Reference values for pulp oxygen saturation as a diagnostic tool in endodontics: a systematic review and meta-analysis

  • Objectives: This systematic review aimed to identify mean oxygen saturation values (SpO2) using pulse oximetry in permanent maxillary anterior teeth. Materials and Methods: The MEDLINE, Scientific Electronic Library Online, Cochrane Central Register of Controlled Trials, EMBASE, and Literatura Latino Americana em Ciências da Saúde electronic databases were searched. Combinations and variations of "oximetry" AND "dental pulp test" were used as search terms. Studies reporting means and standard deviations of SpO2 values were included. Two reviewers independently extracted data following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist. Heterogeneity was assessed using the I2 statistic, and all analyses were performed using R software. Study quality was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2 tool and the Newcastle-Ottawa scale. Results: Of the 251 studies identified, 19 met the eligibility criteria and were included (total sample, 4,541 teeth). In the meta-analysis, the mean SpO2 values were 84.94% (95% confidence interval [CI], 84.85%-85.04%) for the central incisors, 89.29% (95% CI, 89.22%-89.35%) for the lateral incisors, and 89.20% (95% CI, 89.05%-89.34%) for the canines. The studies were predominantly low-quality due to the high risk of bias associated with the index test, unclear risk regarding patient selection, and concerns about outcome assessment. Conclusions: Although most studies were low-quality, the oxygen saturation levels in normal pulp could be established (minimum saturation, 77.52%). Despite the risk of bias of the included studies, the reference values reported herein are clinically relevant for assessments of changes in pulp status.

Effect of direct-fed microbials on culturable gut microbiotas in broiler chickens: a meta-analysis of controlled trials

  • Objective: This meta-analysis was conducted to evaluate the overall effect of direct-fed microbial (DFM) or probiotic supplementation on the log concentrations of culturable gut microbiota in broiler chickens. Methods: Relevant studies were collected from PubMed, SCOPUS, Poultry Science Journal, and Google Scholar. The studies included controlled trials using DFM supplementation in broiler chickens and reporting log concentrations of the culturable gut microbiota. The overall effect of DFM supplementation was determined using standardized mean difference (SMD) with a random-effects model. Subgroups were analyzed to identify pre-specified characteristics possibly associated with the heterogeneity of the results. Risk of bias and publication bias were assessed. Results: Eighteen taxa of the culturable gut microbiota were identified from 42 studies. The overall effect of DFM supplementation on the log concentrations of all 18 taxa did not differ significantly from the controls (SMD = -0.06, 95% confidence interval [-0.16, 0.04], p = 0.228, $I^2=85%$, n = 699 comparisons), but the 18 taxa could be further classified into three categories by the direction of the effect size: taxa whose log concentrations did not differ significantly from the controls (category 1), taxa whose log concentrations increased significantly with DFM supplementation (category 2), and taxa whose log concentrations decreased significantly with DFM supplementation (category 3). Category 1 comprised nine taxa, including total bacterial counts. Category 2 comprised four taxa: Bacillus, Bifidobacterium, Clostridium butyricum, and Lactobacillus. Category 3 comprised five taxa: Clostridium perfringens, coliforms, Escherichia coli, Enterococcus, and Salmonella. Some characteristics identified by the subgroup analysis were associated with result heterogeneity. Most studies, however, were present with unclear risk of bias. Publication bias was also identified. Conclusion: DFM supplementation increased the concentrations of some beneficial bacteria (e.g. Bifidobacterium and Lactobacillus) and decreased those of some detrimental bacteria (e.g. Clostridium perfringens and Salmonella) in the guts of broiler chickens.

좌골신경통에 적용한 추나 치료에 대한 체계적 문헌 고찰 및 메타 분석 (Systematic Review and Meta-analysis of Chuna Therapy for Sciatica)

  • This study aimed to evaluate the effects of Chuna therapy for Sciatica. We searched the following 16 online databases without a language restriction (Pubmed, Cochrane, Embase, CINAHL, Ovid, Kmbase, RISS, NDSL, OASIS, KISS, KNAL, KTKP, DBpia, CNKI, Wangfang, J-stage) to find randomized controlled clinical trials that used Chuna therapy for Sciatica. The methodological quality of randomized controlled clinical trials (RCTs) were assessed using the Cochrane risk of bias tool and meta-analysis were performed. Among 496 articles that were searched, 15 RCTs were finally selected for systematic review. 14 studies showed that Chuna therapy has positive effect on sciatica. Two studies noted that there were side effects, and the difference between the intervention group and the control group was statistically insignificant. One study noted no side effects and the rest of the study, there was no mention of side effects. Meta-analysis showed positive results for Chuna single therapy in terms of efficiency rate compared to painkiller, herb medicine excepting acupuncture. When comparing Chuna therapy plus acupuncture and acupuncture, Chuna therapy plus acupuncture had a more positive result than acupuncture in terms of efficiency rate. Cochrane Risk of Bias (RoB)evaluation method, most of the studies's selection, performance, detection and reporting bias were unclear. The studies showed that Chuna therapy can significantly effective on sciatica. However, most of the studies's Risk of Bias included in the analysis were not low enough. In the future, to prove the level of evidence of Chuna therapy, more high-quality studies will be needed.

한약 복용이 산후 유즙분비에 미치는 효과에 대한 체계적 문헌고찰 (A Systematic Review and Meta Analysis of Randomized Controlled Trials of the Clinical Effect on Taking a Herbal Medicine on Postpartum Lactation)

  • Objectives: The purpose of this study is to assess the effect of taking herbal medicine for the postpartum lactation. Methods: We searched 9 databases for recent three years, that contained four english, two chinese, one japanese and two korean database from September 17, 2016 to December 31, 2020. Randomized controlled trials (RCTs) were eligible. Measurement of outcome included total curative effective rate, volume of lactation, volume of milk supplement, and serum prolactin concentration. The risk of bias was assessed by two independent authors using the Cochrane risk of bias tool. Results: Total 725 of studies was screened, 11 RCTs were finally selected. Number of participants per study ranged from 60 to 257. The treatment group of taking a herbal medicine is effective for improving total curative effective rate, volume of lactation, volume of milk supplement, and serum prolactin concentration compared with control group. Because most of the studies had considerable heterogeneity in terms of type of intervention and comparison for outcome measurement, meta analysis for quantitative analysis was impossible. Conclusions: This studies showed that taking a herbal medicine is effective on increasing volume of lactation and speeding up the start of the first breastfeeding. However, included studies suffered from incomplete reporting, high or unclear risk of bias and substantial heterogeneity between studies. In the Future, further high-quality RCTs are needed to prove effectiveness of herbal medicine for breastfeeding and reduce the risk of bias.

한국 방송의 팩트 체크 뉴스 공정성 비교 분석 (A Comparative Analysis of Fact-Checking News Fairness in South Korean Broadcasting)

  • 객관보도의 한계를 극복하고자 출발한 방송의 팩트체크 뉴스가 공정성을 담보하고 있는지를 비교하기 위해 20대 대선과 지방선거전이 치러진 2022년 1월1일부터 2022년 5월31일까지 KBS와 MBC SBS, TV조선과 JTBC MBN, YTN의 메인뉴스에 방영된 팩트체크 뉴스 227건을 비교 분석했다. 분석결과 방송사에 따라 팩트체크 검증대상과 서술방식에 뚜렷한 차이를 보였다. 대체로 MBC와 JTBC YTN이 민주당 등 진보 진영에 우호적인 서술이 많았던 반면 TV조선은 국민의 힘 등 보수 진영에 우호적인 서술이 많았다. MBN은 비교적 중립적인 서술태도를 보인 것으로 나타났다. KBS는 외견상 중립을 지키려는 흔적이 역력했다. SBS와 TV 조선이 팩트체크에 가장 적극적이었으나 이슈를 폭넓게 다루면서 사실여부를 명확하게 가리기보다는 시청자들이 궁금해 하는 이슈를 대상으로 맥락 설명에 치우친 것이 특징이다. 팩트체크 서술에서 방송사별로 이념적 편향성이 투영되는 것은 극복해야할 과제이다.

국내 한의학 학회지에 게재된 질적 연구 보고 분석 (Analysis on Reports of Qualitative Research Published in Domestic Korean Medicine Journals)

  • Objectives This study was aimed to analyze the reports of qualitative research published in domestic Korean medicine journals.Methods Eleven published qualitative research studies were selected for analysis. Selected reports were analyzed by Consolidated Criteria for Reporting Qualitative Research (COREQ), a reporting guideline for qualitative research.Results In the 1st domain, descriptions of personal characteristics, interviewer's bias or assumption were insufficient. In the 2nd domain, descriptions such as presence of non-participants, research questions for the interview guidelines, data saturation and transcript return to participants were insufficient. In the 3rd domain, descriptions of items related to number of data coder, participants evaluating the finding, and clarity of minor themes were insufficient.Conclusions Qualitative studies in Korean Medicine have increased. COREQ should be considered by investigators who report qualitative research, in order to improve the quality of research.

  • Purpose: The purpose of this study was to analyze reports of qualitative research published in Journal of Korean Academy of Fundamentals of Nursing (JKAFN). Methods: Twenty four qualitative research studies using in-depth interviews and focus group interviews published in JKAFN from 1994 to 2014 were selected for analysis. Chosen reports were analyzed by consolidated criteria for reporting qualitative research (COREQ), which is a reporting guideline for qualitative research. COREQ consists of 32 items and 3 domains. Results: In the 1st domain, the relationship of researchers and research participants, bias and understanding of the researchers on the study phenomenon were analyzed less than other items. In the 2nd domain, methodological research was carried out across various qualitative studies, but the description of the research environment and research questions for the interview guidelines was insufficient. In the 3rd domain, research results and data were consistent, but data showing opposing content and explanations of negative practices were not done at all. Conclusion: These results indicate a need for the development of evaluation tools for qualitative research and items should be added to improve the reliability and validity, research should be related to the ethics in the framework of the COREQ.

24시간 회상법을 사용한 식이섭취조사에 나타난 한국 여대생의 과소응답 분석 (Under-Reporting in Dietary Assessment by 24-Hour Recall Method in Korean Female College Students)

  • Underreporting of dietary intake is common and might distort analysis and interpretation of dietary surveys. This study was designed to investigate the degree of underreporting and characterastics of under-reporting group in Korean college female students. Dietary survey of 1-day 24-hour recall method was conducted on 379 college students in Seoul and Chonan areas. Physical activity and life style were aquired from questionnnaires. Underreporting was defined as energy intake(EI) lower than 0.9BMR(based metabolic rate), since EI<0.98BMR is statistically judged as bias in 1-day 24 hour recall. BMR was calculated from Schofield's equation. Proportion of underreporting was 18.7% and it's not so different from one of American or European women. Intake of nutrients except vitamin A by underreporting group was lower than other groups(p<0.001). Proportions of subjects with nutrient intake level less than 75% of Korean RDA were more than 80% in protein, Ca, Fe, vitamin A, riboflavin, niacin, zinc as well as energy. Dietary quality of underreporting group was also worse than other groups. Proportion of subjects less than 3 food groups among 5 food group was higher in underreporting group. The number of foods eaten by underreporting group were also less than those of other groups. BMI and body weight were the largest in underreporting group(p<0.05) and the trial of weight reduction was shown higher trend(p<0.01). Different in PAC and other characteristics between underreporting group and other group were not significant. Not only dietary quantity but also dietary quality were worse in the underreporting group. Furthermore underreporting in college female students seemed to be affected by body weight and concern for weight reduction.

Effects of Foot Reflexology on Fatigue, Sleep and Pain: A Systematic Review and Meta-analysis

  • Purpose: The purpose of this study was to evaluate the effectiveness of foot reflexology on fatigue, sleep and pain. Methods: A systematic review and meta-analysis were conducted. Electronic database and manual searches were conducted on all published studies reporting the effects of foot reflexology on fatigue, sleep, and pain. Forty four studies were eligible including 15 studies associated with fatigue, 18 with sleep, and 11 with pain. The effects of foot reflexology were analyzed using Comprehensive Meta-Analysis Version 2.0. The homogeneity and the fail-safe N were calculated. Moreover, a funnel plot was used to assess publication bias. Results: The effects on fatigue, sleep, and pain were not homogeneous and ranged from 0.63 to 5.29, 0.01 to 3.22, and 0.43 to 2.67, respectively. The weighted averages for fatigue, sleep, and pain were 1.43, 1.19, and 1.35, respectively. No publication bias was detected as evaluated by fail-safe N. Foot reflexology had a larger effect on fatigue and sleep and a smaller effect on pain. Conclusion: This meta-analysis indicates that foot reflexology is a useful nursing intervention to relieve fatigue and to promote sleep. Further studies are needed to evaluate the effects of foot reflexology on outcome variables other than fatigue, sleep and pain.

Effects of Elastic Band Resistance Training on Muscle Strength among Community-Dwelling Older Adults: A Systematic Review and Meta-Analysis

  • The purpose of this study was to investigate the effectiveness of elastic band resistance training for muscle strength among community-dwelling older adults. The systematic review and meta-analysis was conducted by following the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA). Data were pooled using fixed effect models. Sit to stand, arm curl, and grip strength were analyzed for main effects. Heterogeneity between studies was assessed using the I2 statistics and publication bias was evaluated by funnel plots. Twelves studies were included representing 611 participants. Elastic band resistance training was effective for lower (d=3.89, 95% CI: 3.03, 4.75) and upper extremity muscle strength (d=4.08, 95% CI: 2.94, 5.23). Heterogeneity was moderate and no significant publication bias was detected. Based on these findings, there is clear evidence that elastic band resistance training has significant positive effects on muscle strength among community-dwelling older adults. Further study will be needed to perform subgroup analysis using number of sessions and exercise intensity as predictors.
