완전 무치악 환자의 가철성 보철 수복 치료에서 잔존 치조제의 흡수가 심한 경우 임플란트를 이용하면 부가적인 지지, 유지 및 안정을 제공할 수 있다. 환자의 상태에 대한 정확한 진단 및 평가 없이 보철물을 제작하게 되면 생역학적, 기능적, 심미적으로 불리한 치료 결과를 야기하게 된다. 본 증례의 환자는 개인치과의원에서 상·하악 임플란트 피개의치를 제작하였으나 연조직 통증 및 기존 의치 유지력 저하로 재치료를 위해 내원하였다. 진단 결과 기존 의치의 수직고경이 다소 낮게 설정되어 있어 이를 거상하고, milled bar와 부가적인 attachment를 이용한 임플란트 피개의치를 제작하였다. 이를 통해 증진된 구강위생과 기능적, 심미적인 면에서 만족할 만한 결과를 얻었기에 보고하는 바이다.
Statement of problem: Removable partial denture and complete denture often require denture base relines to improve the fit against the tissue-bearing mucosa because of gradual changes in edentulous ridge contours and resorption of underlying bone structure. Purpose: This study was performed to investigate the effect of surface design on bond strength of relining denture base resins to denture base acrylic resin. Materials and method: Heat curing resin(Lucitone 199, Dentsply U.S.A. and Vertex, Dentimex, Holland), self curing resin(Tokuso rebase, Tokuyama, Japan), and visible light curing resin(Triad, Dentsply, U.S.A.) were used in this study. The surface designs were classified as butt, bevel and rabbet joint and the bond strengths were measured by Universial Testing Machine (Zwick 2020, Zwick Co., Germany). Results and Conclusion: The obtained results from this study were as follows ; 1. The bond strength of Vertex resin was higher than those of Tokuso rebase and Triad. 2. The bond strength of rabbet and bevel joint was higher than that of butt joint. 3. The failure mode of Triad and Tokuso rebase was mainly adhesive, but cohesive failure was shown mainly in vertex.
구순구개열 환자에게 가철성 보철치료는 중요한 치료 선택 중 하나이다. 구순구개열 환자에서 만족스러운 기능적 및 심미적 결과를 얻기 위해서는 전통적인 보철 치료법에서 약간의 수정이 필요할 수 있는데, 치주 질환에 이환되어 있으며 소수의 구치만 잔존해 있는 구개열 환자의 경우 이중관에 연결된 가철성 보철이 치료 선택의 대안이 될 수 있다. 이중관이 가철성 국소의치에 연결되면 보철적 안정성과 유지를 증가시키고 치아 장축에 유리한 힘 전달을 최적화하며 심미성도 향상시킨다. 그리고 이중관 의치의 횡단치열궁안정(cross arch stabilization)은 구순구개열 환자의 갈라진 치조제의 안정화에도 도움을 준다. 본 증례는 성인 구순구개열 환자에서 마찰핀(friction pin)을 사용한 혼성 이중관 가철성 국소의치를 제작하여 유지 및 안정성과 함께 심미성도 향상시켜 만족할 만한 결과를 나타낸다.
PURPOSE. The purpose of the study was to assess the influence of build orientations and density of support structures on the trueness of the 3D printed removable partial denture (RPD) frameworks. MATERIALS AND METHODS. A maxillary Kennedy class III and mandibular class I casts were 3D scanned and used to design and produce two 3D virtual models of RPD frameworks. Using digital light processing (DLP) 3D printing, 47 RPD frameworks were fabricated at 3 different build orientations (100, 135 and 150-degree angles) and 2 support structure densities. All frameworks were scanned and 3D compared to the original virtual RPD models by metrology software to check 3D deviations quantitatively and qualitatively. The accuracy data were statistically analyzed using one-way ANOVA for build orientation comparison and independent sample t-test for structure density comparison at (α = .05). Points study analysis targeting RPD components and representative color maps were also studied. RESULTS. The build orientation of 135-degree angle of the maxillary frameworks showed the lowest deviation at the clasp arms of tooth 26 of the 135-degree angle group. The mandibular frameworks with 150-degree angle build orientation showed the least deviation at the rest on tooth 44 and the arm of the I-bar clasp of tooth 45. No significant difference was seen between different support structure densities. CONCLUSION. Build orientation had an influence on the accuracy of the frameworks, especially at a 135-degree angle of maxillary design and 150-degree of mandibular design. The difference in the support's density structure revealed no considerable effect on the accuracy.
The extent and direction of movement of removable partial dentures during function are influenced by the nature of the supporting structures and and the design of the prosthesis. Since forces are transmitted to the abutment teeth through occlusal rests, guide planes and direct retainers during functional movements, proper design based on the avaialble research data will maintain the health of abutment teeth and their supporting structures. The purpose of this in vitro study is evaluating stress distribution clinically around the abutment teeth prepared following 4-type clasping systems for unilateral free-end removable partial dentures. Three-Dimensional Photoelastic Stress Analysis method was used because it shows a visual display of stresses of the simulated abutment teeth and residual ridges and reveals stress concentration that can be read at any given points in terms of direction and magnitude. For this study, the author fabricated 4 mandibular photoelastic epoxy models missing left 1st and End molar. Epoxy models were duplicated and 4 unilateral removable partial dentures were construe- ted in accordance with 4-type direct retainers. Unilateral free-end removable partial dentures were positioned on their own models. 6kg force was loaded on the every removable partial dentures of the epoxy model on the central fossa of mandibular left 1st molar vertically by the loading device. After the stress was frozen in a stress freezing furnace, 6 specimens of 6-mm thickness were made from every epoxy model and examined with the circular polariscope. The results were as follows : 1. Generally I-bar clasp revealed the most favorable stress distribution around the abutment teeth. 2. At the end portion of the free-end ridge, Back action clasp showed the highest stress concentration at the bucco-lingual and top portions of the residual alveolar ridge. 3. At the distal area of the abutment teeth, Akers clasp and Roach clasp showed higher stress concentration bucco-lingually and apically than the others. 4. To the abutment tooth, I-bar clasp showed the least stress distribution bucco-lingually but the others showed irregular stress distribution. 5. At the mesial area of the abutment teeth, the order of effective stress distribution was I-bar clasp, Back-action clasp, Akers clasp and Roach clasp. There was big difference of stress distribution between them. 6. At the right 2nd premolar and 1st molar, the stress concentration of Akers clasp was a little high but that of I-bar clasp was low.
구치부 지지가 붕괴되면 전치부 마모, 교합 고경 감소 등의 문제가 나타나고, 이로 인하여 측두 하악 관절 장애나 저작근의 문제, 비심미적인 안모, 저작 효율의 감소 등의 문제가 초래되기도 한다. 광범위한 수복을 요하는 이러한 환자에서는 정확한 진단과 치료 계획의 수립을 통하여, 기능과 심미 회복 및 근신경계와 악관절의 안정과 적응을 이루어 주어야 한다. 본 증례의 환자는 63세의 남자 환자로 전반적인 치아 마모로 인한 저작 곤란과 심미적 불만족을 주소로 내원하였으며 구강 검사, 방사선 검사, 모형 검사 결과를 종합한 수직 고경 평가를 통하여 수직 고경의 증가를 동반한 가철성 국소의치 수복을 결정하였다. 진단 wax-up을 통해 잠정적으로 4 mm 거상하여 12주간 overlay 형식의 가철성 임시 보철물을 장착하였으며 만족할 만한 적응을 보여 치아 삭제 후 임시 치관 장착 및 새로운 임시 가철성 국소의치를 장착하였다. 16주 후 안정적인 적응을 보여 최종 보철물을 제작하였다. 7개월 간 경과 관찰 결과 안정적인 교합을 이루고 있는 것을 관찰하였고, 이를 통해 환자의 기능적, 심미적인 만족을 얻었기에 이를 보고하는 바이다.
기존의 후방 연장 국소의치는 재이장을 위한 잦은 내원의 필요성, 클래스프에 의한 지대치로의 과도한 응력, 클래스프의 노출에 의한 비심미성 등의 단점이 있다. 이에 임플란트를 이용하여 부가적인 지지나 유지를 얻는 Implant-assisted RPD(IARPD)는 국소의치의 안정성과 심미성, 그리고 저작 효율 증진에 도움을 줄 수 있다. 또한 의치의 교합면을 지르코니아로 교체한다면 인공치 마모로 인한 대합치의 정출 및 교합평면의 부조화를 예방할 수 있을 것이다. 본 증례는 엇갈린 교합을 가진 상하악 부분 무치악 환자에서 임플란트 고정성 보철물과 임플란트 유지 국소의치를 제작하여 전악 재건 시행한 증례로 주기적인 내원을 통해 기능적, 심미적으로 만족할만한 결과를 얻었기에 보고하고자 한다.
넓은 영역의 무치악 부위를 수복하기 위한 전통적인 방법은 의치를 이용한 치료다. 하지만, 임플란트를 활용할 경우 보철물의 안정과 유지, 그리고 지지의 측면에서 더 유리하며, 저작효율이 향상되는 등 기능적으로 더 양호한 결과를 얻을 수 있다. 이 같은 임플란트를 이용한 치료방법은 크게 가철성과 고정성 방식으로 분류될 수 있으며, 치조골의 흡수정도, 악간관계, 환자의 선호도, 사회 경제적 요인 등을 고려하여 적절한 접근 방식을 선택하여야 한다. 한편, 지르코니아의 물성이 크게 개선됨에 따라, 임플란트 지지형 고정성 보철물의 재료로서 단일구조 지르코니아가 각광을 받고 있다. 단일구조 지르코니아는 지르코니아 코어에 세라믹을 비니어링 한 보철물이나 금속 도재관보다 파절과 chipping의 빈도가 적으며 지르코니아 자체가 충분한 두께를 가질 수 있어 구조적으로 좋은 물성을 지닐 수 있다. 최근에는 투명도가 증진된 단일구조 지르코니아도 출시되어, 전치부 보철물을 위한 재료로도 널리 사용되고 있다. 본 증례의 남환은 상악 양측 구치부의 결손 및 다수 잔존치의 중등도 이상의 전반적 만성 치주염 이환으로 인해, 하악 양측 견치 및 좌측 제1, 2 소구치를 제외한 모든 치아를 발치한 후, 상악 8개 및 하악 3개의 임플란트 식립을 동반한 전악 수복 치료를 받았다. 보철물 장착 후 1년 간의 경과 관찰 기간 동안 기능적, 심미적으로 만족할만한 결과를 얻어 이를 보고하는 바이다.
PURPOSE. This study evaluated the adhesion of 10-MDP containing self-etch and self-adhesive resin cements to dentin with and without the use of etch-and-rinse technique. MATERIALS AND METHODS. Human third molars (N=180) were randomly divided into 6 groups (n=30 per group). Conventional (Panavia F2.0, Kuraray-PAN) and self-adhesive resin cements (Clearfil SA, Kuraray-CSA) were bonded to dentin surfaces either after application of 3-step etch-and-rinse (35% $H_3PO_4$ + ED Primer) or two-step self-etch adhesive resin (Clearfil SE Bond). Specimens were subjected to shear bond strength test using the universal testing machine (0.5 mm/min). The failure types were analyzed using a stereomicroscope and quality of hybrid layer was observed under a scanning electron microscope. The data (MPa) were analyzed using two-way ANOVA and Tukey's tests (${\alpha}$=.05). RESULTS. Overall, PAN adhesive cement showed significantly higher mean bond strength ($12.5{\pm}2.3-14.1{\pm}2.4$ MPa) than CSA cement ($9.3{\pm}1.4-13.9{\pm}1.9$ MPa) (P<.001). Adhesive failures were more frequent in CSA cement groups when used in conjunction with two-step self-adhesive (68%) or no adhesive at all (66%). Hybrid layer quality was inferior in CSA compared to PAN cement in all conditions. CONCLUSION. In clinical situations where bonding to dentin substrate is crucial, both conventional and self-adhesive resin cements based on 10-MDP can benefit from etch-and-rinse technique to achieve better quality of adhesion in the early clinical period.
Relining and rebasing are essential for long-term success and oral health in removable prosthodontics. Major features of failures between metal base and relining resins are adhesive failure due to lack of chemical bonding. The purpose of this study was to find a better metal primer and metal surface treatment method that enhance the bonding strength with relining resin materials. The surfaces of ticonium alloys were treated with $25{\mu}m$ sandblasting (Group A), stone wheel(Group B), stone wheel and EZ oxisor(Group C), $75{\mu}m$ sandblasting(Group D) and EZ oxisor application after $75{\mu}m$ sandblasting(Group E). They were subdivided into no primer application (Group I), MR bond application(Group II) and Metafast bonding liner (Group III). Then specimens were completed though being bonded with relining resins. The specimens were stored in $38^{\circ}C$ water for 48 hours and tensile strength was measured using the universal testing machine. The results were as follows, 1. Primer application groups showed higher bond strength than no primer application group(p<0.05). 2. In comparison with primer application groups, MR bond group showed higher bond strength than Meta fast bonding liner application group(p<0.05). 3. In comparison with surface treatment methods, Bond strengths of group A and B were significantly different with group C, D, and E, and group C were significantly different with group D, and E in no primer application group()(0.05). In primer application groups, group A, B, C were significantly different with group D and E(p<0.05). According to results of this study, Metal primer application and metal surface roughening were considered to be advantageous for relining of metal base dentures.
