An Epidemiological Investigation on Severity of Cerebro-Vascular Accident Patients in Korea (한국인의 뇌졸중 위중도에 관한 역학적 분석)

  • A study on severity of cerebro-vascular accident patients in Korea was conduced in order to determine the risk factors affecting the severity of stroke patients. This study was performed by interviewing a total of 477 hospitalized and ambulatory patient of CVA in Seoul, Taejon, and Taegu and Pusan areas from April 1, 1998 through June 30, 1998. The results are as follows; 1. Two hundred and sixty seven $(56\%)$ at Four hundred and seventy seven subjects were manes, md two hundred and ten $(44\%)$ were females. $28.3\%$ of the subjects' ages were 50's and $27.3\%$ were 60's and $18.5\%$ were 70's. $22.6\%$ of the subjects' occupations were home makers, $17.8\%$ were farming. $66.4\%$ of the subjects spent their childhood in urban areas and $33.1\%$in rural areas. $41.7\%$ of the subjects became violin of CVA in Spring, $35.0\%$ in Winter. 2. On physical and mental conditions of the subjects at the occurrence of CVA, $28.7\%$ of the subjects were engaged in physical activities. $22.6\%$ were in a rest stale $19.5\%$ were in sleeping and $18.9\%$ were mentally shocked. $79.4\%$ of the male subjects and $14.8\%$ of female subjects smoked cigarettes. $82\%$ of male subjects drank coffee. $81.1\%$ of the subjects did not exercise regularly. $45.9\%$ of the subjects had the systolic blood pressures in the range of 160 to 199 mmHg and $5.6\%$ of the subjects had hypertension before the occurrence of stroke and $11.7\%$ had diabetes. 3. Of the types of strokes, cerebral hemorrhage was the highest $(49.1\%)$. cerebral infarction was the second $(41.1\%)$. Severe strokes were found in cerebral hemorrhage cases $(52.0\%)$ and cerebral infarction cases $(40.1\%).\;50.9\%$. of the male subjects were moderate cases, $50.9\%$ were severe cases. In females, moderate cases were $72.4\%$, severe cases $15.2\%$. $37.5\%$ of the subjects who had preceding diseases were severe cases, and $15.6\%$ of the subjects without preceding diseases were severe cases. $50.7\%$ of the subjects whose family members had strokes had severe strokes. $34\%$ of the subjects sleeping less than 6 hours a day, $42.4\%$ of the subjects with irregular eating habits, $33.3\%$ of the subjects who liked meat, and $42.3\%$ of the subjects who liked salty foods had severe strokes. $35.9\%$ of the subjects with hot temper, $27.6\%$ of the subjects with moderate temper and $14.5\%$ of subjects with mild temper were severe cases. 4. The correlation coefficient between obesity and blood pressure was 0.094.

The Socio-Psychiatric Study on Attempted Suicides (일부(一部) 자살기도자(自殺企圖者)에 대한 사회정신의학적(社會精神醫學的) 조사연구(調査硏究))

  • The socio-psychiatric study and personality analysis by Minnesota Multiphasic Personality Inventory on attempted suicides with drug intoxication in Seonsan-Gun and Cheongdo-Gun of Kyoungsang buk-Do was attempted to identify overall picture of attempted suicides and to detect some characteristics of personality. The total of 93 suicide attempted cases which were brought to the local clinics for the past 3 years from February, 1981 to February,1984 were sampled among them. The socio-psychiatric study and MMPI were performed with 45 attempters possible to be interviewed. The personalities of a group which answered "Still have the desire occasionally" or "Don't know about the present attitude toward suicide was compared with those of the other group who answered "Will not do it agagin". The results were as follows; 1) The sex ratio of the total attempters (93 cases) were 138.5. The highest rate by age group was found in the 3rd decade to be 27 cases. On the other hand, the sex ratio of interviewed cases were 136.8 and the highest rate was found in the age group of 50-59. 2) For the common characteristics of majority other than sex, 55.7% of cases were graduated primary school, and 62.2% were engaged in agriculture. 3) Except the fact that more of the attempted suicides happened during summer, there were no seasonal variation among other seasons. 4) The places selected for the attempted suicides were the houses in majority of cases (98%). Most of cases (53.3%) were found immediately after the attempt and 24.4% within one hour. For the poison taken, 86.7 took a sort of herbicides or insecticides, 55.6% were admitted to local clinics for only one day and 20% for two days. 5) The most common precipitating factors of the attempted suicides were family trouble and financial poverty. 6) The psychodynamic motives of the attempters were impulsive hostility(35.6%), abandonment(33.3%), guilt feeling (11.1%), manipulation and attention seeking (8.9%) and others. 7) In regard to their present attitude toward suicide, 29 cases (64.4%) expressed that they would never do such an act again, 37.8% of cases expressed that they really wanted to die at the time when they attempted. 8) The Psychopathic deviate, Paranoia, Depression, Schizophrenia scores were significantly higher in the attempters group who answered "Don't know" or "Possibly" for the question of "Will you attempt suicide again?" Than the group who answered" will not do it again".

Study on the Centipede Bite Cases Who Visited a Medical Center (일개 의료원을 방문한 지네 교상 환례에 대한 조사)

  • Thirty six patients, female 21(58.4%), male 15(58.4%), with centipede bite visited emergency room of Gimcheon Medical Center, from September 1998 to August 1999. The number of patients accompanying the seasons were increased once in June and were high in August and September; peaked in September, which conforms to the habitude that centipede likes the warmer climate. Frequency accompanying the time was high after sunset(from 6 p.m. to 6 a.m. next morning) than before sunset, which conforms to the habitude that centipede is nocturnal. Distribution of the bite sites was finger(30%), neck(28%), foot(25%) and hand(14%) as same as the previous researches showed. In case of finger and hand, patients got bitten while they were wearing gloves or doing works; foot, wearing shoes. They got bitten the neck while sleeping. Among the localized symptoms were localized pain: 36 patients(100%), swelling: 36 patients(100%), erythema: 26 patients(72.2%), paresthesia: 5 patients (13.9%). Systemic symptoms were dizziness 2 patients(5.6%) and nausea 1 patient(2.8%). Mostly the localized pain was improved within 48 hours, swelling in 48 hours and within 72 hours in case of paresthesia. Systemic symptoms such as dizziness and nausea were improved within 24 hours. Treatment and prognosis of scolopendra have similarity in many countries. We suggest that Korean standard treatment manual is needed so that the localized symptom, systemic symptom and external wound can be cured soon.

A Comparative Study on Physique and Health status of Elementary School Children between Ethnic Koreans in the People's Republic of China and Kojae Area in Korea (중국 연변지역 조선족 아동의 보건의료 및 건강상태 비교 - 한국 경남 거제지역과의 비교 -)

  • The purpose of this study is to compare the health status of Korean and Chosun-Jok elementary children. To accurately achieve the purpose, a survey was conducted in Yanbian area in China and Kojae in Korea during the period of June 15 to July in 1995. This survey was performed by using two survey methods. The first was the parents' survey method. It asked structured questions about their children. The second method focused on the actual health of the children. It used the collection of children's physical records in school. Guided by the school teachers, each child distributed the questionnaires to their parents. We used stratified-cluster sampling method to determine subjects. 1,083 questionnaires of 1,749 were used to analyze the data (666 questionnaires were incomplete and were not used in the analysis). Each questionnaire matched the data of their physical record. : Body Weight, Body Height, Chest-Circumstance, Eyesight, Dental Health. Using the data, we compared the BMI(Body Mass Index) the Koreans and Chosun-Jok in China. The results of this study were as follows : Comparing the general average physique of contained body height, body weight and chest circumstance of Korean and Chosun-Jok, the general physique of Chosun-Jok is inferior to that of Korean regardless of age and sex. Meanwhile, the average physical constitution of Korean compared the Chosun-Jok (i.e. eyesight and dental hygiene), revealed that the physical constitution of Chosun-Jok is superior to that of Korean without concern of age and sex. Average BMI of Chosun-Jok is lower than that of Koreans. But, it seemed that most of the students in both groups maintain an adequate health level. In the case of children from 10 to 12 years old, females are superior to males through all data contained of the body weight, the chest circumstance, and the body height. It seems that females and males have a different maximum growth age. Most of the parents preferred a good physique as a good health condition for their children. The physique of each child was affected with some variables, including the number of family members, and the educational level of the parents. According to the above results, the students' physique in Korea is superior to that of Chosun-Jok in China. But, Koreans are inferior to Ethnic Koreans in China in the students' physical constitution. In conclusion, we consider Chosun-Jok in China to maintain an adequate health level in their physique and physical constitution.

  • This study aimed to modify behavior regarding liver fluke and cholangiocarcinoma prevention in Chumphuang district, Nakhon Ratchasima province, Thailand through integrated learning. A total of 180 participants were included through purposive selection of high-risk scores on verbal screening. Participants attended the health education program which applied the health belief model included family based, knowledge station based, academic merit based and community based learning. Data were collected using a questionnaire composed of 4 parts: 1) personal information, 2) knowledge, 3) perceived susceptibility, severity, benefits, and barriers, 4) practice regarding liver fluke and cholangiocarcinoma prevention. The result revealed that the majority were female (79.9%), age ${\geq}60$ years old (33.2%), primary school educational level (76.1%), and agricultural occupation (70.1%). The mean scores of knowledge, perception, and practice to liver fluke and cholangiocarcinoma prevention, before participated the integrative learning were low, moderate, and low, respectively. Meanwhile, the mean score of knowledge, perceived susceptibility, severity, benefits, and barriers, and practice regarding liver fluke and cholangiocarcinoma prevention, were higher with statistical significance after participation in the integrated learning. This finding indicates that health education programs may successfully modify health behavior in the rural communities. Therefore they may useful for further work behavior modification in other epidemic areas.

  • Objectives: To assess knowledge of HPV and attitudes towards HPV vaccination among the general female population, government officials, and healthcare providers in China to assist the development of an effective national HPV vaccination program. Methods: A cross-sectional epidemiologic survey was conducted across 21 urban and rural sites in China using a short questionnaire. 763 government officials, 760 healthcare providers, and 11,681 women aged 15-59 years were included in the final analysis. Data were analyzed using standard descriptive statistics and logistic regression. Results: Knowledge of HPV among the general female population was low; only 24% had heard of HPV. Less than 20% of healthcare providers recognized sexually na$\ddot{i}$ve women as the most appropriate population for HPV vaccination. There was high acceptance of the HPV vaccine for all categories of respondents. Only 6% of women were willing to pay more than US $300 for the vaccine. Conclusions: Aggressive education is necessary to increase knowledge of HPV and its vaccine. Further proof of vaccine safety and efficacy and government subsidies combined with increased awareness could facilitate development and implementation of HPV vaccination in China.

A study on the use of dental hygiene devices in Korean adolescents (청소년의 구강관리보조용품 사용에 관한 연구)

  • Objectives: The objective of the study was to investigate the use of dental hygiene devices in Korean adolescents. Methods: The study subjects were 70,362 adolescents in 800 schools who completed the 2015 Korean Youth Risk Behavior Web-based survey. Dependent variables included usage of dental floss, interdental brushes and mouthwash solutions. Independent variables included demographic characteristics of the subjects, health state and behaviors, and oral health behaviors and experience of oral diseases. Results: The related factors of usage of dental hygiene devices included gender, where females showed higher usage (OR=1.10) compared to males, father and mother's level of education where usage was higher in above university graduates (OR=1.20, OR=1.14) compared to less than high school graduation, economic status where usage was higherin high and middle (OR=1.93, OR=1.26) compared to low, vigorous physical activity where usage was higher in those who responded yes (OR=1.35) compared to no, subjective weight recognition where usage was higher in normal (OR=1.07) compared to under weight, sleep time where usage was higher in enough (OR=1.12) compared to not enough, number of toothbrushing (day) where usage was higher in 2 times or over 3 times (OR=1.35, OR=1.75) compared to below 1, oral health education experience (OR=1.10), sealant experience (OR=1.17) and scaling experience (OR=1.45) where usage was higher in those who responded yes compared to no, school where usage was lower in high school (OR=0.64) compared to middle school, residential type where usage was lower in rural area (OR=0.74) compared to metropolitan area, living form where usage was lower in other (OR=0.77), compared to with family, smoking (OR=0.93), and alcohol drinking (OR=0.90) where usage was lower in those who engage in the activities compared to those who didn't, BMI where usage was lower in normal (OR=0.87) and over weight (OR=0.98) compared to under weight. Conclusions: To expand the use of dental hygiene devices in the adolescents, it is necessary to improve the continuing education program for need and motivation of dental hygiene device usage.

Mean Fasting Blood Glucose Level and An Estimated Prevalence of Diabetes Mellitus Among A Representative Adult Korean Population (우리나라 성인 인구의 평균 공복혈당치와 당뇨병의 추정 유병률)

  • This study was carried out along with the 1990 6th National Tuberculosis Prevalence Survey in order to estimate the prevalence rate of diabetes mellitus among a representative adult Korean population. Fasting blood glucose was measured by diastix (Ames) using glucometer II for seventy seven percent of the population (30 year old or above) residing in 190 enumeration districts randomly sampled from 146,944 general ED. The prevalence of diabetes mellitus of the population was estimated by projecting the ratio of ${\geq}200mg/dl\;PP_2$ of fasting blood sugar level below 120 mg/dl by sex to the study population. Fasting blood glucose and 2 hr. postprandial blood glucose were measured on about 3000 subsampled individuals, and diabetes mellitus was defined by the WHO criteria-FBG${\geq}120mg/dl\;or\;PP_2{\geq}200mg/dl$ when FBG is below 120mg/dl. The results obtained are as follows : 1. Estimated prevalence(age adjusted) of diabetes mellitus was 4.6% for male and 8.1% for female. The age adjusted mean FBG was $93.9{\pm}26.2mg/dl$ for male and $102.9{\pm}31.5mg/dl$ for female. 2. The prevalence increased as age advanced with peak in $60{\sim}69$ years old age group for both sexes. 3. The mean FBG and estimated prevalence rate of diabetes mellitus varied considerably among the populations of fifteen cities and provinces ; it vaired from $87.0{\pm}17.7\;to\;104.6{\pm}34.5mg/dl$ and 1.2% to 8.9% for males, and from $93.3{\pm}25.3\;to\;116.7{\pm}38.6mg/dl$ and 3.4% to 20.8% for females. 4. The mean FBG and prevalence rates were higher among the rural population than those residing in metropolitan areas. 5. The proportion of the people previously treated among the diabetics was estimated to be a little less than one fifth. 6. Factors strongly associated with FBG were age, sex, family history of D. M., BMI, area and educational level among eleven variables.

Measles Infections and Measles Vaccinations Rates for the Past 10 Years in Kang Wha -A Cohort Observation- (강화 지역주민의 과거 10년간 홍역이환율 및 예방접종율의 변화 -출생년도별 코호트 분석-)

  • The objective of the study is to measure the changes in measles infection and measles vaccination rates for the past 10 years in a rural area, Kang Wha. The study population were the entire children who were born between 1971 and 1950 in three townships (Sunwon, Naegae, Buleun) in Kangwha County. Two interview surveys were carried out during the 10 years of period, one in 1977 and the other in 1981. The data were collected by Family Health Workers through interview with structured questionnaires. The diagnosis of measles was mainly based on histories, symptoms and sighs of the disease. If a mother had reported measles history of her child, a public physician reviewed and decided the final diagnosis of the reported case. A retrospective cohort observation was done in order to see the trends of measles infections and measles vaccinations. The major findings were as follows; 1. The 5 year prevalence rate of measles vaccinations was 51.3% between 1971 and 1975 and 71.9% between 1976 and 1980 respectively. The difference between two periods was statistically significant (P<0.05). The secular trend of measles vaccinations showed increasing tendency from 1971 to 1978 and since then kept maintained. 2. In the birth cohort analysis of measles vaccinations, the vaccination rates, in general, were higher in the later cohort groups than that of earlier cohort groups. 9. The 5-year experience rates for measles infections were 24.3% between 1971 and 1975 and 17.2% between 1976 and 1980 respectively. This difference was statistically significant(P<0.05). The secular trend of experience rates for measles infections showed decreasing tendency from 1971 to 1980 except an outbreak in 1976. 4. The birth cohort analysis of experience rates for measles infections showed that the rate was higher in the later cohort groups than that of the earlier cohort groups. This decreasing tendency was prominent between $1973{\sim}1974$ and $1976{\sim}1977$. 5. The distribution of age specific incidence rates for measles infections showed unimodal curve with the peak at the age of 12 to 18 months. This findings were same in both two surveys. 6. Seasonal variations of the measles infections showed two peaks, one major peak in March through May and the another minor peak in September through December. 7. The 5-year reduction rate for measles infections among those vaccinated was 90.4% between 1971 and 1975 and 88% between 1976 and 1980.

The culture conditions for mycelial growth and sclerotial formation of Polyporus umbellatus

  • Polyporus umbellatus (Syn. Grifola umbellata) is a sclerotium forming mushroom belongs to family Polyporaceae of Polyphorales, Basidiomycota. The sclerotia of P. umbellatus have long been used for traditional medicines in China, Korea and Japan. This study was initiated to obtain the basic data for artificial sclerotial production of P. umbellatus. Here, we investigated the favorable conditions for mycelial growth of P. umbellatus and its symbiotic fungus Armillaria mellea. We also evaluate the favorable carbon and nitrogen sources for sclerotial formation in dual culture between P. umbellatus and A. mellea. The favorable conditions for mycelial growth of P. umbellatus were $20^{\circ}C$ and pH 4, while optimal conditions for mycelial growth of A. mellea were $25^{\circ}C$ and pH 6. The carbon sources for optimal mycelial growth of P. umbellatus were fructose and glucose, while carbon sources for favorable mycelial growth of A. mellea were also fructose and glucose. The nitrogen sources for favorable mycelial growth P. umbellatus were peptone and yeast extract, while optimal mycelial growth of A. mellea were obtained in peptone and yeast extract. When P. umbellatus and A. mellea were dual cultured on carbon sources, sclerotia were induced on basal media supplemented with glucose, fructose and maltose at pH 4~6, while nitrogen sources inducing sclerotia were basal media supplemented with peptone and yeast extract for 60 days at $20^{\circ}C$ under dark condition.
