폐부종 및 신부전을 동반한 개와 고양이에서 체외초미세여과법을 이용한 치료 (Application of Extracorporeal Ultrafiltration Therapy Given to a Dog and a Cat with Pulmonary Edema and Renal Failure)

  • 1년동안 ACVIM C단계의 심부전 및 IRIS 4단계의 신부전으로 관리 중이던 13세의 중성화 잡종견과 급성으로 발병한 신부전으로 인한 요독증과 이에 속발한 형태의 폐부종이 동반된12세의 중성화 암컷 잡종 고양이가 본원에 내원하였다. 본 환자들에 체외초미세여과법을 이용한 투석요법을 적용하여 폐부종, 요독증 및 이에 동반된 전해질 및 산염기 불균형 등이 개선되었다. 본 증례는 초미세여과법을 이용한 투석요법이 이뇨제 저항성 및 심한 전해질 불균형이 있는 심장신장증후군 환자와 이뇨제를 이용한 폐부종 치료 도중 급성으로 신장기능 이상이 발생한 환자 등에서 생명을 구하는 치료옵션으로 고려될 수 있음을 보여준다.

  • 8세 남아가 호흡곤란과 기면증을 보이며 응급실에 내원하였다. 극도의 호흡부전을 보이고 있었고 고유량의 산소 공급을 함에도 불구하고 88-90%로 밖에 유지되지 않았고 단순 흉부 방사선 검사에서 전 폐야에 불투과도가 증가하였고 중등도의 흉수를 보였다. 마이코플라스마 폐렴 진단 하에 정맥 macrolide 를 포함한 항생제 치료를 시작하였으나 2병일 째 간, 신장에 다기관 부전 및 급성 호흡부전 증상을 보였다. 정맥-정맥 체외순환막성산소화기를 삽입하였고 지속적 신대체요법도 병행하였다. 18병일 째 성공적으로 체외순환막성산소화기에서 이탈하였고 저산소성 뇌 손상 없이 성공적으로 치료되었기에 본 사례를 보고한다.

Clinical Relevance of Cystatin C as a Renal Marker in Dogs with Chronic Mitral Valve Insufficiency

  • Cystatin C is a low molecular weight 13 kilodalton protein. It is known to be a more sensitive marker of glomerular filtration rate than creatinine in humans. The purpose of the present study was to demonstrate the changes of renal markers including cystatin C according to the severity of chronic mitral valve insufficiency (CMVI) and to investigate the clinical relevance of cystatin C as an early renal marker in dogs with CMVI. A retrospective study was performed to assess renal function according to International Small Animal Cardiac Health Council (ISACHC) system classification of heart failure in dogs with CMVI. Thirty seven dogs were divided into a group 1 (healthy dogs ; n = 10), a group 2 (ISACHC I ; n = 10) and a group 3 (ISACHC II-III ; n = 17). In all dogs, serum concentrations of bun (sUr), creatinine (sCr) and cystatin C (sCys-C) were measured with an automated analyzer. In dogs with CMVI, sCys-C concentrations were significantly correlated with sCr concentrations and were independent of age, BW, SBP, and sex. Renal dysfunction tended to occur more frequently as the severity of CMVI increases. In dogs with mild CMVI, only sCys-C concentrations were statistically higher than in healthy dogs. This study demonstrates the clinical relevance of sCys-C. sCys-C may be a valuable renal marker for early diagnosis of renal dysfunction in dogs with CMVI.

소아 신장 동맥 폐색에서의 풍선 혈관성형술 (Balloon Angioplasty in a Pediatric Renal Artery Occlusion)

  • 신장 동맥 손상은 둔상의 드문 합병증이나, 신장의 혈류 차단을 일으켜 신부전증을 일으킬 수도 있어 조기 진단과 치료가 필요하다. 신장 동맥 손상의 치료 방법은 아직 논란이 남아 있기는 하지만, 최근 연구들에서 혈관 내 스텐트를 이용한 성공적인 치료 결과가 보고 되었다. 그러나 아직 소아 환자에 대해서는 표준 치료법이 정해지지 않았다. 우리는 16세 여자 환아에서 발생한 4등급 간 열상과 동반된 우측 신장 동맥 폐색 증례를 보고하고자 한다. 환아는 풍선 혈관형성술만을 이용한 치료를 시행 받았고, 후에 신장의 실질 혈류 관류가 뚜렷하게 개선되었으며 신기능이 정상화 되었다. 따라서 소아 환자에서 신장 동맥 손상이 발생했을 경우, 풍선 혈관형성술을 이용한 시술이 치료의 한가지 방법이 될 수 있다.

복잡선천성 심장기형 완전교정수술후 시행한 복막투석의 임상적 고찰 (Peritoneal Dialysis after Correction of Complicated Congenital Heart Disease in Children)

  • 본 연세대학교 심장혈관센타에서는 1992년 I월부터 1993년 12월가지 체외순환을 시행하여 완전교정 을 시행한 복잡심장기형 환아 198명중 22예에서 복막투석을 시행하였다. 이들의 나이는 10일에서 6세 (평균 14.8$\pm$ 17.8개월)이었으며 복막투석의 적응증으로는 수분과부하상태가 10예, 혈장칼릅치가 5. Smmo111 이상인 경우가 3예, 그리고 충분한 양의 이뇨제를 투여함에도 소변양이 Imllkglhr 이하가 시간 이상 지속된 경우 9예에서 시행하였다. 체외순환이 90분 이상인 경우와 그 이하인 경우를 비교할 때 90분 이상 시행한 경우에 의미있게 많이 복막투석을 필요로 하였다(P<0.05). 1개월 미만 환아 13예중 5예(38%)에서 시행하였으나 그 이상의 나이와 비교할 때 의미있게 높지는 않았다(p : 0.08). 16예에서 신기능이 회복되었으나 4명은 호흡부전 또는 폐혈증으로 사망하였고 신기능이 회독되지 않 은 6예는 저심박출증이나 폐부종 등으로 모두 사망하였다. 아직도 사망률은 높으나 체 외순환 후 빈뇨, 수분과부하, 또는 고칼륨증이 있을 때에는 조기에 복막투석을 시행하는 것이 환자 치료에 도움이 되리라 사료된다.

택사산(澤瀉散) 추출물(抽出物)이 Cisplatin으로 유발(誘發)된 흰쥐의 급성신부전(急性腎不全)에 미치는 영향(影響) (Nephroprotective Effects of Taeksa-san Aqueous Extracts on Cisplatin-induced Rat Acute Renal Failure)

  • Objectives : This study was to observe the nephroprotective effects of the traditional prescription, Taeksa-san(TSS). TSS has generally been used for treating various renal diseases, including renal failure. Methods : Three different dosages of TSS extract were orally administered once a day for 28 days, At the 23rd day after TSS extract treatment, cisplatin was also treated. Then, 5 days after cisplatin treatment, all the rats (6 groups of 8 rats each) were sacrificed. Changes in the body weight, kidney weight, serum BUN and creatinine levels were observed with changes on the kidney MDA and GSH contents. The results were compared with captopril 100mg/kg, of which the effects on cisplatin-induced acute renal failures are already confirmed. Results : Dramatic decrease of body weight, increase of kidney weight, increase of serum BUN and creatinine levels were detected in the cisplatin control as compared with the intact control. In addition, marked increase of kidney MDA contents and decrease of kidney GSH contents were also detected in the cisplatin control as compared with the intact control. This means that cisplatin-induced ARF is induced by oxidative stress and related lipid peroxidation in the present study. However, these ARFs and inhibition of antioxidant effects induced by cisplatin were dose-dependently reduced by treatment with all three different dosages of TSS extracts. Conclusion : This study suggests that TSS extracts show favorable effects on cisplatin-induced rat ARF.

Methods : At first, three different dosages of MJMTS extract were orally administered once a day for 28 days. On the 23rd day after MJMTS extract treatment, cisplatin was treated. Then, 5 days after cisplatin treatment, all the rats (6 groups of 8 rats each) were checked in the present study. The changes to the body weight, kidney weight, serum BUN and creatinine levels were observed with changes on the kidney MDA and GSH contents. The results were compared with captopril 100mg/kg in which the effects on cisplatin-induced acute renal failures were already confirmed. Changes of body weight, kidney weight, serum BUN and creatinine levels were observed, as well as changes to the kidney MDA and GSH contents. The results were compared with captopril 100 mg/kg, of which the effects on cisplatin-induced acute renal failures were already confirmed. Results : Cisplatin-induced ARF were induced by oxidative stress and related lipid peroxidation in the present study. However, these ARF and inhibition of antioxidant effects induced by cisplatin were dose-dependently reduced by treatment of OPS extracts except in the 100 mg/kg treated group. The effects of OPS extracts at 500 mg/kg were similar to those of 100 mg/kg of captopril. Conclusion : This study suggests that OPS extracts 500 mg/kg showed favorable effects on the cisplatin-induced rat's ARF as similar to 100 mg/kg of captopril.

홍화자약침액(紅花子藥鍼液)이 수은(水銀)에 의한 가토(家兎)의 신세뇨관(腎細尿管) 물질이동(物質移動) 변화(變化)에 미치는 영향(影響) (Effect of Carthami Semen Aquacupunture(CSA) on Mercury-Induced Alterations in Tubular Transport Function in Rabbits)

  • Objective : This study was undertaken to determine if Carthami Semen Aquacupunc- ture(CSA) exerts protective effect against alterations in membrane transport function rabbits with mercury chloride(HG)-induced acute renal failure. Methods : The administration of Hg at a subcutaneous single dose of 10 mg/kg caused a reduction in GFR and an increase in fractional Na excretion, indicating generation of acute renal failure. When CSA were given for 7 days prior to Hg administration, such changes were significantly attenuated. The fractional excretion of glucose and phosphate was increased in rabbits treated with Hg alone. Results : The increase in rabbits treated with Hg following CSA are significantly lower than that in animals treated with Hg alone. Uptakes of glucose and phosphate in purified isolated brush-border membrane and Na-K-ATPase activity in microsomal fraction were inhibited in rabbits treated with Hg alone. Such changes were prevented by CSA. Uptakes of organic ions, PAH and TEA, in renal cortical slices were inhibited by the administration of Hg, which was prevented by CSA. Exposure of renal cortical slices to Hg in vitro caused an increased LDH release and lipid peroxidation, which was significantly prevented by CSA extract. Conclusions : These results indicate that the administration of Hg causes impairment in reabsorption of solutes in the proximal tubule via the generation of reactive oxygen species. CSA provides the protection against the impairment in proximal reabsorption, and its effect may be resulted from its antioxidant effect.

Quatification of Flavonoid Contents in Chungsimyeonja-tang, a Multi-Herbal Decoction, and Its Protective Effect against Cisplatin-induced Nephrotoxicity

  • Chungsimyeonja-tang (CSYJT) is an herbal decoction that consists with 12 herbal medicines. CSYJT has been shown to have anti-stress, anti-allergic and anti-oxidant effects. The aim of this study was to determine flavonoid contents in CSYJT and evaluate its protective effect against cisplatin induced nephrotoxicity using both in vitro (porcine renal epithelial cell; PK15 cell) and in vivo (Sprague Dawley rat) experiments. In the present study, thee mean contents of baicalin, wogonoside and baicalein in CSYJT were 14.65, 5.27 and 0.02 mg/g, respectively. The CSYJT extract treatment attenuated the following alteration in porcine renal epithelial (PK15) cell: the increase in reactive oxygen species (ROS), the glutathione depletion and the increase in p53 expression induced by cisplatin treatment. In the in vivo study, rats were orally treated with CSYJT extract once a day for 28 days. Five days before the last treatment, cisplatin (5 mg/kg) was intraperitoneally injected to induce acute renal failure. Increased blood urea nitrogen (BUN) and creatinine (CRE) levels after cisplatin treatment were ameliorated by pretreatment of CSYJT extract. In addition, lipid peroxidation was decreased and antioxidant enzyme (glutathione) was recovered in CSYJT pretreated kidney tissue. In histopathological examination, CSYJT pretreated group showed ameliorated pathological alteration after cisplatin injection with decreased apoptosis. Taken together, pretreatment of CSYJT could ameliorate cisplatin-induced nephrotoxicity.
