A Study of Motor Functional Evaluation in Stroke Patients (뇌졸중환자의 운동기능평가에 대한 연구)

  • Purpose: The purpose of this study was to evaluate the effects of exercise on functional status in stroke patients. The understanding of the course of recovery after stroke and factors affecting outcomes are important in planning and evaluation of stroke rehabilitation. Methods: To predict the outcom of stroke patient, we measured in the beginnig of rehabilitation, on 36 patients. The variables were gender, age, duration of rehabilitation treatment, motor assessment scale(MAS). Results: The patient age in 50-59 years was 41.7%. The stroke left side motor weakness were found 55.6%. The duration of rehabilitation treatment in 2-3 months was 50.0%. In the difference 24.63 for motor assessment scale after a rehabilitation treatment had significantly higher than 10.86 for motor assessment scale at initial. Gender by motor assessment scale after a rehabilitation treatment was 25.7 in male and 23.2 in female(P<0.05). Past history by motor assessment scale after a rehabilitation treatment was 26.7 in hypertension, 24.6 in cardiac disorder and 21.8 in diabetes mellitus(P<0.05). Conclusion: The subjective symptoms of motor assessment scale after a rehabilitation treatment was significantly associated with gender.

The Effect of Dong-gi Acupuncture(DGA) on Rehabilitation after Stroke (중풍(中風) 편마비(偏痲痺) 환자(患者)의 기능적 회복에 있어 침시술(鍼施術) 시(時) 동기요법(動氣療法)의 효과)

  • Objectives : The purpose of this study is evaluate of the effect of Dong-gi Acupuncture(DGA) on rehabilitation after stroke. Methods : 32 patients who had a stroke of paralysis were randomized into two groups. 17 patients were treated with DGA on sound side combined active or passive exercise on affected side. The other 15 patients were exclusive DGA group and the method retaining needles on acupoints of affected side for about 20 minutes were applied. We evaluated by Manual muscle test, Range Of Motion(ROM), Visual Analog Scale(VAS) and Modified Barthel index(MBI) before treatment, after 10 days and after 20 days. Results : In terms of power of lower limb, active ROM of shoulder abduction and MBI, the sample group showed statistically significant improve. In terms of power of lower limb ROM of hip joint abduction and affected side pain, significant difference between two groups was not shown. Conclusions : In this clinical study, Dong-gi Acupuncture therapy can be used for improving the power of upper limb, active ROM of shoulder abduction and MBI score after hemiplegic stroke.

Home Based Dysphagia Rehabilitation for Stroke Patients Using Information and Communication Technology

  • Background: Recently, a new home-based dysphagia rehabilitation method using information and communications technology (ICT) has been reported, but clinical evidence is still lacking. Objectives: To investigate the effects of home-based dysphagia rehabilitation using ICT on tongue muscle strength and volume in patients with developed dysphagia after stroke. Design: Randomized controlled trial design. Methods: Twenty patients who developed dysphagia after stroke were enrolled. The experimental group received dysphagia rehabilitation in the form of ICT-based home care. In contrast, the control group received traditional rehabilitation based on swallowing under the supervision of occupational therapists. All interventions were conducted five times a week for four weeks. Results: Both groups showed statistically significant increases in tongue muscle strength and volume after the intervention (P<.05, both). However, there were no significant differences in tongue strength or volume between the two groups after the intervention (P>.05, both). Conclusion: Home-based dysphagia rehabilitation using ICT showed effects similar to those of conventional swallowing rehabilitation in patients who developed dysphagia after stroke. These findings suggest that dysphagia rehabilitation can be conducted at home without the help of a therapist.

  • Objective: Final goal of nerve advancement therapy is to provide maximum ability to function independently in life to patients. This paper appraises and describes basic concepts of the virtual reality (VR) based exercise program to improve functional movement for neurologically impaired patients. Design: Review article. Methods: Stroke patients from the physical therapy department while wearing comfortable clothing receive therapy and also VR based motion therapy administered by the therapist in charge. After evaluation of stroke patients, therapy includes an exercise program that is suitable for use with stroke patients; stroke patients wear head-mounted display while in front of the computer, where the camera is located; they follow the action on the screen and the computer perceives the operation of the stroke patients according to subject accomplishment. Results: According to obstacle condition of stroke patients using the method, which is various environments after setting, in stroke patients, there is a possibility of presenting suitable therapeutic environments. The display presentation of the method, which is identical, causes difficulty for all stroke patients. According to subject accomplishment; stroke patients result in execution of repetition training and deepening study, which leads to mobility. Conclusions: The VR based rehabilitation training programs is a difference of the existing video training program, is immediate feedback and compensation method. It will provide rehabilitation training services for the family of the patient whose condition could be improved with rehabilitative therapy where it is a continuous circumstance as a matter of the social welfare facility therapy.

  • Purpose : The purpose of present study was to determine effects of action observation training on upper limb function after stroke. Training was progressed to imitation and intensive training after observation to required action in ADL. Methods : Among the single case study was used to ABA design. pre base line(A) was only collected participant information without intervention in 5 times. action observation intervention(B) was carried out 10 times and 5 times to base lime(A) after intervention. Results : Results indicated that 10-second test, box and block test, manual function test was increased when compared action observation intervention(B) to pre base line(A). Conclusion : To stroke action observation training was evaluated gross manipulation, dexterity and upper limb function in related with ADL. action observation training benefits were maintained after intervention(B) and showed improvement on upper limb function of stroke.

Clinical Application of AR System in Early Rehabilitation Program After Stroke: 2 Case Study

  • Purpose: To investigate the effect of an augmented reality (AR) system on muscle strength and function level of the paretic lower limb and the balance ability in the early rehabilitation program of acute stroke patients. Methods: The participants (30 or fewer days after stroke) were randomly assigned to receive intervention with an early rehabilitation program using an AR system (n=1) or an early rehabilitation program consisting of functional electrical stimulation and tilt table use (n=1). Patients in both subjects received interventions 4-5 times a week for 3 weeks. Results: In the paretic limb muscle strength, AR subject was increased from 15 to 39.6 Nm and Control subject was increased from 5 to 30.2 Nm. The paretic limb function of AR subject motor function was increased from 8 to 28 score and Control subject motor function was increased from 6 to 14 score. But sensory function was very little difference between the two subjects (AR subject: from 4 to 10 score, Control subject: from 3 to 10 score). In the balance ability, AR subject had more difference after intervention than control subject (AR subject: 33 score, Control subject: 22 score). Conclusion: The early rehabilitation program using the AR system showed a slightly higher improvement in the motor function of the paretic lower limb and balance ability measurement than the general early rehabilitation program. The AR system, which can provide more active, task-oriented, and motivational environment, may provide a meaningful environment for the initial rehabilitation process after stroke.

  • Purpose: The purpose of this study was to identify factors contributing to health-related quality of life in patients under rehabilitation after stroke. Methods: A descriptive correlational study design was used. Ninety-two stroke patients under rehabilitation from a university hospital were included. Data were collected on March and April, 2013 using structured questionnaires. Hierarchial multiple regression analysis was used to identify factors influencing health-related quality of life. Results: There were significant differences in health-related quality of life according to type of impairment. The health-related quality of life had significant correlations with age, functional dependency, and stress. Factors influencing health-related quality of life for stroke patients under rehabilitation were stress, functional dependency, type of impairment, and age, which explained about 34.5% of total variance. Conclusion: To promote health-related quality of life for stroke patients under rehabilitation, stress and functional dependency should be managed in this population, especially for older adults. Also these results can be utilized in the development of program for helping rehabilitation of stroke patients. The effect of depression and anxiety on health-related quality of life should also be addressed in future research.

The effects of a task-oriented circuit training program of lower limb on walking ability after stroke (순환식 하지 훈련이 뇌졸중 환자의 보행능력에 미치는 영향)

  • Purpose : The purpose of present study was to determine effects of a task-oriented circuit training(TOCT) for lower limb on walking ability after stroke. Methods : Twenty one chronic stroke patients participated. Participants were randomly divided into either TOCT group or control group(11 experimental, 10 control). All of participants were in-patients at local rehabilitation centre and had been receiving a traditional rehabilitation program, five days a week. TOCT group have additionally undergone for four weeks, three days a week, the TOCT program but control group was not received any additional program except the traditional rehabilitation program. The 10 m walking test (10MWT), the 2 min walking test (2MWT), the step test (ST) and the figure-8 walking test (F8WT) to measure a walking ability were carried out twice before and after training. Results : After participation in the program, subjects of TOCT demonstrated a significant improvement in the scores of the 10MWT, 2MWT, the ST, the F8WT. The control group had no change on the any tests. After the training, the results to improve significantly in TOCT group compared to post-test of control group were the time of 10MWT and the time and the step of curved walking of F8WT. Conclusion : The present study suggests that the TOCT program may become a useful strategy for enhancing walking ability in the rehabilitation of stroke patients.

Health beliefs of patients and caregivers of stroke rehabilitation in the implementation and impact the ability to perform everyday actions (뇌졸중 환자 및 보호자의 건강신념이 재활치료 이행 및 일상생활동작 수행능력에 미치는 영향)

  • Purpose : This study attempts to assist stroke patients lead independent daily life movements by providing basic data for stroke patients' successful rehabilitation program and understanding the relationship between health beliefs of stroke patients and their guardians, and patients' rehabilitation performance level and daily life movement fulfillment ability. Methods : 23 stroke patients receiving treatments at rehabilitation specializing hospital after getting diagnosed with a stroke, along with 23 guardians were selected as research subjects, and health belief was measured by modifying supplementing Byun Young-Hee(2002)'s health belief measurement tool for leg movement disorder patients. Research findings revealed the following relationship between health beliefs of stroke patients and their guardians, and patients' rehabilitation performance and daily life movement fulfillment ability. Results : The patients group's initial MBI score was $33.13{\pm}3.46$, and measurement after four weeks was $38.43{\pm}3.47$. As health beliefs got stronger, MBI score increased significantly(p<.05). Correlation analyses of factors that affect rehabilitation attendance rate showed that perceived susceptibility, seriousness and usefulness were statistically significant, and perceived disability was not statistically significant(p<.05). Guardians' health belief index(susceptibility, seriousness, usefulness, disability) had no correlation with rehabilitation attendance rate. Conclusion : In order to increase daily life movement fulfillment ability of patients who are being treated with stroke, health beliefs must be raised to increase changes in daily life movement fulfillment ability. Therefore, it is believed that managing programs that can increase health beliefs of stroke patients can allow patients obtain positive health beliefs, further increase rehabilitation performance rate of stroke patients as well as independent daily life ability.

Conclusions : Overuse sound tends after the stroke occurrence in the early stage in order to recompense for stroke, and at the time of a new aspect called learned nonuse syndrome by a movement of a paralysis part dusting off wealth with this step thing later. Constraint induced movement using self efficacy could be an effective for improving function of stroke.
