The Effect of Traditional Korean Medicine Treatment and Herbal Network Analysis in Postoperative Hip Fracture Inpatients (고관절 골절 수술 후 한의 입원치료 효과 및 다빈도 처방 약재 네트워크 분석)

  • Objectives This study aimed to evaluate the effects of Integrative treatment of traditional Korean medicine (TKM) on 7 hospitalized patients after hip fracture surgery, and to identify significant herbs and co-prescribed herbs by using network analysis and association rule mining. Methods A retrospective chart review of the 7 hospitalized patients treated for postoperative hip fractures between January and December 2021 was performed. All TKM treatments for the patients were identified and Wilcoxon signed-rank test was performed to compare hip pain and mobility on admission and discharge. We visualized the network of herbal medicines and complications. By using network analysis, we also identified the significant herbs (high centrality of degree, eigenvector, and sub-graph). Co-prescription patterns for the hip fracture patients were further analyzed by association rule mining. Results We found that TKM treatment significantly relieved hip pain and improved mobility. Accompanying symptoms reported by the patients were general weakness, anorexia, dizziness, delirium, edema, sputum, sore throat, cough, rhinorrhea, and chills. Herbs composed of Sagunja-tang and Samul-tang showed high centralities and high associations with other herbs. In addition, Gupan, Nokyong, Yukjongyong, Useul, and Hyunhosaek were identified as important herbs for postoperative hip fracture patients. Conclusions This study provides evidence for clinical TKM use as an effective postoperative treatment for pain relief and improvement of mobility in patients with hip fractures. In addition, herbs that can be considered in the treatment of patients after hip fracture surgery were identified through network analysis and association rule mining.

Reports of 63 Patients Treated with Herbal Medicine to Relieve Symptoms of COVID-19 at One Korean Medicine Clinic: A Retrospective Chart Review (일개 한의의료기관에서 COVID 19 증상 완화를 위해 한약이 투여된 환자 63례에 대한 보고 : 후향적 차트 리뷰 연구)

  • Objectives: The purpose of this study was to report on the cases and prescriptions of non-face-to-face care performed to alleviate COVID-19 symptoms and to present their results. Methods: This study was conducted on 63 COVID-19 patients who visited the Ilsan Hamsoa Korean Medicine Clinic from January 24 to June 11, 2022. We retrospectively analyzed all 63 patient charts and amassed COVID-19-related symptoms and their changes according to clinical characteristics and outcomes. Results: All 63 COVID-19 patients complained of symptoms, such as coughing, sore throat, sputum, fever, headache, myalgia, rhinorrhea, nasal obstruction, postnasal drip, and voice change. In total, 30 patients were administered Eunma-san, 6 patients were administered Eungyo-san plus Mahaenggamseok-tang, 10 patients were administered Galgeunhaegi-tang, 7 patients were administered Eungyo-san, 6 patients were administered Maekmundong-tang, and 4 patients were administered Jugyeopseoggo-tang. Improvements in symptoms due to herbal medicine included the disappearance of COVID-19 fever (22 cases) within 3.59±4.2 days, sore throat (32 cases) within 4.37±2.98 days, and cough (36 cases) within 4.86±3.1 days. Conclusion: Various herbal medicine prescriptions were effective COVID-19 treatments, indicating that herbal medicines may be alternative treatment options for COVID-19. However, the evidence should be supplemented with better designed prospective research on individual prescriptions.

  • Objectives: This study aims to report a case of atopic dermatitis with otitis media with effusion and chronic rhinitis improved by oral administration of Galgeungabanha-tang. Methods: The patient of this case had symptoms such as erythema, abrasions, scales, and itching. She also complained of rhinorrhea, nasal obstruction, cough, sputum, and ear fullness. Considering the fact that she was sensitive to heat, but did not sweat well, and respiratory symptoms often occurred, Galgeungabanha-tang was prescribed. Results: After medication, the symptoms of atopic dermatitis significantly improved from 10 to 2 point. The subjective symptoms of chronic rhinitis and otitis media with effusion also improved considerably. In addition, nasal swelling and purulent exudate almost disappeared. The tympanic membrane, which was swollen due to amber exudate, also changed to a normal gray-white color. Conclusions: Atopic dermatitis accompanied by chronic rhinitis and otitis media was improved after 45 days of medication.

A Case Report of Suspected Allergic Rhinitis in a Patient with LBP Hospitalized at a Korean Medicine Hospital and Treated with Korean Medicine (한방병원에 입원한 요통을 동반한 알레르기 비염 의증 환자에 대한 한방치료 1례)

  • Objective: The purpose of this study is to report the efficacy of using Korean medical treatment for suspected allergic rhinitis. Methods: We used Korean medical treatments (herbal medicines and acupuncture) to treat a hospitalized patient with suspected allergic rhinitis. To evaluate the treatment, we used Total Nasal Symptom Scores to measure sneezing, rhinorrhea, itching, and obstruction. The degree of rhinitis was also evaluated using the Visual Analogue Scale, and the patient's generic health status was measured using the European Quality of Life 5 Dimensions Scale. Results: Visual Analogue Scale and European Quality of Life 5 Dimensions Scale scores improved after treatment. The change in Total Nasal Symptom Scores showed that the patient's suspected allergic rhinitis symptoms were relieved. Conclusions: The results of this study suggest that Korean medicine therapies may be effective for treating suspected allergic rhinitis.

  • Coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) mainly causes respiratory symptoms such as fever, cough, sputum, and rhinorrhea, as well as digestive symptoms such as diarrhea, vomiting, and abdominal pain in children. In this report, we describe a case of a child with a SARS-CoV-2 infection who presented with epigastric pain and was subsequently diagnosed with acute pancreatitis without any concomitant infections in other organs. The epigastric pain was relieved with goal-directed vigorous fluid therapy for acute pancreatitis for 24 hours, and the serological and radiological findings normalized after two months. Acute pancreatitis should be considered as a differential diagnosis when a child with a history of COVID-19 visits the hospital with epigastric pain.

Effects of Korean Medicine Treatment at Public Health Center for Post-acute COVID-19 Patients: A Retrospective Case-series of 11 patients (일개 보건지소에 내원한 급성기 후 코로나-19 환자들의 특성 및 한의 치료 효과 분석: 사례군 연구)

  • Objectives: The purpose of this study is to determine the clinical features of post-acute COVID-19 syndrome and the effectiveness and safety of Korean medicine treatment at the Public health center. Methods: The study was conducted among 11 patients with post-acute COVID-19 syndrome who attended a single public health center from January to December 2022. We retrospectively analyzed the charts of 11 patients and collected clinical characteristics, previous treatments, Korean medicine treatments, outcome variables (Numeral Rating Scale (NRS), Leicester Cough Questionnaire (LCQ), Visual Analog Scale (VAS)), adverse events, etc. Results: Of the 11 patients, six were women, and the average age of all patients was 68.27±12.31 years. The most common symptom were cough(n=9, 81.82%) and sore throat(n=9, 81.82%), followed by sputum, fatigue, rhinorrhea, and loss of appetite, etc. All 11 patients were treated with herbal medicine, with Samso-eum(n=6, 54.55%), Yeonkyopaedok-san(n=5, 45.45%), and Haengso-tang(n=3, 27.27%) being the most commonly used. After herbal medicine treatment, the median cough NRS decreased from 5 to 1, and the median sore throat NRS decreased from 4 to 1, both of which were statistically significant. One patient reported adverse event of dyspepsia and heartburn, but it was mild. Conclusions: The study presented the clinical features of the post-acute COVID-19 syndrome and suggested that Korean medicine treatment at public health centers may be effective and safe in alleviating associated symptoms.

Change of Prevalence and Clinical Features of Mycoplasma Pneumonia in Infants Younger than 2 Years (2세 미만아에서의 Mycoplasma 폐렴의 발생 비율 변화 및 임상 양상)

  • Purpose : Recently, it has been reported that the prevalence of mycoplasam pneumonia in infants is increasing. We studied the change of prevalence and clinical features in infants for recent three years. Methods : We evaluated the clinical records of 206 patients diagnosed as mycoplasma pneumonia during Mar. 2000~Feb. 2003. We retrospectively analyzed epidemiologic, clinical, serologic and radiologic difference between children younger than 24 months and others. Results : Among 206 patients, 111 were boys and 95 were girls. Mean age of onset was $5.12{\pm}2.91$ years and lowered annually(P>0.05). 28 patinets(13.6%) were younger than 24 months and annual prevalence in this group increased(P<0.05). Main clinical features included cough, fever, coarse breathing sound(=sputum), rhinorrhea and dyspnea. There was no clinical difference between children younger than 24 months and others, except for rhinorrhea and dyspnea which more developed frequently in children younger than 24 months (P<0.05). There was also no serologic and radiologic difference for these groups. Conclusion : Annual mean age of onset lowered and annual prevalence of younger than 24 months increased with mycoplasma pneumonia. Therefore, we need careful attention to differentiate Mycoplasma pneumoniae as causal organism of pneumonia in children younger than 24 months.

When the pathogenesis of hoarseness is evil Ki(穢濁), the treatment of resuscitation(開竅) with Arctii Fructus(牛蒡子), Lasiosphaera Seu Calvatia(馬勃), Curcumae Radix(鬱金), etc can be applied. When the pathogenesis of headache is stasis of both Ki and blood(氣血瘀痺), the treatment of resuscitation(開竅) with Cnidii Rhizoma(川芎), Asari Herba Cum Radice(細辛), Scorpio(全蝎), moxibustion(灸), etc can be applied. 9) When the pathogenesis of lacrimal hypersecretion, visual disturbance, mydriasis, tinnitus, hearing disturbance, sinusitis, epistaxis, hoarseness and cough are deficiency of Yin(陰虛), deficiency of kidney(腎虛), deficiency of both liver and kidney(肝腎虛), deficiency of both heart and kidney(心腎虛), brain discharge by deficiency of Yin(陰虛 腦熱), exuberance of Yang in liver(肝陽上亢), overexertion(勞損), the treatment of Yin-replenishing(滋陰) with Yukmijihwanghwan(六味地黃丸), Hojamhwan(虎潛丸), Jeobutang(猪膚湯), Lycii Fructus(枸杞子), Polygoni Multiflori Radix(何首烏), Rehmanniae Radix(生地黃), Schizandrae Fructus(五味子), Liriopis Tuber(麥門冬), Asini Gelatinum(阿膠), etc can be applied. 10) When the pathogenesis of ophthalmalgia, mydriasis, vertigo and headache are deficiency of Yin in liver(肝陰虛), exuberance of Yang in liver(肝陽上亢), endogenous wind(內風), excess in upper and deficiency in lower part(上實下虛), the treatment of Yin-replenishing(滋陰) and endogenous wind-calming(熄風) with Rehmanniae Radix Preparat(熟地黃), Lycii Fructus(枸杞子), Polygoni Multiflori Radix(何首烏), Paeoniae Radix Alba(白芍藥), Ostreae Concha(牡蠣), Saigae Tataricae Cornu(羚羊角), Chrysanthemi Flos(菊花), etc be applied. 11) When the pathogenesis of mydriasis, sinusitis, hoarseness, headache, cough and asthma are exhaustion of vital essence(精氣無收藏), brain discharge(腦髓不固), floating Yang(陽虛浮), exsanguination(失血), deficiency of both Yin and Yang(陰陽不足), overexertion(勞損), deficiency of Yang in kidney(腎陽虛), the treatment of Yang-restoring and exhaustion-arresting(回陽固脫) with Yangyeongtang(養營湯), Cheonjinhwan(天眞丸), Bokmaektang(복맥탕), Geonjungtang(建中湯), Dogihwan(都氣丸), Singihwan(腎氣丸), Jinmutang(眞武湯), Ostreae Concha(牡蠣), Nelumbinis Semen(蓮子肉), etc can be applied. 12) When the pathogenesis of lacrimal hypersecretion, vertigo and headache are deficiency of stomach and endogenous wind(胃虛內風), endogenous wind with phlegm(內風挾痰), liver check of stomach(肝木橫擾), the treatment of concomitant-treating of both liver and stomach(肝胃同治) with Paeoniae Radix Alba(白芍藥), Uncariae Ramulus Et Uncus(釣鉤藤), Gastrodiae Rhizoma(天麻), Astragali Radix(황기), Pinelliae Rhizoma(半夏), etc can be applied. When the pathogenesis of asthma is failure of kidney to promote inspiration(腎不納氣), the treatment of kidney-tonifing and inspiration-promoting(補腎納氣) with Singihwan(腎氣丸), Psoraleae Fructus(補骨脂), Juglandis Semen(胡桃), Aquilariae Resinatum Lignum(沈香), etc can be applied. When the pathogenesis of asthma is deficiency of Ki(氣虛), the treatment of Ki-reinforcing(補氣) with Sagunjatang(四君子湯), Insamgeonjungtang(人參建中湯), etc can be applied.

A Case of Bronchus-Associated Lymphoid Tissue(BALT) Lymphoma Treated with Lobectomy (폐엽절제를 통한 BALT 림프종 치료 1 예)

  • The bronchus-asociated lymphoid tissue(BALT) lymphoma is a low-grade primary malignant lymphoma that originates from bronchus associated lymphoid tissue. A 67-year-old woman was admitted for evaluation of cough, sputum, rhinorrhea which had persisted for one month. Physical examination showed decreased breathing sound on the left upper lung field. High resolution chest computed tomography demonstrated consolidation which showed air-bronchogram and surrounding ground glass opacity in left upper lobe. These findings implicated inactive tuberculosis, organizing pneumonia, or bronchiolo-alveolar carcinoma. The histologic findings from percutaneous needle aspiration biopsy revealed aggregated atypical small lymphoid cells with lymphoepithelial lesions. With immunohistochemical staining, the atypical lymphoid cells reacted positively with CD 20 antibody and negatively with CD 3 antibody. Thus, we could diagnosed her as a patient with BALT lymphoma. After left upper lobectomy, she has been well without recurrence of the disease for 14 months. In this country of Republic of Korea, it was the 1st case of BALT lymphoma surgically treated when histological diagnosis had been done. Based on this case, we wanted to demonstrate the importance of early histological diagnosis and treatment of BALT lymphoma.

Clinical Study on the Effect of Moxa-pellet Treatment in Allergic Rhinitis Patients (알레르기 비염 환자에 대한 압봉치료의 임상효과 연구;증상과 삶의 질을 중심으로)

  • Objectives : The purpose of this study is to evaluate the clinical effect of Moxa-Pellet treatment for nasal symptoms and quality of life(QOL) in patients with allergic rhinitis. Methods: Subjects were voluntarily recruited through newspaper and internet advertisement. In this randomized, single blind and controlled study, experimental group (n=19) received real Moxa-Pellet treatment and control group(n=20) received sham Moxa-Pellet treatment for 3 weeks. Acupuncture points used were $LI_4$, $ST_{36}$, $LU_7$, $GV_{14}$ and $GB_{20}$. Patient's nasal symptoms and QOL were assessed before and after 3 weeks treatment by the Nasal Symptom Scores(NSS) and the Medical Outcomes Study 36-Item Short-Form Health Survey(SF-36). Results : The results were as follows; 1. Among items of NSS, Sneezing' Rhinorrhea' Itching scores and Total Nasal Symptom Scores(TNSS) were significantly improved in experimental group after 3 weeks Moxa- Pellet treatment(p<0.05). In control group, there was no significant difference in all items except Headache item of NSS. 2. Among 8 domains of SF- 36, experimental group showed significant difference in Role-Emotional(RE), Mental Health(MH) and Bodily Pain(Bp) after 3 weeks Moxa- Pellet treatment(p<0.05). Control group showed no significant difference in all domains except one domain(Role limitation-physical). Conclusions: These results suggest that Moxa-Pellet treatment can be applicable to improve nasal symptoms and QOL in the patients with allergic rhinitis. Further long tenn studies on the Moxa-Pellets treatment's sustaining power and safety is needed.
