Non-tuberculous Mycobacterial Lung Disease Presenting as a Solitary Pulmonary Nodule (고립성 폐결절로 발현된 비결핵성 마이코박테리움 폐질환 1예)

  • We report a case of Mycobacterium intracellulare pulmonary infection presenting as a solitary pulmonary nodule (SPN). A 35-year-old male was admitted due to a SPN in the right upper lobe which was detected on the chest radiography being examed due to recurrent cough for 1 year. The computed tomography (CT) revealed a spiculated nodule containing air-bronchogram, which was suspicious of malignancy. We performed transbronchial biopsy and the pathology showed granulomatous inflammation with caseous necrosis. Under the presumptive diagnosis of pulmonary tuberculosis, we started anti-tuberculous medication including isoniazid, rifampin, ethambutol, and pyrazinamide. In one month, however, the sputum culture was positive for Mycobacterium intracellulare. The follow-up chest CT showed slight aggravation of the previous lesions. Under the final diagnosis of Mycobacterium intracellulare pulmonary infection presenting as a solitary pulmonary nodule, we changed the regimen to rifampin, ethambutol, and clarithromycin. The follow-up chest CT after the completion of treatment, revealed resolution of the previous lesions.

A Case of Advanced Gastric Cancer with Virchow's Node and Lung Metastasis Successfully Resected after Combined Chemotherapy of Taxotere, CDDP, and 5-FU (선행화학요법으로 원격전이의 관해 후 위절제를 시행한 원격전이를 동반한 위암 1예)

  • We report a case of advanced gastric cancer with Virchow's node and lung metastasis that responded remarkably to preoperative chemotherapy. A 47-year-old female patient was diagnosed as having incurable advanced gastric cancer with Virchow's node and multiple lung metastasis. Preoperative chemotherapy with Taxotere, CDDP and 5FU was carried out. After four courses of the regimen, the Virchow's node and the lung metastasis had disappeared, and a marked reduction of the gastric lesion was observed on the CT scan. Consequently, the patient underwent a total gastrectomy with D2 lymph node dissection. On histopathological examination, cancer cells were found to have infiltrated up to the muscle layer of the gastric wall, and 42 out of 60 resected lymph nodes were found to be metastatic. Efficacy and Safety Profile of TS-1 or TS-1/CDDP in Patients with Advanced Gastric Cancer (진행성 위암환자에 있어 TS-1 또는 TS-1/CDDP의 항암효과 및 안정성)

  • Purpose: Although several chemotherapy regimens used against advanced gastric cancer (AGC) have been studied extensively in an attempt to further improve the prognosis of patients, to date, no standard chemo-therapeutic regimens have been established. The aim of this study was to determine the anti-tumor efficacy and safety of TS-1 or TS-1 plus cisplatin (CDDP). Material and Methods: We treated 78 patients with AGC either with $80\;mg/m^{2}$ of TS-1 for 28 days, which was followed by a 2-week rest, or with $80\;mg/m^{2}$ of TS-1 for 21 days and $80\;mg/m^{2}$ of CDDP on day 8 every 5 weeks. Results: Tumor response rates in the neoadjuvant chemotherapy group and in the recurrent or post-palliative surgery group were 87.5% and 32.4%, respectively, and they were 28.6% and 48.4%, respectively, in the TS-1 group and the TS-1 plus CDDP group. The survival rates in the recurrent and the post-palliative surgery group were significantly different according to the degree of tumor response (P=0.0016), but the one-year survival rates according to the kinds of regimens (TS-1 or TS-1/CDDP group) were not significantly different. The incidences of grade 3 or 4 adverse effects in the TS-1 and the TS-1/CDDP groups were 14.3% and 36.8%, respectively. Conclusion: The anti-tumor efficacy and safety of TS-1 and TS-1 plus CDDP in Korean patients with AGC seemed to be high with modest adverse effects, thus suggesting the possible use of this regimen as a standard chemotherapy for gastric cancer.

A Comparative Study of the Effects of Public Health Nursing: Home Visits to Patients with Tuberculosis (결핵환자 간호에 있어서 가정방문이 미치는 영향에 관한 조사 연구)

  • Home visiting as a Public Health Horsing function is believed to be of therapeutic value to, the patient. However, home visiting is time consuming and expensive. Is the gain in knowledge and treatment for patients with Tuberculosis in Korea enough to make the necessary outlay in finances and personnel worthwhile\ulcorner While this study does not attempt to completely answer this question it does, under the following objectives, attempt to answer part of the question. The objectives of the study were to l) ascertain if there is a difference between patients, who receive home visits from the public Health Nurse and those who do not, in the following areas: a) their compliance with medical regimen, b) their ability to answer general questions about Tuberculosis, and c) their compliance with medical advice concerning prevention (B.C.G. immunization) and early diagnosis (contact X-rays), and 2) to determine if there is any correlation between the patient's answers to questions about Tuberculosis and his action both in the areas of treatment and prevention. The patients participating in the study were all newly diagnosed patients at Kwangju Christian Hospital. A control group and an experimental group were selected. The patients in the control group were seen according to the regular schedule at the Kwangju Christian Hospital except that they received no home visits from the Public Health Nurses. The patients in the experimental group were visited on an average of three times during the first two months of their treatment by the investigator, a Public Health Nurse. At the end of two months the patients in the two groups were compared as to compliance both in the treatment and preventive areas. They were also compared according to their answers to a questionnaire regarding both the prevention and treatment of Tuberculosis. The following results were obtained : 1. Patients in the experimental group (68.2%) showed a significantly higher compliance rate for medical treatment than patients in the control group (43.2%). 2. Patients in the experimental group (87.5%) showed a higher compliance rate for B.C.G. immunization than those in the control group (40%). 3. Women patients in the experimental group showed a higher mean score (7.2$\pm$2.6) on the questionnaire than did women in the control group (6.2$\pm$3.4). The results of this study seem to indicate that while home visiting is beneficial to the patient with Tuberculosis as far as treatment is concerned, something more concrete needs to be done if home visiting is to help the patient learn more about Tuberculosis and its prevention. Further study is indicated in the following areas: 1) A similar type of study over a longer period of time involving more subjects and using Korean Public Health Nurses to make the home visits. 2) Study to develop an adequate approach to education considering the problems unique to patients with Tuberculosis living in Korea.

Effects of Feeding Frequency on Grwoth, Feed Efficiency and Body Composition of Juvenile Korean Rockfish (Sebastes schlegeli) (배합사료 급여 횟수가 조피볼락의 성장, 사료효율 및 체성분에 미치는 영향)

  • A 15-week feeding experiment was conducted to determine the effects of feeding frequency from 2 different formulated diets on growth, feed efficiency and body composition in juvenile Korean rockfish (Sebastes schlegeli). Duplicate groups of the 30 fish averaging 25 g were fed the control diet containing $58\%$ fish meal and the experimental diet containing $40\%$ fish meal, $10\%$ meat meal, $3\%$ corn gluten meal, $3\%$ soybean meal, and $3\%$ blood meal as protein sources at different feeding frequencies of 2 times daily, once a day in the morning or afternoon, once in 2 days, or given no food. Growth and feed efficiency of the fish were affected by diets (P<0.05), whereas these values were unaffected by feeding frequencies (P>0.05). Feed intake (${\%}$ of body wt.) of the fish fed once in 2 days was lower than that of the fish fed 2 times daily and once a day (P<0.05), Moisture, protein, lipid and ash contents of liver and muscle were not influenced by different diets and feeding frequencies (P>0.05). These results indicate that once a day or once in 2 days feeding regimen must be more effective and economical than that of 2 times daily for juvenile Korean rorkfish.

EVALUATION OF BIODEGRADABILITY, BIOCOMPATIBILITY AND TISSUE REGENERATIVE CAPACITY OF SYNTHETIC BIODEGRADABLE MEMBRANES IN BEAGLE DOGS (수종의 생분해성 차폐막의 생채분해도, 생채친화도 및 조직재생유도 능력에 관한 실험적 연구)

  • The purpose of this investigation was to evaluate on the biodegradability, biocompatibility and tissue regenerative capacity of synthetic bioabsorbable membranes in beagle dogs. For animal study, 9 adult beagle dogs were used to examination, on the surgical implantation of membranes and histological analysis. In each animal, the 3rd and 4th premolars of the both sides of the mandible were selected as test teeth. Two types of bioresorbable membranes including "Guidor membrane", "S-membranes" were used to examining for biological activity, and also Gore-tex membranes was used for positive control. Surgically created defects were made in 2 premolars of both sides of the mandible at $3{\times}4mm^2$ in size and tested membranes were implanted in the defected area. A plaque control regimen was instituted with daily tooth brushing with a 0.1% chlorhexidine digluconate during experimental periods. All the experimental animals were sacrificed after 2, 4, and 8 weeks from surgery and undecalcified slides were prepared using the "sawing and grinding" technique described by Donath and Breuner". In biodegradability, all the membranes were started their biodegradation from two weeks after implantation and gradually demolished of their frame morphology from eight weeks. However, demolition of membranes in 8 weeks after implantation was highest in Guidor membranes and followed by S-membranes. Biocompatibilityof two kinds of biodegradable membranes including Guidor and S-mambrane were shown to be well tolerated to the surrounding tissue, and were minimal accumulation of inflammatory cell infiltration around the implanted membranes to compare with Gore-tex membrane. Regeneration of defected alveolar bone was initiated from two weeks of membrane implantation and new bone formation was gradually increased from that time. However, pattern of new bone formation on the defected areas of two kinds of biodegrable membranes was almost similar and quite competitive comparing with Gore-tex membrane. These results implicate that bioresorbable membranes should be highly useful tool for guided tissue regeneration of periodontal defects.

A Case Report of Tuberculous Brain Abscess and Tuberculous Peritonitis Developing Due to Paradoxical Reactions (역설적 반응에 의하여 동시에 결핵성 뇌농양 및 결핵성 복막염이 발생한 증례 1예)

  • While receiving appropriate treatment, patients with tuberculosis occasionally have unusual, paradoxical reactions, with transient worsening of lesions or the development of new lesions. This report is a case of tuberculosis brain abscess and tuberculosis peritonitis with intra-abdominal abscess that developed during appropriate anti-tuberculosis chemotherapy. A 45-year-old male patient had been diagnosed as with all-drug susceptible pulmonary tuberculosis with pleurisy. Subsequently, the patient underwent standard treatment with anti-tuberculosis therapy; the pulmonary lesions improved. Three months after initial treatment, the patient developed brain abscesses and peritonitis. With the addition of corticosteroid treatment, the patient's neurologic symptoms were relieved. Exploratory laparotomy with surgical drainage was performed and a diagnosis of tuberculosis peritonitis was confirmed on biopsy. Anti-tuberculosis therapy was continued for 19 months, the patient improved eventually without further complications, although the therapeutic regimen had not been altered. In this case, the paradoxical response to treatment may have been involved in the pathogenesis of disease.

Effects of the Prunus persica Batsch var. davidiana Max. Extract on the Lipid Compositions and Enzyme Activities in Hyperlipidemic Rats (돌복숭아(Prunus persica Batsch var. davidiana Max.) 추출액이 고지혈증 흰쥐의 생체 내 지질성분 및 효소활성에 미치는 영향)

  • The purpose of this study was designed to observe the effects of the feeding Prunus persica Batsch var. davidiana Max. extract on the concentrations of the lipids and blood glucose in the S.D. rats fed the experimental diets for 5 weeks. Concentrations of total cholesterol, atherosclerotic index, LDL, LDL-cholesterol, free-cholesterol, cholesteryl ester, triglyceride(TG), phospholipid(PL) and blood glucose in serum were significantly higher in the cholesterol administration groups (groups BCG (cholesterol+water), BCPG (cholesterol+ Prunus persica 5.0 g% extract) than those in the control group (group BG, basal diet+water). But the concentrations of total cholesterol, atherosclerotic index, LDL, LDL-cholesterol, free-cholesterol, cholesteryl ester, TG, PL and blood glucose in serum were remarkably lower in the group BCPG than those in the group BCG. In the ratio of HDL-cholesterol concentration to total cholesterol and HDL-cholesterol concentration, Prunus persica 5.0 g% extract administration group was higher percentage than in the group BCG. The activities of aspartate aminotransferase (AST), alanine aminotransferase(ALT), lactate dehydrogenase(LDH) and alkaline phosphatase (ALP) in serum were rather lower in the Prunus persica 5.0 g% extract administration group(group BCPG) than in the cholesterol diet group(group BCG). From the above research, Prunus persica Batsch var. davidiana Max. were effective on the improvement of the blood glucose, lipid compositions in serum of dietary hyperlipidemic rats. And particularly, Prunus persica Batsch var. davidiana Max. was more effective therapeutic regimen for the control of metabolic derangements in adult disease.

Head and neck extra nodal NHL (HNENL) - Treatment Outcome and Pattern of failure - A Single Institution Experience

  • Background: Extra nodal lymphoma (ENL) constitutes about 33 % of all non-Hodgkin's lymphoma. 18-28% develops in the head and neck region. A multimodality treatment with multi-agent chemotherapy (CT) and radiotherapy (RT) is considered optimum. Materials and Methods: We retrieved the treatment charts of patients of HNENL treated in our institute from 2001-2012. The charts were reviewed and the demographic, treatment details and outcome of HNENL patients were retrieved using predesigned pro-forma. Results: We retrieved data of 75consecutive patients HNENL. Median age was 47years (Range: 8-76 years). Of the 75 patients 51 were male and 24 were female. 55patients were evaluable. The patient and tumor characteristics are summarized in Table 1. All patients were staged comprehensively with contrast enhanced computed tomography of head, neck, thorax, abdomen, pelvis and bone marrow aspiration and biopsy 66 patients received a combination multi-agent CT with CHOP being the commonest regimen. 42 patients received 4 or lesser number of cycles of chemotherapy whereas 24received more than 4 cycles chemotherapy. Post radiotherapy, 41 out of 42 patients had a complete response at 3 months. Only 21patients had a complete response after chemotherapy. All patients received radiation (mostly involved field radiation) as a part of the treatment. The median radiation dose was 45 Gray (Range: 36 Gray-50 Gray). The radiation was planned by 2D fluoro simulation based technique in 37cases and by 3 Dimensional conformal radiation therapy (3DCRT) in 36 cases. Two patients were planned by the intensity modulated radiation therapy (IMRT) technique. IMRT was planned for one thyroid and one nasal cavity primary. 5 patients experienced relapse after a median follow up of 19 months. The median survival was not reached. The estimated two and three year survival were 92.9% (95%CI- 68.6- 95.35) and 88% (95%CI- 60.82 - 92.66) respectively. Univariate analysis revealed higher stage and poorer baseline performance status to be significantly associated with worse progression free survival. 5 patients progressed (relapse or primary disease progression) after treatment. Of the 5 patients, two patients were primary orbital NHL, two patients had NHL nasal cavity and one was NHL thyroid. Conclusions: Combined modality treatment in HNENL confers excellent disease control with acceptable side effects.
