Implant assisted removable partial denture using bilateral single implant-supported surveyed crown: a case report (양측성 단일 임플란트 지지 서베이드 크라운을 이용한 하악 임플 란트 보조 국소의치 수복 증례)

  • Implant assisted removable partial denture (IARPD) has been practiced in various forms for a long time, and among them, implant surveyed crown RPD is gaining predictability as well as being considered as a treatment option for patients with anatomical and financial disadvantages. The position of implant could be divided as posterior placement or anterior placement according to the purpose of the treatment and should be planned in consider to the alveolar ridge of patient, anticipated prognosis of remaining teeth, and opposing dentition. This case report describes a treatment for mandibular Kennedy class I partial edentulous patient with two implant-supported surveyed crown and implant assisted removable partial denture. Given the difficulty of posterior placement in this patient and the prognosis of the residual teeth, the plan was to place two implants in close proximity to the residual teeth, which were placed in the planned position, angle, and depth using guided surgery. The process of fabricating the fixed prosthesis was carried out in parallel with the maxillary edentulous tooth arrangement process to increase predictability, and when fabricating the localized tooth, the implant was designed in a form that allows the patient to perform functional movements by preventing excessive loading as the last supporting tooth, and was fabricated through a secondary impression process. Each treatment procedure was proceeded as planned, with aesthetically and functionally satisfactory results for both patient and operator.

  • When attempting to restore the oral function of a partially edentulous patient, there are a number of prosthetic treatment options available, depending on the structure of remaining teeth. For example, when only one set of maxillary and mandibular teeth are diagonally in place across from each other, it is difficult to gain stable occlusion. In this case, implants can be put in place at the corresponding edentulous area to achieve balance. By doing so, a stable occlusion can be achieved. For this case report, a patient with crossed occlusion after extraction was treated with maxillary RDP (removable dental prosthesis) and mandibular implant-supported RDP (removable dental prosthesis). Moreover, an implant fixture was placed under the posterior molar of the distal extension base diagonally across from the remaining maxillary teeth. Then, magnetic attachment was implemented. According to the patient who received the treatment, the result was functionally and aesthetically satisfactory.

  • Computer-aided design-computer-aided manufacturing technology has been widely used in the manufacture of fixed prostheses including implants, but in the case of removable dentures, the analog method is still being used due to the errors such as a lack of fusion and over-fusion in selective laser meting process. With the recent development of CAD software, virtual surveying and framework design are made possible, and the designed file can be manufactured by milling or 3D printing. It replace the analog method of waxing and denture curing process and also can reduce the production time and cost. Therefore, this case is reported because good clinical results were obtained by digitally surveying on CAD software to produce a surveyed metal restoration and framework on maxillary and mandibular removable dentures.

  • With the recent development of computer-aided design-computer-aided manufacturing technology and 3D printing technology, and the introduction of various digital techniques, the accuracy and efficiency of top-down definitive prosthetic restoration are increasing. In this clinical case, stable occlusion support was obtained through the placement of a total of 9 maxillary and mandibular posterior implants in patient with anterior-posterior crossed occlusion. The edentulous area of the maxillary anterior teeth, which showed a tendency of high resorption of the residual alveolar bone, was restored with a Kennedy Class IV implant assisted removable partial denture to restore soft tissue esthetics. Computed tomography guided surgery was used to place implants in the planned position, double scan technique was used to reflect the stabilized occlusion in the interim restoration stage to the definitive prostheses, and metal 3D printing was used to manufacture the coping and framework. This clinical case reports that efficient and predictable top-down full mouth rehabilitation was achieved using various digital technologies and techniques.

  • An implant-supported fixed dental prosthesis (ISFDP) or an implant-supported overdenture (IOD) are good options when treating a completely edentulous jaw opposing natural teeth. However, an ISFDP for a full arch requires sufficient bone quality and quantity, which limits its application. Meanwhile, using an ISFDP as an abutment of a removable partial denture has been considered recently. This clinical report discusses the treatments applied to two patients with edentulous maxillas and opposing natural teeth: one was treated with an IOD and the other was treated with an ISFDP and removable partial denture. Follow-up and management were performed for 8 years.

  • Traditional denture treatment is often performed in patients with the completely edentulous maxilla and the bilateral edentulous posterior mandible. However, with a long-term prognosis, a complication called 'combination syndrome' may occur in this oral environment. In this case, by applying an implant-assisted removable partial denture (IARPD), it is possible to preserve the alveolar bone and prevent combination syndrome. In this case, when additional implants are placed in the edentulous area, the stress applied to the abutment due to the movement of the removable denture can be reduced, so it is possible to design an esthetic clasp using resin rather than metal, and flexible denture base resin is used. As a treatment for maxillary complete denture and mandibular flexible IARPD, it showed an esthetic and functional long-term prognosis with minimal cost and surgery.

  • Purpose: To evaluate the effects of implant location and length on stress distribution and displacement in osseointegrated-implants that were associated with mandibular distal extension removable partial dentures (DERPD). Materials and Methods: A sagittally cut model with the #33, #34 teeth and a removable partial denture of the left mandible was used. Seven models were designed with NX 9.0. Models A, B, C had implants with lengths of 11, 6, 4 mm, respectively, under the denture base of the #37 artificial tooth. Models D, E, F had implants with lengths of 11, 6, 4 mm, respectively, under the denture base of the #36 artificial tooth. Model G did not have any implants. Axial force (250 N) was loaded on #36 central fossa. The finite element analysis was performed with MSC Nastran. Von Mises stress maps were plotted to visualize the results. Results: The models of #37 implant placement showed much lower stress concentration on the surrounding bone of the implant compared with #36. The #36 implant position tended to reduce displacement more than #37. Conclusion: When an IARPD is designed, the distal positioning of implant placement has more advantages in the edentulous bone of DERPD on the prognosis of short implants and the stress distribution of edentulous alveolar bone. Using implants with longer lengths are important for stress distribution. However, Additional studies are necessary of the effects of length on implant survival.

  • In this clinical report, a simple and convenient conversion of a fractured metal-ceramic surveyed crown into a complete contour zirconia surveyed crown by using computer-aided design and computer-aided manufacturing technology for an existing partial removable dental prosthesis is described. The duplication of the original contours, morphology, and the rest seat of the existing metal-ceramic surveyed crown, into a complete contour zirconia surveyed crown under anticancer treatments were reported.

FLEXURE STRENGTH AND MICROSTRUCTURE OF IPS EMPRESS 2 GLASS-CERAMIC ON HEAT-PRESSING AND HEAT TREATMENTS (열가압 및 열처리에 따른 IPS Empress 2 세라믹의 굴곡강도와 미세구조)

  • This investigation was designed to determine whether heat-pressing and/or simulated heat treatments affected the flexure strength and the microstructure of the lithium disilicate glass-ceramic in the IPS Empress 2 system. Four groups of the specimens were prepared as follows: group 1 - as-received material, group 2 - heat-pressed material; group 3 - heat-pressed and simulated initial heat-treated material; group 4 - heat-pressed and the simulated heat-treated material with full firings for a final restoration. The three-point bending test and the scanning elec-tron microscope (SEM) analysis was conducted for the purpose of this study. The flexure strength of group 2 was significantly higher than that of group 1. However, there were no significant differences in strength among group 2, 3, and 4, and between group 1 and 4. The SEM micrographs of the lithium disilicate glass-ceramic showed the closely packed, multi-directionally interlocking microstructure of numerous lithium disilicate crystals protruding from the glass matrix. The crystals of the heat-pressed materials (group 2, 3, and 4) were a little denser and about two times bigger than those of the as-received material (group 1). This change of microstructure is more obviously exhibited particularly between group 1 and 2. However, there was no a marked difference among group 2, 3, and 4 after the heat-pressing procedure. Although there were significant increase of the strength and some changes of the microstructure after the heat-pressing operation, the combination of the heat-pressing and the simulated subsequent heat treatments did not produce the increase of strength of IPS Empress 2 glass-ceramic.

Clinical and radiographic evaluations of implants as surveyed crowns for Class I removable partial dentures: A retrospective study

  • PURPOSE. The purpose of this study was to evaluate survival rates and marginal bone loss (MBL) of implants in IC-RPDs. MATERIALS AND METHODS. Seventy implants were placed and used as surveyed crowns in 30 RPDs. The survival rates and MBL around implants based on multiple variables, e.g., position, sex, age, opposing dentitions, splinting, type of used retainer, and first year bone loss, were analyzed. Patient reported outcome measures (PROMs) regarding functional/esthetic improvement after IC-RPD treatment, and complications were also inspected. RESULTS. The 100% implant survival rates were observed, and 60 of those implants showed MBL levels less than 1.5 mm. No significant differences in MBL of implants were observed between implant positions (maxilla vs. mandible; P = .341) and type of used retainers (P = .630). The implant MBL of greater than 0.5 mm at 1 year showed significantly higher MBL after that (P < .001). Splinted implant surveyed crowns showed lower MBL in the maxilla (splinted vs. non-splinted; P = .037). There were significant esthetic/functional improvements observed after treatment, but there were no significant differences in esthetic results based on implant position (maxilla vs. mandible). Implants in mandible showed significantly greater improvement in function than implants in the maxilla (P = .002). Prosthetic complication of IC-RPD was not observed frequently. However, 2 abutment teeth among 60 were failed. The bone loss of abutment teeth was lower than MBL of implants in IC-RPDs (P = .001). CONCLUSION. Class I RPD connected to residual teeth and strategically positioned implants as surveyed crowns can be a viable treatment modality.
