

  • In this paper, the research has been done to develop the inclination test items to form the desirable online learning community in which social interaction may be maximized, dropout rate lowered and learners' feeling of isolation eliminated. Once developed, the inclination test items have been classified into homogeneous ones and heterogeneous ones. And on the basis of the results of this research, Intelligent agent based Inclination Test Grouping e-learning System(IIGS) has been developed, which can perform automatic grouping of online leaning community by intelligent agent. The results of this research with 1,000 teachers in reality by means of developing the grouping system have shown that 151 groups are automatically formed. Among them, 34% have shown very high degree of learning satisfaction and intended to maintain the groups in the future.

Breast Reduction through an Inframammary Incision (유방밑주름절개식 유방축소수술)

  • Purpose: Reduction mammaplasty is a procedure with a relatively high patient satisfaction rate, however, associated scarring around the areola can be a serious problem. This study proposes a new modification of the breast reduction procedure by means of an inframammary incision alone. Methods: The breast is marked out preoperatively with standing position. Under the general anesthesia, an inframammary incision of approximately 7 - 8 cm is done. The subcutaneous plane is made in the lower pole of the breast, then the subglandular plane is entered and a sharp dissection is made up to 2 cm below the areola. The breast is mobilized from the chest wall and a cone-shaped parenchyme is removed in en-block except from the retroareolar central part. The remaining both pillars are gathered together with absorbable sutures and the base of the gland is narrowed to project the breast forward. The wound is closed in a layered fashion and taping of the breast mound is applied to redistribute the breast skin. Results: 21 patients (36 breasts) underwent this procedure from December 2004 to December 2009. Average follow up was 9 months (ranged from 6 months to 12 months). No major complication occurred. Most patients were pleased with the breast size, shape, and scars. However, 2 patients complained their hypertrophic scars which were corrected by revision. Conclusion: This technique is a simple approach to mild to moderate breast reduction through an inframammary incision alone. And, this technique provides an option with minimal complications and invisible scarring, which is especially important in the young patient group.
