Genetic and Epigenetic Biomarkers on the Personalized Nutrition

  • Nutritional genomics is a new field of study of how nutrition interacts with an individual's genome or individual responds to individual diets. Systematic approach of nutritional genomics will likely provide important clues about responders and non-responders. The current interest in personalizing health stems from the breakthroughs emerging in integrative technologies of genomics and epigenomics and the identification of genetic and epigentic diversity in individual's genetic make-up that are associated with variations in many aspects of health, including diet-related diseases. Microarray is a powerful screen system that is being also currently employed in nutritional research. Monitoring of gene expression at genome level is now possible with this technology, which allows the simultaneous assessment of the transcription of tens of thousands of genes and of their relative expression of pathological cells such tumor cells compared with that of normal cells. Epigenetic events such as DNA methylation can result in change of gene expression without involving changes in gene sequence. Recent developed technology of DNAarray-based methylation assay will facilitate wide study of epigenetic process in nutrigenomics. Some of the areas that would benefitfrom these technologies include identifying molecular targets (Biomarkers) for the risk and benefit assessment. These characterized biomarkers can reflect expose, response, and susceptibility to foods and their components. Furthermore the identified new biomarker perhaps can be utilized as a indicator of delivery system fur optimizing health.

Urban Quality of Life Assessment Using Satellite Image and Socioeconomic Data in GIS

  • This paper evaluates and maps the quality of life in the Atlanta, Georgia metropolitan area in 2000. Three environmental variables from Landsat TM data, four socioeconomic variables from census data, and a hazard-related variable from toxic release inventory (TRI) database were integrated into a geographic information system (GIS) environment for the quality of life assessment. To solve the incompatibility problem in areal units among different data, the four socioeconomic variables aggregated by zonal units were spatially disaggregated into individual pixels. Principal components analysis (PCA) was employed to integrate and transform environmental, socioeconomic, and hazard-related variables into a resultant quality of life score for each pixel. Results indicate that the highest quality of life score was found around Sandy Springs, Roswell, Alphretta, and the northern parts of Fulton County along Georgia 400 whereas the lowest quality of life score was clustered around Smyma of Cobb County, the inner city of Atlanta, and Hartsfield-Jackson International Airport. The results also reveals that normalized difference vegetation index (NDVI) and relative risk from TRI facilities are two versatile indicators of environmental and socioeconomic quality of an urban area in the United States.

Assessment of interhospital transport care for pediatric patients

  • Background: Many critically ill patients require transfer to a higher-level hospital for complex medical care. Despite the publication of the American Academy of Pediatrics guidelines for pediatric interhospital transportation services and the establishment of many pediatric transport programs, adverse events during pediatric transport still occur. Purpose: To determine the incidence of adverse events occurring during pediatric transport and explore their complications and risk factors. Methods: This prospective observational study explored the adverse events that occurred during the interhospital transport of all pediatric patients referred to the pediatric intensive care unit of Ramathibodi Hospital between March 2016 and June 2017. Results: There were 122 pediatric transports to the unit. Adverse events occurred in 25 cases (22%). Physiologic deterioration occurred in 15 patients (60%). Most issues (11 events) involved circulatory problems causing patient hypotension and poor tissue perfusion requiring fluid resuscitation or inotropic administration on arrival at the unit. Respiratory complications were the second most common cause (4 events). Equipment-related adverse events occurred in 5 patients (20%). The common causes were accidental extubation and endotracheal tube displacement. Five patients had both physiologic deterioration and equipment-related adverse events. Regarding transport personnel, the group without complications more often had a physician escort than the group with complications (92% vs. 76%; relative risk, 2.4; P=0.028). Conclusion: The incidence of adverse events occurring during the transport of critically ill pediatric patients was 22%. Most events involved physiological deterioration. Escort personnel maybe the key to preventing and appropriately monitoring complications occurring during transport.

Statistical Assessment on Cancer Risks of Ionizing Radiation and Smoking Based on Poisson Models

  • In many epidemiological and medical studies, a number of cancer mortalities in categorical classification may be considered as having Poisson distribution with person-years at risk depending upon time. The cancer mortalities have been evaluated by additive or multiplicative models with regard to background and excess risks based on several covariances such as sex, age at the time of bombings, time at exposure, or ionizing radiation, cigarette smoking habits, duration of smoking habits, etc. An interest herein is to examine an additive, synergistic, or antagonistic relationship between radiation exposures and cigarette smoking habits for cancer mortalities. The results revealed a highly significant antagonistic in uence for cancer mortalities from all non-hematologic findings, lung and respiratory system with negative interaction between radiation exposures and cigarette smoking amounts.

Caenorhabditis elegans as a Biological Model for Multilevel Biomarker Analysis in Environmental Toxicology and Risk Assessment

  • While in some instances, loss of diversity results from acute toxicity (e.g. major pollution incidents), in most cases it results from long-term sub-lethal effects that alter the relative competitive ability and fitness of certain organisms. In such cases the sub-lethal effects will cause a physiological response in the organism that ultimately leads to community level changes. Very sensitive tools are now available to study sub-lethal responses at the molecular level. However, relating such laboratory measurements to ecological effects represents a substantial challenge that can only be met by investigation at all scales (molecular, individual organism and community level) with an appropriate group of organisms. Among the various in vertebrates which can be used as model organisms in such a way, the soil nematode, Caenorhabditis elegans appear to be a promising biological model to diagnose environmental quality. This paper reviews the current status of multilevel biomarkers in environmental toxicology, and C. elegans as promising organisms for this approach.

  • The floor slip test method using O-Y·PSM was developed based on the risk assessment and sense of slip by the users implementing actions such as changing walking direction on a floor. This test method is regulated under the Korea Industrial Standard KS M 3510, and in the Korea Industrial Standard KS F 3230, the article of KS M 3510 is cited. Yet, in the standard, the surface condition of test or slip adjustment method is merely mentioned or difficult to be found, and thus it creates confusion in floor slip test using O-Y·PSM. Therefore, this study is to provide the useful data to revise the relative standard through the standardization study including various surface conditions of sample and slip adjustment method used in floor slip test method using O-Y·PSM.

신경망 분리모형과 사례기반추론을 이용한 기업 신용 평가 (Corporate Credit Rating using Partitioned Neural Network and Case- Based Reasoning)

  • The corporate credit rating represents an assessment of the relative level of risk associated with the timely payments required by the debt obligation. In this study, the corporate credit rating model employs artificial intelligence methods including Neural Network (NN) and Case-Based Reasoning (CBR). At first we suggest three classification models, as partitioned neural networks, all of which convert multi-group classification problems into two group classification ones: Ordinal Pairwise Partitioning (OPP) model, binary classification model and simple classification model. The experimental results show that the partitioned NN outperformed the conventional NN. In addition, we put to use CBR that is widely used recently as a problem-solving and learning tool both in academic and business areas. With an advantage of the easiness in model design compared to a NN model, the CBR model proves itself to have good classification capability through the highest hit ratio in the corporate credit rating.

선 자세에서의 인체 관절의 지각 불편도 측정 (Measurement on Perceived Human Body Joints Discomfort in the Standing Posture)

  • In workplace design, an ergonomic solution should ensure low postural stress in the operator during his/her work. Stress caused by awkward working postures of the trunk, shoulders and legs can result in fatigue, discomfort, musculo-skeletal disorders and nerve entrapment syndromes. Since discomfort and musculo-skeletal disorders are both related to exposure to biomechanical load on the musculo-skeletal system, minimization of discomfort will contribute to reduction of the risk for musculo-skeletal disorders as well. Therefore, in this study, perceived discomfort on the human body joints was measured in the standing postures using the magnitude estimation in order to have a standardized numerical scale for joint discomfort. Nine healthy graduate students participated voluntarily in the laboratory study. The results revealed that perceived discomfort of all the joints increased as the joints deviated from neutral position. Especially, it showed drastic increment on perceived discomfort when deviation from neutral position in each human body joint increased from 75% to 100%. in terms of relative range of motion(R0M). On the basis of these experimental results, a preliminary ranking for assessment of stressfulness of non-neutral postures around the human body joints was suggested.

Diabetes and depression

  • Diabetes and depression are serious chronic conditions. As a result of their increasing prevalence, diabetes and depression, together with population growth and aging, are public health issues. The rate of depression in adults with either type 1 diabetes or type 2 diabetes is high relative to the general population. The coexistence of diabetes and depression has attracted much interest. Although it is unclear whether diabetes and depression are causally linked, most studies have shown that the association between diabetes and depression might be bidirectional. Currently, emotional well-being is becoming an increasingly important aspect of diabetes care and self-management. Psychiatric disorders and diabetic distress among people with diabetes may increase the risk of diabetes complication and mortality. Thus, assessment and appropriate management of depression in people with diabetes should be considered to achieve psychological well-being and optimize medical outcomes.

MTHFR Polymorphisms and Pancreatic Cancer Risk:Lack of Evidence from a Meta-analysis

  • Objective: Methylenetetrahydrofolate reductase (MTHFR) gene polymorphisms have been reported to be associated with pancreatic cancer, but the published studies had yielded inconsistent results.We therefore performed the present meta-analysis. Methods: A search of Google scholar, PubMed, Cochrane Library and CNKI databases before April 2012 was conducted to summarize associations of MTHFR polymorphisms with pancreatic cancer risk. Assessment was with odds ratios (ORs) and 95% confidence intervals (CIs). Publication bias were also calculated. Results: Four relative studies on MTHFR gene polymorphisms (C667T and A1298C) were involved in this meta-analysis. Overall, C667T(TT vs. CC : OR = 1.61, 95%CI = 0.78 - 3.34; TT vs. CT : OR = 1.41, 95%CI = 0.88-2.25; dominant model: OR = 0.68, 95%CI = 0.40-1.17; recessive model: OR = 0.82, 95%CI = 0.52-1.30) and A1298C(CC vs. AA:OR=1.01, 95%CI=0.47-2.17; CC vs. AC: OR=0.99,95%CI=0.46-2.14; dominant model: OR=1.01, 95%CI = 0.47-2.20; recessive model: OR = 1.01, 95%CI = 0.80-1.26) did not increase pancreatic cancer risk. Conclusion: This meta-analysis indicated that MTHFR polymorphisms (C667T and A1298C) were not associated with pancreatic cancer risk.
