A Study on the impact of rural older adults residence type and social activity participation on health-promoting behaviors : a gender perspective

  • 본 논문에서는 농촌 노인의 건강증진행위에 영향을 미치는 요인들을 성별에 따라 제안한다. 본 연구는 농촌진흥청의 '농촌 노인 맞춤형 돌봄프로그램 개발 수요조사' 자료를 활용하여 분석하였다. 분석대상은 농촌에서 거주하는 만65세 이상 502명의 자료를 활용하였다. 분석방법은 SPSS 25.0 프로그램을 활용하여 기술통계, 교차분석, 상관관계분석, 위계적 회귀분석을 사용하였다. 분석결과 건강증진행위 실천에 있어서 남성과 여성의 차이가 있는 것으로 나타났다. 남성의 경우 연령이 낮고 교육수준이 낮을수록, 배우자와 함께 거주할 때, 사회적 활동에 참여할 때 건강증진행위 수준이 더 높았고, 여성의 경우 연령이 많고, 주관적 건강상태가 좋을수록, 사회적 활동에 참여할 때 건강증진행위 수준이 더 높은 것으로 나타났다. 본 연구는 농촌 노인의 성별에 따라 건강증진행위를 촉진할 수 있는 기초자료가 될 것이며, 이를 바탕으로 정책적·실천적 접근 방안에 대한 시사점을 제시하였다.

한국인 상용 식품의 아연함량표를 보완하여 평가한 한국농촌성인의 아연 섭취 실태 (Supplementation of Zinc Nutrient Database and Evaluation of Zinc Intake of Korean Adults Living in Rural Area)

  • This study was conducted for two purposes ; (1) to develop a database for zinc levels in commonly usef Korean food items ; and (2) to calculated the zinc intake fo Korean adults living in a rural area. The currently used Korean food compositinotable was supplemented in term sof zinc content using several methods ; (1) analyzing 98 Korean Food items frequently consumed by Korean adults living in rural area. ; (2) adapting values from U.S Minnesota for 71 items ; and (3) imputing values from similar food for 282 items. A new zinc nutrient databse was constructed including zinc contentrs of 1,195 food items. Zinc intake of rural Korean adults was estimated by a 240hours recall method from 2 ,037 adults over 30 over 30years of age in Yeonchon -gun , Kyunggi province of Korea. Mean daily zinc intake of all subjects was 61mg an dmean intake level of males (7.0mg/day, 46.85 of RDA) was significantly thigher than females(5.2mg/day, 43.0% of RDA). Subjects in their 40's had the highest zinc intak ewhile those over 70 years of age consumed the least amount of zinc. The food group that contributed most to the dietary ainc intake of subjects was cereals and grain products supplying 38% of total zinc intake. The next most important group for zinc intak ewas the meat, poultry , and product group supplying 26% ot total intake. This group was followed by fishes and shellfishes, legumes and their products, and vegetales . For individual food items , reicecontribued most, supplying 27% of total zinc intake follwoed by beef(10%) and prok(9%) . Altogether, plant foods supplied 68% of zinc intake suggesting that the bioabailability of dietary ainc is low. In conclusion, these results show ethat the zinc intake of rural Korean adults is low and that sources of dietary zinc are mainly plant foods, suggesting low bioavailability . Further studies are needed to determine zinc intake and status of Korean population. The zinc database developed in this study will be very valuable for such studies.

식품안정성 수준에 따른 청년의 영양섭취상태: 국민건강영양조사 2008-2018년 자료 활용 (Nutritional Status of Young Adults by Food Security Level: Analysis of Data from the Korea National Health and Nutrition Examination Survey (2008-2018))

  • This study was conducted to examine the status of food consumption and nutrition intake of young adults according to food security using data from the Korea National Health and Nutrition Examination Survey (KNHANES) (2008~2018). A total of 10,655 young adults aged 19~34 years participated in the study. The subjects in the moderately/severely food insecure group consumed less fruits, vegetables, meat, nuts and seeds than those in food secure/mildly food insecure group after adjusting for confounding factors. The moderately/severely food insecure group significantly decreased mean adequacy ratio (MAR) and nutrient adequacy ratio (NAR) for protein, vitamin A, riboflavin, niacin, vitamin C, calcium, phosphorus and iron compared to the food secure/ mildly food insecure group. The percentages of the participants consuming a daily diet with carbohydrate, protein, vitamin A, thiamin, riboflavin, niacin, vitamin C, phosphorus and iron less than the estimated average requirement (EAR) increased in the moderately/severely food insecure group compared with the food secure group. This study suggested that the nutrition intake of young adults was related to food security status. These findings can be used as basic data for developing nutritional policies for young adults.

How Much Do Older Adults Living Alone in Rural South Korea Know About Dementia?

  • Objectives: This study aimed to examine the level of dementia knowledge of older Korean adults living alone in rural areas and to identify related factors. Methods: A cross-sectional descriptive design was applied. The participants were 231 older adults living alone who were recruited from 12 of the 13 primary health care posts in the rural area of Chuncheon. Participants' level of dementia knowledge was assessed using the Dementia Knowledge Scale. Data were analyzed using descriptive statistics, and the t-test, analysis of variance, chi-square test, and Mann-Whitney test were applied. Results: Participants' mean age was $77.3{\pm}5.4$ years, and women comprised 79.7% of the sample. Over half of the participants (61.9%) had no formal education, and all the participants were enrolled in Medical Aid. The participants' average percentage of correct answers was 61.6%. The highest rate (94.4%) was for the item "Dementia can change one's personal character." The item with the lowest proportion of correct answers was "Dementia is not treatable" (23.4%). Dementia knowledge was significantly associated with age, education, health coverage, source of living expenses, and dementia risk. Conclusions: Dementia knowledge among Korean rural older adults living alone was relatively low. Participants' misconceptions about symptoms and treatment could hinder them from seeking early treatment. The results of this study suggest the need for active outreach and health care delivery for rural older adults living alone in South Korea.

도시와 농어촌 노인의 고독감과 죽음불안: 사회적 자본의 조절효과를 중심으로 (Loneliness and Death Anxiety among Older Adults Living in Urban and Rural Communities: The Moderating Effect of Social Capital)

  • 본 연구의 목적은 도시와 농어촌 노인의 고독감과 죽음불안, 사회적 자본에 대한 인식을 비교하고 고독감과 죽음불안 간 관계에서 사회적 자본의 조절 효과가 있는지 검증하는 데에 있다. 이를 위해 SSK 고령사회와 사회자본 연구센터에서 전국 만 65세 이상의 노인을 대상으로 2018년도에 실시된 「노인의 건강한 노화 및 웰다잉에 관한 연구」 설문 데이터를 바탕으로 서울특별시 및 6개의 광역시에 거주하는 총 839명의 도시 노인과 군단위 지역에 거주하는 322명의 농어촌 노인을 연구 대상자로 사용하였다. 주요 통계분석방법으로는 인구학적 특성과 주요 변수에 대한 도시와 농어촌 노인의 비교를 위해 기술 통계 및 t-test와 χ2 test를 실시하였고, 고독감과 죽음불안과의 관계와 사회자본의 조절효과를 검증하기 위하여 집단 별 이항 로지스틱 회귀분석을 실시하였다. 연구 결과, 주요 연구 변인에서 도시와 농어촌 노인들의 유의미한 차이가 발견되었다. 도시 노인은 농어촌 노인에 비해 죽음불안을 보고한 노인의 비율이 높았고, 고독감의 수준도 높게 나타났지만, 사회적 자본에 대한 인식의 경우에는 농어촌 노인들이 전반적으로 도시 노인보다 더 높게 나타났다. 고독감과 죽음불안 사이의 사회적 자본의 조절효과는 도시 노인들에게는 발견되지 않았지만 농어촌 노인들에게는 사회응집력과 사회적 지원이 고독감과 죽음불안과의 관계를 조절하는 효과가 있었다. 본 연구의 의의는 도시와 농어촌 노인이 고독감, 죽음불안, 사회적 자본에 대한 상이한 인식을 가지며, 고독감과 죽음불안 간의 관계를 조절하는 사회적 자본의 역할 또한 도시와 농어촌 노인 집단에서 서로 다르게 나타남을 검증한 것이다. 이러한 연구 결과는 노인의 고독감 및 죽음불안 감소를 위한 사회적 자본 확충 및 관련 개입방법을 고려할 때 도시와 농어촌의 지역적 차이 또한 검토해야 함을 시사한다.

농촌마을단위 건강지표 개발을 위한 주민건강 측정 및 진단에 관한 기초연구 (A Fundamental Study on Survey and Diagnostic of Residents Health for Development of Villages' Health Indices)

  • The purpose of this study was to survey and diagnostic the self-related health cognition, stress, culture life and health-related fitness for residents of rural area. For this study, Total 126 people answered by a written questionnaire and took part in health-related fitness test for old adults in rural area. According to the normal distribution, the indexes were divided into five grades(very poor 5, poor 4, average 3, good 2, very good 1). The self-related health cognition(SF-36) was third grades. The index was as same as average old adults in rural area. The old adults stress level was third grades. The degrees of stress were as same as average old adults in rural area. The participation in the culture life was fourth grades. The health-related fitness test were third-fifth grades. The results of this experiment could be functioned as a very important fundamental source in order to establish satisfying health system, social welfare for the old people in rural area. Taken together, it seemed that self-related health cognition, stress level, participation in the culture life and health-related fitness have to be considered and improved. The indexes should be further investigated and some practical method should be developed for the olded people in rural area.

Determinants of Physical Frailty among Old-Old Adults in an Urban-Rural Complex Community in Korea

  • This study aimed to identify the determinants of physical frailty among the old-old adults in rural Korean communities. A total of 191 individuals aged 75 and older were included in the study, with the majority being female. Participants were classified into healthy (n=47), pre-frail (n=54), and frail (n=90) groups. Significant differences were found across these groups in terms of age, gender, education level, depression, and nutritional status. Multiple logistic regression analysis revealed that age (OR=1.16), depression (OR=0.21), malnourishment (OR=10.85), and short physical performance ability (OR=0.70) were significant predictors of physical frailty. These findings underscore the multifaceted nature of physical frailty among old-old adults in urban-rural complex communities and highlight the need for comprehensive and integrated interventions. Such interventions should consider not only physical factors but also broader health conditions and socio-demographic influences impacting the elderly. Further research is needed to develop and evaluate interventions that address these determinants and promote health equity among the elderly population in urban-rural complex communities

보건진료소 중심 치매 쉼터 프로그램 사례 (A Case Report on the Shelter for Dementia Programs based on the Primary Health Care Post)

  • Purpose: This study developed a shelter for dementia programs based on primary health care posts; and examined its effectiveness in rural older adults. Methods: This study used a case report provided by a primary healthcare post in Jeonnam, Korea, in 2023. A single group pre-test post-test design was used; for three months, and 10 older adults participated in the shelter for dementia programs based on the primary health care post. The effectiveness of the intervention was measured immediately after entering the shelter for dementia programs. Results: There were significant differences between the pre-test and post-test on the cognitive impairment screening test (p=.005). Conclusion: The shelter for dementia programs based on primary health care posts was effective in improving cognitive impairments of rural older adults.

읍면지역 성인의 여가시간 신체활동에 영향을 미치는 개인적, 사회적 환경 요인 (Personal and Social Environmental Factors Affecting Leisure Time Physical Activity in Adults Living in Rural Areas)

  • Purpose: This study was conducted to examine the personal and social environmental factors affecting leisure time physical activity (LTPA) among adults living in rural communities. Methods: A cross-sectional study was conducted with a convenience sample of 282 adults living in three rural towns of Chungcheongbuk province. Leisure time physical activity was assessed through the Korean version of the Global Physical Activity Questionnaire (GPAQ), and social influence and social cohesion of social environment were assessed using reliable and validated measures. Multiple logistic regression analysis was conducted to explore personal and social environmental factors associated with LTPA. Results: The overall prevalence of leisure-time physical activity (LTPA) was 40.0%. Multiple logistic regression analysis showed that LTPA participation was associated with gender (OR=2.60, 95% CI=1.47-4.58), healthy eating (OR=1.15, 95% CI=1.01-1.31), unhealthy eating (OR=0.70, 95% CI=0.53-0.93) and social influence for physical activity (OR=1.11, 95% CI=1.03-1.20). However, social cohesion was not correlated with engaging in LTPA. Conclusion: Both personal and social environmental factors are associated with LPTA among rural adults living in Eup-Myen regions. Interventions to promote LTPA would likely benefit from considering healthy dietary intake and social influence for physical activity.

사회자본이 농촌 거주 노인의 우울 상태에 미치는 영향: 2019년도 지역사회건강조사를 이용한 단면연구 (Influence of Social Capital on Depression of Older Adults Living in Rural Area: A Cross-Sectional Study Using the 2019 Korea Community Health Survey)

  • Purpose: This study aimed to investigate the influence of social capital on the depression of older adults living in rural areas. Methods: Data sets were obtained from the 2019 Korea Community Health Survey. The participants were 39,390 older adults over 65 years old living in rural areas. Indicators of social capital included trust, reciprocity, network, and social participation. Depression-the dependent variable-was measured using the Patient Health Questionnaire-9 (PHQ-9). Hierarchical ordinal logistic regression was conducted to identify factors associated with depression after adjusting the data numbers to 102,601 by applying the Synthetic Minority Oversampling Technique (SMOTE). Results: The independent variables-indicators of social capital-exhibited significant association with the depression of older adults. The odds ratios of depression were higher in groups without social capital variables. Conclusion: To reduce depression, we recommend increasing social capital. Factors identified in this study need to be considered in older adult depression intervention programs and policies.
