미국과 한국의 Single Institutional Review Board와 Human Research Protection Program의 비교와 함의 (Comparison and Implications of Single Institutional Review Board and Human Research Protection Program in the United States and Korea)

  • In the United States (US), due to the Common Rule, completely revised in 2017, single Institutional Review Board (IRB) review has become mandatory for government-sponsored multi-institutional research since 2020 regardless of the number of participating institutions. The goal of these changes is to reduce redundant reviews by the IRB at each institution and better protect research participants. In this paper, single IRB and Human Research Protection Program (HRPP) in the US and Korea were compared and considered, and their implications were discussed. A comparison of the HRPP evaluation and certification systems of the US and Korea includes that of SMART IRB in the US and Korea Central IRB, aiming at single IRB review for efficient review with support from the country and building a more efficient national human subject research network in the future. Its comparison and analysis will be helpful in deriving future tasks and development directions of single IRB and HRPP in Korea.

과학기술 학회의 개방형동료심사에 관한 인식 연구 : 한국과학기술총연합회 산하 학회를 중심으로 (A Study on Science and Technology Scholarly Societies' Understanding on Open Peer Review)

  • 개방형동료심사는 투고된 학술논문을 투명하게 심사할 목적으로 심사자의 신원, 심사의견, 심사결과를 저자를 포함한 일반 독자에게 공개하는 심사 방식이다. 개방형동료심사 인식에 관한 선행연구가 진행되었지만, 국내 과학기술 분야 학술지 편집담당자 관점에서 이러한 인식을 조사하고 분석한 연구는 찾아볼 수 없었다. 구체적으로, 학술지 편집위원의 개방형동료심사 적용 의향, 적용 시기 및 새로운 심사 방식을 채택할 때 예상되는 도전과 기회, 개방형동료심사 지원을 위한 투고심사시스템 요구사항에 관한 조사 및 분석은 부족한 실정이다. 이에 본 논문은 개방형동료심사에 대한 학술지 편집담당자의 인식을 조사하고 이러한 심사를 편리하게 지원하는 투고심사시스템의 핵심적인 요구사항을 이해하기 위해, 설문문항 준비, 설문조사 실시, 응답 결과를 분석하였다. 또한, 일부 과학기술 학술지 편집위원장을 대상으로 개방형동료심사에 관한 표적집단인터뷰를 수행하였다. 이후, 개방형동료심사 인식에 관한 본 연구의 분석 결과, 표적집단인터뷰 결과 및 국외 유사 설문조사 사례를 비교, 분석하고 시사점을 도출하였다. 본 연구 결과는 국내 학술단체에서 새로운 논문심사 방식의 정책 수립 및 시행, 투고심사시스템의 개방형동료심사를 위한 기능 탑재 및 선택 등의 의사결정에 도움이 될 것으로 기대된다.

의약품 처방·조제지원서비스(Drug Utilization Review)사업이 병용금기 처방률에 미치는 영향 (The Effect of Korean Prospective Drug Utilization Review Program on the Prescription Rate of Drug-Drug Interactions)

  • Background: Since December 2010, online computerized prospective drug utilization review (pDUR) has been implemented in Korea. pDUR involves the review of each prescription before the medication is dispensed to the individual patient. The pDUR is performed electronically by Health Insurance Review & Assessment Service (HIRA), which is a Korean governmental agency, and then HIRA provides medical institutions and pharmacies with information that can be helpful to them in preventing potential drug problems such as drug/drug interactions or ingredient duplication. The aim of this study was to assess the impact of the Korean pDUR implementation on the proportion of drug-drug interactions (DDIs) using claims data from HIRA. Methods: A before-after comparison of the prevalence of DDIs between prescription was conducted, using HIRA administrative claims data of medical institution from January 2010 to December 2011. The analysis unit was the prescription issued and pairs before and after. The main outcome measures were the proportion of DDIs within- (control group) or between- physician encounters. To examine the difference, a paired t-test was applied. Results: We found that DDIs proportion between prescription decreased significantly (t=3.04, p=0.0026) after the implementation of pDUR, whereas there is no significant reduction within prescription (t=1.15, p=0.2518). With respect to the prevalence of DDIs between drug groups, the most dramatic reduction was occurred between 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors and anti-fungal agents. Conclusion: It seems effective that giving a direct feedback to prescribers by a prospective DUR. Further research is needed to assess the impact of DUR to final outcomes such as hospitalization.

Imbalance in Cardiovascular Surgery Medical Service Use Between Regions

  • Background: This study uses the relevance index to understand the condition of regional medical service use for cardiovascular surgery and to identify the medical service use imbalance between regions. Methods: This study calculated the relevance index of 16 metropolitan cities and provinces using resident registration address data from the Ministry of Government Administration and Home Affairs and the 2010-2014 health insurance, medical care assistance, and medical benefits claims data from the Health Insurance Review and Assessment Service. We identified developments over the 5-year time period and analyzed the level of regional imbalance regarding cardiovascular surgery through the relative comparison of relevance indexes between cardiovascular and other types of surgery. Results: The relevance index was high in large cities such as Seoul, Daegu, and Gwangju, but low in regions that were geographically far from the capital area, such as the Gangwon and Jeju areas. Relevance indexes also fell as the years passed. Cardiovascular surgery has a relatively low relevance index compared to key types of surgery of other fields, such as neurosurgery and colorectal surgery. Conclusion: This study identified medical service use imbalance between regions for cardiovascular surgery. Results of this study demonstrate the need for political intervention to enhance the accessibility of necessary special treatment, such as cardiovascular surgery.

COVID-19 International Collaborative Research by the Health Insurance Review and Assessment Service Using Its Nationwide Real-world Data: Database, Outcomes, and Implications

  • This article aims to introduce the inception and operation of the COVID-19 International Collaborative Research Project, the world's first coronavirus disease 2019 (COVID-19) open data project for research, along with its dataset and research method, and to discuss relevant considerations for collaborative research using nationwide real-world data (RWD). COVID-19 has spread across the world since early 2020, becoming a serious global health threat to life, safety, and social and economic activities. However, insufficient RWD from patients was available to help clinicians efficiently diagnose and treat patients with COVID-19, or to provide necessary information to the government for policy-making. Countries that saw a rapid surge of infections had to focus on leveraging medical professionals to treat patients, and the circumstances made it even more difficult to promptly use COVID-19 RWD. Against this backdrop, the Health Insurance Review and Assessment Service (HIRA) of Korea decided to open its COVID-19 RWD collected through Korea's universal health insurance program, under the title of the COVID-19 International Collaborative Research Project. The dataset, consisting of 476 508 claim statements from 234 427 patients (7590 confirmed cases) and 18 691 318 claim statements of the same patients for the previous 3 years, was established and hosted on HIRA's in-house server. Researchers who applied to participate in the project uploaded analysis code on the platform prepared by HIRA, and HIRA conducted the analysis and provided outcome values. As of November 2020, analyses have been completed for 129 research projects, which have been published or are in the process of being published in prestigious journals.

  • OECD는 투자위원회를 중심으로 각국 NCP들이 OECD 가이드라인을 효과적으로 이행하기 위한 일환으로 NCP 간 동료평가(Peer Review)를 강조하는 추세이다. 각 NCP로서는 동료평가가 자발적인 성격이라고는 하지만 부정적인 평가를 받게 되는 경우 국제적으로 많은 비난을 받을 소지가 크고 국가 이미지에도 심각한 타격을 입을 가능성이 높다. 더군다나 지난 2017년 3월, 기업책임경영에 관한 작업반(Working Party on Responsible Business Conduct) 회의에서 한국 NCP에 대하여 2019년 동료평가를 시행하기로 결정하였다. 이에 본 논문에서는 NCP 동료학습의 의의와 현황, 최근 시행되었던 덴마크와 벨기에 동료평가 사례들을 검토함으로써 우리 정부가 2019년 동료평가에 어떻게 대비해야 하는지 정책적 제언을 하고 있다.

독성병리 Peer Review의 중요성과 실제적 접근방법 (Importance of Peer Review in Toxicological Pathology and Its Practical Approach)

  • Evaluation of toxicological pathology is to some extend subjective judgments by pathologist and the accuracy of pathologist's works is based on the individual training and experiences. It has been required to establish a peer review system for toxicologic pathology and these review system has been employed by various practice of toxicological pathology. It would be pointed out that the possible causes of drifts in pathology are due to 1) lack of knowledge or experience of individual pathologists, 2) poorly maintained consistence of grading among animals in study, 3) different interpretation of findings between pathologists, or 4) pathology data processing. Example cases of diagnostic errors and current practice of peer review including tissue selection criteria, documentation and problem resolution for short-term and carcinogenicity studies were introduced. For sound regulatory system and high integrity of practice in toxicological pathology, current approaches of peer review system were reviewed.

WebReview : 웹 기반 논문심사 지원 시스템 (WebReview : Web-Based Paper Review Support System)

  • There has been a complaint that current editorial and refereeing system of the traditional journal systems has something to be desired due to long time lag between submission and publication, improper(unfair in some sense, because no communication is allowed between reviewers and authors) review of submitted papers sometimes, and so on. This paper introduces a Web-Based Paper Review Support System, called WebReview, The system helps to reduce the time lag and allows reviewers and authors to communicate anonymously to clarify the research results, if there is any ambiguity. WebReview is implemented in Java, using Oracle as an underlying DBMS, to capture interactions between users and the system.

감사보고서 감리와 이익반응계수 (Audit Review and Earnings Response Coefficient)

  • Audit review is the system to review the audit procedures performed and the audit opinion expressed by the independent auditors. The public audit review performed by Financial Supervisory Service (FSS) is regarded as one of the most important means to maintain the reliability of accounting information in Korea. We examinate the effect of audit review on earnings quality by using ERC (earnings response coefficient). We find that ERC of the sample group which issued the audit error reports after they were reviewed by SSB is more effective and higher than one of the non-reviewed group, matching group. We infer that the audit review system has the effectiveness in Korea.

인체유래물을 사용하는 연구와 Institutional Review Board 심의 (Research Using Human Biological Materials and Institutional Review Board Review)

  • Recently, with the development of biotechnology and molecular genetics, research using human biological materials has been receiving attention, and the demand for the use of human- biological materials has also increased significantly. In particular, human biological materials contain the personal information of the person providing them, so special care must be taken to protect personal information throughout the entire process from drafting the human biological materials research plan, Institutional Review Board (IRB) review, and research progress. In addition, it is important to appropriately utilize residual human biological materials obtained during medical diagnosis and treatment at medical institutions to achieve scientific results, while protecting the rights of human biological materials providers and improving the method of expressing consent so as not to damage the trust of human biological materials providers in researchers. To this end, it is necessary to provide sufficient information and education so that researchers and IRB members can understand and apply related laws such as the Bioethics Act and the Personal Information Protection Act.
