In this study, we compared the efficiency of osteoblast differentiation media (ODM) containing three distinct reagent combinations in osteoblastic differentiation of human bone marrow-derived mesenchymal stem cells (hBMSCs) in monolayer culture. In addition, we analyzed growth and differentiation of hBMSCs on silk scaffolds and examined the bone-forming activity of a nanofibrous silk scaffold in a tibia diaphysis defect model of a rat hind limb with intramedullary nailing. Although all three ODM increased alkaline phosphatase activity to a comparable extent, the ODM containing bone morphogenetic protein-2 (BMP-2) was found to be significantly less effective in promoting mineral deposition than the others. Growth of hBMSCs on sponge-form silk scaffolds was faster than on nanofibrous ones, while osteoblastic differentiation was apparent in the cells grown on either type of scaffold. By contrast, bone formation was observed only at the edge of the nanofibrous scaffold implanted in the tibia diaphysis defect, suggesting that use of the silk scaffold alone is not sufficient for the reconstitution of the long bone defect. Since silk scaffolds can support cell growth and differentiation in vitro, loading MSCs on scaffolds might be necessary to improve the bone-forming activity of the scaffold in the long bone defect model.
The purpose of this paper is to develop the software for the diagnosis and evaluation of the nutritional ingestion status of inviduals or a group of family members who have inhabited in rural and urban, based on the suggested amount of nutrition and foods. This software can be used for the computation for the needed amount of nutrition, which is suitable to the economic conditions of individuals and group of family members, and for the reduction of effort to plan the balanced table menu and effort to manage the diets. However, those commercial softwares were hard to be purchased and expensive for the users, and even if they were purchased, it needs much time and efforts to operate and run the program. Especially those commerical software were not possible to revise when the user has faced with the need to revise and complement the input data according to his situations such as the individuals and the group of family members. The software which was developed to treat the table menu and the nutrition was very limited to the particular persons. That is, the software was not for the evaluation of the nutrition synthetically and reasonably, because it was planed to use for the patients and the group of hospital and research centers. However this software has a merit of providing the table menu and the nutritional diagnosis in that it can show the suggested amount of nutrition, the evaluation of nutrition, the methods of diet, and management of nutrition individually according to the age, sex, amount of exercise, and experience of sickness. The expected effect can be predicted to calculate the lacking amount of nutrition which the individual has taken, and be provided to the suitable table menu. In addition to this, the various table menu is based on the foods which can be purchased easily and inexpensively at any times.
Purpose: This study is to investigate the low back pain, social support, impediment in daily living activities and to identify factors affecting impediment in elderly farmer' daily living activities. Methods: The participants were 128 elderly farmers who had received nerve block. Data were collected using a structured questionnaire from February to March, 2018. They were analyzed using t-test, ANOVA, pearson's correlation coefficient, and linear multiple regression. Results: The score of low back pain was $6.27{\pm}1.69$ (10 points), that of social support $2.92{\pm}0.76$ (1~5 points), and that of impediment in activity of daily living $2.01{\pm}0.82$ (0~5 points). Factors affecting impediment in activity of daily living were found to include age (p=.017), daily hours of farm work (p<.001), fear for the nerve block (p<.001), low back pain (p<.001), and social support (p<.001); the explanatory power of these variables was 58.8%. Conclusion: This study has found the controllable factors affecting impediment in activity of daily living among the rural elderly engaging in farm work include low back pain, social support, and daily farming hours. Therefore, to reduce impediment in activity of daily living among them, it is necessary to develop nursing interventions that can improve impediment in activity of daily living through reduction of daily farming hours using local resources. It is also desirable to improve their health status by reducing low back pain, and develop and apply social supports with health education programs that fit the local resources and the needs of the rural elderly.
Objectives: We attempted to study sleep habits and insomnia-associated factors in Korean rural adult population. Methods : In 1,441 adult subjects of three rural communities selected by cluster sampling, we administered an epidemiologic survey using questionnaire methods from July 14, 1996 to July 28, 1996. Results : 1) Mean sleep latency and mean time to sleep again after awakening during nocturnal sleep were longer in females than in males. Females suffered more frequently from insomnia symptom, awakening during nocturnal sleep. morning headache, dysphoric mood in the morning, and the feeling of 'not refreshed' on waking-up than males. 2) The older age group (defined as those older than 65 years) was found to have earlier bed-time, longer sleep latency, more frequent nocturia, longer time to sleep again after awakening during nocturnal sleep, and more frequent insomnia symptom. 3) The presence of insomnia symptom more than once a week was significantly associated with suffering from physical illness, being divorced-separated-widowed, frequently feeling depressed, nocturia, or low education level. 4) In multiple logistic regression analysis, old age, being female, low education level, frequently feeling depressed, and nocturia were independently associated with the presence of insomnia symptom more than once a week. Conclusion : We conclude that, in a Korean rural adult population, insomnia symptom increases with age and females usually suffer more from insomnia symptom than males. In addition, low education level, feeling depressed mood frequently, and nocturia are found to be significantly associated independently with the presence of insomnia symptom more than once a week.

Objectives: The purpose of this study was to identify the level of quality of life and related factors in the elderly with dementia living in their houses of rural area who are receiving dementia partners' supporting activities. Methods: The study participants were 55 elderly people with dementia living in their houses who were participating in the dementia partner pilot project among those registered in the public health center in one area and 55 dementia partners. Data were analyzed by independent t-test, ANOVA, Pearson correlation analysis and multiple regression analysis. Results: The factors that had a significant effect on quality of life included depression to predict the quality of life in the elderly with dementia. And its explanatory power was 46.8%. Conclusions: Based on these results, it is crucial not only to maintain physical functions but also to decrease depression by emotional support activities so as to enhance their quality of life, therefore, it is required to develop and apply the integrative supporting programs in rural area. From the result that dementia partner's self-compassion would lower the depression in the elderly with dementia significantly, it is considered to develop the programs to enhance dementia partners' self-compassion.

Objectives: The purpose of this study was to suggest the direction of the Healthy Village project for rural residents in accordance with the prolonged COVID-19 by investigating the digital environment for major health problems, the role of a health leader, necessary projects, and non-face-to-face projects for Healthy Village members in the COVID-19 epidemic. Methods: Telephone interview surveys were conducted with 585 residents from November 30, 2020 to December 21, 2020. Results: Health problems perceived by residents were in the order of concerns about infection (48.5%), depression (32.5%), difficulties in using medical services (9.4%), and lack of exercise (7.7%). The role of the health committee in the COVID-19 situation was "encouraging people to follow quarantine rules" with 91.3%. As a necessary health village project, there was a high demand for the provision of health products and mental health projects. 17.9% said that there is a computer or smart device connected to the Internet in their home, and 42.2% said that there is someone in the village who can easily get help if there is a problem in accessing and using Internet information. 36.9% were able to watch videos, and 22.2% were able to use the Internet through public facilities. Conclusion: In a public health crisis, where the provision of public health and medical services to rural residents is not smooth, it is necessary to manage health and quarantine through health leaders in the village, and it is required to establish a digital environment infrastructure that can conduct community participatory health village projects in a non-face-to-face environment.

The authors surveyed the inhabitants of a rural area to assess the patterns of medical utilization on dermatoses. Seven hundred and sixty new outpatients of dermatoses were examined and surveyed with formed questionnaire from March 1997 to February 1999. The results are as follows; 1. Among 760 new outpatients, the number of male patients was 283(37.2%) and that of female patients was 477(67.3%). 2. The most common dermatoses was Tinea pedis(34.9%), and follows senile pruritus, contact dermatitis, housewife eczema, seborrheic dermatitis, numular dermatitis, atopic dermatitis, Tinea corporis, Tinea ungium, acne vulgaris, impetigo, keratolysis exfoliativa, chronic urticaria, Tinea cruris and Molluscum contagiosum in orders. 3. Drug store was the most frequent places where patients initially visited for their skin diseases(39.6%) and followed by non dermatologic clinic, dermatologic clinic and general hospital in orders. 4. One hundred and twenty one(15.9%) patients have been experienced folk treatment. It was founded that the topical vinegar application or soaking was the most common method. Many patients felt no symptom improvement after the folk treatment(48.8%). Seventeen point four percent of patient felt symptom worsened. The results of this study suggest that many of the rural inhabitants are lack of understanding on their dermatoses. So many physician who are in charge of the primary care in rural area have to pay attention to the common dermatoses and educate patients on their medical utilization.

Injury in agriculture is a serious public health issue with a major impact on the lives of Korean farmers. It is one of the leading causes of death and is also a major cause of longand short-term disability. In 2001, the social cost of one accident in agricultural machinery was estimated as 97.7-97.8 million won that is 4 fold of farm household income in Korea. Effective prevention and control of injuries requires a system of surveillance that monitors the incidence of injuries, their causes, treatment and outcomes. This requires an integrated system of data collection, analysis and interpretation and communication. Creating effective injury surveillance system in Korea requires to establish a framework for a national agenda. Discussions regarding the development of the framework should address, but not be limited to issues related to Data Holdings and Linkages; Capacity and Skills; Communication; Interconnection; and Surveillance Products. Ideally, an injury surveillance system would meet the information requirements across all sectors, while allowing each to have the ongoing information it needs for its policy and programming needs. This study was carried out to develop a surveillance system of agricultural injuries in Korea. Study subjects were residents who lived in a typical agricultural area (Yangpyung area in Kyung-gi province). The main data sources were reports of village headmen, compared with data of 'National Emergency Management Agency', 'National Health Insurance Corporation', 'Insurance of National Agricultural Cooperative', and 'Emergency Medical Centers'. Each data were reviewed to validate the strengths and weaknesses.

Before introducing the national long-term care insurance in 2008, the want for long term care service has to be estimated and analysed. This study estimates the demand and analyses what determines the want of long term care service. This study investigated data of 3f6 elderlies, that was collected by age stratified random sampling. The elderies resided in Onyang 4 - dong (urban area) and Dogo-myun (rural area) In the city of Asan. The researchers visited the elderlies and their care giver, and assessed their demand for the long term care service and examined physical, mental, socio-economic status by the assessment tools for Korean Long-Term Care System. $64\%$ of the those who are entitled to be served refuse the long term care service. $26.7\%$ of them wants for home care service and $7.9\%$ want facility care service. It is estimated that the want of home care service are three or four times as much as that of facility care service. The demand for long term care service is 5.155 times higher for those who live in rural area (p=0.000), 3.040 times higher for those who do not have spouse(p=0.057), and 3.356 times higher for the people who is in medicaid than medical insurance(p=0.029). However, income(p=0.782), means(p=0.614), living alone(p=0.223), number of family to live with (p=0.341) and age of the elderly(p=0.420) are not related with the demand of long term care service. The assessment tools for Korean Long-Term Care System for need evaluation of the long term care service can reflect the demand well.(p=0.024) If medical care will cover $80\%$ of total cost, the willingness to pay of the out of pocket money of the people with medical insurance is 67,400 Korean Won(66.77 US$) for the home care service and 182,500 Korean Won(180.78 US$) for the facility care service. There is possibility that long term care demand is still small after Introducing the long term care Insurance due to the care given by family members. When developing service delivery system of long term care insurance, rural area has to be given more consideration than urban area because of the higher demand. The people who do not have spouse or are in medicaid have to be given special consideration as well.

The purpose of this study was to verify the validity of Mealworms as a hospital meal with increased nutrition density. We provided a meal for postoperative patients and conducted analysis of dietary intake and nutritional status of patients and assessment of acceptability of the meal. This study was carried out as a randomized control trial. Patients were supplied either a hospital meal using Mealworms (Experimental group) or a regular hospital meal (Control group). We investigated the administration amounts of parenteral nutrition (PN) and food intake of patients after surgery and measured anthropometry, body composition, and blood tests before surgery and at hospital discharge. We included 34 postoperative patients who were admitted to Gangnam Severance Hospital from March to September. In the groups of patients not supplied with PN, the experimental group ($964.68{\pm}284.6kcal$, $38.82{\pm}12.9g$) had significantly higher dietary calorie and protein intake than the control group ($666.62{\pm}153.7kcal$, $24.47{\pm}4.9g$)(P<0.05). Additionally in the group of patients not supplied with PN, the experimental group (1.37%) showed a significantly higher increase in fat free mass index than the control group (-3.46%)(P<0.05). In all subjects, calorie density and protein density were significantly higher in the experimental group (P<0.001), and acceptability of calorie (P=0.036) and protein (P=0.001) was also significantly higher in the experimental group. Therefore, the results of this study support the validity of the introduction of hospital meals using Mealworms.
