이중경화법을 이용한 열개시제 및 광개시제가 배합된 황칠도료의 경화속도 촉진 및 물성향상 연구 (Improving Curing Rate and Physical Properties of Korean Dendropanax Lacquer with Thermal and Photo Initiator by Dual Curing)

  • 한반도 남부 일대에서 자생하는 황칠나무(Dendropanax morbifera Lev.) 수액으로부터 추출한 천연 도료인 황칠도료(Korean Dendropanax lacquer)는 예로부터 귀중한 예술품이나 투구, 화살, 활 등의 전쟁도구를 찬란한 황금색으로 도장하는데 사용되어 왔다. 경화 후 황금색의 투명한 도막을 형성하여 우수한 색상특성을 지니고 있을 뿐만 아니라 내후성, 내수성, 내식성 등이 우수하여, 보호도장으로써도 훌륭한 가치를 지니고 있다. 그러나 이러한 많은 장점에도 불구하고, 현대적인 여러 적용분야에 사용되지 못하고 있는 이유는 황칠도료의 생산량이 적고, 이로 인하여 가격이 매우 고가이고, 장시간의 경화시간이 소요되는 문제도 큰 원인으로 작용하고 있다. 한편 황칠 내에 광중합이 가능한 conjugated diene을 포함한 모노머가 있으며, 이러한 모노머는 일광 조사 조건등에 의하여 짧은 시간에 단단하고 황금색을 띠는 도막을 형성할 수 있음이 보고된 바가 있다. 따라서 본 연구에 서는 전통적인 황칠도료의 경화방법을 개선하고 경화속도를 촉진하기 위하여, 열개시제를 도입하여 열경화를 촉진하는 방법과 열개시제 및 광개시제를 동시에 도입하고 이중경화(dual curing)에 의해서 경화속도를 촉진하고자 하였다. 이러한 경화 속도 및 경화 거동은 적외선분광분석기(FT-IR)을 이용하여 -C=C- 이중결합 특성피크의 변화를 관찰하거나, 진자경도계(pendulum hardness tester)를 이용하여 표면 경도의 변화를 관찰함으로써 평가하였다. 또한 강체진자물성측정기(RPT)에 spot UV curing 장비를 도입하여 이중경화에 의한 경화 속도 진을 평가하였다. 본 연구의 결과 열개시제에 의하여 열경화가 촉진될 수 있을 뿐만 아니라, 이중경화에 의하여 경화속도를 향상시킬 수 있음을 확인하였다. 또한 이러한 황칠도료 경화 속도 촉진 방법을 활용하여 전통적인 역에만 사용되어온 황칠도료를 현대적인 여러 적용분야에도 확대하여 사용할 수 있는 가능성을 제시하였다.

일개 신생아중환자실 반코마이신 저항 장구균(VRE)의 유행 양상과 조절 (Colonization Rate and Control of Vancomycin-Resistant Enterococci in the Neonatal Intensive Care Unit)

  • 목 적:부산대학교병원 신생아 중환자실에서 2006년 3월 VRE 유행이 나타나 입원 환아를 대상으로 대변VRE 감시 배양을 시행하였다. 본 연구는 VRE 감염의 정착정도를 파악하고, 위험인자를 분석하여 향후 VRE의 출현을 예방하고자 본 연구를 시행하였다. 방 법: 2006년 3월부터 2007년 3월까지 본원 신생아 중환자실 환아 192명을 대상으로 주 1회 대변 감시배양 검사를 시행하였다. VRE가 분리된 환아의 위험인자를 규명하고자 성별, 재태 기간, 기저 질환, 이전 치료에 사용된 항생제의 종류, 침습적 처치의 유무 등을 의무기록을 바탕으로 후향적으로 조사하였다. 결 과:총 192명의 환아 중 VRE 양성군은 48명(25%), VRE 음성군은 189명(75%)이었다. VRE 양성 환아 중 본원 출생아는 12명(25.0%), 타병원 출생아는 36명(75.0%)이었다. VRE 양성 환아의 기저 질환으로는 선천성 심질환이 25명(52.1%)으로 VRE 양성 빈도가 유의하게 높았다(P =0.005). 항생제 중 VRE 양성군에 서 3세대 cephalosporin 사용(45.8% vs. 15.3%, P < 0.001)과 vancomycin 사용(95.8% vs. 40.9%, P < 0.05)의 기왕력이 통계적으로 유의하게 높음을 알 수 있었다. 또한 VRE 양성군에서 중심 정맥 도관(41.7% vs. 15.3%, P <0.001)이나 인공 호흡기(41.7% vs 25.0%, P =0.017)를 사용한 예가 많았고, 수술 여부(41.7% vs 16.7%, P =0.001)나 흉관 삽입의 기왕력(10.4% vs 2.7%, P =0.021)이 통계적으로 유의하게 높았다. VRE양성군 48명 중 11명(22.9%)에서 VRE 음전을 확인 하였고 VRE의 음전기간의 중앙값은 101일 이었다. 결 론: VRE 감염은 신생아 중환자실에서 중요한 원내 감염으로 부각되고 있다. 이러한 VRE 집락을 차단하기 위해서는 철저한 감염 관리와 격리를 해야 하며 침습적인 기구의 사용 및 불필요한 항생제의 사용을 줄이려는 노력이 필요하다.

폐결핵의 외과적 치료 (Clinical Study of Pulmonary Resection for Tuberculosis(V))

  • 화학요법이 확립되고 유병율이 점진적인 감소에도 불구하고 폐결핵은 여전히 수술의 대상이 되고 있다. 이에 저자등은 폐결핵으로 수술을 시행한 예들을 분석, 검토함으로써 향후 폐결핵 치료에 도움이 되고자 한다. 방법 : 국립의료원 흥부외과에서 1991년1월부터 2000년12월까지 10년간 폐절제술을 시행한 144례를 토대로 본과에서 기왕에 발표된 4보까지와 비교관찰하여 폐결핵의 외과적 치료에 대한 최근 경향 및 그 결과를 검토,분석하였다. 방법으로 연간수술빈도 및 연령분포, 술전 결핵병소범위, 객담도말검사, 수술적응 및 술식의 종류, 합병증, 술후 추적관찰 결과들을 분석하였다. 결과 : 연간 수술빈도는 감소추세를 보였으며, 남녀성 비는 2:1로 남자환자가 많았고, 20대 이하의 감소와 50대 이상이 현저히 증가함으로써 연령의 고령화 현상을 볼 수 있다. 수술 전 병변의 범위는 대부분 중증 및 중등도에 속하였으며, 술전 항결핵제를 사용한 기간을 보면 3년 이상의 예가 16.0%에서 55.6%로 현저히 증가하는 추세를 보이고 있다. 한편 술전 결핵균에 대한 객담도말검사 양성율은 91%에서 27%로 급격한 감소 현상을 보였다. 수술적응에 있어서는 전폐파괴 및 폐부분파괴가 대부분을 차지하였으며, 폐실질 병소를 동반한 농흉이 4.0%에서 20.1%로 유의한 증가를 보이고 있다. 술식의 종류에 있어서 편측 폐절제술 및 폐엽 절제술이 대부분을 차지하고 있다. 술후 합병증으로는 농흉의 증가와 잔여 결핵 병소의 재발의 증가가 특징적으로 관찰되었다. 술후 추적관찰결과 완치율이 70% 이상을 나타내는 가운데 지속적으로 항결핵제 치료를 받고 있는 예가 5.8%에서 18.0%로 점진적 증가추세임을 나타냈다. 절론 : 최근 추세를 보면 화학치료에 내성을 보이는 환자가 증가하고, 늑막결핵의 증가와 중증의 병변환자의 증가가 특징적으로, 내과적 요법만으로 치유하기 어려운 병소를 정확히 판단하고 다제 내성균이 발생하기 전에 적극적으로 외과적 요법을 고려하는 것이 바람직하겠다.

침습적 및 비침습적 감염 환자에서 분리된 A군 연구균의 T 혈청형과 emm 유전자형의 비교 연구 (Differences in T Serotypes and emm Genotypes of Group A Streptococci Obtained from Invasive and Non-invasive Streptococcal Infections)

  • 목 적 : A군 연구균은 인두염, 성홍열과 같은 경증감염으로부터 심각한 치명률을 보이는 중증 침습적 감염의 원인균이 된다. 역학, 항생제 내성발생 등을 조사, 연구하는 데에 혈청형과 유전자형이 이용되고 있다. 이에 침습적 질환에서 혈청형과 유전자형이 비침습적 질환과 비교하여 어떤 차이점이 있는지 알아보기 위하여 본 연구를 시행하였다. 방 법 : 2004년 1~12월간 신촌세브란스병원에서 침습적 A군 연구균 질환으로 진단된 환자의 검체에서 39개의 균주를 동정, 미국 미네소타 대학의 WHO Collaboratory Center에 보내어 T혈청형 및 emm 유전자형을 분석하였고, 이를 1998~2002년 및 2003~2004년에 비침습적 질환에 대해 본 기관에서 연구하였던 결과와 비교하였다. 결 과: 2003~2004년 비침습적 질환대상 연구결과와 비교해 T2/28, 3, 5/27/44, 9, NT형은 상대적으로 많았는데, NT형이 통계적으로 유의하게 많았다(P=0.019). emm 유전자형은 emm 3, 9, 18, 44, STG485 형이 상대적으로 많았는데, STG485형은 통계적으로 유의하게 많았다(P=0.000). 높은 EM 내성과 관련된 T12, 4는 17.9%에 불과하였으며, 이는 비침습적 질환대상 연구 결과인 47%, 47.9%보다 통계적으로 유의하게 낮은 결과였다(P=0.002, P=0.000). 류마티스열의 발생과 재발에 관련하는 emm(M) 1, 3, 5, 6, 16, 18형은 41%로 비침습적 질환 연구결과인 26%(2003~2004) 보다 높았으나 통계적 의미는 없었다. 새로운 emm 유전자형이 발견되었다. 결 론 : T혈청형 분석으로 추정한 EM 내성률은 침습적 질환군에서 더 높지는 않을 것으로 생각되었다. emm 유전자형 결과 류마티스열의 발생과 재발 가능성이 더 높을 수도 있어 균주의 감시와 인두염의 철저한 치료가 필요할 것으로 사료된다. 향후에도 항생제 내성균 감시 및 새로운 형의 발견을 위해 혈청형 및 유전자형 분석을 계속해야 할 것이다.

수면관련 호흡장애 환자에서의 수면중 주기성 사지운동장애의 동반이환율 (The Comorbidity of Periodic Limb Movements Disorder in Patients with Sleep-Related Breathing Disorder)

  • 연구배경: 주기성 사지운동장애(PLMD)는 여러 수면장애와 함께 공존하는 경우가 많으며, 특히 수면관련 호흡장애(SRBD)와의 관계는 밀접하다고 알려져 있다. PLMD는 반복적이고 상동증적인 사지의 움직임에 의하여 전기생리적인 각성을 일으키기 때문에 불면증과 주간 졸립증을 초래하므로 이에 대한 관심이 필요하다. 본 연구는 SRBD 환자에서 PLMD의 빈도를 조사하고, PLMD로 진단된 환자군의 특성을 조사함으로써, SRBD 환자에서 PLMD의 동반 이환에 대한 이해를 높이는데 있다. 대상 및 방법: 진단적인 수면다원 검사로 수면관련 호흡장애의 진단이 내려진 106명을 대상으로 이들의 수면기록을 후향적으로 조사하였다. 수면다원 검사의 기록과 판독은 표준적인 방법을 사용하였다. OSA 의 진단은 RDI가 10 이상인 경우 내렸고, UARS는 낮 동안에 심한 졸림증을 호소하고 수면다원 검사상 RDI가 5 이하이나 호흡노력의 증가에 이어서 짧은 각성이 반복되는 경우 내렸다. 수면중 주기성 사지운동장애의 진단은 주기성 사지운동지수가 5 이상일 때 내렸다. 자료처리는 SPSS/$PC^+$를 이용하였다. 곁 과: 평균연령은 49.5세였고, 성별은 남자 76명, 여자 30명이었다. 진단은 폐쇄성 무호흡증 78명, 상기도저항증후군 28명이었다. 전체 연구대상의 평균 수면효율은 76.3%, 평균 호흡장애 지수는 22.3/hr, 평균 혈중 산소포화도는 84.9%로 중등도의 수면중 호흡장애가 있음을 알 수 있다. 평균 주지성 사지운동 장애지수는 13.1/hr였으며 70% 이상의 환자들이 32/hr 이하에 분포하였고 개인차이가 많았다. 주기성 사지운동 장애지수가 5 이상으로 수면중 주기성 사지운동장애를 동반한다고 판단된 군이 47명으로 전체의 44.3%를 차지하였다. 수면중 주기성 사지운동장애를 동반하는 군과 동반하지 않는 군의 비교에서, 두 군간에 나이, 무호흡 지수, 성별, 1 단계 수면의 백분율에서 유의한 차이가 있었다. 결 론: 본 연구의 결과는 이전의 타 연구들보다 수면관련 호흡장애에서의 주기성 사지운동장애의 동반이환율이 더 높다. PLMD를 동반하는 군의 나이가 유의하게 더 많고, 여성에서의 동반율이 더 높고, 무호흡 지수(apnea index)가 유의하게 더 낮다. 수면중 주기성 사지운동장애는 수면관련 호흡장애와 함께 불면증과 주간 졸림증을 초래하고, 본 연구에서 뒷받침 하듯이 수면중 호흡장애 환자에서 흔히 함께 동반되기 때문에, 수면관련 호흡장애 환자의 진단 및 가료시 이에 대한 배려가 필요하다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The authors suggests that this system based on down-sized computer could provide a progressive approach to total LIS based on local area network, and the implemented system could serve as a model for other hospital's LIS for routine hematology. 30609 T00401030609 ^x To develop an artificial bone substitute that is gradually degraded and replaced by the regenerated natural bone, the authors designed a composite that is consisted of calcium phosphate and collagen. To use as the structural matrix of the composite, collagen was purified from human umbilical cord.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction.
