  • One hundred among 320 patients who underwent splanchnic nerve block were evaluated retrospectively by telephone or letters. After splanchnic nerve block most of the patient experienced relatively good pain relief until death. The results were as follows 1) The duration of pain relief and survival time were well correlated. 2) The duration of survival time after the block was varied from 35 days to 240 days. These data suggest that the splanchnic nerve block is a satisfactory and reliable method for terminal cancer pain and results in a painless life until death.

  • For robot navigation and visual reconstruction, structure from motion (SFM) is an active issue in computer vision community and its properties arc also becoming well understood. In this paper, when using stereo image sequence and direct method as a tool for SFM, we present a new method for overcoming bas-relief ambiguity. We first show that the direct methods, based on optical flow constraint equation, are also intrinsically exposed to such ambiguity although they introduce robust methods. Therefore, regarding the motion and depth estimation by the robust and direct method as approximated ones. we suggest a method that refines both stereo displacement and motion displacement with sub-pixel accuracy, which is the central process f3r improving its ambiguity. Experiments with real image sequences have been executed and we show that the proposed algorithm has improved the estimation accuracy.

  • Digital surface model generation from IKONOS stereo imagery is a new challenge in photogrammetric community, especially when the satellite company does not provide the raw data as well as their ancillary ephemeris data. In this paper we utilized an estimated relief displacement azimuth and the nominal collection elevation data included in the metadata file to correct the relief displacement of GCPs, together with a linear transformation for geometric modeling of IKONOS imagery. Space intersection is performed by the trigonometric intersection assuming a parallel projection of IKONOS imagery due to its small FOV and frame size. In the experiment, less than 2-meters of RMSE in orbit modeling is achieved denoting the potential positioning accuracy of the IKONOS stereo imagery.

  • Background: Some clinical trials have reported that a new analgesic combination of tramadol and acetaminophen provides good efficacy in various pain models. For the more clinical uses of this agent, comparisons about the onset of analgesia and analgesic efficacy in the acute state of pain with the other drugs known as strong analgesics were needed. Purpose: The goal of this study was to compare the times to onset of analgesia and the other analgesic efficacy of 75 mg tramadol/650 mg acetaminophen and 20 mg codeine/500 mg acetaminophen/400 mg ibuprofen in the treatment of acute pain after oral surgery. Patients and Methods: Using a randomized, single-dose, parallel-group, single-center, and active-controlled test design, this clinical study compared the times to onset of analgesia using a two-stopwatch technique and the other analgesic efficacy of the single-dose tramadol/acetaminophen and codeine/acetaminophen/ibuprofen. These were assessed in 128 healthy subjects with pain from oral surgical procedures involving extraction of one or more impacted third molars requiring bone removal. From the time of pain development, the times to onset of perceptible and meaningful pain relief, pain intensity, pain relief, an overall assessment, and adverse events of the study medications were recorded for 6 hours. Results: The demographic distribution and baseline pain data in the two groups were statistically similar. The median times to onset of perceptible pain relief were 21.0 and 24.4 minutes in the tramadol/acetaminophen and codeine/acetaminophen/ibuprofen groups respectively and those to onset of meaningful pain relief were 56.4 and 57.3 minutes, which were statistically similar. The other efficacy variables such as mean total pain relief (TOTPAR) and the sum of pain intensity differences (SPID) were also similar in the early period after pain development and drug dosing. The safety of tramadol/acetaminophen was well tolerated and very comparable to that of codeine/acetaminophen/ibuprofen. Conclusions: In this acute dental pain model, the onset of analgesia and analgesic efficacy of tramadol/acetaminophen was comparable to that of codeine/acetaminophen/ibuprofen. These results showed that tramadol/acetaminophen was recommendable for fast and effective treatment in the management of postoperative acute pain.

  • Purpose: This study collects basic data on the awareness of evacuation methods and evacuation facilities in the event of a radiological disaster of residents living in the emergency planning zone. Method: The residents of emergency planning zone were sampled using a random sampling method. A 1:1 interview was conducted using a structured questionnaire, and statistical analysis was performed using the minitab program. Result: First, the survey subjects showed a relatively low and negative awareness of the local government's work on radioactive disasters. Second, in terms of resident safety education, they had little experience in education, but they felt it was necessary and wanted education on evacuation methods, action tips, and the location of relief centers. Third, the location of the relief centers related to radioactive disasters was not well known, and there were many responses that they did not receive any guidance, and that they would be with their families when using the relief centers. Satisfaction levels were generally low with regard to the relief facilities. Fourth, the necessary priorities in preparation for radioactive disasters were education and training for radioactive disasters, facility supplementation, and supply of protective chemicals. Conclusion: The residents of emergency planning zone perceived the policies and tasks of the government or local governments relatively negatively in preparation for the occurrence of radioactive disasters, and their satisfaction was low. Regarding the matters pointed out as a priority, the government and local governments should publicize and educate the residents of accurate information and policies on radioactive disasters.

  • This study observed the temperature and humidity within the green zone to understand the effect that land coverage and the structure of forests have on the relief of micro-climate control. Based on this set of data, this study interpreted, through the regression analysis, the relevance of land coverage of the green zone with temperature and distribution of humidity, as well as the amount of green with the relief of microclimate control. The results of the study demonstrated that high temperature regions were formed in barren areas, and low temperature regions in forests or near the water. In particular, low temperature was found in areas covered with tall and small trees, the water surrounded by forests and areas enclosing small rivers. Furthermore, mechanisms causing low temperature were, among others, the ratio of land coverage (forest, grassland, water). In fact, the temperature reduction effect varied in accordance with the types and ratios of the land coverage. Humidity also showed a close correlation with the distribution of temperature high temperature areas had low humidity and low temperature areas had high humidity. Such a phenomenon.

  • Regional sympathetic blockade is the most effective treatment for reflex sympathetic dystrophy (RSD). Radiofrequency thermocoagulation provides longer duration of pain relief than local anesthetics and less complication than chemical neurolytic agents for lumbar sympathectomy. Spinal cord stimulation (SCS) is thought to be an effective modality yieding good results in treating intractable neuropathic pain. Therefore RSD might be a good indication for SCS. We treated a patient with RSD who responded well to lumbar sympathetic blockade (LSB) with radiofrequency thermocoagulation and SCS. The patient had a left ankle sprain requiring a case for the lower leg for 2 weeks. The patient suffered increasing pain and swelling on the lower part of that leg. We thought to block the lumbar sympathetic chain utillzing radiofrequency thermocoagulation 2 days after LSB with local anesthetics. The results provided accepatable pain relief (VAS $8{\rightarrow}15$) but the patient still could not walk due to remaining pain which was further aggravated by walking. After SCS, pain relief improved (VAS $5{\rightarrow}13$) and patient could walk without assistance.

  • Improving the formal objection system regarding reviewing medical expenses requires authority and confidence in the aspect of well-functioning the health insurance review and assessment system, legally and appropriately. The purposes of improvement of the formal objection system should aim for protecting the people's right of health. On handling the formal objections, the disputes of the rights should be settled economically and promptly by fairness, specialty, and objectivity in the health insurance review and assessment administration. Therefore, in order to promote the administrative specialty of health insurance, the formal objection committee needs to be organized independently and guaranteed expertly. Under the current formal objection system, however, the organization of committee lacks right-relief function, recognition and public relation as a health insurance appeal system, and related professional man powers. It is also analyzed that there are several controversial points, such as mass deliberation to the formal objection committee and its conference procedure. As a measure of improvement, it is analyzed that the committee needs to be organized independently with a proper number of professional man powers. The strict deliberation procedures and the prohibition of the decision-making by non-conference are also required to be empowered. The formal objection procedure provides the beneficiaries and the claims legitimately, so that it secures the legal relations on the health insurance system. Therefore, on the conference process of formal objection, the expert and guaranteed protection should be provided promptly, and its procedures to the appellants should also be assisted kindly.

  • A various ${\alpha}$-arylmethylpropionic acids(profen) have been widely used as non-steroidal anti-inflammatory drugs for the relief of acute and chronic rheumatoid arthritis and osteoarthritis, as well as for other connective tissue disorders and pains. Example is fenoprofen, ibuprofen, ketoprofen, and naproxen. All are chiral and, except for naproxen and ibuprofen, are marketed in racemic form. Enantioseparations of profens have been of considerable interest becaus their anti-inflammatory and analgesic effects have been attirbuted almost exclusively to their (S)-enantiomer. (omitted)
