Development of Home Care Nursing Intervention Protocols for Spinal Cord Injury Patients Based on NIC (NIC에 근거한 척수손상환자의 가정간호 중재 프로토콜 개발)

  • Spinal cord injury patients need rehabilitation after the acute stage. They should be cared for at home to decrease hospitalization. Home care nurses play an important role in making spinal cord injury patients who have physical. and psychosocial problems do their best. For effective care, home care nurses need standardized nursing intervention protocols for spinal cord injury patients, but they are rarely developed. Therefore, this study was conducted to develop home care nursing protocols, based on NIC, that are applicable to spinal cord injury patients at home. Forty home care nursing charts of spinal cord injury patients registered in a home care nursing agency from July 1st, 1994 to August 31st, 1999 in S city were analyzed. Fifteen home care nurses participated in this study as a user validity validation group, The results of this study are as follows. 1. Fifteen nursing diagnoses were classified through the frequency analysis of home care nursing charts and previous literature for 40 spinal cord injury patients: altered urinary elimination, constipation, high risk for impaired skin integrity, chronic pain, impaired skin integrity, impaired social interaction, knowledge deficit, bowel incontinence, high risk for injury, altered role performance, care giver role strain, impaired physical mobility, sexual dysfunction, dysreflexia, and ineffective breathing pattern. 2. Based on validation by experts and user validities, 93 nursing interventions which were above ICV(Index of Content Validity) .80 were chosen. 3. Nursing intervention protocols which showed above ICV .80 were developed.

A Survey on Cancer Screening Among Nurses at General Hospital in Busan (일 지역 종합병원 간호사의 암 조기검진 수검실태에 관한 조사연구)

  • Purpose : Cancer is th e most frequent cause of death in Korea. Cancer screening can save lives through early detection. This study was to investigate major cancer screening rates and the reasons for not having been screening. Method : 210 participants of registered nurses from 5 general hospitals in Busan, Korea were completed a structured self-administered questionnaire. Data were analyzed using descriptive statistics with SPSS WIN 10.0. Results : The cancer screening rates of the subjects were 53.8% in Pap smear test for cervical cancer, 41.0% in gastroendoscopy for stomach cancer, 15.7% in mammograpy for breast cancer, and 3.8% in colonoscopy. And the higher proportions of having regular screening were 9, 5% in Pap smear test and 2.9% in gastroendoscopy. The primary reason related to not having a screening test was 'seems to be healthy'. Conclusion : The findings showed the necessity of recognizing and educating Korean nurses to have a regular cancer screening for their health management.

Effects of sense of humor and optimism on the nursing performance of the clinical nurse in the advanced general hospital (상급종합병원 임상간호사의 유머감각과 낙관성이 간호업무성과에 미치는 영향)

  • This study was done to explore the correlation among sense of humor, optimism, and nursing performance and identify factors related nursing performance among nurses. A survey was conducted for April 2018 with self-report questionnaire. Participants were 181registered nurses working in three tertiary hospitals. Sense of humor was positively correlated optimism, and nursing performance. Factors affecting nursing performance included educational background, work shift, clinical career. Nursing performance had a positive correlation with sense of humor and optimism. And work shift, sense of humor and optimism accounted for 38.6% of the variance in nursing performance. Therefore, quality management for developing nursing professionalism support for improving nursing performance and sense of humor and optimism . Future research will be needed to clarify the effects of the sense of humor and optimism promotion program on nursing performance.

Mediation Effect of Job satisfaction in the Relationship between Nurses' violence experience and turnover intention (임상간호사의 폭력경험과 이직의도 관계에서 직무만족의 매개효과)

  • This study investigated how clinical nurse's violence experience influenced turnover intention and job satisfaction and how job satisfaction mediates the relationship between violence experience and turnover intention. In order to get the answers, a survey of 213 registered nurses from 3 hospitals in U-city and analyzed with SPSS/WIN 25.0 version. The survey was conducted using a self-reporting questionnaire February 2019 to April 2019. Nurse's violence experience was negatively correlated job satisfaction but was positively correlated with turnover intention. Hierarchical regression analysis showed violence that explained 16.5% of job satisfaction and explained 6.1% of turnover intention. Job satisfaction worked as a partial mediation between violence and turnover intention. It is necessary to reduce violence and to increase job satisfaction for reduce turnover intention in strategies of hospital.

  • The concept of parish nursing began in the late 1960s in the United States when increasing numbers of churches employed registered nurses (RNs) to provide holistic, preventive health care to the members of their congregations. Parish nursing role was developed in 1983 by Lutheran chaplain Granger Westberg, and provides care to a variety of church congregation of various denominations. The parish nurse functions as health educator, counselor, group facilitator, client advocate, and liaison to community resources. Since these activities are complementary to the population-focused practice of community health' CNSs, parish nurses either have a strong public health background or work directly with both baccalaureate-prepared public health nurses and CNSs. In a Midwest community in U.S.A., the Healthy People 2000(1991) objectives are being addressed in health ministries through a coalition between public health nurses and parish nurses. Parish nursing is in the beginning state in Korea and up untill now, there has been no research was conducted on concrete role of korean parish nurses. The main purpose of this study was to identify, classify and analyze activities of parish nurses. The other important objective of this study was to establish an effective approach and direction for parish nursing and provide a database for korean parish nursing model through analysis and' classification of the content of the nursing record which included nursing activities. This study was a descriptive survey research. The parish nurses were working in churches where the demonstration project developed on parish nursing. The study was done on all nursing records which were working in churches where the demonstration project developed on parish nursing. The study was done on all nursing records which were documented by parish nurses in three churches from March, 1995 to February, 1996. Namsan, Taegu Jeei and Nedang presbyterian churches in Taegu and Keimyung nursing college incooperated together for the parish nursing demonstration project. The data analysis procedure was as follows: First, a record analysis tool was developed and second, the data was collected, coded and analyzed, the classification for nursing activities was developed through a literature review, from which the basic analysis tool was produced and cotent validity review was also done. The classification of the activities of parish nurses showed 7 activitity categories. 7 activity categories consisted of visitation nursing, health check-ups, health education, referring, attending staff meetings, attending inservices and seminar, volunteers coordinating. The percentage of activities were as follows: Visitation nursing(A: 51.6%, B: 55%, C: 42.6%) Health check-ups(A: 13.5%, B: 12.1%, C: 22.3%) Health education(A: 13.5%, B: 13.2%, C: 18.2%) Referring(A: 1.4%, B: 4.2%, C: 2.4%) Attending staff meeting(A: 18.8%, B: 13.0%, C: 12.2%) Attending inservices and seminar(A: 1.5%, B: 2.2%, C: 2.1%) Volunteers coordinating(A: 0.3%, B: 0.4%, C: 0.0%) To establish and develope parish nursing delivery network in Korea, parish nurses role, activities and boundaries of practice should be continuously monitored and refined every 2 years. Also, It is needed to develope effective nursing recording system based on the need assessment research data of various congregation members. role, activities and boundaries of practice and arrangement of the working structure, continuing education, cooperation with community resources and structuring and organizing parish nursing delivery network. Nurses' Knowledge, Attitude and Education Needs for Sexuality of the Elderly (간호사의 노인의 성에 대한 지식, 태도 및 성교육 요구도)

  • The purpose of this study was to investigate nurses' sexual knowledge, attitude and sex education needs of the elderly. The participants were 235 nurses who were working at hospitals, geriatric hospitals and nursing homes at I city, K city and J city in Jeollabukdo province. A survey was conducted using self questionnaire scale from December 1, 2012 to December 10, 2012. First, the mean score of sexual knowledge was $15.91{\pm}6.43$. It was statistically significant according to marital status(p=.024), workplace(p<.001), duration of work experience(p=.006), sex education needs(p<.001), sex education acceptance(p<.001). Second, the mean score of attitude was $38.26{\pm}4.41$. It was statistically significant according to workplace(p=.033), living with the elderly(p=.009), sex education needs(p<.001) and sex education acceptance(p<.001). Third, the mean score of sex education needs was $35.97{\pm}6.28$. It was statistically significant according to age(p=.023), living with the elderly(p=.014), sex education needs(p<.001) and sex education acceptance(p<.001). Nurses' knowledge on the elderly's sexuality showed a significantly positive correlation with attitude(p<.001) and sex education needs(p<.001). This study indicated that general education on sexuality of the elderly for undergraduate nursing students and registered nurses is essential. More active nursing care on the elderly's sexuality is required when providing them with health assessment and nursing interventions.

Factors Influencing the Retention intention of Home visiting nurses with Long-term care insurance system in Korea: Focusing on Professionalism, Job satisfaction and Resilience (노인장기요양보험 방문간호사의 재직의도에 영향을 미치는 요인 : 전문직업성, 직무만족도, 회복탄력성을 중심으로)

  • This is a descriptive research study that aims to identify the effects of professionalism, job satisfaction, and resilience on the retention intention of home visiting nurses from the long-term care insurance system of Korea. Data were collected from self-administered surveys completed by 141 home visiting nurses of registered visiting-nursing institutions in the long-term care insurance database (April 1-May 31, 2019). Analyses were independent t-tests, one-way analysis of variance (ANOVA), the Scheffé test, Pearson's correlation analysis using SPSS/WIN 22.0, and multiple hierarchical regression analyses. Results showed that the mean professionalism score was 3.51/5, 3.27/5 for job satisfaction, 3.79/5 for resilience, and 4.04/5 for retention intention. A positive correlation existed between retention intention and professionalism (r = .272), job satisfaction (r = .201), and resilience (r = .530). The final regression model showed that resilience (β = .455, p < .001) and job satisfaction (β = .175, p = .016) significantly affect retention intention. The variables' model explanation power was 32.0% (F = 11.968, p < .001). The results show the need for strategies to improve resilience, followed by strategies to improve job satisfaction to increase the retention intentions of these home visiting nurses.

Factors affecting PTSD symptoms among hospital nurses during the COVID-19 pandemic in Korea (코로나19 팬데믹 상황 중 병원 간호사의 PTSD 증상에 영향을 미치는 요인)

  • This study is to investigate the factors affecting post-traumatic stress disorder (PTSD) symptoms among hospital nurses during the COVID-19 pandemic in Korea. Cross-sectional, descriptive design is used in this study. Data collection was completed through an online self-administered survey from December 2020 to January 2021 among 180 registered nurses dealing with COVID-19 patients at hospitals. This survey includes socio-demographic questions, including a 22-item PTSD questionnaire, a 14-item type D personality questionnaire, a 25-item resilience questionnaire, and a 23-item Social Support Scale questionnaire. 56.1% of the subjects in this study were at risk of PTSD. In the high-risk group for PTSD, resilience and social support were lower than those in the low-risk group for PTSD. But there was no statistically significant difference in both variables (resilience t=0.21, p=.836, social support t=1.07, p=.287). However, education (OR = 2.23, p= .041) and type D personality (OR = 3.67, p < .001) were significant factors for PTSD symptoms. The results of the study can be utilized to recognize PTSD in nurses by identifying factors influencing PTSD during epidemics such as COVID-19, and to apply management systems such as psychological programs to help overcome them.

A Study on Differences of Opinions on Home Health Care Program among Physicians, Nurses, Non-medical personnel, and Patients. (가정간호 사업에 대한 의사, 간호사, 진료관련부서 직원 및 환자의 인식 비교)

  • The government has adopted a policy to introduce Home Health Care Program, and has established a three stage plan to implement it. The three stage plan is : First, to amend Article 54 (Nurses for Different Types of Services) of the Regulations for Implementing the Law of Medical Services; Second, to tryout the new system through pilot projects established in public hospitals and clinics; and third, to implement at all hospitals and equivalent medical institutions. In accordance with the plan, the Regulation has been amend and it was promulgated on January 9,1990, thus establishing a legal ground for implementing the policy. Subsequently, however, the Medical Association raised its objection to the policy, causing a delay in moving into the second stage of the plan. Under these circumstances, a study was conducted by collecting and evaluating the opinions of physicians, nurses, non-medical personnel and patients on the need and expected result from the home health care for the purpose of help facilitating the implementation of the new system. As a result of this study, it was revealed that: 1. Except the physicians, absolute majority of all other three groups - nurses, non-medical personnel and patients -gave positive answers to all 11 items related to the need for establishing a program for Home Health Care. Among the physicians, the opinions on the need for the new services were different depending on their field of specialty, and those who have been treating long term patients were more positive in supporting the new system. 2. The respondents in all four groups held very positive view for the effectiveness and the expected result of the program. The composite total of scores for all of 17 items, however, re-veals that the physicians were least positive for the- effectiveness of the new system. The people in all four groups held high expectation on the system on the ground that: it will help continued medical care after the discharge from hospitals; that it will alleviate physical and economic burden of patient's family; that it will offer nursing services at home for the patients who are suffering from chronic disease, for those early discharge from hospital, or those who are without family members to look after the patients at home. 3. Opinions were different between patients( who will receive services) and nurses (who will provide services) on the types of services home visiting nurses should offer. The patients wanted "education on how to take care patients at home", "making arrangement to be admitted into hospital when need arises", "IV injection", "checking blood pressure", and "administering medications." On the other hand, nurses believed that they can offer all 16 types of services except "Controlling pain of patients", 4. For the question of "what types of patients are suitable for Home Health Care Program; " the physicians, the nurses and non-medical personnel all gave high score on the cases of "patients of chronic disease", "patients of old age", "terminal cases", and the "patients who require long-term stay in hospital". 5. On the question of who should control Home Health Care Program, only physicians proposed that it should be done through hospitals, while remaining three groups recommended that it should be done through public institutions such as public health center. 6. On the question of home health care fee, the respondents in all four groups believed that the most desireable way is to charge a fixed amount of visiting fee plus treatment service fee and cost of material. 7. In the case when the Home Health Care Program is to be operated through hospitals, it is recommended that a new section be created in the out-patient department for an exclusive handling of the services, instead of assigning it to an existing section. 8. For the qualification of the nurses for-home visiting, the majority of respondents recommended that they should be "registered nurses who have had clinical experiences and who have attended training courses for home health care". 9. On the question of if the program should be implemented; 74.0% of physicians, 87.5% of non-medical personnel, and 93.0% of nurses surveyed expressed positive support. 10. Among the respondents, 74.5% of -physicians, 81.3% of non-medical personnel and 90.9% of nurses said that they would refer patients' to home health care. 11. To the question addressed to patients if they would take advantage of home health care; 82.7% said they would if the fee is applicable to the Health Insurance, and 86.9% said they would follow advises of physicians in case they were decided for early discharge from hospitals. 12. While 93.5% of nurses surveyed had heard about the Home Health Care Program, only 38.6% of physicians surveyed, 50.9% of non-medical personnel, and 35.7% of patients surveyed had heard about the program. In view of above findings, the following measures are deemed prerequisite for an effective implementation of Home Health Care Program. 1. The fee for home health care to be included in the public health insurance. 2. Clearly define the types and scope of services to be offered in the Home Health Care Program. 3. Develop special programs for training nurses who will be assigned to the Home Health Care Program. 4. Train those nurses by consigning them at hospitals and educational institutions. 5. Government conducts publicity campaign toward the public and the hospitals so that the hospitals support the program and patients take advantage of them. 6. Systematic and effective publicity and educational programs for home heath care must be developed and exercises for the people of medical professions in hospitals as well as patients and their families. 7. Establish and operate pilot projects for home health care, to evaluate and refine their programs.

EXPLORATION OF OPINIONS OF KOREAN NURSES ABOUT THE REGISTERED NURSE BACCALAUREATE CONCEPT (등록간호원 학사편입제도에 대한 한국 간호원의 의견 조사연구)

  • 사회 구조가 날로 복잡해짐에 따라 현대는 보다 노련하고 보다 높은 지식층의 개인에 대한 요구가 점차 증가하게 되었으며 이러한 사회적 경향은 전문직의 발전에 박차를 가하게 되었다. 본 논문의 목적은 한국 간호원들의 등록 간호원 학사 편입제도에 대한 의견을 조사 발표하는 것으로 질문지를 이용한 서술적 방법에 의한 연구 조사이다. 본 논문의 대상이 되었던 113명의 한국 간호원들 중 특히 학사 출신 간호원들은 간호 교직을 그들의 생의 목표로 삼는 경향이 더욱 두드러 지는데 비해 3년제 간호학교 출신 간호원들은 간호직 외에 보다 넓은 분야에서 그들의 생의 목표를 찾는 경향이 보였다. 질문에 응해준 간호원들의 대부분은 한국에 있어서 등록 간호원 학사 편입제도의 가치와 필요성에 대해서 지대한 관심과 갖고저 하는 희망을 보여 주었고 특히 3년제 간호학교 출신 간호원들은 이 제도에 대한 시급한 필요성과 큰 가치에 적극 호응하는 가장 높은 교육열을 보여 주었다.
