  • A case of repeated bovine abortion caused by Neospora caninum is described. The 3-year-old Holstein cow, which was seropositive to N caninum antibody by indirect immunofluoresent assay, aborted consecutively within a 10-month period. Two fetuses of 6- or 5-month of gestation were aborted in June 1996 and March 1997, respectively. Histologically, multifocal necrotizing encephalitis, nonsuppurative periportal hepatitis, myocarditis, and myositis were observed in both fetuses. Tachyzoites or tissue cysts detected in the brain, heart and liver were positive to N caninum but negative to Toxoplasma gondii by immunohistochemical method. The results of our study demonstrate that repeated abortion due to N caninum actually occurs in Korea. This is believed to the first report of repeated abortion associated with N caninum in Korea.

  • Objectives : The prevalence of infertility is reported to be 23.3%, which is a serious social problem. Habitual abortion is one of the important complications during pregnancy. The purpose of this study is to help the treatment of habitual abortion by observing and reporting the process in which four women who did not have children due to habitual abortion make healthy birth through treatment. Methods : We administered Jokyungjongok-tang gamibang as a herbal medicine treatment method and treated with acupuncture, moxibustion and uterine steam treatment. Treatment was done once or twice a week but it was also varied depending on the patient's circumstances. The herbal medicines were taken 30 minutes after meals and 3 times a day. While taking the Korean medicine, they were prohibited from eating flour, pork, liquor, tobacco and coffee. Patient status was assessed by consultation through pulse, tongue and abdomen. A detailed questionnaire was performed for each treatment. In some cases, they have tested hormone levels at the hospitals to know ovulation dates. The diagnosis of pregnancy was confirmed by ultrasonography at hospitals and the treatment for habitual abortion was judged based on healthy birth. Results : As a result of herbal medicine, acupuncture, moxibustion and uterine steam treatment, the coldness of the body became weak, the fatigue decreased and the digestive condition gradually began to improve. The bad condition of the uterus caused by the repeated administration of heritage has been improved with Boheosaenghwa-tang gamibang. After administration of Jokyungjongok-tang gamibang, they became pregnant and overcame the condition of addictive abortion and gave birth to healthy children. Conclusion : Jokyungjongok-tang gamibang is effective in overcoming the symptoms of habitual abortion and giving birth to a healthy child for a woman who had no children due to her habitual abortion

  • We performed a systematic review and meta-analysis to evaluate whether intralipid administration improved the outcomes of in vitro fertilization. Online databases (PubMed, Cochrane Library, Medline, and Embase) were searched until March 2020. Only randomized controlled trials (RCTs) that assessed the role of intralipid administration during in vitro fertilization were considered. We analyzed the rates of clinical pregnancy and live birth as primary outcomes. Secondary outcomes included the rates of chemical pregnancy, ongoing pregnancy, and missed abortion. We reviewed and assessed the eligibility of 180 studies. Five RCTs including 840 patients (3 RCTs: women with repeated implantation failure, 1 RCT: women with recurrent spontaneous abortion, 1 RCT: women who had experienced implantation failure more than once) met the selection criteria. When compared with the control group, intralipid administration significantly improved the clinical pregnancy rate (risk ratio [RR], 1.48; 95% confidence interval [CI], 1.23-1.79), ongoing pregnancy rate (RR, 1.82; 95% CI, 1.31-2.53), and live birth rate (RR, 1.85; 95% CI, 1.44-2.38). However, intralipid administration had no beneficial effect on the miscarriage rate (RR, 0.75; 95% CI, 0.48-1.17). A funnel plot analysis revealed no publication bias. Our findings suggest that intralipid administration may benefit women undergoing in vitro fertilization, especially those who have experienced repeated implantation failure or recurrent spontaneous abortion. However, larger, well-designed studies are needed to confirm these findings.

  • This study sought to find out the level of perception of the effect on women's physical, mental, socio-culture health of the artificial abortion. The research design was a descriptive correlational study and selected by convenient sampling. The subjects were 510 female and male adults who live in Seoul, Korea. Data were collected from 20th September, 1999 to 28th October, 1999. The data were analyzed through the SPSS program by use of frequency, percentage, t-test, ANOVA and Scheffe test as a post hoc contrast, $x^2$-test, Pearson correlation coefficient and stepwise multiple regression. The results of the study are as follows. 1. The subject who experienced artificial abortion was 24.3% and 468% of these had experienced repeated artificial abortion. 54.4% was using the contraceptives and there was a significant difference in the contraceptives between that women wish to use and that women are really using. 2. For the opinion on the artificial abortion, 77.3% of respondents was pro-abortionists and the most acceptable reasons of pro-choice were" In case of unmarried" and "For the women's health". 3. On the whole, the subjects perceived that artificial abortion had a negative effect on the women's health, especially, on the women's mental health. Among the items, 'guilty feeling about the baby' was the highest. 4. The related factors to the level of perception of the effect on women's health of the artificial abortion were in sex, age, occupational status, marital status, educational level, the number of children, abortion history, contraceptives women use, the frequency of the consideration on the problem of artificial abortion and the position toward abortion such as pro or anti. The main influencing factors on the level o0f perception were artificial abortion history, occupational status and the frequency of the consideration on the problem of artificial abortion. Theses factors explained 35% of total variance of the level of perception 5. The variables related to the experience of artificial abortion were age, occupational status, marital status, religion, educational level, the number of children and whether women practice contraceptives or not.

  • The purpose of this study is to investigate the experience of abortion and practice of contraception in view of preventive in order to analysis the degree of family planning according to the sex & number of actual children. We collected data on married conceptive women who are resident in Seoul and from 20 age to 49 age. This was carried out from 26 July, 1993 to 25 August, 1993. This collected data was analyzed through x² Logistic regression using SAS/PC/sup +/and SPSS/PC/sup +/. The analyzed results are as follow. 1. Those who answered "son is not positively necessary"are only 15.7% among investigated women. 2. The percentage of practicing contraception was 68.2%, in case of 30s' women it was 74.3% and most high. Also, in view of religion, the percentage of practicing contraception of Buddihist was most low as 61.54% (p<0.05). 3. The portion of those who had sons more than four who used permanent. method of contraception is most high(83.3%), in terms of composition, while the ratio of those who have only son is 60.7%, the ratio of those who have only daughter was only 14.3%. 4. In order to investigate the independent variable affecting practice of contraception, we analysed data with Logistic regression analysis. The result of this analysis is as follows: the older(p<0.01) woman is and the higher the level of education of woman(p<0.05), the more it affects significantly practice of contraception. 5. In abrotion experience, the number of experienced women was 241(50.3%) and including repeated abortion, the total number of abortion is 317. The main reason of abortion was that they didn't want more child. The number of abortion due to such reason was 153(48.3%), Especially, the 181(43.0%) cases among last pregnancy 421 cases were the undesired pregnancy. So, the 147 cases of them came to abortion. 6. In the number of actual children, the ratio of those who have three children occupied high portion(56.7%, p<0.05), for the composition of children, although the case of those who have both son and daughter showed most high ratio of experience of abortion, but it was statistically insignificant. 7. In order to investigate independent variable affecting experience of abortion, we used Logistic regression analysis. The result of the analysis was that the older women was(p<0.01) and the more they had actual children(p<0.01), it significantly affected abortion.

  • Objective : To report the prevalence of lupus anticoagulants and anticardiolipin antibodies in patients with recurrent spontaneous abortion and infertility. Material and Method: Lupus anticoagulants and anticardiolipin antibodies were analyzed by Diluted Russell's Viper Venom Test (DRVVT) and solid phase enzyme immunoassay, respectively. Results : In 200 patients with infertility, there were 6 cases (3%) with positive lupus anticoagulants or anticardiolipin antibodies. Of these, 3 patients (1.5%) showed positive lupus anticoagulants and anticardiolipin antibodies, respectively. In 120 patients with recurrent spontaneous abortion, there were 13 cases (10.8%) of positive lupus anticoagulants or anticardiolipin antibodies. Of these, one patient (1%) showed lupus anticoagulants and 12 patients (10%) showed anticardiolipin antibodies. But in two groups, there was no cases with positive lupus anticoagulants and anticardiolipin antibodies. Conclusion: Lupus anticoagulants and anticardiolipin antibodies are definite cause of recurrent spontaneous abortion. There has been a speculation that they might be associated with infertility and repeated IVF failures. But it was found that the role of lupus anticoagulants and anticardiolipin antibodies in these cases are not clear.

  • The aim of present study was to determine the efficacy of immunotherapy with paternal lymphocytes for unexplained infertility. It has been apparent that reproductive success may be affected by the presence of abnormal autoantibodies. Unexplained infertility and repeated pregnancy wastage has been reported in the presence of abnormal autoantibodies. These data suffest that abnormal immune function may be an important pathologic entity contributing subfertility in patients with unexplained infertility. Therefore, immunotherapy may be a possible treatment modality for patients with unexplained infertility. Some investigators have reported that a proportion of infertile couples with repeatedly unsuccessful ET showed close histocompatibility similar to those of spontaneous recurrent abortion. Recently, it has been noted that immunotherapy with paternal lymphocytes achieves a high efficacy in preventing subsequent abortion in women with primary recurrent abortion of unknown cause, which was mediated by immune reaction including blocking antibody. To substantiate the hypothesis, we applied immunotherapy preceding Peritoneal Oocyte and Sperme transfer (POST) to 43 patients, 47 cycles of 82 patients, 89 cycles with at least three previous IUI failure from April, 1993 to February, 1995. There were no significant differences between treatment and control group in clinical response and hormonal response to controlled hyperstimulation. there was no significant difference between treatment and control group in pregnant rates per cycles (42.6% versus 28.6%), but a significantly lower abortion rate per pregnancy in treatment group, with 10.0% (2/20) compared with 50.03% (6/12) in control group. This study may suggest that immune therapy for patients with unexplained infertility with paternal lymphocytes might be beneficial.

  • A reveiw of 85 patients with uterine anomalies was made in respect to the incidence, chief complaints, the reason of infertility, fetal wastage rate, pregnancy complications, fetal presentations and obstetric outcome after metroplasty from 1980 to 1985. The results were summarized as follows: 1. Incidence of uterine anomaly was 0.18% among all outpatients (85/48,240). 2. Of the 85 patients, there were 36 with bicornuate deformities (42.3%), 21 septate (24.7%), 18 uterus didelphys (21.2%), 8 arcuate (9.4%) and 2 patients with unicornuate anomalies (2.4%). 3. Uterine anomalies were diagnosed by hysterosalpingogram (54.1%), pelvic examination (14.2%) and other operative procedures. 4. Chief complaints were primary infertility (41.2%), secondary infertility (15.3%), repeated pregnancy loss (12.9%), antenatal care (11.8%) and menstural disturbance (10.6%), etc. 5. Twenty-nine patients with uterine anomalies had primary infertility. The cause of infertility was proved nonuterine in 26 cases and remained unknown in 3 cases. 6. The obstetric outcome of 104 pregnancies was spontaneous abortion in 51.0%, premature delivery in 11.50/0 and fetal loss in 57.7%. 7. Complications of 41 present pregnancies were threatened abortion (22%), premature rupture of membrane (12%) and premature labor (10%), etc. The frequency of abnormal presentation was 35.3% and 64.7% of deliveries was made by Cesarian section. 8. Metroplasty was performed in 13 patients who didn't have a baby because of repeated miscarriage and unknown cause of infertility. Subsequently 8 patients had 9 successful pregnancies: 6 patients had 7 healthy babies and 2 patients are now in pregnancy without any complications.

  • Primary maternal infection with toxoplasmosis during pregnancy is frequently associated with transplacental transmission of the parasite to the fetus. This study was conducted to test the utility of PCR assay to detect recent infections with Toxoplasma in aborted women at various gestational ages who referred to Obstetrics and Gynecology Department of Alavi Hospital in Ardabil during 2014 and 2016. Two hundred women with a history of single or repeated abortion were investigated in this study. Blood samples were tested for specific anti-Toxoplasma IgM and IgG antibodies by ELISA. According to the results, 53.5% of the women under study were positive for anti-Toxoplasma antibodies: 4.0% of them had IgM, 43.0% had IgG, and 6.5% had both IgM and IgG. Subsequently, Nested-PCR analysis was used to detect T. gondii DNA in the placenta of subjects. In 10.5% of the women, the results were positive for 529 bp element of T. gondii. Among them, 5 (23.8%) cases were IgM positive, 1 (4.8%) case was IgG positive, and 11 (52.4%) were both IgM and IgG positive. In 4 (19.0%) patients, none of the antibodies were found to be positive. In total, 16 patients had positive results in both ELISA and PCR methods, and 174 cases had negative results for new infection. The findings of this study revealed that T. gondii might be one of the significant factors leading to abortion, and that the analysis of placenta can be important in order to achieve increased detection sensitivity.

  • Objectives: Spontaneous pregnancy is a rare event after repeated failure of in vitro fertilization (IVF) or Intrauterine insemination (IUI) treatment. So this paper is to report the natural pregnancy of Gamiseogagjihwang-tang treatments on two infertile patients who were repeatedly failed in IVF or IUI. Methods: One patient was a 35-year-old patient diagnosed with the adenomyosis and contralateral tubal obstruction, treated with 90 cc Gamiseogagjihwang-tang two times per day for 6 weeks after second failure of IVF. The other patient was a 34-year-old patient undergone induced abortion by three times, treated with 90 cc Gamiseogagjihwang-tang two times per day for 4 weeks after second failure of IUI. Results: Through taking Gamiseogagjihwang-tang without assisted reproduction techniques, two cases got pregnant naturally. Conclusions: These cases suggest that Gamiseogagjihwang-tang is effective in treating infertile female after failure in IVF or IUI and inducing spontaneous pregnancy. Therefore, there needs to be more trial on infertile patients treated with Seogagjihwang-tang.
